[Chylopericardium as a rare cause of a "large" heart shadow--2 case reports].

Two cases with the diagnosis of asymptomatic chylopericard were evaluated. One was a 20-year old girl who had been operated on for intrathoracic lymphangioma and chylopericardium that existed since birth, and who developed a chylothorax after the operation. The second case report concerns a 22-year old man who had been suffering from a posttraumatic asymptomatic chylopericardium for two years and who is being treated conservatively "Low-fat diet").

[Sonography in salivary calculi--indications and status].

In 23 patients with sialolithiasis the value of real time sonography was investigated. The calculi were correctly diagnosed in 91% of the patients. 3 of 4 calculi could be localized in the main duct of the salivary gland. Because of the high accuracy and the possibility to detect non opaque stones, sonography should be performed, if on plain film the localisation of the calculi is questionable, before sialography is done. In sonolithotrypsy, sonography can be used to document the extraglandular localisation of intraductal calculi.

[Occlusion or stenosis of the splenoportal axis. Supplementary information with duplex sonography?].

In 21 patients with sonographic signs of occlusions or stenoses of the splenoportal axis additionally pulsed Doppler measurements in the region of the splenic and portal veins were conducted with a Duplex system. Examinations using the pulsed Doppler Duplex system yield functional data in respect of blood flow in addition to the analysis of morphological changes. It is possible to differentiate between cystic structures and collaterals as well as to assess residual perfusions or cavernous transformations in vascular occlusions. Besides qualitative information on blood flow in a vessel and determination of the direction of blood flow, quantitative data on the acceleration of blood flow in stenosis are also possible.

[Questionable indication for radical neck dissection in carcinomas of the tongue and floor of the mouth. Results after 4 years].

Based on a 4-year-study the use of ultrasound and preoperative treatment of the neck area is evaluated. The results suggest that there is no indication for radical neck dissection. The therapeutic approach described resulted in a considerable increase in the survival rate during the first 4 years as against the figures published by the DOSAK in 1988.

Duplex sonography of normal renal allografts.

Thirty-five patients with normal renal allografts as well as 10 healthy volunteers with normal native kidneys were examined by duplex sonography. Blood flow measurements were performed in the main renal artery as well as in segmental, interlobar and arcuate arteries. In both groups the normal range of variation in blood flow was defined by using the resistive index (RI). Doppler wave forms and RI of main renal arteries were both similar to those of their branches. In renal allografts as well as in native kidneys the blood flow pattern showed a rapid up-slope in systole followed by a gradual down-slope in early diastole, with a persistent flow throughout diastole. In addition, there was no significant difference in the RI between allografted and native kidneys. The interobserver variability was less than 10 per cent. The mean RI was 0.68 using all data points. Vascular rejection in 11 patients could be discriminated significantly (p less than 0.001). Duplex sonography is useful in evaluating renal blood flow. This simple and non-invasive technique seems to be useful in long-term follow-up of renal allograft recipients.
