Severe and recurrent purpura fulminans developed in a Chinese boy at one day of age. Results of coagulation studies performed on the patient during attacks were compatible with the diagnosis of disseminated intravascular coagulation. Subsequent investigations have revealed that the patient is homozygous and that his parents are heterozygous for protein C deficiency. Cryoprecipitate and fresh frozen plasma induced a remission, and administration of warfarin has been successful in preventing recurrence of attacks for as long as 8 months without infusion of any plasma components. None of the family members who are heterozygous for protein C deficiency have had thrombotic episodes.

Thrombomodulin, a cell surface cofactor for thrombin-catalyzed activation of protein C, was isolated from human placenta by a combination of affinity chromatography and gel filtration. Molecular weight of purified human thrombomodulin, estimated by sodium dodecyl sulfate electrophoresis after reduction, was 88,000. Its isoelectric point was around pH 4. The thrombomodulin was soluble only in the presence of detergent. The purified thrombomodulin was inactivated by disulfide bond reduction, but was stable to heat, pH extremes and protein denaturants. The thrombomodulin served as a cofactor of thrombin-catalyzed protein C activation in human as well as bovine enzyme systems.
Unique CT findings were obtained in four cases of acute toxic encephalopathy, including two cases of clinical Reye's syndrome in the acute stage. Common findings in these cases were symmetrical low density in the thalami and the cerebellum (mainly the dentate nuclei) and similar changes in the surroundings of the lateral ventricles.
The author analyzed the appearance on computed tomography after infusion of contrast medium of cerebellar infarction in three patients, whose diagnosis was made on the basis of clinical presentation, cerebral angiography, and sequential computed tomographic scans. Their computed tomographic appearance was not always the same as seen in the supratentorial counterpart, in showing reminiscence of an arteriovenous malformation, a leptomeningeal dissemination of a tumor or meningitis, or a neoplasm, respectively. For these reasons, it was concluded that it is important to notice the contrast-enhanced computed tomographic appearance in cerebellar infarction, when making a differential diagnosis between various pathologies in the posterior fossa.

Interferon (IFN)-induced suppression of the lectin-stimulated lymphoproliferative response was studied comparatively with human IFN-alpha, IFN-beta and IFN-gamma, using an equal unit of their antiviral activity ranging from 31.25 to 1000 IU/ml. Both IFN-alpha and IFN-beta inhibited phytohemagglutinin (PHA) and pokeweed mitogen (PWM)-stimulated lymphoblastogenic response similarly in a dose-related fashion, but the IFN-gamma effect was far less. Indeed, the PWM-stimulated lymphocyte blastogenesis in the cultures incubated for 7 days was enhanced in the presence of IFN-gamma at a concentration of 62.5 IU/ml. The enhancing effect was found to be highest at the lowest concentration of IFN-gamma examined. The IFN-gamma induced enhancement of lectin-stimulated blastogenesis was found mainly in the PWM cultures incubated for 7 days but less in cultures incubated for 5 days or in PHA cultures incubated for 3 days, suggesting that the observed effect might be caused by the activation of interleukin production.
We report a case of a primary yolk sac tumor of the lower abdominal wall with inguinal lymph node metastases. To our knowledge, this is the first documented case originating in the lower abdominal wall. Radical surgery was performed, followed by combination chemotherapy consisting of bleomycin, vinblastine and cis-platinum. The patient has been free of disease for more than 2 years.
