Hard-copy provisional appliances in the rehabilitation of the edentulous patient.

A technique is described whereby a diagnostic appliance simulating an implant-supported fixed prosthesis for the edentulous patient can be made quickly and relatively inexpensively. Such an appliance gives the patient an understanding of the physical limitations of various modalities of prosthodontic restoration. It is a tool that permits both the patient and the dentist to assess functional, esthetic, and phonetic criteria in the realistic setting of everyday usage. Important decisions regarding future treatment can thus be finalized before a huge financial commitment is made. It is a highly cost-effective technique, both financially and emotionally.

The role of the overlay denture in the prevention of mandibular ridge atrophy.

The overlay denture concept may be used to postpone and in some cases avoid the atrophied mandibular ridge syndrome. The co-operation of the patient is a vital factor for success and the treatment regimen is divided into a series of self-contained stages to allow the patient to demonstrate his mastery of each before proceeding to the next. Function in the edentate can be enhanced by the use of this type of denture.

Overlay dentures. Philosophy and practice. II.

The practical stages in the construction of mandibular overlay dentures, with and without precision attachment anchors, are described and illustrated in detail. The importance of post-insertion maintenance, continuing effective oral hygiene and patient care, as a prerequisite for a successful overlay denture experience, is emphasized.

The pulpless abutment.

The use of preformed metallic anchorages inserted in satisfactorily treated teeth for increased stabilization and retention in complete and partial dentures is demonstrated. An additonal advantage is the retention of proprioceptive guidance in mastication with complete dentures. A number of clinical examples are illustrated.

Imaging for predictable maxillary implants.

Whereas other radiographs may be adequate for assessment of mandibular anterior implant sites, the use of computerised tomography is recommended for evaluating potential implant sites in the maxillae. A complete denture replica with appropriate radiopaque markers aids in the assessment. Hounsfield densitometric measurements provide additional information on the quality of available bone.
