[Tumor marker: practical usage of tumor marker for recurrent breast cancer--our experience].

The point of practical usage of monitoring tumor marker, CEA and CA15-3, for breast cancer patients was indicated and discussed. We presented several cases with recurrent breast cancer patients and verified the practical usefulness of monitoring tumor marker, CEA and CA15-3. Fundamental changing pattern of these markers in serum on semi-logarithmic graph was exhibited to show linear pattern and the two curves of CEA and CA15-3 kept basically parallel except several conditions, when both markers were positive. From the graphical changes of markers we could made early and correct judgement of the effect of the treatment. However, there existed several cases who showed strange and paradoxical changing pattern of these markers apart from clinical courses. That is the situation that heterogenetic changes of the tumor or tumor lysis syndrome are taken place. It was emphasized that the value of serum tumor markers should be taken notice of the changes of markers in these situations.

[Analysis of prognostic factors of node negative breast cancer].

Prognostic factors of 229 patients with node negative breast cancer were analysed using Cox's proportional hazard regression model. From 11 pathological factors, four factors nuclear grade, pectoral invasion, fat infiltration and tumor size were selected under condition of p less than 0.05. When the three factors (moderate or poorly differentiated nuclear grade, marked fatty infiltration and tumor size 2 cm) were provided as poor prognostic factors, node negative breast cancer was successfully classified from the number of the poor prognostic factors: namely, the recurrence rate of subgroups with the number of 0,1,2,3 was 2%, 7%, 21%, 39%, respectively.

Association of elevated expression of the c-erbB-2 protein with spread of breast cancer.

We analyzed amplification and expression of the c-erbB-2 gene in human breast cancers. Southern blot hybridization analysis demonstrated amplification of the c-erbB-2 gene in 10 out of 50 tumor DNAs examined. The degree of amplification was three- to twenty-fold relative to normal placenta. The c-erbB-2 protein could be analyzed in 39 tumor tissues of the 50 samples by immuno-blotting, and elevated expression of the c-erbB-2 protein was found in 15 cases. On the other hand, expression of the c-erbB-2 products was not detected in normal breast tissues either by immuno-blotting or by immuno-histological analysis. These data indicate that transcriptional and/or translational activation of c-erbB-2 might occur in some breast cancers in addition to activation by gene amplification. The elevated expression of the c-erbB-2 protein was most strongly correlated with lymph-node metastasis (P less than 0.001), suggesting that c-erbB-2 expression is involved in promotion of the lymph-node metastasis of human breast cancers. Therefore, immuno-histological diagnosis with anti-c-erbB-2 antibody might be useful as an indicator to predict lymph-node involvement in breast cancer.

Recovery from foetal hypothyroidism: evidence for the safety of breast-feeding while taking propylthiouracil.

We assessed the post-natal thyroid function in eight infants of mothers with Graves' disease whose thyroid function at birth was suppressed by maternal ingestion of propylthiouracil during pregnancy. These mothers continued taking propylthiouracil after delivery and breast-fed exclusively (two mothers supplemented their breast milk with a small amount of baby food). The cord free T4 level was slightly but uniformly below the normal range in all eight infants, and the cord TSH level was above the normal in seven infants. The dose of propylthiouracil after delivery ranged from 50 to 300 mg daily, which was equal to, or higher than, that before delivery. All these abnormal values normalized in the infant after birth. Serum samples, from seven of the eight mothers, taken at delivery were examined for TSH receptor antibodies; all were positive. The antibody titre, however, was too low, and/or free T4 and TSH levels were examined too long after delivery, for the antibodies to be the cause of the restoration of the infants' thyroid function. These results assure the safety of breast-feeding for the infants of mothers with Graves' disease taking propylthiouracil.

Mean cellular volume of urinary red blood cells in investigation of hematuria.

The mean cellular volume (MCV) of urinary red blood cells (RBCs) of pediatric patients with glomerular (group I; n = 52) and nonglomerular (group II; n = 21) hematuria was determined using an automated blood-cell analyzer. Group I patients had a significantly lower MCV than group II (61 +/- 8 vs. 88 +/- 15 microns 3, mean +/- SD; p less than 0.001). With a MCV of 75 microns 3 taken as the dividing line between glomerular and nonglomerular hematuria, correct assessment of the site of bleeding was made in 67 of the 73 (92%) patients studied. Determination of the MCV of urinary RBCs is a simple noninvasive test for localizing the site of hematuria in pediatric patients.

[Hypothalamic glioma with diencephalic syndrome and following precocious puberty--a case report].

A 1-year 11-month-old girl was admitted for losing her weight and gait disturbance. At 4 months of age, she began to become emaciated inspite of normal food intake. Physical and neurological examinations were normal except for a marked lack of subcutaneous fat, irritability and nystagmus. CT scans demonstrated a large tumor occupied in the third ventricle and marked dilatation of the lateral ventricles. Endocrinological studies revealed high levels of plasma growth hormone (GH) in contrast with normal levels of somatomedin-C. The basal value of GH returned to normal with a subnormal response to insulin subsequently after VP-shunt. Then, a pilocytic astrocytoma was partially resected with transcallosal approach. Postoperative course was uneventful and her growth rate returned to normal range. CT scans after radiation therapy of 49 Gy showed marked decrease in size of the tumor. At 3 years and 6 months of age, enlargement of her breast was pointed out although MRI indicated no enlargement of the tumor. Basal value of LH, FSH, E 1 and E 2 elevated and LHRH test showed over-response of LH and FSH. Other hypothalamic-pituitary functions were partially preserved. Case of precocious puberty following diencephalic syndrome associated to the hypothalamic and/or optochiasmatic glioma is quite rare in the previous literature. Mechanisms of diencephalic syndrome and following puberty are unclear. However, endocrinological and radiological findings observed in the present case suggest that hormonal disfunction might be due to the failure of inhibition on GH and LHRH secretion mechanism in the anterior hypothalamus.

[CD33-positive acute lymphoblastic leukemia with breast tumor and cavernous sinus tumor].

A 38-year-old woman was diagnosed as acute lymphoblastic leukemia (L2) in Oct. 1985. After VP and AdVEMP therapy, complete remission was obtained. In Oct. 1987, she noticed bilateral breast tumors and leukemic cell infiltrations were shown in a biopsy specimen of the breast tumor. Bone marrow was occupied with 94 percent blasts. The second complete remission was achieved by the AdVP therapy. In Nov. 1988, she developed double vision and photophobia. The examinations of CT and MRI showed cavernous sinus tumor, and 20 percent blasts were recognized in a bone marrow aspirate. The leukemic cells were negative for peroxidase, but were positive for both lymphoid and myeloid cell surface markers (CD2, CD5, CD7, CD33). The two color flowcytometry showed that CD5 and CD33 were simultaneously expressed on the leukemic cells.

[A case of fused pelvic kidney (lump kidney) with invasive bladder tumor].

A case of fused pelvic kidney, in a 67-year-old woman is presented. The patient also had a bladder tumor that had invaded the bladder wall. The renal anomaly was confirmed by drip infusion pyelography, computed tomography scanning, (CT-scan) and angiography. The CT-scan showed lump kidney with an aplastic right kidney anterior to the sacrum. Two renal arteries derived from the right common iliac artery were demonstrated by pelvic-angiography. The right ureter was not identified during the total-cystectomy. The left ureter was fixed firmly to the surrounding tissues and its free portion was less than 7 cm long. Thus left ureterocutaneostomy was selected for the formation of the urinary diversion.

Complex glycerol kinase deficiency: molecular-genetic, cytogenetic, and clinical studies of five Japanese patients.

Five male Japanese patients with complex glycerol kinase deficiency (CGKD) and their relatives were studied clinically, cytogenetically, and molecular-genetically. All patients had muscular dystrophy or muscle weakness, mental retardation, congenital adrenal hypoplasia, and glycerol kinase deficiency. High-resolution GTG-banded chromosomes showed a microdeletion in the Xp21 region in all four patients examined and in all five mothers. Southern hybridizations, after digestions by restriction endonucleases, with various cloned DNAs (D2, 99-6, B24, C7, L1-4, cDMD13-14, J66-HI, P20, J-Bir, ERT87-30, ERT87-15, ERT87-8, ERT87-1, XJ-1.1, 754, cx5.7, and OTC-1) that are located around Xp21 also showed a deletion in the genome of all patients and mothers. Although the deletion differed in size among patients, a segment commonly absent was located between the genomic sequences corresponding to L1-4 and cDMD13-14. This finding indicated that the gene coding for glycerol kinase (GK) is located within this segment. A comparison of the clinical manifestations of the present five patients and reported CGKD or Duchenne muscular dystrophy (DMD) patients with DNA deletion suggests the existence of a certain gene responsible for gonadotropin deficiency (GTD). The result of the present study and results of previous studies suggest that genes for ornithine transcarbamylase (OTC), DMD, and GK and putative genes responsible for congenital adrenal hypoplasia (AHC) and GTD are arranged from telomere to centromere as pter--GTD--AHC--GK--DMD--OTC--cen.

Prenatal DNA analysis in four embryos/fetuses at risk of 21-hydroxylase deficiency.

Prenatal diagnosis of 21-hydroxylase deficiency (21-OHD) in two unrelated embryos and two fetuses was attempted with the Southern hybridization method using the 21-hydroxylase (21-OHase) complementary DNA as a probe. The two embryos whose genomic DNA was extracted from their chorionic villi both had four TaqI fragments (3.7 kb, 3.2 kb, 2.4 kb and 2.3 kb) identical to those of their respective parents and normal controls, while the DNA from each proband of these two families lacked with the 3.7 kb and the 2.3 kb fragments corresponding to the functional 21-OHase gene (21-OHase B gene). These findings indicated that none of the embryos examined were deletion homozygotes for the 21-OHase B gene. In the two fetuses, only amniotic fluid cells were available for prenatal diagnosis. The results of Southern hybridization analysis were uninformative since all family members, including the probands and fetuses, had all four TaqI fragments. Linkage studies between 21-OHD and human leukocyte antigen (HLA) haplotypes and those between the disease and restriction fragment length polymorphisms of the 4th complement gene revealed that the fetus of one family was normal. The other fetus could not be diagnosed because a recombination between the class I HLA and the 21-OHD loci had occurred in this family.

Haemolytic uraemic syndrome associated with Yersinia enterocolitica infection.

Haemolytic uraemic syndrome (HUS) associated with Yersinia enterocolitica gastroenteritis is reported in a 6-year-old girl. Y. enterocolitica of biotype 03 was isolated from the patient's initial stool sample and was subsequently identified as serotype 03 based on the rising agglutinin titres. This paper shows that yersiniosis should be suspected as a possible cause of HUS, and investigations should include the measurement of serum agglutinin titres against antigen preparations of the genus Yersinia.

Glomerular basement membrane thickness in recurrent and persistent hematuria and nephrotic syndrome: correlation with sex and age.

The glomerular basement membranes (GBM) were measured in 264 patients with recurrent and persistent hematuria (152 males, 112 females), 47 patients with minimal change nephrotic syndrome (26 males, 21 females), and 91 patients with focal segmental glomerular sclerosis and nephrotic syndrome (55 males, 36 females). The average number of glomeruli measured was 2.5 per case. Analysis of the data showed GBM thickness to be significantly greater in males (n = 233) than in females (n = 169) (Student's t-test, P less than 0.01). There was no difference in GBM thickness between the two groups of nephrotics; the data were subsequently combined and the group referred to as idiopathic nephrotic syndrome. There was also no difference in GBM thickness among the immunofluorescence-defined subcategories of recurrent hematuria, and these groups were combined. GBM thickness was significantly greater in the idiopathic nephrotic syndrome than in recurrent hematuria (Kruskal-Wallis H test, Po = less than 0.001). Analysis of the data showed, in both conditions, a gradual increase with age throughout the span of the study (1-69 years). Regression analysis of GBM thickness and age in recurrent hematuria showed Ro = 0.43 and Po = less than 0.001, in idiopathic nephrotic syndrome Ro = 0.61 and Po = less than 0.001. The differences between nephrotic syndrome and recurrent hematuria may be based on the inclusion in the recurrent hematuria group of patients with genetically thin GBM; they may also be based on the inclusion in the idiopathic nephrotic syndrome group of patients with subendothelial thickening, a relatively frequent occurrence.(ABSTRACT TRUNCATED AT 250 WORDS)

Increased platelet thromboxane synthesis in renal glomerular diseases.

Washed platelets were prepared from healthy children and adults, and patients with renal glomerular diseases, and incubated with [1-14C] arachidonate to measure the generation capacities of thromboxane (Tx) A2 and 12-hydroxyeicosatetraenoate (12-HETE). Tx generation capacity of platelets was significantly higher in patients with chronic glomerulonephritis, purpura nephritis and lupus nephritis than in healthy control subjects. There was no significant increase in minimal change nephrotic syndrome. 12-HETE showed a decreasing tendency in the glomerular diseases, which was restored to normal level by in vitro addition of indomethacin. Such increased Tx generation capacity of platelets may cause abnormal enhancement of platelet functions and conceivably constitute an aggravating factor of glomerular and microvascular damage in the affected kidney.

Neonatal onset of medium-chain acyl-CoA dehydrogenase deficiency in two siblings.

Two male siblings with medium-chain acyl-CoA dehydrogenase deficiency were reported, in whom the enzyme activity was essentially undetectable and the symptoms and signs, including cyanosis, apnea, low body temperature, hypoglycemia and hyperammonemia, appeared within 48 hours of life. Muscle weakness and cardiomegaly in association with morphological abnormalities of mitochondria in skeletal and cardiac muscles, respectively, were found on electron microscopic examination in one of them. These observations suggest that the patients suffered from the most severe form of the disease, which has not been described in the literature.

Percutaneous renal biopsy well-visualized by orthogonal ultrasound application using linear scanning.

We attempted 24 percutaneous renal biopsies under orthogonal ultrasonic guidance in 23 children with renal diseases aged 1 to 17 years old. We used a 5 MHz linear-array transducer which was positioned on the side of the abdomen and aligned parallel to the biopsy needle so that projected ultrasound reaches the needle orthogonally. Thus, we could clearly and continuously monitor the biopsy needle and transverse plane of the kidney during the biopsy. We obtained diagnostically sufficient renal specimens in all subjects and found no significant complications, including macroscopic hematuria, in any case. We found that our method simultaneously visualized the biopsy needle and kidney much more clearly and made the biopsy far easier and safer than any other method previously proposed and we recommend the procedure be widely used in adult as well as child patients.
