Fracture of the sesamoid bones of the hallux is rare. A case of fracture of the medial sesamoid bone is presented. The X-ray finding of an irregular line dividing the medial sesamoid bone together with typical local symptoms and signs established the diagnosis in a 17-year-old female athlete. Nonunion of the fractured sesamoid was successfully treated by extraction of the sesamoid fragments. Complete relief of pain was followed by the patients' return to full athletic activity. The diagnostic criteria and indications for operation are discussed briefly.

The use of the double compression medullary nail in 5 cases of fracture of the femur is reported. The results are very promising as the nail gives excellent stability, affords efficient internal fixation of fractures of the proximal and distal thirds of the shaft, allows very early weight bearing and in this small series has brought about very early strong union with a minimal time in hospital and a short period away from work. There were no complications and no difficulties were encountered in nail extraction. We believe that this nail may prove to be the solution to the various biomechanical limitations of medullary nailing and may be the universal nail for fractures of the femoral shaft.
An anomalous musculotendinous portion of the triceps muscle lying in the ulnar groove behind the medial humeral epicondyle is described. It was the cause of snapping elbow and ulnar neuritis and was mistaken for a snaping ulnar nerve.
Patients suffering from the restriction of their locomotor function due to disease of the weight-bearing joints or ischaemic intermittent claudication, but who are otherwise relatively healthy, are enabled to achieve a remarkable improvement in the quality of their lives by the use of a new weight-relieving walker-scooter. The device is described. Simple experiments using the walker-scooter are recorded. The importance of ambulation is stressed, and the place of various walking aids is discussed.
The common infra-isthmal fracture of the shaft of the femur is defined. The difficulties in its management are discussed and various methods of treatment are critically reviewed. The operative treatment of 21 such fractures with a single plate by the AO method is described. Strict adherence to every detail of the operative and postoperative technique and a cooperative patient are essential for a successful result.
Flexor profundus is a mass action muscle so that when we fully extend one finger including full extension of the distal joint the entire muscle is pulled distally. By fully flexing the injured or operated finger at the metacarpophalangeal joint and fully extending the adjacent fingers, flexor digitorum profundus of the operated finger is made so redundant as to abolish all tension at the suture line. The position of the operated finger: metacarpophalangeal joint flexion with interphalangeal joint extension is ideal for the preservation of joint mobility and therefore ideal for the restoration of movement when the repaired flexor tendon is mobilized. It is permissible to splint a healthy finger in full extension for three weeks.
