[Autoimmune hemolytic anemia reactivated by alpha-interferon therapy in a case of chronic active C-type hepatitis].

A case with autoimmune hemolytic anemia (AIHA) induced by interferon-alpha (IFN-alpha) is presented. A 40-year-old male who had a previous history of autoimmune hemolytic anemia, agranulocytosis and thrombocytopenia was admitted to our hospital because of chronic C type hepatitis. Liver biopsy was performed, which diagnosed chronic active hepatitis and IFN-alpha was administrated at a dose of 3 Meg unit per day. 11 days after the initiation of the therapy he developed hemolytic anemia, but Coombs tests were negative. Although IFN was withdrawn 15 days later, anemia became progressively more serious. 20 days later, both direct and indirect Coombs tests became positive. He was diagnosed as AIHA and treated with methylprednisolone pulse therapy, then he recovered soon afterward. Further analysis of Coombs tests revealed that he had both cold type and warm type (IgG) autoantibodies which was the same type of antibodies for AIHA he suffered 10 years ago. In conclusion, latent AIHA may be reactivated by the treatment with IFN-alpha.

[Endoscopic mucosal resection for radical treatment of esophageal cancer].

Clinico-pathological results of patients with superficial esophageal cancer was reviewed to determine the indications of endoscopic mucosal resection (EMR) for esophageal cancer as a radical treatment and to evaluate clinical results of EMR. The analysis on eighty-seven cases with superficial esophageal cancer who underwent esophagectomy revealed no lymph node metastasis in any 0% of case with cancer confined to the lamina propria mucosae, 10% of cancer reaching the muscularis mucosae and 43% of cancer infiltrating the submucosa. These results suggested that the endoscopic mucosal resection should be indicated for patients with esophageal cancer confined to the lamina propria mucosae. The accuracy rate for estimating depth of invasion of mucosal cancer of the esophagus was 96%. We early established "the double channel technique" for resection of mucosal lesion of the esophagus with a major part of the submucosa, we used it for sixty-nine cases, and all were eventually discharged. Immediate complications of EMR were noted in 12.9% of all cases (mediastinal emphysema: 2.9%, ulcer bleeding: 10%) and the late complication in 7.2% (esophageal stricture due to scar formation: 5.8% and ulcer bleeding 5 days after EMR: 1.4%). All cases who developed stricture had mucosal defect over 3/4 the circumference. The cumulative 5-year survival rate of patients with esophageal mucosal cancer treated by EMR (86%) showed no significant difference from those treated by esophagectomy (83.2%). We to conclude that endoscopic mucosal resection is indicated for the patient with mucosal cancer confined to the lamina propria mucosae. One can expect an excellent prognosis by less invasive treatment than esophagectomy.

Sarcoglycan complex is selectively lost in dystrophic hamster muscle.

We recently reported that the dystrophin-associated glycoprotein (DAG) complex is biochemically divided into two subcomplexes: one is the dystroglycan complex comprised of 156DAG and 43DAG and the other is the sarcoglycan complex comprised of 50DAG, A3b, and 35DAG. A3b is a novel dystrophin-associated glycoprotein with an approximate molecular mass of 43 kd but is distinct from 43DAG. In the present study, we examined the striated muscles of the dystrophic hamster with anti-A3b antibody in addition to anti-50DAG, anti-43DAG, anti-35DAG, anti-dystrophin, and anti-laminin antibodies by both immunohistochemistry and immunoblot analysis and found that 50DAG, A3b, and 35DAG are selectively lost. This selective defect of the sarcoglycan complex in dystrophic hamster muscles may give rise to dystrophic changes in striated muscles. Thus, the differentiation of the dystrophin-associated glycoprotein complex into the dystroglycan and sarcoglycan complexes is important not only from a biochemical standpoint but also in understanding the cause of muscular dystrophy in the hamster. Our findings further show that the dystrophic hamster may serve as an animal model for a human disease, severe childhood autosomal recessive muscular dystrophy, which has recently been shown to result from a selective defect in the sarcoglycan complex.

[Clinical effects of carmofur (Mifurol) on advanced and recurrent breast cancer in a cooperative study. Research association for re-evaluation of direct effects of Mifurol on breast cancer].

The direct effect and safety of carmofur (Mifurol) in cases of advanced and recurrent breast cancer were evaluated as a cooperative study at twenty-two facilities nationwide. Carmofur 12 mg/kg/day within the maximum daily dose of 600 mg was administered orally for eight weeks or longer. Of 42 patients registered for the study, 38 met the eligibility criterial (qualified patients), of whom 31 completed all tests for evaluation (completely qualified patients). The response rates were 18.4% with the qualified patients (PR, 7; Long NC, 3; NC, 11; PD, 12; NE, 5; ie, 7/38), and 22.6% with the completely qualified patients (PR, 7; Long NC, 2; NC, 10; PD, 12; ie, 7/31). The interval before the onset of PR after administration of carmofur was started was a minimum 29 days, a maximum 190 days and a median 82 days. Side effects were observed in eighteen patients (42.9%). Frequent micturition, loss of appetite, dizziness or feverish feeling were major symptoms, but none were serious.

Interleukin-1 gene expression in transient retinal ischemia in the rat.

PURPOSE: To determine in rats whether there are time-dependent changes in interleukin-1 alpha (IL-1 alpha) and interleukin-1 beta (IL-1 beta) gene expression in transient retinal ischemia and to localize their mRNAs in the retina. METHODS: Retinal ischemia was induced in Sprague-Dawley rats by ligating the optic nerve. Two hours later, the ligature was released and reperfusion occurred. The levels of IL-1 alpha and IL-1 beta gene expression in the sensory retina were then compared at various times after reperfusion by a semiquantitative polymerase chain reaction method. Localization of their expressed mRNAs was examined by in situ hybridization histochemistry. RESULTS: Little expression of IL-1 alpha and IL-1 beta genes was observed in normal retina. IL-1 alpha gene expression rapidly increased (about 30-fold greater than that of the control) as early as 1 hour after cessation of ischemia, reached a peak (about 50-fold) at 3 to 12 hours, and then gradually decreased to near baseline levels. IL-1 beta gene expression began to increase 2 hours later than did that of IL-1 alpha and had two peaks. IL-1 beta gene was found by in situ hybridization histochemistry to be expressed by retinal glial cells, endothelial cells, and neutrophils infiltrating the retina and vitreous. No gene expression was found in the control retinas. CONCLUSIONS: Expression of IL-1 alpha and IL-1 beta genes was dramatically upregulated during reperfusion after induced retinal ischemia. IL-1 beta gene was expressed by retinal glial cells, endothelial cells, and neutrophils recruited into the retina. From these results, it appeared that IL-1 may have an important role in retinal ischemia-reperfusion injury.

[Two siblings of familial amyotrophic lateral sclerosis with multisystemic degeneration characterized by mild involvement of the middle root zone of the posterior column, Clarke's nuclei and spinocerebellar tract].

The aim of this study is to clarify the clinicopathological characteristics of the multisystem degeneration seen in two male siblings with familial amyotrophic lateral sclerosis (FALS). A similar neurological disorder affected their elder sister and paternal uncle, but not their parents. The older brother (case 1) developed muscular weakness at 50 years of age and the younger brother (case 2), at 42 years of age. The duration of illness was 19 months in case 1 and 31 months in case 2. The clinical picture was the common (suspended) form in case 1 and the pseudopolyneuritic form in case 2. Pyramidal tract sign was obscure in both cases and cerebellar sign, sensory disturbance, sphincter disturbance and oculomotor palsy were not observed in either case. Neuropathological examination revealed similar findings in the two cases: 1) marked loss of lower motor neurons in the spinal anterior horn and motor nuclei of the lower brain stem in both cases, with neuronal loss of Onuf's nuclei in case 2; 2) very mild involvement in Clarke's nuclei, the dorsal and ventral spinocerebellar tracts and the middle root zone of the posterior column; 3) relatively well preserved Betz cells in the upper motor cortex with the appearance of a few macrophages, and mild changes in the pyramidal tract of the spinal cord; and 4) mild degenerative changes in the pallidoluysian system and the dentatorubral system. The most characteristic pathological findings common to both cases were the extremely mild involvement of the middle root zone of the posterior column, Clarke's nuclei and spinocerebellar tracts. The pattern of lower motor neuron system degeneration paralleled the development of clinical features. Genetic studies demonstrated no mutations in exons 1, 2 and 4 of Cu/Zn-binding superoxide dismutase gene. We emphasized the existence of mild involvement of middle root zone of posterior column, Clarke's nuclei and spinocerebellar tract in FALS with multisystemic degeneration.

Adaptation of the resettled Kenyah Dayak villagers to riverine environment in east Kalimantan: a preliminary report.

The Kenyah Dayak in East Kalimantan (Indonesia), who migrated from their mountainous homeland to a riverine village in the 1940s, have subsisted on slash-and-burn rice cultivation. To cope with rapidly increasing population, the villagers have not changed their farming practice to increase land productivity but instead have exploited fields in remote riverbanks, using motorized canoes.

[Role of neutrophils in lung injury after intestinal ischemia and reperfusion in dogs].

We studied the role of neutrophils in lung injury after intestinal ischemia and reperfusion (I/R) in anesthetized dogs with lung lymph fistulae. One group was subjected to 2.5 hours of balloon occlusion of the superior mesenteric artery without laparotomy, followed by 3 hours of reperfusion (I/R group, n = 7). The other group was subjected to the same procedures, except for intestinal I/R (sham operation group, n = 6). In the sham operation group, lung fluid balance, hemodynamics, extravascular water volume (Qw 1: ml/g BFDW), myeloperoxidase activity in the lung (MPO: unit/g DW), H2O2 production by neutrophils in blood (mean DCF/cell), and migration of neutrophils into the lung lymphatic system did not significantly change. In the I/R group, both lung lymph flow (Jv) and protein clearance (Qp) increased more than 2.5 fold as compared with the baseline values, while capillary pressure (Ppc) and the ratio of lymph to plasma protein concentration (CL/Cp) remained almost the same as the baseline values. Qw1 also moderately increased. MPO activity, H2O2 production, and migration of neutrophils into the lung lymphatic system increased after I/R, and were more remarkable than in the sham operation group. These results suggest that activation and migration of primed neutrophils contributes to lung injury after intestinal ischemia and reperfusion.

Effects of biological factors on cell growth and anthocyanin formation by suspended cultures of Perilla frutescens cells.

Effects of biological factors (i.e., size of cell aggregates, subculture period, and inoculum density) on cell growth and production of anthocyanin pigment (a secondary metabolite) by suspended cultures of Perilla frutescens plant cells were investigated on a flask scale. It was found that anthocyanin content was lower in a culture inoculated with cells of size greater than 250 microns, compared with unscreen-sized cells or cells smaller than 250 microns. Subculture periods of 7 and 10 days were suitable for the cell cultures since favorable growth and a high level of anthocyanin content were maintained during a long period of subculture. An inoculum density (wet cells) of 50 g/L was the best for both the cell growth and pigment accumulation in flask culture of P. frutescens cells.

A case report of mixed P- and C-ANCA positive patients with pauci-immune crescentic glomerulonephritis.

We describe 2 patients with pauci-immune crescentic glomerulonephritis and elevated titers of perinuclear and cytoplasmic anti-neutrophil cytoplasmic antibody (P-ANCA and C-ANCA) which were associated with both 29 and 59 kD bands on Western blots. The presence of C-ANCA corresponds with a clinical and/or histological diagnosis of Wegener's granulomatosis, but although both patients suffered from pulmonary complications there was no clinical nor histological evidence of granulomatosis. Transbronchial lung biopsy showed only interstitial pneumonia and old hemorrhage. Therefore, the patients' conditions were interpreted as microscopic polyarteritis nodosa or hitherto undefined vasculitis associated with ANCA.

Analysis of organic compounds in a case of suicide by ignition with lacquer thinner.

An autopsy case of suicide by ignition using lacquer thinner is presented. A wholly-charred body of a 54-year-old man and a can of lacquer thinner were found at the burnt driver's seat of a truck. Organic solvents in blood, urine, lung tissue, trachea and gastric contents and of the remaining clothes have been analyzed by gas chromatography/mass spectrometry (GC/MS). High levels of toluene, ethyl acetate and butyl acetate were detected in his clothes. The concentrations of toluene in the left and right heart blood, urine, gastric contents, squeeze sample and a block of lung tissue were 0.309, 0.226, 0.018, 0.051, and 0.268 mg/ml, and 0.340 mg/g, respectively. The ethanol levels in these samples were 1.89, 1.71, 1.58, 13.88 and 1.39 mg/ml, and 1.49 mg/g, respectively, and the profile suggested that the source of the ethanol was mainly drinking. The carboxyhemoglobin concentration in the left and right heart blood was 43.3 and 36.1%, respectively. The GC/MS data on organic solvents are consistent with the idea that he used the lacquer thinner contained in the can found in his truck for ignition. The high levels of toluene in his blood suggest that not only burns but also toluene poisoning contributed to his death.

[Effect of theophylline on disturbances of ventilation and of gas exchange during exercise in patients with pulmonary emphysema].

Factors limiting exercise were studied in 23 patients with pulmonary emphysema, and the effects of theophylline on exercise tolerance and on hypercapnic ventilatory response were studied in 16 patients with pulmonary emphysema. Exercise in these patients was limited by inability to increase tidal volume and by disturbances of gas exchange, as indicated by low PaO2, high AaDO2, and high PaCO2 during exercise, and by low diffusing capacity. Theophylline increased tidal volume during exercise, but it did not improve oxygenation during exercise. Theophylline improved exercise tolerance and hypercapnic ventilatory response. The percent increases in VEmax and in VO2max after theophylline administration correlated significantly with the percent increase in the ratio of mouth occlusion pressure to PACO2 during rebreathing. These results indicate that exercise tolerance in patients with pulmonary emphysema can be improved by theophylline, probably because of an increase in ventilatory drive.

Establishment of the novel B acute lymphoblastic leukemia (FAB L3) cell line KHM-10B with a 13q34 abnormality and constitutive expression of c-myc and max during cell cycle.

We established and characterized a new acute lymphoblastic leukemia (ALL-L3 according to FAB classification, or Burkitt's type) cell line, KHM-10B. The morphology of the patient's lymphoblasts and KHM-10B cells corresponded to that of ALL-L3 cells. The cells were positive for HLA-DR, CD19 and surface immunoglobulin (mu, lambda). Southern blot analysis revealed that the fresh lymphoblasts and KHM-10B shared the same immunoglobulin gene rearrangement. Conventional cytogenetic analysis of fresh lymphoblasts from the patient and KHM-10B cells revealed the 13q34 abnormality, the second most common additional abnormality in Burkitt's lymphoma, but no detectable 8q24 involvement. Rearrangement of the c-myc oncogene was not detected by Southern blot analysis. However, a fluorescence in situ hybridization (FISH) assay identified a t(8;22)(q24;q11). The KHM-10B cells were arrested at S phase with hydroxyurea and thymidine, and the synchronized cells progressed through the cell cycle in drug-free medium. The expression of c-myc and max was observed throughout the cell cycle, as was found in the Burkitt's lymphoma cell in Raji. Our findings indicate that FISH analysis is of diagnostic value in detecting obscure chromosomal translocations and that max, as well as c-myc, is expressed constitutively in ALL-L3 and Burkitt's lymphoma cell lines.

Plasma levels of type II phospholipase A2 and cytokines in patients with sepsis.

Plasma levels of type II phospholipase A2 (type II PLA2), cytokines and endotoxin were determined in patients with sepsis to investigate their interrelations and their role in the patient's prognosis. Type II PLA2 was measured by radioimmunoassay, tumor necrosis factor-alpha (TNF-alpha), interleukin 6 (IL-6), and IL-8 were each measured by enzyme-linked immunosorbent assay (ELISA). Endotoxin was determined by a method based on an endotoxin-specific synthetic substrate. Plasma levels of type II PLA2 were significantly higher in the patients who died of sepsis than in those who survived the illness. There was a significant correlation between type II PLA2 and TNF-alpha and IL-6. Type II PLA2, TNF-alpha, IL-6, and IL-8 may be useful as indices of disease severity. The results suggest that TNF-alpha and IL-6 stimulate the production of type II PLA2 in the plasma of patients with sepsis.

Blood levels of endothelin-1 and thrombomodulin in patients with disseminated intravascular coagulation and sepsis.

We evaluated the roles of plasma endothelin-1 and plasma thrombomodulin in the development of disseminated intravascular coagulation (DIC) in patients with sepsis. Plasma endothelin-1 was measured by radioimmunoassay (RIA). Plasma thrombomodulin and tumor necrosis factor-alpha (TNF-alpha) were measured by enzyme-linked immunosorbent assay (ELISA), and serum protein C (protein C) was measured by the synthetic substrate method. Endotoxin was measured by the Endospecy test, a synthetic substrate method. A new perchloric acid method was used for the pretreatment of plasma. Blood levels of endothelin-1 and thrombomodulin were significantly higher in patients with DIC than in those without DIC (p < 0.0001). Endothelin-1 and thrombomodulin levels were positively correlated (r = 0.8645, p = 0.0001), as were endothelin-1 and TNF-alpha levels (r = 0.5441, p = 0.0002). Thrombomodulin and protein C levels were negatively correlated (r = -0.5627, p = 0.0001). Endotoxin was elevated above the normal level 14.3% (6/42) for these patients. TNF-alpha is involved in the production of endothelin-1 and thrombomodulin, which play a role in the pathogenesis of DIC and whose blood levels reflect its severity.

[Reappraisal of amyotrophic lateral sclerosis with dementia].

Amyotrophic lateral sclerosis (ALS) with dementia is characterized by rapidly progressive dementia and motor neuron involvement. The age at onset in 12 sporadic cases ranged from 43 years to 78 years. The initial symptoms are dementia, such as uninhibited behavior and personality change in most patients. Both dementia and motor neuron involvement appear within 1 or 2 years of the onset. The clinical picture of motor neuron disturbance was bulbar-type ALS. In ALS-dementia, lower motor neuron sign is predominant than upper motor neuron sign. The pattern of dementia indicated impaired frontal lobe function, confirmed by frontal sign such as perseveration, forced grasping and utilization behavior. These neurological signs are significant in association with the frontal lesion on CT, MRI and single photon emission computed tomography. The pathological findings show frontotemporal atrophy, mild non-specific neuronal loss of cortical superficial layer, fibrous gliosis of subcortical white matter, degeneration of substantia nigra and motor neuron involvement. The clinicopathological findings resembled those of dementia of frontal type and are distinct from those of Alzheimer's disease. We thus consider that the combination of motor neuron disease and dementia is a new clinicopathological entity, quite distinct from ALS or other dementias, as Yuasa and Mitsuyama proposed earlier.

[The role of interstitial hyaluronan in acute lung injury].

The pulmonary interstitium may affect the movement of water, macromolecules, and inflammatory cells between capillaries and lymphatics. Hyaluronan (hyaluronic acid), a glycosaminoglycan of the interstitial matrix, helps to retain water in the lung interstitium and to exclude proteins. Marked reduction of interstitial hyaluronan by infusion of hyaluronidase decreased the resistance to fluid transport and decreased water retention in the interstitium, and thus accelerated lymphatic removal of fluid filtered across capillary walls. However, reduction of hyaluronan increased the accumulation of neutrophils in the lung and exacerbated acute lung injury caused by pancreatic elastase. Interstitial hyaluronan may protect against acute inflammatory changes in elastase-induced lung injury.
