[Metastatic calcification associated with malignancy].

From 1975 to 1986, 15 cases (2%) of metastatic calcification associated with an underlying malignancy were found in a review of 702 autopsied cases with histories of malignancy. These underlying malignancies included 7 cases of lung cancer, 6 cases of malignant lymphoma, one case of breast cancer, and one of urinary bladder cancer. Squamous cell carcinoma was of the histological type most often associated with metastatic calcification in lung cancer, and ATL in malignant lymphoma. Hypercalcemia was found in 10 (83%) out of cases, and almost all were accompanied by renal dysfunction. Calcium deposits were most frequently observed in the kidneys and the lungs. It has been suggested that metastatic calcification in the lungs and kidneys of a patient with a history of malignancy showing hypercalcemia is sometimes accompanied by respiratory and renal dysfunction, causing the patient's condition to deteriorate.

Veno-occlusive disease of the liver in a patient with allergic granulomatous angiitis.

A 45-yr-old woman, with asthma and skin erythema, was diagnosed as having allergic granulomatous angiitis. Leukocytosis accompanied by hypereosinophilia and characteristic histological findings of skin lesions also were present. Upon laparoscopy and liver biopsy, the liver showed a congestive appearance and, histologically, showed thickening of the central vein walls, as well as narrowing of the lumens with collagenous fibers, and pericentral hemorrhage with hepatocyte degeneration. A diagnosis of veno-occlusive disease of the liver was made. This is the first report on a case of veno-occlusive disease of the liver occurring in association with allergic granulomatous angiitis.

Strong promoting activity of reversible uracil-induced urolithiasis on urinary bladder carcinogenesis in rats initiated with N-butyl-N-(4-hydroxybutyl)nitrosamine.

The tumor-promoting effect of uracil-induced calculi on rat urinary bladder carcinogenesis was investigated in male F344 rats pretreated with N-butyl-N-(4-hydroxybutyl)nitrosamine (BBN). Since uracil-induced calculi and papillomatosis of the bladder are reversible, uracil was given for a limited period after the treatment with BBN. Animals were given 0.05% BBN in their drinking water for 4 wk and then treated with uracil as 3% of the diet for 8 or 16 wk. After the uracil treatment, rats were given basal diet without uracil until Wk 28 of the experiment. Animals were killed from each group at the end of either Wk 12, 20, or 28. The incidence of carcinoma of the bladder was 40% after only 8 wk of uracil treatment following BBN initiation and increased to 100% when uracil treatment was extended to 16 wk. After discontinuation of uracil treatment, the papillomatosis disappeared, but the incidence of carcinoma steadily increased with increasing time. In the control group given BBN alone, only 1 of 16 rats had carcinoma at Wk 28. The present findings clearly demonstrate that uracil-induced urolithiasis had a strong promoting activity on BBN bladder carcinogenesis.

Compared promoting potential of D-galactosamine, carbon tetrachloride and partial hepatectomy in rapid induction of preneoplastic liver lesions in the rat.

Effectiveness of two different chemically induced stimuli for hepatocellular proliferation was compared with regard to that of commonly used partial hepatectomy (PH), for the purpose of developing short-term protocol for the assay of promoting agents of hepatocarcinogenesis. Enhancing effect of D-galactosamine (DGA) and carbon tetrachloride (CCl4) given during the promotion procedure by 2-acetylaminofluorene (2-AAF) was compared along with PH in rats initiated by diethylnitrosamine (DEN), using preneoplastic glutathione S-transferase positive (GST-P+) hepatocyte foci as an end-point marker lesion. The number of GST-P+ foci per cm2 was largest in the group given CCl4 followed by DGA, no treatment (2-AAF alone) and PH. In contrast, the area (mm2) per cm2 and mean diameter of the focus were largest in the PH group then DGA followed by CCl4 and no treatment. The results indicate that the number of GST-P+ foci were not clearly affected by 3 different treatments whereas area and size of foci which represented the result of promoting effect were clearly influenced by those treatments, indicating they caused differential proliferation of initiated cells. In this respect, even though PH is the most potent procedure, DGA is also efficient and preferred to CCl4 for the non-surgical enhancing method.

Regression of simple hyperplasia and papillomas and persistence of basal cell hyperplasia in the forestomach of F344 rats treated with butylated hydroxyanisole.

The reversibility of forestomach lesions induced in rats by butylated hydroxyanisole (BHA) was examined. F344 rats were given a 2% BHA diet for 24, 48, or 72 wk followed by a basal diet for the remainder of the 96-wk experiment. Two other groups of rats were given a 2% BHA diet or basal diet alone for 96 wk. The forestomach lesions at wk 24, 48, 72, or 96 were compared histopathologically. The results showed that exophytic epithelial proliferation (simple hyperplasia or papilloma) induced by BHA was reversible, while endophytic proliferation of basal cells (basal cell hyperplasia) persisted after withdrawal of BHA administration. This suggests that simple hyperplasia and papilloma of the forestomach induced by BHA are not autonomous and need continuous feeding of BHA to develop further.

Analysis of human glutamyl- and lysylplasminogen by high-performance affinity chromatography.

An analytical system for the fibrinolytic system in the blood is described, based on high-performance affinity chromatography and a newly devised specific proenzyme detection procedure. Plasminogen subspecies in human plasma without any pretreatment (less than 100 microliters) were specifically separated on an high-performance affinity column. For detection they were continuously activated by urokinase, and their elution profile was determined by assay of the plasmin activity using a specific fluorogenic substrate. Preliminary data on plasma from a patient are presented.

Different modifying response of butylated hydroxyanisole, butylated hydroxytoluene, and other antioxidants in N,N-dibutylnitrosamine esophagus and forestomach carcinogenesis of rats.

The modifying effects of antioxidants were examined in a carcinogenesis system after N,N-dibutylnitrosamine treatment. Male F344 rats were given 0.05% N,N-dibutylnitrosamine in their drinking water for 4 wk and then treated with basal diet containing 2% butylated hydroxyanisole (BHA), 1% butylated hydroxytoluene (BHT) with 7 ppm vitamin K, 0.8% ethoxyquin, 5% sodium L-ascorbate, 5% sodium erythorbate, or no added chemical for 32 wk. BHA enhanced forestomach carcinogenesis but did not enhance esophageal carcinogenesis. BHT enhanced esophageal carcinogenesis but did not enhance forestomach carcinogenesis. Ethoxyquin significantly enhanced esophageal tumorigenesis. Neither esophageal nor forestomach carcinogenesis was affected by the other antioxidants evaluated. BHA significantly increased DNA synthesis of the forestomach epithelium, whereas BHT tended to increase that of the esophageal epithelium. Thus, BHA and BHT showed different modifying responses in carcinogenesis of the esophagus and forestomach.

Combination of high-performance affinity chromatography and specific detection of proenzyme applicable to the analysis of the fibrinolytic system of human plasma.

A procedure for the analysis of the fibrinolytic system in human blood was devised by combining high-performance affinity chromatography (HPAC) and specific detection of proenzyme. Components of the fibrinolytic system were separated by HPAC using Asahipak GS gel coupled with p-aminobenzamidine, and they were specifically detected by means of an on-line enzyme assay system. This system made it possible to quantitate not only Glu-plasminogen (Glu-Plg) but also Lys-plasminogen (Lys-Plg) in human plasma in a short time without pre-treatment. The effect of urokinase on the state of components of the fibrinolytic system in blood was studied. It was clearly shown that Lys-Plg is more susceptible to activation by urokinase than Glu-Plg (both in vitro and in vivo).

Alteration of microfibrils in the conjunctiva of patients with exfoliation syndrome.

We electron-microscopically studied 15 specimens of upper limbal conjunctiva obtained from 14 patients with exfoliation syndrome and clinical evidence of glaucoma. Of 15 specimens, four (three patients) contained exfoliation materials in the stroma of the conjunctiva. These exfoliation materials of immature, intermediate, and mature forms were composed of abnormal microfibrils lying adjacent to the fibroblasts and located in close proximity to the elastic and collagen fibers. These observations suggest a sequence of events by which microfibrils develop into exfoliation materials.

Immunogold labeling of blood-group antigens in human salivary glands using monoclonal antibodies and the streptavidin-biotin technique.

We investigated the localization of blood-group antigens A, B, and H in human labial salivary and submandibular glands by applying a postembedding immunogold method using monoclonal antibodies in combination with the streptavidin-biotin bridge technique. The H, A, and B antigens were only detected in mature secretory granules (SGs), which were mainly found in cells in the late phase of the maturation cycle. In immature SGs, which were present in cells in the early or middle phases of the maturation cycle, these antigens were not detected. All other cytoplasmic organelles were not labeled by the monoclonal antibodies used. In blood-group-O secretors, H antigen was present in almost all of the mature SGs. In blood-group-A secretors, the labeling for H antigen exhibited a mosaic-like pattern, i.e. only some of the mature SGs contained H antigen. With respect to the A and B antigens, a similar mosaic-like pattern of staining was observed in blood-group-A and -B secretors, respectively. To the best of our knowledge, this is the first time that the distribution of blood-group antigens A, B, and H in human tissues has been demonstrated at the electron-microscope-level using monoclonal antibodies.

Immunohistochemical observations of keratins, involucrin, and epithelial membrane antigen in urinary bladder carcinomas from patients infected with Schistosoma haematobium.

Squamous cell carcinomas of the urinary bladder and the epithelial lesions associated with infection by Schistosoma haematobium were histopathologically and immunohistochemically described for keratin proteins (TK, 41-65 kDa; KL1, 55-57 kDa; PKK1, 40, 45 and 52.5 kDa), involucrin, and epithelial membrane antigen (EMA). Normal urothelial epithelium was positive for all keratins, and showed absent or slight reactions for involucrin and EMA in superficial umbrella cells. The intestinal type of epithelium was composed of columnar cells and small basal cells; TK was positive in the basal cells, KL1 staining was positive in the columnar cells, whereas PKK1 was negative or slight in the columnar cells. Involucrin was confined to columnar cells. Squamous metaplastic epithelium showed a rather regional keratin distribution: TK was distributed in all layers, KL1 decorated upper spinous and granular layers, but PKK1 did not bind, and involucrin staining existed only in upper spinous and granular cells. Keratin expression in squamous cell carcinomas indicated heterogeneity and its stainability was dependent on the degree of keratinization: The G 1 type revealed strong reaction, the G 2 type showed a similar distribution pattern, but the staining intensity was less, and the G3 type showed irregular staining with decreased intensity. Involucrin staining was limited to keratinized cells of carcinoma as was that for EMA.

Coexistence of pancreas divisum and intestinal malrotation in a patient with cholecystolithiasis--a case report.

An unusual case of a coexistence of pancreas divisum and intestinal malrotation in a patient with cholecystolithiasis is described herein. The diagnosis of pancreas divisum was established by endoscopic retrograde pancreatography, and the intestinal malrotation was diagnosed by duodenography and barium enema. An operation was performed for the cholecystolithiasis. The pancreas was soft, and its shape was almost normal. Cholecystectomy and prophylactic appendectomy were performed, however nothing was done to the pancreas.

Response of osteoblastic clonal cell line (MC3T3-E1) to [Asu]eel calcitonin at a specific cell density or differentiation stage.

Clone MC3T3-E1 cells isolated from newborn mouse calvaria is an osteogenic cell line which retains an ability to differentiate into osteoblastic cell in vitro. The effect of [Asu]eel calcitonin (ECT) on clonal MC3T3-E1 cells was investigated at different stages of differentiation. ECT caused an increase in alkaline phosphatase (ALP) activity. The stimulative effect was demonstrated to be dependent upon cell density or differentiation stage. At a cell density of 1.18 X 10(5)/cm2 cells were incubated with ECT for 2 days. The treatment by ECT caused an increase in ALP activity. A specific response to ECT dependent on the cell density was observed in a narrow range of cell density. Moreover this range of cell density responsible to ECT was found to be a rapid differentiation stage of MC3T3-E1 cells. These results suggest that calcitonin stimulates differentiation of osteoblast. In addition to these results, cellular adenosine-3',5'-cyclic monophosphate (cAMP) level was raised by ECT treatment at a cell density of about 1.4 X 10(5) cell/cm2 and this response was also specific for cell density. At cell density lower or higher than this density no stimulative effect by ECT was observed. On the other hand, N6,O2-dibutyryl adenosine-3',5'-cyclic monophosphate (db-cAMP) and theophylline caused an increase in ALP activity in wide cell density range. These results indicate that an increase in ALP activity by ECT is mediated by intracellular cAMP and that the specific response to ECT dependent on the cell density is regulated in the process of cAMP formation and/or in the preceding process of cAMP formation.

Immunohistochemically demonstrated increase in glutathione S-transferase species in propylnitrosamine-induced focal proliferative and neoplastic Syrian hamster pancreatic lesions.

Immunohistochemical investigation of focal proliferative and neoplastic Syrian hamster pancreatic lesions induced by propylnitrosamine administration revealed a distinct pattern of expression of different molecular forms of glutathione S-transferase (GST) during neoplastic development. Initial increase in levels of GST placental (P) and B forms in early ductal/ductular proliferations and atypical (dysplastic) lesions was followed by a drop in the latter during transition to carcinoma. Unequivocal acinar cells observed within so-called 'pseudoductules' did not share the altered phenotype evident in ductular elements, suggesting their non-involvement in the generation of early lesions. However, the fact that component cells were occasionally of abnormal morphology did not allow the exclusion of acinar cell participation in histogenesis. Elevation of GST-A and C forms was limited to stromal elements surrounding the epithelial lesions and since they were associated with benign, cystic as well as atypical lesions and a similar increase was observed after common duct ligation, they appeared to be non-specific. The results indicated independent control of expression of individual GST forms and suggest that biochemical similarities exist between early, putative preneoplastic lesions induced by propylnitrosamines in the hamster lung, liver (both hepatocellular and intrahepatic bile duct) and pancreas.

Altered enzyme expression in propylnitrosamine-induced Syrian hamster lung lesions.

Focal proliferative and neoplastic lung lesions induced in Syrian hamsters by dihydroxy-di-n-propylnitrosamine (DHPN) were investigated using a combined histochemical, autoradiographic and electron microscopic approach. Expression of elevated glucose-6-phosphate dehydrogenase (G6PD) and gammaglutamyl-transpeptidase (GGT) activities and levels of immunohistochemically demonstrable glutathione S-transferase placental form (GST-P) were evident in epithelial cells of focal proliferative populations and bronchioloalveolar neoplasms. Binding for the GST-C form, normally only weak, became very pronounced in the stromal elements of DHPN-induced lesions. Increased labelling with tritiated thymidine was associated with increase in morphological atypia within the tumours. Although the enzyme phenotype findings were equivocal the presence of lamellar bodies in some cells of focal proliferative and neoplastic lesions suggested an origin from alveolar type II cells. The present results regarding changed enzyme phenotype in lung lesions suggest important similarities at the biochemical level for the process of neoplasia in the different target organs of DHPN in the hamster and indicate that GST-P may be a useful 'marker' for lung neoplasia.

Vagus nerve and celiac ganglion lesions in generalized amyloidosis. A correlative study of familial amyloid polyneuropathy and AL-amyloidosis.

To clarify the cause of gastrointestinal disorders in systemic amyloidosis we made pathologic and morphometric studies of vagus nerves, celiac ganglia, stomach and rectum in three autopsied cases with type 1 familial amyloid polyneuropathy (FAP) and two with nonhereditary generalized amyloidosis (AL-amyloidosis). The gastric and rectal walls in all cases were affected in the same way by amyloid deposition. On the other hand, there was a great difference between the two diseases in the severity of vagus nerve and celiac ganglion lesions: the vagus nerves in FAP showed very extensive endoneurial deposition of amyloid with severe loss of myelinated nerve fibers, but in AL-amyloidosis there was no loss of myelinated nerve fibers and only slight amyloid deposition in the endoneurium. Similarly, in the celiac ganglion, intraganglionic deposition of amyloid was prominent in FAP and slight in AL-amyloidosis. It is known that bowel symptoms frequently occur in type I FAP and are less prominent in AL-amyloidosis. This study demonstrated that the gastrointestinal autonomic nerves were more markedly disturbed by amyloid in the former than in the latter, and disorder in neural control of the digestive tract may be responsible for the bowel symptoms in systemic amyloidosis, especially in type I FAP.

Combined chronic toxicity and carcinogenicity studies of morpholine oleic acid salt in B6C3F1 mice.

Morpholine oleic acid salt (MOAS) was administered to groups of 50 male and 50 female B6C3F1 mice in the drinking-water at levels of 0, 0.25 and 1.0%. Both sexes given 1.0% MOAS and females given 0.25% showed growth retardation. Significant increases in blood-urea nitrogen concentrations were only observed in the 1.0% male group. The incidence of squamous-cell hyperplasia of the forestomach epithelium was significantly higher in the males given 1.0% than in the controls. The male mice given 0.25% MOAS had a significantly reduced incidence of hepatocellular carcinoma in comparison with the control group and this trend was indicated also in the 1.0% group. This experiment did not demonstrate any carcinogenic effect of MOAS in mice at levels up to 1.0% in the drinking-water.
