[Strategy for the treatment of esophageal carcinoma].

Esophageal carcinoma is classified as one of the most malignant neoplasms owing to the difficulty of early detection, frequent lymph node metastasis, and high incidence of recurrence even after curative resection of the lesion. According to the increase of early cases of esophageal carcinoma with the progress of the diagnostic technique, endoscopic mucosal resection and blunt resection of the esophagus have come to be performed in cases with quite early carcinoma or poor risk factors. Hyperthermo-chemo-radiotherapy (HCR) is extremely effective for esophageal carcinoma, especially for the advanced cases. The 1-, 3- and 5-year survival rates of the cases treated with preoperative HCR and chemo-radiotherapy (CR) were 64.6, 35.0 and 23.9%, against 46.9, 20.0 and 3.4%, respectively. The prognosis of the cases treated with HCR was significantly better than that for cases treated with CR. Moreover, for far-advanced esophageal carcinoma cases, palliative therapy such as intubation should be performed to assure quality of life (QOL). Strict follow-up including postoperative combined therapies is required for an improved prognosis in esophageal carcinoma.

The association of myasthenia gravis and connective tissue diseases--the role of Sjøgren's syndrome.

The symptoms of myasthenia gravis (MG) reflect the loss of neuromuscular transmission due to the functional loss of the acetylcholine receptor. We reviewed the reported association of MG and connective tissue diseases including rheumatoid arthritis, systemic lupus erythematosus, progressive systemic sclerosis (PSS), polymyositis and dermatomyositis, mixed connective tissue disease (MCTD) and Sjøgren's syndrome (SjS). We found that the association of MG and MCTD or PSS is rare. We also reviewed the role of D-penicillamine, thymus abnormalities and the coexistence of SjS as one of the underlying pathological conditions for the association of MG and various connective tissue diseases.

Efficacy of additive DMARD therapy in patients with rheumatoid arthritis. Double blind controlled trial using bucillamine and placebo with maintenance doses of gold sodium thiomalate.

OBJECTIVE: To examine the efficacy of the addition of small doses of additional disease modifying antirheumatic drugs (DMARD) to ongoing DMARD treatment [additive DMARD therapy (ADT)]. METHODS: A 3-month prospective, double blind, randomized, placebo controlled study was performed using either 100 mg/day of bucillamine (Buc) or an inactive placebo (P1). Two groups of 12 patients each who had experienced an insufficient benefit from gold sodium thiomalate (GSTM) alone were enrolled in the study. RESULTS: The addition of Buc proved more beneficial than P1 regarding improvement in disease activity (p = 0.0032) and drug usefulness (p = 0.0025). A significant within group improvement was observed in joint swelling count, the Lansbury activity index, erythrocyte sedimentation rate and C-reactive protein. However, the difference in the clinical variables between the 2 groups was minimal. CONCLUSION: The usefulness of ADT was suggested by this trial; however, further confirmation by additional studies is still needed.

[A study on bone metabolism in woman with hyperprolactinemia].

We studied bone metabolism in hyperprolactinemia in 21 hyperprolactinemic women, and compared it with 57 normal subjects. Second to fourth lumbar bone mineral density (BMD) was measured by dual energy X-ray absorptiometry (DXA). The mean BMD (g/cm2) in hyperprolactinemic women was 9% less than in normal subjects (0.940 +/- 0.115 (SD) vs. 1.030 +/- 0.106, p < 0.05). A negative correlation was found between BMD and the duration of hyperprolactinemia. The analysis of bone metabolic parameters, bone turnover, bone formation and bone absorption showed that they increase in the hyperprolactinemic state. The main mechanism of BMD loss in hyperprolactinemia is probably due to hypoestrogenemia, but a direct effect of prolactin cannot be excluded.

Hematopoietic supportive function of human bone marrow stromal cell lines established by a recombinant SV40-adenovirus vector.

We have previously reported the establishment of a variety of human bone marrow stromal cell lines using a recombinant SV40-adenovirus vector. Using this vector, we obtained more clonal stromal cells. Here, we have characterized these cells and analyzed their capacity to support the proliferation and differentiation of hematopoietic cells. The stromal cells were cocultured with nonadherent human bone marrow cells used as hematopoietic cells. The total numbers of hematopoietic cells and CFU-GM in culture were counted every week. Two of the six stromal cell lines, AA101 and HAS303, supported the proliferation and differentiation of hematopoietic cells and CFU-GM for more than 9 weeks. Further, granulocytes, macrophages, and megakaryocytes were detected when cocultured with these cells. When hematopoietic cells were cocultured but separated from the two stromal cell lines by a 0.45-microns millipore membrane to prevent their attachment, almost all CFU-GM disappeared within 7 weeks. The supportive stromal cells produced GM-CSF and IL-6. However, other cell lines producing these humoral factors did not support hematopoietic cell proliferation for such a long time. These findings suggest that these established human bone marrow stromal cell lines will be useful, in that analysis of their supportive function in hematopoietic cell proliferation and differentiation through cell-to-cell interaction will shed some light on this area.

Peptide histidine methionine and vasoactive intestinal peptide levels in human cerebrospinal fluid: age-related changes and absence of a correlation with serum prolactin.

Serum levels of prolactin (PRL), peptide histidine methionine (PHM) and vasoactive intestinal peptide (VIP) were measured in 97 subjects and cerebrospinal fluid (CSF) levels of PHM and VIP were measured in 50 subjects by specific radioimmunoassays to investigate correlations between them. The chromatographic studies revealed that PHM and C-terminal extended form of PHM, peptide histidine valine occurred in human serum and CSF. Significant age-related increases of CSF PHM (P < 0.02) were observed in both males and females, whereas an age-related decrease of serum PRL level was found in females (P < 0.01). In contrast, neither serum VIP, serum PHM nor CSF VIP changed significantly with age. There was a significant positive correlation (P < 0.002) between VIP and PHM in serum, but not in CSF. The close relation between VIP and PHM in serum meets one's expectation because of their derivation from a common precursor. In CSF, these two peptides did not change in parallel with each other. These results may reflect the alteration in metabolism of PHM in CSF. Finally, there was no correlation between serum PRL concentrations and either serum or CSF levels of the peptides, suggesting that neither VIP nor PHM levels in CSF as well as in serum can influence the basal PRL secretion in subjects without endocrine disorders.

Complete avulsion of the papilla of Vater and gastroduodenal artery due to blunt abdominal trauma: report of a case.

A case of traumatic avulsion of the papilla of Vater and gastroduodenal artery successfully treated by pancreaticoduodenectomy is presented herein. The mechanism of this rare injury appeared to be a shearing force applied to the common bile duct and gastroduodenal artery. Thus, when the liver is driven cephalad by compression of the abdomen and by the deceleration force, the common bile duct and gastroduodenal artery are avulsed from the fixed duodenum and pancreas. The mechanism of this rare injury is postulated on the basis of operative and histological findings. Our case is thought to be the first of traumatic avulsion of the papilla of Vater and gastroduodenal artery to be reported in Japan.

Plasma concentration of human atrial natriuretic hormone in patients with connective tissue diseases.

Atrial natriuretic peptide (ANP), a peptide released from the cardiac atria, compensates blood volume expansion by its diuretic, natriuretic and vasoactive properties. We measured human plasma ANP(hANP) levels in patients with systemic lupus erythematosus (SLE), rheumatoid arthritis (RA) and progressive systemic sclerosis (PSS) and found that their values were higher than those of healthy controls. In SLE patients, hANP levels correlated with serum creatinine concentration and the patients with proteinuria showed high levels of hANP. Administration of large amount of corticosteroid as a remission induction of the patients with SLE caused high levels of hANP. In patients with PSS, %FEV1 showed strong inverse correlations between hANP levels, and the patients with an enlarged second curvature of the heart had high levels of hANP. In patients with RA, no significant correlation was found between hANP levels and clinical variables including patients' age.

Effect of hormone replacement therapy on spinal bone mineral density and T lymphocyte subsets in premature ovarian failure and Turner's syndrome.

OBJECTIVE: Our purpose was to evaluate the effect of hormone replacement therapy (HRT) on bone metabolism and the immunological change in premature ovarian failure (POF) and Turner's syndrome. METHOD: The study was conducted on 17 POF patients, 10 Turner's syndrome patients and 35 control subjects aged 20-40 years. Bone mineral density (BMD) of lumbar vertebra by dual energy X-ray absorptiometry, serum bone metabolic parameters and T lymphocyte subsets in the peripheral blood were investigated. RESULT: The untreated and treated patients of POF (0.829 +/- 0.077, 0.918 +/- 0.094 g/cm2) and the untreated and treated patients of Turner's syndrome (0.647 +/- 0.037, 0.885 +/- 0.148 g/cm2) showed spinal osteopenia compared with the control group (1.039 +/- 0.107 g/cm2). They showed a significantly lower percentage of CD4 and a ratio of CD4/CD8 in T lymphocytes as compared with the control group before HRT. CONCLUSION: POF and Turner's syndrome patients showed marked spinal osteopenia and abnormal T lymphocytes subsets. HRT slightly improved their BMD and immunological status.

Cineradiographic study of wrist motion after fracture of the distal radius.

Residual dorsal tilt is one of the main deformative factors in the malunion of fractures of the distal radius. We investigated the effect of such tilt on the range of wrist motion and on carpal alignment in a cineradiographic study of wrist motion in 30 patients with extra-articular Colles' fracture. With increased dorsal tilt, the range of wrist motion became more restricted and abnormalities of carpal alignment during wrist motion became apparent. When the dorsal tilt was less than 10 degrees, the effects on the range of motion and on the dynamic carpal alignment were small.

Peptide histidine methionine in cerebrospinal fluid of patients with senile dementia of the Alzheimer type.

Immunoreactivities (IRs) of peptide histidine methionine (PHM) as well as somatostatin and vasoactive intestinal peptide (VIP) in the cerebrospinal fluid (CSF) were measured in patients with senile dementia of the Alzheimer type (SDAT) and age-matched control subjects. We found statistically significant reductions in the PHM-IR and somatostatin-IR levels in the CSF from patients with SDAT, as compared with those of the controls. However, the VIP-IR level in the CSF from SDAT was not different from that of the controls. These results suggest that selective degeneration of neurons containing somatostatin and PHM or the alteration in metabolism of PHM in the CSF might occur in SDAT.

Is an additional vaccination necessary for a successful second pregnancy in unexplained recurrent aborters who were successfully immunized with their husband's lymphocytes before the first pregnancy?

PROBLEM: Is an additional immunotherapy necessary or not for patients who have obtained successful results after initial immunotherapy? METHODS: The successive pregnancy outcome was analyzed in 22 patients out of 29 unexplained recurrent aborters who had undergone immunotherapy with their husband's lymphocytes according to our previously reported protocol (Takakuwa et al., Am J Reprod Immunol Microbiol. 10:1-9, 1986; Takakuwa et al., Am J Reprod Immunol. 23:37-41, 1990) and had obtained successful outcome between January 1983 and December 1989. In addition, the alteration of blocking antibodies (BAbs), which was evaluated by a one-way mixed lymphocyte culture reaction (MLR) blocking assay between the spouses, was analyzed in 26 patients out of 29. RESULTS: None of the 22 patients underwent further immunotherapy because a significantly high titer of MLR-BAbs had been detected before the new pregnancy. In 19 out of the 21 patients (90.5%), pregnancy was successful. CONCLUSION: Additional immunotherapy is not necessary for patients who have obtained successful results after the initial immunotherapy and are positive for MLR-BAbs after their first delivery.

Role of active oxygen, lipid peroxidation, and antioxidants in the pathogenesis of gastric mucosal injury induced by indomethacin in rats.

The roles of active oxygen, lipid peroxidation, and the antioxidative defence mechanism in gastric mucosal injury induced by treatment with indomethacin in rats were investigated. The total area of gastric erosions and concentration of lipid peroxides in the gastric mucosa increased with time after administration of indomethacin (20 mg/kg, orally). The alpha-tocopherol:total cholesterol ratio in serum was significantly decreased and the activity of glutathione peroxidase, an important enzyme to scavenger of lipid peroxides, was inhibited by the administration of indomethacin. Treatments with superoxide dismutase and catalase inhibited the increases in gastric mucosal erosions and lipid peroxides in the gastric mucosa, and the reduction of serum alpha-tocopherol. Treatment with these scavengers did not improve the decreased glutathione peroxidase activity. These findings suggest that active oxygen species and lipid peroxidation play an important part in the pathogenesis of gastric mucosal injury induced by indomethacin, and that the decreased glutathione peroxidase activity aggravated the injury due to accelerated accumulation of hydrogen peroxide and lipid peroxides in the gastric mucosal cell.

A retrospective study of ventral fenestration for disk diseases in dogs.

Sixty-six dogs suffering from intervertebral disk diseases on cervical and/or thoracolumbar disks were treated by a ventral fenestration. The breeds affected with disk diseases were Dachshund (65.1%), Beagle (15.2%), Shih Tsu (4.5%), Pekingese (4.5%), and others (18.2%). Sites suffered from disk diseases and received disk fenestration most frequently were from T10-11 to L2-3. However, other disks were also needed the operation in some cases. An average number of disks received fenestration was 6.0. Days to walk after the fenestration in 51 cases varied 1 to 120 days (average: 14.9 +/- 22.0 days, mean +/- S.D.) depending on their severity. The recovery rates from the follow-up study were as follows: In grade I (3 cases, able to walk with back pain) and grade II (14 cases, slight ataxia and paresis) it was 100%, respectively. The rates in grade III (23 cases, paraplegia with positive deep pain) and grade IV (26 cases, paraplegia with lack of deep pain) were 73.9% and 65.4%, respectively. A few recurrence after the operation were observed in this study. As a result, the ventral fenestration was extremely effective as a treatment for disk diseases in the dog.
