Elevation of laminin and beta-subunit of prolyl 4-hydroxylase in the sera of human subjects with Schistosomiasis mansoni.

Serum levels of several markers for liver fibrosis were measured utilizing three groups of human subjects related with schistosomiasis mansoni in northeast Brazil; (1) 20 Schistosoma mansoni egg-positives, who have never been administered with anti-schistosomal drugs, (2) 29 egg-negative inhabitants in the endemic area of schistosomiasis, and (3) 23 egg-negative Japanese immigrants in the non-endemic area. None of these sera were positive for antibody to the surface antigen of human hepatitis B (HBs) and circulating HBs antigen. There was no significant difference in the serum levels of N-terminal peptide of procollagen type-III between the egg-positive subjects and either of the egg-negative Brazilian or Japanese immigrants, whereas the mean value of serum laminin significantly increased in the egg-positive subjects. A significantly higher concentration of serum immunoreactive beta-subunit of prolyl 4-hydroxylase (IR beta PH) was also observed in the egg-positive subjects only in comparison with that of the egg-negative Brazilian. Serum laminin and IR beta PH concentrations of the egg-positive subjects did not correlate with the absorbance of enzyme-linked immunosorbent assay (ELISA) which utilized crude antigens isolated from schistosome adults or eggs. No significant difference in these two parameters was observed between two subgroups of the egg-negative Brazilian or Japanese immigrants divided according to the serological data by ELISA. These findings suggest that serum laminin and IR beta PH levels are worth further evaluation for their usefulness as the marker for liver fibrosis in schistosomiasis.

[Measuring periodontal pockets with contrast media. 1. Preparation of contrast media].

Periodontal pocket is main clinical symptom of periodontitis. Measurement of periodontal pocket depth is the most important examination. The purpose of this study was to get contrast medium radiography of periodontal pocket. The present study was compared with contrast media of varying concentration. Contrast medium-sample containing 40% Sodium Iothalamate and 30% Meglumine Adipiodone had slight higher radiopacity than the others. These contrast media-sample were adequate to clinical practice over 40% concentration. Using Sodium Iothalamate and Meglumine Adipiodone, radiopacity was able to differ from radiopacity of teeth at 60%-100%.

[The efficacy of combined use of 1 alpha-hydroxyvitamin D3 with calcium supplements in the treatment of osteoporosis].

Synthetic analogue of active vitamin D metabolite, 1 alpha (OH) D3, has been widely used in the treatment of osteoporosis. However, the most effective method of treatment is yet to be established. Importance of calcium supplement to improve the calcium metabolism in osteoporosis is also reported by many authors. We have studied the combined effects of 1 alpha (OH)D3 with calcium supplement in preventing progressive decrease of bone mass in patients with osteoporosis. Sixty-six cases of postmenopausal and senile osteoporosis were divided into two groups: one treated with calcium alone 1,000 mg a day and the other treated with calcium 1,000 mg and 1 alpha (OH)D3 0.5 microgram a day, and both groups were followed for 24 months. Bone mass was evaluated by microdensitometry of the roentgenograms of the second metacarpal bone. The results revealed that the combined use of 1 alpha (OH)D3 with calcium has a significantly more favorable effect than the use of calcium alone in preventing bone loss. Therefore, 1 alpha (OH)D3 with calcium is useful in the treatment of osteoporosis. The conclusion would be further confirmed with longer term study and a more accurate method of measuring bone mass.

Abnormal breathing patterns in patients with mitral stenosis: a possible compensatory role.

The breathing patterns of 37 patients with mitral stenosis were investigated in standing position by Konno-Mead analysis. Hemodynamic parameters were measured by cardiac catheterization, pulmonary function variables by pulmonary function testings and distribution of pulmonary perfusion by Tc-99m-MAA scintigraphy. Seventeen patients displayed rib cage dominant breathing patterns, and 6 patients displayed paradoxical breathing patterns, whereas 14 patients displayed normal breathing patterns. None of these patients exhibited an abdomen dominant breathing pattern. The patients with abnormal breathing patterns (rib cage dominant or paradoxical) displayed significantly higher values of pulmonary arterial pressure (p less than 0.01), capillary wedge pressure (p less than 0.01), total pulmonary vascular resistance (p less than 0.01) and mitral valve gradient (p less than 0.05) as well as smaller mitral valve area (p less than 0.05) and lower values of both in PaO2 (p less than 0.05) and diffusing capacity (p less than 0.05) than patients with normal breathing patterns. Abnormal distribution of pulmonary perfusion with hyperperfusion in the upper zone of the lung were found to be associated with the abnormal breathing patterns. Since rib cage dominant and paradoxical breathing patterns are known to increase ventilation in the upper zone of the lung, these abnormal breathing patterns may represent a compensatory mechanism serving to match ventilation and perfusion.

Natural history of atrial fibrillation.

The clinical significance of atrial fibrillation was analyzed in cases with chronic or acute heart disease and its significance at the acute and chronic stages of the disease was investigated in various disease groups. The types of disease, number of patients, and incidence of atrial fibrillation were: atrial septal defect (92, 14.1%), mitral valve disease (128, 79.7%), nonrheumatic valvular disease (32, 56.2%), aortic regurgitation (71, 2.8%), aortic stenosis (10, 10.0%), hypertrophic cardiomyopathy (181, 11.6%), dilated cardiomyopathy (111, 37.8%), acute myocardial infarction (823, 9.0%), healthy subjects (31,886, 0.3%). A histopathological and electron-microscopic evaluation of the atrial heart muscle revealed that the advancement of the morphological changes was closely related to the occurrence of atrial fibrillation. Decrease in size of F waves in the electrocardiogram correlated well with the extent of right and left atrial fibrosis. Also, it was noteworthy that the atrial fibrillation in cases with dissecting aneurysm (n = 60) was an expression of the myocardial damage due to the infiltration of the bleeding into the right atrium. Intra-atrial electrogram in 48 patients with various heart diseases revealed that the electric potentials obtained from various parts of the atrium varied to a great extent and finally the patient's condition transformed to that of atrial standstill. We conclude that atrial fibrillation is an expression of some important aspect of the progression of heart disease and is not directly associated with hemodynamic overloading to the atrium. A strategy for quinidine treatment was also introduced.

DNA polymerase activity in the n.hexadecane-induced hyperkeratotic epidermis.

DNA polymerase activity was determined in hyperkeratotic epidermis of the guinea pig by topical application of n.hexadecane. Epidermal cells were separated by Percoll gradient centrifugation. DNA polymerase alpha activity was higher in the basal cells, but polymerase beta activity was higher in granular cells than in cells from the other layers. The hyperkeratosis was accompanied by an increase in polymerase alpha activity in both the squamous and basal cells. However, polymerase beta activity decreased in the granular cells and was distributed almost uniformly across the epidermis. The distribution pattern of polymerase activities in the hyperkeratotic epidermis was not simply an enhancement of the pattern in normal epidermis.

Hemagglutination with rhabdoviruses related to bovine ephemeral fever virus.

Three different bovine ephemeral fever group viruses were tested for hemagglutination (HA). One of them, Tortilla Flat virus (CSIRO 368), agglutinated erythrocytes from geese, pigeons, horses, hamsters, mice and guinea-pigs when the concentrated infectious culture fluid was used as a hemagglutinin. The HA was dependent on the pH of phosphate buffered saline used as the erythrocyte diluent when using borate buffered saline (pH 9.0) with 0.4% bovine serum albumin as the antigen diluent. The optimal pH of the phosphate buffer was from 5.2 to 5.8. The HA, however, was not dependent on salt concentration. The incubation temperature did not affect the HA titer significantly. This HA reaction was inhibited by serotype specific antiserum.

Spin labeling study on membrane fluidity of epidermal cell (cow snout epidermis).

Cow snout epidermal cells digested by 0.25% trypsin were separated into three regions of keratinocytes by Percoll density gradient centrifugation. The membrane fluidity of keratinocytes in each region was measured by electron spin resonance using 5-doxyl stearic acid (5-DSA) as a labeling agent. The order parameter(s) showed higher values as the depth of the epidermis decreased: lower region of epidermis, 0.632; upper region, 0.645; and horny cells, 0.680. These data indicated that membrane fluidity of epidermal cells decreased as cells approached the surface.

New theory on the pathogenesis of acute myocardial infarction.

Autopsy studies of hearts from 140 patients who had suffered acute myocardial infarction (AMI) and 26 cases of sudden coronary death revealed two distinct types of myocardial cell death: "kinetic cell death" (KD) and "static cell death" (SD). In KD, the predominant type of cell death in AMI-myofibrils disintegrated through alternating overcontraction and overextension. KD was found not only in patients having died after some time had elapsed from the onset of AMI, but also in cases of sudden coronary death. Muscle fibers in SD, which by contrast began to appear at least seven hours after the onset of AMI, characteristically preserved cross striations, while their nuclei were pyknotic or had already disappeared. Such fibers were observed only in territories peripheral to occlusive coronary thrombus, a secondary rather than a primary event that takes place during the course of AMI. As a result of the above observations, we were able to produce a new experimental model of AMI using mongrel dogs. As a preparatory procedure we first injected them intravenously with 2% calcium chloride at a constant rate for 90 or 120 minutes, and then with a sudden injection of caffeine, calcium chloride and catecholamine in order to induce KD. In contrast, ligation of the intraventricular coronary artery near its origin caused SD of myocardial fibers in the dependent territories. Overall results led us to conclude that AMI is initiated by instantaneous overcontraction of myocardial fibers, resulting in their KD, a phenomenon that could be called "myocardial self-destruction."
