Condyloma acuminatum of the bladder in two autopsy cases.

Condyloma acuminatum is a very rare lesion occurring in the bladder. We report two cases of condyloma acuminatum of the bladder found at autopsy, one in a 96-year-old woman and the other in an 80-year-old man. Both had papillary or polypoid tumorous lesions in the trigone and neck of the bladder. The lesions were composed of nodular or papillary growths of squamous epithelium. The epithelium exhibited koilocytosis and the nuclei showed immunohistochemical staining for human papillomavirus antigen.

Spontaneous tumors in aging (C57BL/6N x C3H/HeN)F1 (B6C3F1) mice.

Spontaneous tumors in untreated (C57BL/6N x C3H/HeN)F1 (B6C3F1) mice used as controls in carcinogenicity tests were recorded. In both sexes, the development of spontaneous tumors was age-related. In 244 male mice, the most common tumors were hyperplastic nodules of the liver, hepatocellular carcinomas, malignant lymphomas/leukemias, lung adenomas, and adenocarcinomas. In 246 female mice, the most common tumors were malignant lymphomas/leukemias, pituitary adenomas, neoplastic nodules of the liver, hepatocellular carcinomas, and lung adenomas. Hepatocellular carcinomas metastasized in 20.3% of the animals with these tumors. Few other tumors except malignant lymphomas and leukemias metastasized. Various tumors of other organs and/or tissues were found at low incidences.

Cytochemical localization of blood group substances in human salivary glands using lectin-gold complexes.

We investigated localization of blood group antigens and their related substances in human labial salivary and submandibular glands by application of a post-embedding cytochemical staining procedure using lectin- or glycoprotein-gold complexes. Surgical tissue was obtained from 10 patients. Blood group-specific lectins, such as Dolichos biflorus agglutinin or Helix pomatia agglutinin (group A-specific), Griffonia simplicifolia agglutinin-I B4 (group B-specific), and Ulex europaeus agglutinin I (group H-specific) could recognize A, B, and H antigens, respectively, only in mature secretory granules (mature SG), which were found preferentially in cells in the late phase of the maturation cycle. In immature secretory granules (immature SG), which were found in cells in the early or middle phase of the maturation cycle, no binding with these lectins was observed. The Golgi complexes and endoplasmic reticula also were not labeled with these lectins. In blood group O and B secretors, blood group antigens were uniformly distributed throughout all the mature SG examined. However, in blood group A secretors, the distribution was heterogeneous, i.e., in some granules only H antigen was demonstrated, whereas in others both A antigens and a small amount of H antigens were detected. Among the blood group-nonspecific lectins, wheat germ agglutinin (WGA) was found to bind more preferentially to immature SG than to mature SG. This was demonstrated irrespective of the blood group and secretor status of the tissue donor, except that in blood group A secretors WGA bound strongly to some mature SG which possessed A antigen. We discuss the significance of cellular and subcellular mosaic distribution of blood group antigens in connection with morphological differences of secretory granules and the maturation cycle of mucous cells.

Effects of alpha-L-fucosidase digestion on lectin staining in human pancreas.

We examined the effects of alpha-L-fucosidase digestion on lectin staining in formalin-fixed, paraffin-embedded human pancreatic tissue from individuals of different blood groups. Digestion with the enzyme resulted in apparent diminished intensity of Ulex europaeus agglutinin-I (UEA-I) staining in the acinar cells. In addition to the decreased intensity of UEA-I staining, reactivity with soybean agglutinin (SBA) was increased in the enzyme-susceptible, UEA-I-reactive cells. The intensity of Griffonia simplicifolia agglutinin-II (GSA-II) staining performed after beta-galactosidase digestion in UEA-I-reactive acinar cells was markedly increased by prior treatment with fucosidase. GSA-II staining following sequential digestion with fucosidase and galactosidase was completely abolished by subsequent digestion with beta-N-acetylhexosaminidase. These results therefore substantiate the previous assumption that SBA-reactive D-galactose-(beta 1-3,4)-N-acetyl-D-glucosamine and GSA-II reactive beta-N-acetyl-D-glucosamine imparted following galactosidase digestion represent precursors of H antigen. The present study further demonstrated that intense peanut agglutinin (PNA) staining was imparted after digestion with fucosidase in UEA-I-reactive sites in secretors. In contrast, nonsecretors showed vivid PNA staining that was usually detected throughout the pancreas without prior enzyme digestion. Here, fucosidase digestion had if any little effect on PNA staining. These results suggest that in secretors a terminal trisaccharide, fucosylated D-galactose-(beta 1-3)-N-acetyl-D-galactosamine exhibiting positive PNA reaction after fucosidase digestion, exists in UEA-I-reactive acinar cells. It is assumed that the secretor gene could control the step of final fucosylation of D-galactose-(beta 1-3)-N-acetyl-D-galactosamine in human pancreas.

Human chorionic gonadotropin beta-subunit in human semen.

The level of human seminal chorionic gonadotropin beta-subunit (hCG beta) was determined by radioimmunoassay (RIA). The mean hCG beta level in 34 normal men was 3.7 +/- 1.6 ng/ml, which was much higher than that in serum. The mean hCG beta level for 20 patients with mild oligozoospermia (20-39 x 10(6) sperm/ml) was 2.5 +/- 0.8 ng/ml, that for 34 patients with severe oligozoospermia (1-19 x 10(6) sperm/ml) was 1.7 +/- 0.5 ng/ml, and that for 21 azoospermia was 1.5 +/- 0.6 ng/ml. Thus, the decrease of sperm count was correlated with the decrease of hCG beta. In 17 cases to which testicular biopsy was applied together with sperm counting, the seminal hCG beta level was found to positively correlate with the germinal cell index (the ratio germinal cell count/Sertoli cell count) and with the testicular volume. The level of seminal hCG beta was also found to correlate negatively with the levels of seminal LH and FSH and positively with the level of seminal testosterone. These findings suggest that the production of seminal hCG beta is a process of spermatogenesis and closely related to spermatogenesis. The level of hCG beta in serum was too low to detect, and no relation to that in seminal plasma could be investigated. However, in 6 cases with testicular tumor, the hCG beta level in serum was high, whereas that in seminal plasma was rather low probably because of unilateral secretion. Enhanced production of hCG beta by tumor tissues and the destruction of the blood-testis barrier by proliferation of tumor cells seemed to be one of the causes of this high hCG beta level in serum. The hCG beta levels in 13 vasectomized seminal plasma and the prostatic fluid samples collected from 3 normal men were 1.5 ng/ml, which was similar to those in azoospermic patients. These findings suggest that the seminal hCG beta level consists of the hCG beta secreted by the testis and about 1.5 ng/ml of hCG beta from the prostate. Based on these results, seminal hCG beta is thought to be secreted by the prostate and the process of spermatogenesis and the value of seminal hCG beta may serve as an effective index for the testicular function.

On the factors influencing the dispersion of radioiodines.

Radioactivities dispersed from 0.1 N sodium hydroxide solutions of 123I, 125I, or 131I were measured, and the factors influencing dispersion of radioiodines were analyzed. The radioiodines dispersed were trapped by glass fiber filters, charcoal filters, and separable paint. The factors influencing the dispersion of radioiodines are discussed in terms of the dispersal rate, which defined as the ratio of total activity of three kinds of traps in a unit time to whole activity. In order to clarify the factors influencing the dispersal rate, we carried out the experiments; on the effect of reducing agent, that of radioactive concentration of radioiodine, that of mixing the 125I solution with 131I, and that of scavenging hydrated electrons and hydroxyl radicals. It could be concluded from experimental results that the principal factors influencing dispersion of radioiodines are the number of atoms of radioiodine per volume and the energy absorbed in solution from radioiodines. In regard to the absorbed energy, we thought that the dispersion of radioiodines is influenced mainly by hydrated electron, not markedly by hydroxyl radical.

[Studies on serum and urinary alpha 1-microglobulin levels as parameter of the renal function--renal function observed after CDDP administration].

To evaluate the clinical usefulness of alpha 1-microglobulin (alpha 1-MG) as parameter of renal function, we determined the levels of alpha 1-MG in the serum and urine of patients who had no malignant tumors, and compared them with the levels of beta 2-microglobulin (beta 2-MG), serum creatinine, urinary N-acetyl-beta-glucosaminidase (NAG) and 24-hour creatinine clearance (24 Ccr). There were significant positive correlations between alpha 1-MG in the serum and urine, and those of beta 2-MG levels. Serum alpha 1-MG and 24 Ccr were inversely correlated. Combined measurements of alpha 1-MG in the serum and urine seemed to be useful to estimate glomerular and tubular renal functions. The renal function in 8 patients with advanced urogenital cancers treated with cis-diammine-dichloroplatinum (CDDP) was examined by measuring 24 Ccr, alpha 1-MG, and beta 2-MG in serum and urine and urinary NAG. Determination of urinary alpha 1-MG was useful for early detection of tubular damage after CDDP administration.

[Property of scirrhous carcinoma of the stomach defined by collagen metabolism].

Scirrhous carcinoma of the stomach is characterized by the extensive deposition of collagen in the desmoplastic stroma. In the present study, we examined which cell types are responsible for the synthesis of collagen in tissue of scirrhous carcinoma. Furthermore, a RIA for carboxyterminal peptide of human type I procollagen (type I C-peptide) was developed and its clinical implication for serodiagnosis of scirrhous carcinoma of the stomach was evaluated. 1) The immunohistochemical localization of type I procollagen was investigated employing an antibody to procollagen. Strong staining was observed in the cytoplasma of scirrhous carcinoma cells. Furthermore northern blot technique using c-DNA probe of type I procollagen was performed. Tumor cells established from scirrhous carcinoma (KATO III) were expressing mRNA-procollagen type I as revealed by this technique. Thus in scirrhous carcinoma of the stomach, at least some portion of the increased collagen content is due to production by the tumor cells. 2) In scirrhous carcinoma of the stomach, serum type I C-peptide level appeared to increase, while in non-scirrhous carcinoma, serum type I C-peptide level stayed within normal range. Serum type I C-peptide levels corresponded to clinical course who underwent either operation or chemotherapy. Collectively, the measurement of serum type I C-peptide concentrations will provide a new means for diagnosis and monitoring the scirrhous carcinoma of the stomach.

[A case of carcinoma of the prostate presenting as abdominal mass].

A 69-year-old man was hospitalized for bilateral abdominal masses and constipation on October 31, 1986. Physical examination revealed firm, rough, movable, hard masses of over fist size, in the bilateral lower abdomen. CT scan disclosed two large masses in the pelvis. The prostate was of walnut size, rough and elastic firm on rectal examination. The serum acid phosphatase was elevated. Needle biopsy of the prostate and open biopsy of the mass showed a poor differentiated adenocarcinoma. Treatment with diethylstilbestrol diphosphate, 5-fluorouracil, estramustine sodium phosphate, chlormadinone acetate, 1-la-tetrahydrofuryl-5-fluorouracil, and cyclophosphamide was effective. The masses-metastasis of intrapelvic lymph nodes have shrunk and serum acid phosphatase has fallen to the normal level. In April 1987, the patient left hospital, and as of October 1987, he is still alive. This case indicates the presence of an unusual type of prostatic carcinoma in which huge lymph node metastasis occurs.

[Clinical evaluation of vesicoureteroneostomy. II. Efficacy of surgery by Boari's method in eight and the psoas-hitch method in eight cases].

Vesicoureteroneostomy was performed by Boari's method on 8 patients and the psoas-hitch method on 8 patients during the 16 years from 1969 to 1986. Although Boari's method without the formation of a submucosal tunnel was employed for the initial 4 patients, the formation of this type of tunnel was a general principle for all patients thereafter. In choosing the surgical method, the psoas-hitch method was assigned to patients in whom it was possible to extend the urinary bladder to the upper part of the common iliac vessel and in whom the ureter allowed the formation of a submucosal tunnel, and Boari's method was assigned to those in whom it is was not possible to reach the ureter or to form a submucosal tunnel or in whom the ureter would be tensed. The mean length of the resected section of ureter was 75 mm for Boari's method and 43.2 mm for the psoas-hitch method. Follow-up observations revealed the presence of VUR in 2 patients who underwent surgery by the initial form of Boari's method and in 1 patient who underwent surgery by the psoas-hitch method. There was no ureteral stenosis or renal hypofunction in any of the patients.
