Renal changes in primary aldosteronism.

The clinical presentations and renal biopsy specimens of 18 patients with primary aldosteronism were reviewed to determine the characteristic pathologic features of the kidney in this syndrome. All patients were hypertensive with a mean blood pressure of 192 nm. Hg systolic and 122 mm. Hg diastolic. The average duration of hypertension was 6.88 years. The mean serum potassium was 2.88 mEq. per l. and the mean plasma carbon dioxide was 31.4 mEq. per l. A significant history of urinary tract disease was noted in 8 patients. Laboratory and diagnostic studies evaluating renal structure and function were abnormal in 11 patients. Renal biopsies from all 18 individuals showed evidence of parenchymal damage. Hypertensive and hypokalemic changes were the most significant abnormalities and were considered moderate to severe in 78 and 89 per cent of the patients, respectively. Histologic evidence of pyelonephritis was noted in 2 patients only and no renal specimens contained characteristic changes of metabolic alkalosis. The preoperatively hypertensive and renal evaluations did not reflect the severity of the renal changes noted histologically. The extent of renal injury caused by hypertension and hypokalemia in these patients emphasizes the consequences of primary aldosteronism. Early diagnosis and treatment of this disorder are essential if these consequences are to be avoided.

The role of orchiectomy in the management of advanced male breast cancer.

Nine men with advanced carcinoma of the breast were evaluated after orchiectomy at the National Institutes of Health over 19 years. Of patients followed for greater than one year, five had complete remissions, one had a partial remission, and two patients did not respond to orchiectomy. The average disease-free interval was 20 months; the average survival from the time of orchiectomy in those who responded was 55.8 months and in nonresponders was 26.7 months. Orchiectomy appears to be a safe and effective way to produce an objective response and prolong survival in over 50 percent of men with advanced breast cancer.

Segmental renal vein renin assay and segmental nephrectomy for correction of renal hypertension.

Selective segmental renal vein catheterization was used to obtain venous blood samples for renin assay. With this technique a localized source of hypersecretion of renin was identified in 2 patients. Using samples from the main renal veins we found no significant difference in renin activity and, therefore, the etiology of the hypertension in these 2 patients would have been missed. Segmental nephrectomy resulted in a cure of the hypertension in both patients who were followed for a minimum of 14 months.

Cryosurgery of the ureter in dogs.

The effect of cryosurgery on the ureter has been studied in dogs. A single 1- or 2-min cryosurgical exposure produced stricture in two of eight animals. A tumoricidal cryosrugical dose consisting of three, 2-min exposures caused stricture in six of eight dogs. Histopathologic examination of the ureter after repetitive cryosurgery showed progressive, persistent loss of smooth muscle, fibrosis of the lamina propria, and early loss with subsequent regeneration of the transitional epithelium. Although there was no loss of ureteral integrity, the possibility of permanent ureteral damage should be considered with the use of cryosurgery.

Total unilateral renal destruction caused by irradiation for Hodgkin's disease.

Total unilateral renal destruction has been observed in a patient who previously received irradiation for Hodgkin's disease. The clinical features, after a six-year asymptomatic interval, included recurrent calculi and infection in what was subsequently demonstrated radiographically to be a nonfunctioning kidney. The gross and microscopic characteristics of the nephrectomy specimen have been described and correlated with the magnitude of irradiation and the clinical course.

Sigmoid conduit urinary diversion.

The courses of 11 patients with sigmoid conduit urinary diversion following total pelvic exenteration were reviewed in detail and the results presented. The sigmoid conduit appears to be a safe alternative to the ileal conduit for urinary diversion, and offers in addition the possible advantage of an effective antirefluxing ureterocolic anastomosis.
