[Cefotiam concentration in exudates from the drainage of patients with acute peritonitis following intravenous administration].

Cefotiam dihydrochloride (CTM) in a dose of 2 g was given by intravenous bolus injection to 15 patients operated upon for acute or subacute abdominal organs. They were included to 5 cases of acute localized peritonitis due to perforative appendicitis, 3 of diffuse peritonitis due to perforative duodenal ulcer, 1 of diffuse peritonitis with intestinal obstruction, 1 of perforative sigmoid colon cancer, 2 of gastric cancer or polyp with cholelithiasis, and 3 of cholangitis with cholecyst-choledocholithiasis. The materials of exudate from drain were taken at intervals by sterilized paper disc and determined by paper disc bioassay method with Proteus rettgeri ATCC 9250 or Proteus mirabilis ATCC 21100 as the test organisms to CTM concentrations. CTM concentrations in purulent exudate from drain of patients with acute peritonitis, they were increased quickly after intravenous bolus injection, and reached higher levels at early time after injection in cases 2 or 3 days after operation. CTM concentration in purulent exudate from drain was tended to increase in proportion to the severity of the inflammation. The CTM levels in infected exudate were higher than the MICs against clinically isolated organisms for a long time after administration. Therefore, CTM was very useful drug when used for chemotherapy of postoperative peritonitis.

The influence of family history of cancer, irradiation and anticancer medication (mitomycin C), on the occurrence of multiple primary neoplasms with breast cancer--statistical analysis by the person-year method.

The influence of family history of cancer, radiation therapy and anticancer drug therapy (mitomycin C) on the occurrence of multiple primary neoplasms, following treatment of a first primary cancer of the breast, was analyzed by the person-year method in 1,359 patients, in Japan. During 14,371.8 person-years of observation, 111 multiple primary neoplasms including bilateral breast cancers were found in 109 patients. The incidence rate of multiple primary neoplasms were 0.00772 per person-year. The incidence in patients with a family history of cancer was 1.29 times greater than that in patients without such a family history, and the incidence in patients with a family history of breast cancer was about three times greater than that in those without it (p less than 0.01). Radiation therapy raised the occurrence of subsequent primary neoplasms 1.28-fold (or 1.62 fold after 5 years), and mitomycin C (a total dose of 0.8 mg/kg) therapy caused no increase in the occurrence of subsequent primary cancers, after an observation of 10 years or so.

[Cefminox concentration in tissues and clinical efficacy of cefminox in acute peritonitis].

Cefminox sodium (CMNX, MT-141), a new semisynthetic cephamycin, having marked resistance to beta-lactamase, and a broad spectrum of antibacterial activity against various bacterial species, including Haemophilus influenzae, Serratia marcescens and Citrobacter freundii, CMNX has higher activity in vivo than in vitro. For therapeutic purpose, CMNX was given in a daily dose of 0.5 g (0.5 g X 1) to 2 g (1 X 2) by intravenous drip infusion for 4 to 8 days to 24 cases with acute peritonitis (17 cases with acute appendicitis, 1 with localized peritonitis after gastrectomy, 1 with diffuse peritonitis due to perforative duodenal ulcer and 5 with panperitonitis due to intestinal obstruction). The clinical response was rated excellent in 9 cases, good in 14 cases and fair in 1 case and poor in none. No adverse effect was observed. There were 29 strains isolated organisms included 12 Escherichia coli, some Enterococcus faecalis and Pseudomonas aeruginosa. These isolated organisms were eradicated after CMNX treatment, except a strain of E. faecalis was decreased. In 19 cases of them, 16 cases with acute peritonitis due to acute appendicitis and 3 cases with acute panperitonitis due to intestinal obstruction, CMNX was administered intravenously in a dose of 1 g (1 case was 0.5 g) before or during the operation, and tissue specimens and body fluids samples were taken during the operation. CMNX concentration was determined to a bioassay with Escherichia coli NIHJ or Vibrio vercolans ATCC 8461 as the test organisms. CMNX concentrations in purulent ascites were 47.2 +/- 38.5 micrograms/ml (n = 23), those in infected appendix wall were 32.2 +/- 21.7 micrograms/g (n = 16), that in pus in appendix were 22.1 +/- 24.3 micrograms/ml (n = 8) and that in other non infected tissues were 24.3 +/- 22.0 micrograms/g (n = 8). CMNX concentrations in infected tissues were higher than the non infected tissues. In the 3 cases with empyemic appendicitis, CMNX levels in pus in appendix were more higher than that in appendix wall itself. Therefore, CMNX sodium appears to be a very useful drug when used for chemotherapy on acute peritonitis.

Low mobility T cells in the peripheral blood of cancer patients: surface modification and mobility.

To elucidate how low-mobility T cells emerge in the peripheral blood of cancer patients, lymphocytes from normal individuals were treated with cancer patients' plasma, immunosuppressive (IS) substance with or without interleukin-2, or neuraminidase. A delay in cell mobility was induced by IS substance and neuraminidase treatments, which degrade the cell surface charge. Compared to the normal mobility pattern of lymphocytes, patterns induced by these treatments were characterized by a general shift of the fast cell peak to the left side. These patterns were different from those of cancer patients in terms of the inverse relationship between slow and fast peaks. These results indicate that, though humoral factors in the peripheral blood of cancer patients take a partial role, other mechanisms might operate to induce the characteristic mobility pattern of lymphocytes. One of the possibilities, the emerging of a special T cell subset recruited from the thymus, is discussed.

Urinary polyamines as a tumor marker.

We recently established a new simple enzymatic assay method for measuring total urinary polyamines. To evaluate the clinical usefulness of measuring total urinary polyamines as a tumor marker, we have applied this method to the assay of polyamines in the urine of cancer patients. Elevation above 3 SD of the normal mean was found in 116 of the 181 patients with cancer (stomach 49/72 [68.1%], colon 22/32 [68.8%], lung 16/24 [66.7%], blood 15/27 [55.6%], liver 3/14 [21.4%], gallbladder 4/4 [100%], and esophagus 7/8 [87.5%]). Total urinary polyamine levels were determined before and after surgery in 36 patients with gastrointestinal cancer (stomach 22 and colon 14). Urinary polyamine levels fell to within the normal range after successful surgery in 23 of 30 patients who had showed elevated levels of urinary polyamines before surgery. Our data indicate that the determination of total urinary polyamines by our new assay may be useful as a tumor marker.

[Clinical studies on aztreonam following intravenous drip infusion].

Aztreonam (AZT), a new synthetic monocyclic beta-lactam antibiotic, which is resistant to beta-lactamase and has a strong and specific activity against aerobic Gram-negative bacteria including Pseudomonas aeruginosa. The patients of 13 cases with localized peritonitis due to acute appendicitis, 3 cases with panperitonitis (1 case with perforative appendicitis, 1 with acute cholecystitis and 1 with pancreatic necrosis) and 4 cases with skin and soft tissue infection (anal fistula and abdominal abscess etc.) were treated by AZT. AZT was administered in a dose of 1 g twice a day by intravenous drip infusion using 100 ml-volume bottle preparation with saline for 4 to 10 days. Clinical efficacy was rated excellent in 2 cases, good in 16 cases, fair in 1 case and poor in 1 case (efficacy rate 90.0%). Adverse effects were small skin rash in 1 case, and increased GOT and GPT in 1 case. No adverse effect was recognized in other cases. Therefore, AZT appears to be very useful drug when used for chemotherapy of infectious diseases in surgery.

Development of retinohypothalamic projections in the chick embryo.

The retinal projection to the hypothalamus was investigated in embryonic as well as hatched chick using wheat germ agglutinin-conjugated horseradish peroxidase as an anterograde tracer. The presence of an aggregation of anterogradely labeled terminals in the hypothalamus was used to identify the suprachiasmatic nucleus (SCN). The first clear labeling of retinal fibers in the SCN was found on embryonic day 16, whereas labeling was found in the other primary visual projections, in the diencephalon and mesencephalon, several days earlier. Thus, the retinohypothalamic projection is the last primary visual pathway to develop in the chick embryo.

A family study of renal dysplasia.

A family study was undertaken to investigate genetic involvement in renal dysplasia, which is defined as abnormal metanephric differentiation. Probands were ascertained through the retrospective examination of necropsy records and the reevaluation of available material, which included microscopic examinations of the kidneys, gross descriptions of organs, and gross photographs. We obtained family histories and performed physical examinations and renal ultrasonography on parents and sibs of the 21 probands. In only one family a sibling with renal dysplasia was discovered; both the proband and the previous stillborn sib had renal dysplasia in association with posterior urethral values. Renal dysplasia could have resulted from urinary tract obstruction secondary to the urethral valves, with inheritance of the valves as the primary abnormality. However, we cannot exclude primary inheritance of the renal abnormality, perhaps with multifactorial determination with a threshold. The empiric recurrence risk of 2.1%, calculated from this family study, was statistically not significantly different from zero. We can assume, therefore, that the multicystic and aplastic types of renal dysplasia, which predominated in this study, are sporadic or rarely familial, but certain other types of renal dysplasia, identified in the literature as familial, probably carry a higher recurrence risk.

Vagal afferent C fibers projecting to the lateral descending trigeminal complex of crotaline snakes.

The primary vagal axons and terminals in the lateral descending trigeminal complex (dlv-DLV complex) in crotaline snakes were studied following HRP injections into the vagal nerve. Labeled fibers and terminals were found in the marginal neuropil, which was made up entirely of unmyelinated fibers, i.e., C fibers. The general features of vagal input to the dlv-DLV complex in snakes with infrared sensitivity (Boidae and Crotalinae) are discussed.

Configurational changes in rat liver nuclear chromatin caused by azo dyes.

The effects of azo dyes on the chromatin configuration of isolated rat-liver nuclei were examined by electron microscopy for elucidation of the mechanism of dye-stimulated in vitro RNA synthesis. Sunset Yellow FCF had no effect on the incorporation of [3H]UTP into RNA but amaranth and Ponceau 3R stimulated incorporation approximately two- and 3.5-fold, respectively. Sunset Yellow FCF did not produce any major alteration in liver nuclei. Isolated liver nuclei treated with amaranth or Ponceau 3R showed dissociation of heterochromatin and the nucleoli lost their fine-granular structure acquiring a coarse-granular configuration. The dense structure of heterochromatin in liver nuclei treated with Ponceau 3R was completely dissociated and appeared as a lace-like configuration. These results suggest that both Ponceau 3R and amaranth stimulate in vitro RNA synthesis by causing the dissociation of heterochromatin in isolated rat-liver nuclei.

Urinary tract infection and blood P1 antigen: preliminary report.

A clinical study was made on the relationship between the blood type P1 antigen and urinary tract infection (UTI). The blood type P1 antigen could be detected in 3 out of 11 healthy Japanese volunteers (27.2%), and in 54% of the UTI patients as a whole. Classified by the type of infection, it could be detected in 3 out of 4 patients with upper UTI (75%) and in 11 out of 22 patients with lower UTI (50%). These incidences were higher than that of healthy volunteers, the difference being statistically significant. The relationship between the annual frequency of UTI and the positive detection of P1 antigen was examined. The patients who had been exposed to UTI twice or more a year proved to have a higher detection rate (61%), than the other group of patients, the difference being statistically significant. Two of the patients with E. coli detected as a clinical isolate proved to have the P1 antigen.

[Urinary tract infection : clinical significance of antibody-coated bacteria].

More attention should be directed to upper urinary tract infections as they cause renal parenchymal damage. Recently, many reports on antibody-coated bacteria (ACB) in urological diseases have been made. We performed ACB tests in 68 cases of urological infectious diseases. The tests were made according to Thomas' method. Anti-human immunoglobulin IgG, IgA, and IgM rabbit sera were used. The results were compared using the chi-square analysis. ACB-positive were 0 approximately 15% of the cases of acute cystitis, 33 approximately 67% of the cases of chronic cystitis, 30 approximately 60% of the cases of prostatitis and urethritis, and 60 approximately 90% of the cases of pyelonephritis. The percentage was higher in the patients who had indwelling catheters than in those who did not. The ACB test was suggested to be helpful in diagnosing upper urinary tract infection in the female patients who did not have indwelling catheters. But there was the danger of making an incorrect diagnosis with this test on male patients who had indwelling catheters, and those with prostatitis or urethritis, and on female patients with indwelling catheters.

[Influence of family history, irradiation and anti-cancer drug (mitomycin C) on the occurrence of multiple primary neoplasms in breast carcinoma patients].

The influence of family history, irradiation and anti-cancer drug (Mitomycin C) on the occurrence of multiple primary neoplasms was analysed using the person-year method in 1359 Japanese breast carcinoma patients. There were 111 multiple primary neoplasms, including bilateral breast cancer, in 109 patients; the incidence rate was 0.0072 per person-year. The incidence rate in patients with a family history of cancer was 1.29 times higher than in those without. In the bilateral breast cancer group there was about a 3 times higher frequency of family history of breast cancer. Irradiation therapy raised the occurrence of multiple primary neoplasms 1.28 fold, and Mitomycin C (40 mg) had no effect on the occurrence of neoplasms during a 10-year observation period.
