Methodology of ECG interpretation in the Nagoya program.

The outline of the Nagoya Program, the most representative software package for computerized electrocardiography in Japan, is described. First, the history and current status of research and its transfer to commercial products for computerized interpretation of electrocardiograms is briefly reviewed. Second, development and testing of the Nagoya Program is depicted, referring to a gold standard for evaluation and the database being used for this purpose, and to its participation in the CSE study. Third, a description of signal acquisition and wave identification is given, followed by the detection of rhythm disturbances. Next, algorithms for contour classification, which consist of decision-tree logic established through trial and error and making use of the database of ECGs, are described. Finally, special features and future plans of the Nagoya Program are mentioned.

Development of central retinal vein occlusion in dural carotid-cavernous fistula.

We investigated a 46-year-old woman with central retinal vein occlusion complicating dural carotid-cavernous fistula, resulting in severe loss of visual acuity. Venous stasis retinopathy observed on the first examination progressed severely so that central retinal vein occlusion with retinal neovascularization developed 3 months later. A transvascular embolization discontinuing the feeders from the external carotid artery improved the retinal circulation and the visual acuity. These results indicate that the cause of the progression from venous stasis retinopathy to central retinal vein occlusion is the elevation of pressure in the cavernous sinus.

Quantitative analysis of the oculocardiac reflex by traction on human extraocular muscle.

The oculocardiac reflex was quantitatively studied in 15 patients with strabismus. The reflex was observed in all patients when the medial rectus and inferior oblique muscles were stretched; the medial rectus muscle had a lower threshold than the inferior oblique. Bradycardia was evoked in 7 of the 15 patients when the lateral rectus was tractioned with tensions of 50 g and 600 g. The oculocardiac reflex was a graded phenomenon as a function of tension applied to the extraocular muscles. As tension was increased, bradycardia occurred rapidly and became deep. Systemic administration of atropine prevented completely the bradycardia from occurring. The results suggest that the response of the extraocular muscles to stretch are critically mediated through a polysynaptic path to the heart, resulting in suppression of the heart rate.

[Usefulness of a radiolucent spacer in radiation therapy for cancer of the tongue].

A radiolucent silicone spacer was developed for use in interstitial irradiation for cancer of the tongue. It keeps about eight millimeters of distance between the lower gingiva and the lesion of the tongue, and the average radiation dose (TDF) to the lower gingiva was 45% of the minimal tumor dose in the lingular lesion. The total TDF value of the lower gingiva by both external and interstitial irradiation was between 66 and 114, which is not considered to be responsible for the occurrence of osteoradionecrosis of the mandible.

Clinical evaluation on 5 years' experience of automated phonocardiographic analysis.

A mini-computer system for PCG interpretation has been functioning since 1971. The system can analyze as many as 100 PCG's in 3 hours. Based on our 5 years' experience, computer interpretation of PCG's has proved itself valuable for the rapid mass-screening of cardiovascular disease. It also provides more objective and quantitative information than human auscultation, though it is still at a developmental stage because of the present technical limitations. The solution of these shortcomings should achieve an automated PCG analysis of high accuracy.
