Effect of glycine and glucose on sodium and water adsorption in patients with cholera.

Electrolyte solutions containing glucose, glycine, or a combination of the two were absorbed sufficiently well from the intestine to supply maintenance fluid and the electrolytes required by cholera patients. Data on net absorption and duration and volume of diarrhoea show that a solution containing both glucose and glycine provides more effective therapy than solutions containing either glucose or glycine alone.

Is acute watery diarrhoea an important cause of morbidity and mortality among rural Bangladeshi children?

To assess the relative importance of acute watery diarrhoea (AWD) and other types of acute diarrhoea as causes of morbidity and mortality among infants and 1-4 years old children, we examined 3 different data sources from the Matlab field project of the International Centre for Diarrhoeal Disease Research, Bangladesh. In infants, prevalence rates for AWD and non-watery diarrhoeas were similar. In children, prevalence of AWD was 1.8 times lower than prevalence of other acute diarrhoeas. In infants, admission rate to a diarrhoea hospital was 4.1 times higher for AWD than for other acute diarrhoeas (P less than 0.001). In children, admission rate was only 1.7 times higher for AWD than for acute diarrhoeas (P less than 0.001). Infant mortality was 1.7 times higher for AWD than for other acute diarrhoeas, but child mortality was 3 times lower for AWD. These data suggest that, while diarrhoeal disease control programmes should give more importance to oral rehydration therapy in infants, field management of the other types of acute diarrhoea should receive more emphasis in children.

Utilization of medically-trained birth attendants in rural Bangladesh.

Using case-control methodology, this study compared the characteristics of women who requested medically-trained birth attendants at home with those who did not, in order to identify constraints to service delivery and suggest program changes to increase service utilization in a rural area of Bangladesh. By several different measures, prior contact with medical professionals were much more common among cases than controls, with the greatest differences observed in the frequency of antenatal visits. At least one antenatal visit was the strongest predictor that a woman would call a medically-trained birth attendant, though primiparity, proximity to the provider, previous contact with the clinic, and education (both of the mother and her spouse) were also predictive. Recommendations include increasing contact through antenatal visits, and extending midwife coverage through training and supervision of traditional birth attendants. This study demonstrates the usefulness of case-control methodology in evaluating health care utilization.

Health profile of lock factory workers in Aligarh.

In order to assess the health problems and its relationship to the nature of work of lock factory workers, a field based cross sectional study was carried out in sixty randomly selected lock factories in urban industrial areas of Aligarh city. Nine hundred fifty seven lock factory workers were included in the study out of them 45.7 percent were found to be suffering from one or more than one diseases. Detailed study of the workers showed that 73.05 percent were suffering from respiratory tract diseases, a majority of them having upper respiratory tract infection. Diseases showed a definite trend either increasing (Chronic bronchitis, Emphysema etc) or decreasing (upper respiratory tract infection, Bronchial asthma, Injuries & accidents), according to the age and duration of work. Polishing & filing appeared to be the most hazardous type of the work as shown in the proportion of chest diseases, (56.6%) & (38.3%) in polishing & filling workers respectively.

Transferring health and family planning service innovations to the public sector: an experiment in organization development in Bangladesh.

The International Centre for Diarrhoeal Disease Research, Bangladesh, has launched a field experiment in two rural thanas of Bangladesh to test the transferability of its successful health and family planning experiment in Matlab to the Ministry of Health and Population Control service system. This paper reviews the Matlab experiment with particular attention to its organization and identifies elements for transfer. The intervention strategy and operations research design of the new experiment are discussed. The proposed design follows an organization development strategy in which collaborative diagnostic research is used to foster institutional change.

Ulnar deviation of the fingers in gout simulating rheumatoid arthritis.

Two cases of gout with marked ulnar deviation (UD) are reported. Both cases were previously diagnosed rheumatoid arthritis (RA) at referral, but gout was clinically suspected because of atypical features of RA, and subsequently confirmed by demonstration of monosodium urate crystals under the polarized microscope. No definitive evidence of associated RA was present in either case. Although UD in gout is distinctly rare, it is important to recognize this deformity in gout in order to avoid inappropriate therapy. Mechanical factors might have contributed to the hand deformities in both cases.
