Morphometric studies of renal lesions in Itai-itai disease: chronic cadmium nephropathy.

Renal cortex in 15 cases with Itai-itai disease was morphometrically examined, i.e. the cross-size of tubuli in the renal cortex was measured using a color image analyzer (Olympus CIA-102) and the ratio of preserved urinary tubuli occupying renal cortex was calculated by means of the point-counting method. The former and the latter decreased gradually together as atrophy of kidney advanced. However, their decrease was stopped at about 60 g of kidney weight and it was inferred to be the limit of histopathologic changes of the kidneys with Itai-itai disease. Comparing the cross-size of the tubuli of the outer cortex area [= S(o)] with one of the inner cortex area [= S(i)], changes were more marked in S(o). This revealed that the outer cortex area was more sensitive to Cd intoxication and it was more affected by systemic atherosclerosis than the inner cortex area. It was concluded that when atrophy of kidneys as seen with Itai-itai disease was extremely progressing, decreases in the number and volume of tubuli in the cortex usually occurred at the same time and in a regular manner.

Assembly of cardiac C-protein during myofibrillogenesis in myogenic cells in culture.

To examine the function of C-protein, a thick filament-associated protein of vertebrate striated muscles, during myofibrillogenesis, the cDNA encoding chicken cardiac C-protein and the truncated cDNA were subcloned into a expression vector and introduced into mouse C2 myogenic cells. The expression and assembly of the C-protein was investigated by immunofluorescence methods. When the cDNA containing the entire open reading frame was introduced, in C2 myoblasts, the transiently expressed exogenous cardiac C-protein existed only diffusely in the cytoplasm, but it became localized in striated structures together with sarcomeric myosin heavy chains (MHC) in myotubes. To clarify the functional domains of C-protein, the cDNA constructs that lack the regions encoding the C-terminal immunoglobulin (Ig) C2 motif or the N-terminal Ig C2 motif were introduced into C2 cells to produce mutant proteins. The truncated chicken cardiac C-protein, which lacked the C-terminal Ig C2 motif, apparently lost the ability to bind to myosin filaments; the protein was not assembled into myofibrils but diffused in the cytoplasm even in the myotubes. The protein without N-terminal Ig C2 motif, however, was assembled into sarcomeric structures just as complete protein molecules. From these results, we conclude that 1) the assembly of sarcomeric MHC into myofibrils in myotubes is accompanied with that of cardiac C-protein, and 2) the C-terminal Ig C2 motif is necessary for assembly of cardiac C-protein in sarcomeric structures in the cytoplasm.

Changes of conductivity in patients with second- or third-degree atrioventricular block after pacemaker implantation.

UNLABELLED: The purpose of this study was to examine the long-term course of conductivity in patients with second- or third-degree atrioventricular (AV) block after pacemaker implantation. Fifty-four patients (30 males and 24 females, mean age 59.5 years) had a 12-lead electrocardiogram (ECG) recorded prior to pacemaker implantation and again after a follow-up of more than 5 years (mean 108 months). The degree of AV block and the morphology of the QRS complex in the two ECGs were compared. Twenty-eight (85%) of the 33 patients with complete AV block and 15 (71%) of the 21 patients with second-degree AV block before implantation showed complete AV block after the follow-up period. The progression of AV block was recognized only in patients with intra-His (BH) or infra-His (H-V) block. In 29 patients, escape QRS complexes were recorded in both ECGs. A change in the QRS complex was noted in 6 patients (21%), including 2 with new bundle branch block (BBB), 2 with wider BBB, 1 with new left axis deviation, and 1 with a change to another BBB. This change was particularly notable in the H-V block group (44%). IN CONCLUSION: (1) Most patients with complete AV block did not recover AV conductivity after the long-term follow-up. (2) Second-degree BH and H-V block tended to progress to complete AV block.

Effects of 6-Hydroxydopamine on the antibody response to the hapten dinitrophenyl in the chicken.

Antibody responses to the hapten dinitrophenyl (DNP) and the concentration of catecholamine in chickens received a single injection of 6-Hydroxydopamine (6-OHDA) into the chorioallantoic cavity at the embryonal stage were evaluated. A significantly enhanced anti-DNP IgA response and a markedly decreased level of noradrenaline in the peripheral blood were observed in chickens treated with 400 micrograms of 6-OHDA at 14 days of incubation. These results suggested the immunomodulatory influence of the sympathetic nervous system.

Effect of directly ultraviolet-irradiated allografts of fetal pancreas in experimental diabetic dogs.

The proliferative responses of UV-irradiated islets from fetal pancreas decreased to 53.8 +/- 4.7% (mean +/- SEM) compared to that of UV-irradiated islets by allogeneic mixed islet-cell lymphocyte culture. In 5 pancreatectomized dogs, UV irradiated fetal dog pancreas was transplanted either into omentum pouches or the spleen without immunosuppressive agents. The diabetic status (daily insulin requirement for hyperglycemia, decrease in body weight, urine glucose) improved in dogs after allo-transplantation. The survival time after total pancreatectomy was significantly longer in allografted dogs than those treated only with daily insulin injections.

Enhancing effects of chicken egg white derivatives on the phagocytic response in the dog.

Immunostimulative effects of chicken egg white derivatives (EWD) on phagocytic responses of peripheral blood mononuclear cells (MNC) and polymorphonuclear cells (PMN) in dogs were evaluated by flow cytometric analysis. Peripheral blood leukocytes (PBL) cultured with EWD showed the enhanced phagocytic response. The response was maximal when PBL were cultured with 100 - 400 micrograms/ml of EWD for 3 - 12 hr. Furthermore, significantly increased phagocytic responses were also induced even when PBL were cultured with protein components (200 micrograms/ml) of EWD such as conalbumin, flavoprotein and ficin-papain inhibitor for 3 hr. In addition, the enhancing effect of EWD on the phagocytic responses was also observed in MNC cultured with EWD (200 micrograms/ml) for 4 hr but not in PMN cultured with EWD in the same procedures. The supplement of the supernatant (20%) of MNC cultured with EWD (200 micrograms/ml) for 24 hr at 37 degrees C to PBL and MNC resulted in the enhancement of their phagocytic responses. In contrast, the supernatant of PMN cultured with EWD for 24 hr at 37 degrees C did not show any significant enhancing effect on the phagocytic responses of PBL, MNC and PMN. These results suggest that EWD has an enhancing effect on phagocytosis of MNC and PMN, which may be mediated through active humoral substances produced by EWD-stimulated MNC.

Apoptotic cells in the human endometrium and placental villi: pitfalls in applying the TUNEL method.

Apoptotic cells were histochemically demonstrated by the TdT-mediated biotinylated dUTP nick end-labeling (TUNEL) method in formalin-fixed and paraffin-embedded sections of the human endometrium and placental villi. In 53 endometrial biopsy specimens, labeled nuclei were identified in 16 samples showing a desquamating change, associated with menstruation, functional bleeding or adenocarcinoma. Cells in the normal proliferative and secretory phases were unlabeled. The labeled nuclei in the gland and stroma corresponded well to the so-called apoptotic bodies. Placental tissues at various stages of gestation were obtained by spontaneous abortion, intrauterine fetal death or normal delivery. Syncytiotrophoblastic cells in an early gestational stage (7-12 weeks) and in the term placenta were focally labeled, and the labeled cells possessed pyknotic nuclei and densely eosinophilic cytoplasm. In the early gestational chorionic villi with marked hydropic degeneration or in hydatidiform mole, the stromal cells were frequently labeled. Villous cells in coagulation necrosis (infarction) also revealed strong signals. The apoptotic bodies were not recognizable histologically in these labeled villi. The placenta at the 20th to 33rd week of gestation lacked labeling. From a technical point of view, it should be noted that cells in the foci showing ischemia or coagulation necrosis were labeled positively.

Extensive spreading carcinoma of the esophagus with invasion restricted to the submucosa.

OBJECTIVES: The aim of this study was to examine the clinicopathological features of extensive spreading type of squamous cell carcinoma (SCC) of the esophagus with invasion restricted to the submucosa. METHODS: Thirty-eight submucosal SCC were studied histopathologically, and five of these 38 submucosal SCC, which demonstrated an extensive spreading-type SCC in which the size of the SCC was over 5 cm in longitudinal length while extending entirely across the esophageal lumen in a circumferential spread, are described in detail. RESULTS: The extent of SCC depended mainly on the extent of intraepithelial carcinoma (IEC) and ranged from 50 to 135 mm in longitudinal length. IEC existed continuously in four cases, and multiple IEC existed separately in one case. An epithelium showing dysplasia was present in all cases, and the involvement of the glands and ducts by IEC was present in two cases. There was only one invasive part of SCC observed in one case, and three to 10 invasive parts of SCC were observed in the other four cases; various features of invasion, including an invasive SCC from the esophageal glands and ducts, were also present. CONCLUSIONS: The extensive spreading-type SCC of the esophagus, which was considered to result from the widespread or multiple carcinomatous changes of the esophageal epithelium, was found to not be as rare as generally thought. The multiple and various features of the invasive parts of SCC suggested the simultaneous development of a downward invasion of SCC at different areas in extensive IEC.

[Correlation of intratumor DNA ploidy distribution pattern and Ki-67 index in large bowel carcinomas].

Seventy cases of surgically resected specimens of large-bowel carcinomas were used. DNA ploidy was determined by flow cytometry for step-wise sections in each carcinoma. Intratumor DNA ploidy distribution pattern was classified into 5 types: Type A showing diploidy in all sections; Type B showing aneuploidy in all sections with the same DNA index (DI); Type C showing diploidy in the majority of the sections and aneuploidy in a part; Type D showing aneuploidy in the majority of the sections and diploidy in a part; and Type E showing aneuploidy in all sections with different DI in some parts. Ki-67 in cell nuclei was stained immunohistochemically for the 4 microns-thick paraffin section using MIB1 monoclonal antibody, and the Ki-67 index expressing the growth fraction was calculated. Ki-67-index (Mean +/- SD) of 4 cases in Type A showed 20.6 +/- 4.88, that of 16 cases in Type B showed 37.5 +/- 9.62, that of 2 cases in Type C showed 42.4 +/- 0.21; that of 26 cases in Type D showed 41.8 +/- 10.6 and that of 22 cases in Type E showed 41.7 +/- 8.58. Thus, the aneuploidy group (type B, C, D, E) revealed a significantly higher Ki-67 index than the diploidy group (Type A) (p < 0.01). In the predominantly diploidy group (Type A, C), the cases in Type C also revealed a significantly higher value than the cases in Type A (p < 0.01). Therefore, there seemed to be some difference in the Ki-67 index or proliferative activity of cancer cells in large-bowel carcinomas relating to the intratumor DNA ploidy distribution pattern.

A giant thigh mass in a patient with total knee arthroplasty for Charcot joint.

A painless and progressively growing mass consisting of fibronecrotic quadriceps femoris muscles with a giant multilobular cystic cavity formed 15 years after total knee arthroplasty for Charcot joint. It was considered to be a cyst from the knee joint accompanied by compartmental muscular necrosis. The painless and progressively growing nature of the mass seemed to be largely attributable to a neuropathic joint. Computed tomography proved to be helpful for diagnosis. Conservative management was unsuccessful in relieving the symptoms.

The effects of Tripterygium wilfordii extract on adjuvant arthritis in rats.

The effects of the extract of Tripterygium wilfordii. (TWE) on experimental adjuvant arthritis (AA) in the rat was studied. Lewis rats induced with AA were administered with TWE at 50 mg/kg/day for 10 weeks. The paw volume, its ratio to the body weight (paw ratio) and the radiologic changes in the feet of the rats with AA taking TWE were compared with those in the rats taking indomethacin (0.5 mg/kg/day) and no additional drugs. The rats taking TWE showed a smaller paw volume, a lower paw ratio and milder radiologic changes than the rats taking no drugs, however, the beneficial effects were weaker than those of indomethacin. We concluded that the beneficial effects of TWE on rats with AA was suggested. However, these results still need to be further confirmed by additional experiments.

[The significance of bone mineral density measurement of premenopausal women].

We examined the starting age for measuring bone mineral density (BMD) in premenopausal women to detect osteopenia as soon as possible. BMD of the second to fourth lumbar spine vertebrae was measured by dual-energy X-ray absorptiometry to determine normal values for 500 healthy premenopausal women (aged from 17 to 55). We found the rate for low BMD premenopausal women, whose BMD was under the peak bone mass-2SD. And we compared normal BMD women with low BMD women with reference to menstrual history, obstetrical history, and life style, by means of a questionnaire. The peak bone mass of premenopausal women was reached between 35 and 39 years of age (1.059 +/- 0.097g/cm2). The low BMD rate was 7% in all premenopausal women, and increased to 13.5% at from 45 to 49 years. On the other hand, it was only from 5.7 to 7.4% even at from 20 to 29 years. There was the positive correlation between BMD and the body mass index (BMI). There was no statistically significant difference between the low BMD women and the normal BMD women in menstrual history, obstetrical history or life style (parity was 1.7 +/- 1.6 and 1.1 +/- 1.2, respectively, p < 0.05). We concluded that premenopausal women should have their BMD measured after aged 45, and be measured once from 20 to 40 years of age if possible.

[Study on the relationship between hormonal cytology and bone mineral density in postmenopausal women].

We assessed the relationship between bone mineral density (BMD) and hormonal cytology in 181 postmenopausal women. BMD was measured by dual energy X-ray absorptiometry (DXA). Age, height, weight, body mass index (BMI), age at menopause, hormonal cytology (eosinophilic index (EI), karyopyknotic index (KPI) and maturation value (MV)) and estradiol were investigated. A positive correlation was found between BMD and hormonal cytology (EI r = 0.17 p < 0.05, KPI r = 0.19 p < 0.05 and MV r = 0.37 p < 0.005). The low BMD group (BMD < 0.8g/cm2 n = 87) had significantly lower hormonal cytology than the normal BMD group (BMD > or = 0.8g/cm2 n = 94). The receiver operating characteristic curve analysis showed MV at 30 was available for the cutoff point in predicting osteopenia. MV < 30 detected osteopenia (BMD < 0.8g/cm2) as 0.76 in sensitivity and 0.65 in specificity. These findings suggest that hormonal cytology is useful for screening osteopenia in post-menopausal women.

[Effect of dexamethasone on intercellular adhesion molecule-1 expression on cultured bronchial epithelial cells stimulated by inflammatory cytokines].

Bronchial asthma is characterized as a chronic inflammation of the airway that causes an infiltration of lymphocytes and eosinophils. Cell to cell interaction or cell to tissue interaction is essential for infiltration of eosinophils to underlying tissues. These phenomena are closely related to the expression of intercellular adhesion molecule-1 (ICAM-1). Inhalation of steroids, such as beclomethasone dipropionate, is commonly used to cure airway inflammation. In this study, we investigated the effect of cytokines on ICAM-1 expression on human bronchial epithelial cell lines, NCI-H292. Moreover, the effect of dexamethasone on ICAM-1 expression stimulated by IL-1 beta, TNF-alpha and IFN-gamma was observed. Treatment with IL-1 beta, TNF-alpha and IFN-gamma dose-dependently increased ICAM-1 expression on NCI-H292 cells. Inhibitory effects were exerted by dexamethasone on ICAM-1 expression in cells stimulated by IL-1 beta and IFN-gamma in a dose-dependent manner, but not in cells stimulated by TNF-alpha. These results suggest that the inhibition of ICAM-1 expression could be related to the pharmacological action of steroid drugs.

The multicentric occurrence of squamous epithelial dysplasia and squamous cell carcinoma in the esophagus.

BACKGROUND: The biologic significance of esophageal dysplasia has not yet been completely elucidated, especially regarding the process of multiple occurrences of squamous cell carcinoma. METHODS: The multiplicity of dysplasia in 73 patients with solitary carcinoma of the esophagus (Group I) and 21 with multiple carcinomas (Group II) was compared in surgically resected specimens. RESULTS: Thirty-nine second carcinomas were identified in 13 patients of Group II, and all were superficial. The incidences of five or more isolated dysplasias in cases without continuity to a carcinomatous lesion, was 6.8% and 66.7% in Groups I and II, respectively (P < 0.01). In a case with three or more carcinomas, the incidence increased to 84.6%. The coexistence of all grades of isolated dysplasias was observed in 10 patients (47.6%) in Group II but in only 6 patients (8.2%) in Group I (P < 0.01). CONCLUSIONS: These findings suggest that various degrees of evolving biologically related lesions, such as dysplasia and carcinoma, can occur multicentrically in the same esophagus.

Univariate and multivariate analyses of the prognostic significance of discontinuous intramural metastasis in patients with esophageal cancer.

A review of 167 cases of esophageal carcinoma without preoperative treatment revealed 24 (14.4%) to have intramural metastasis (IM) within the esophagus. Among the clinicopathologic factors, the length of the lesions (P < 0.01), lymph node metastasis (P < 0.001), and the depth of the invasion of the tumor (P < 0.0001) were found to be statistically significant different factors between the two groups of patients both with and without intramural metastasis. The survival curve for patients with IM was significantly lower than that for patients without IM (P < 0.0001). A univariate analysis revealed that the depth of invasion, lymph node metastasis, IM (P < 0.0001), and the length of the lesion (P < 0.001) all had a significant correlation with the prognosis. Moreover, in a multivariate analysis, the depth of the invasion (< 0.001), length of the lesion (0.001), and IM (0.049) were all determined to be significant prognostic factors. Therefore, IM is considered to be one of the independent significant prognostic factors for predicting a poor prognosis in esophageal cancer.
