Comparison of the haemodynamic effects of the selective bradycardic agent UL-FS 49, with those of propranolol during treadmill exercise in dogs.

1. To clarify whether the bradycardic agent UL-FS 49 exhibits a positive inotropic effect even in the absence of improvement in regional myocardial function of an underperfused myocardial area, this study was undertaken in dogs with unimpaired coronary flow. 2. We also investigated the haemodynamic and functional effects of the negative chronotropic and inotropic beta-adrenoceptor blocker propranolol. 3. UL-FS 49 did not depress total or regional myocardial performance. Moreover, an increase in positive left ventricular dp/dt max at rest suggests a positive inotropic effect of UL-FS 49. 4. Propranolol, in contrast to UL-FS 49, led to a marked reduction in positive dp/dt max, stroke volume and systolic wall thickening at rest and during exercise. Additionally, propranolol decreased the exercise values of cardiac output, left ventricular work and left ventricular power to a far greater extent than UL-FS 49. 5. In contrast to propranolol, the selective bradycardic agent UL-FS 49 did not decrease total or regional ventricular performance and caused less reduction in cardiodynamic parameters during exercise. 6. These results suggest that patients with moderate coronary insufficiency or patients with coronary vessel disease and mild left ventricular failure may attain a higher exercise limit under selective bradycardia with UL-FS 49 in comparison to that possible with a beta-adrenoceptor antagonist, such as propranolol.

Chicken epidermal growth factor (EGF) receptor: cDNA cloning, expression in mouse cells, and differential binding of EGF and transforming growth factor alpha.

The primary structure of the chicken epidermal growth factor (EGF) receptor was deduced from the sequence of a cDNA clone containing the complete coding sequence and shown to be highly homologous to the human EGF receptor. NIH-3T3 cells devoid of endogenous EGF receptor were transfected with the appropriate cDNA constructs and shown to express either chicken or human EGF receptors. Like the human EGF receptor, the chicken EGF receptor is a glycoprotein with an apparent molecular weight of 170,000. Murine EGF bound to the chicken receptor with approximately 100-fold lower affinity than to the human receptor molecule. Surprisingly, human transforming growth factor alpha (TGF-alpha) bound equally well or even better to the chicken EGF receptor than to the human EGF receptor. Moreover, TGF-alpha stimulated DNA synthesis 100-fold better than did EGF in NIH 3T3 cells that expressed the chicken EGF receptor. The differential binding and potency of mammalian EGF and TGF-alpha by the avian EGF receptor contrasts with the similar affinities of the mammalian receptor for the two growth factors.

Erythromycin therapy for subclinical intrauterine infections in threatened preterm delivery--a preliminary report.

Evidence suggests that subclinical intrauterine infections which can be indicated by elevated maternal CRP-values may cause preterm labor and that Ureaplasma urealyticum might play a role in the pathogenesis of preterm delivery. Since these organisms are sensitive to erythromycin, 43 patients with threatened preterm labor were at the time of tocolysis treated with erythromycin, or placebo in a randomized study. Treatment with erythromycin resulted in a greater mean delay of delivery for 9 days than among the placebo-treated women. This effect is statistically significant in patients with intact membranes and cervical dilatation: in contrast to the placebo group these women achieved an increased delay of 23 days before delivering. Moreover, successful erythromycin treatment was significantly associated with the isolation of ureaplasmas from vaginal swabs. Our results emphasize that in cases with cervical dilatation, vaginal microorganisms, especially ureaplasmas, are more liely to ascend and might be involved in the initiation of preterm labor. Therefore, additional treatment with erythromycin was beneficial only in these women.

[Conservative treatment of chronic venous insufficiency of the legs (I)].

Surgical treatment is applied to something between five and ten per cent of all patients with chronic venous diseases. Conservative treatment has proved to be justified for decompensated forms with venolymphatic insufficiency. Compression dressing is the mainstay of all measures of conservative treatment. Sufficiently high and high-stability working pressure is of decisive importance to its effectiveness. This requirement can be met best by Unna's paste dressing. The compression stocking is suitable for after-care and to a lesser extent for prophylaxis.

On the role of the transmembrane anchor sequence of influenza hemagglutinin in target cell recognition by class I MHC-restricted, hemagglutinin-specific cytolytic T lymphocytes.

We have examined the requirement for the transmembrane hydrophobic anchor sequence of the influenza hemagglutinin (HA) in the formation of the antigenic moiety on the surface of target cells recognized by class I MHC-restricted murine CTL. For this analysis we have used a line of CV-1 monkey epithelial cells that express the transfected murine H-2Kd gene product as target cells and have used recombinant SV40-based late replacement vectors to achieve expression of genes encoding wild-type and mutant forms of HA. We have found that the majority of Kd-restricted HA-specific CTL clones recognize target cells that express a secreted HA molecule that lacks the transmembrane and cytoplasmic domains of the parent glycoprotein. Several Kd-restricted CTL clones that recognize subtype-specific and crossreactive epitopes on HA fail to recognize the anchor-negative, secreted HA or chimeric HA molecules containing the transmembrane and cytoplasmic domains of unrelated glycoproteins. These CTL clones appear to be directed to antigenic epitopes located within the transmembrane domain of HA, as defined by their capacity to recognize target cells sensitized with a synthetic 23-amino-acid peptide corresponding to sequences within this domain. The implications of these results for class I MHC-restricted CTL recognition are discussed.

Pediatric infiltrating lipomas: case report and review of the literature.

A case of pediatric infiltrating lipoma and a collected literature review of four pediatric patients with well-documented infiltrating lipomas are presented. These lesions tend to recur after resection but there are no reported incidences of distant metastases or degeneration into frankly malignant lesions. Treatment consists of excising the lesion with a margin of normal muscle.

[Interpretation of the stress pressure profile in the diagnosis of female stress incontinence].

In 94 stress pressure profiles the electronically measured occlusion pressure under stress with a bladder filling of 100 ml in dorsosacral position and 300 ml in sitting position was compared with the computed value in the proximal and medial thirds of the urethra. Electronic measurement furnishes low, statistically confirmed occlusion pressure values. With a bladder filling of 300 ml in sitting position, urodynamic assessment of incontinence on the basis of the electronic occlusion pressure furnishes corresponding findings in over 70% of patients, while the extent of agreement with the computed values seems unsatisfactory. In stress pressure profiles, in accordance with clinical considerations and radiological studies, the proximal urethra should also be assessed as the first portion of the continence-preserving zones. The occlusion pressure, electronically measured under routine conditions, is the parameter which furnishes the most information for the urodynamic diagnosis of stress incontinence.

Suspected intracardiac masses: evaluation with MR imaging.

Electrocardiographically gated magnetic resonance (MR) imaging was used to examine 34 patients believed or known to have intracardiac masses on the basis of results from two-dimensional (2D) echocardiography. Cardiac masses were confirmed in 15 patients on the basis of MR imaging results. In seven patients, MR imaging confirmed the absence of an intracardiac mass but demonstrated an anatomic variant or other abnormality that had been interpreted as a possible mass on the echocardiogram. In 12 patients, MR demonstrated neither an intracardiac mass nor an anatomic variant that was likely to have been misinterpreted as a mass on the echocardiogram. Clinical follow-up in these patients at 10 months to 2 years and repeat 2D echocardiography have not indicated a definite mass. In six patients tissue characterization of the mass with MR imaging added some specificity to the MR diagnosis. Thus, MR imaging can be used to verify intracardiac masses found on 2D echocardiograms and to exclude a mass as the cause of equivocal findings on 2D echocardiography.

[Epidemiologic studies of the microbial colonization of severely burned patients].

Bacteria isolated between 1/1/1983 and 12/31/1984 from the burns of 122 patients in a burns unit at the Ruhr-Universität Bochum were studied. Grampositive bacteria were predominant in colonizing the burn wounds (62.5% of all strains isolated). In the patients with more than 40% of total body surface area (TBSA) burn, isolation of Staph aureus was most frequent. The exogenous colonization rate with Staph. aureus was 86%. Coagulase negative Staphylococci were identified in 89.6% of all patients (71.4% of the patients with more than 40% TBSA burn). There was a constant decline in detecting Pseudomonas aeruginosa from the second half of 1983. Microbial sensitivity testing was performed in 834 cases. Gramnegative strains of bacteria resistant to Ampicillin, Mezlocillin, Piperacillin and Ticarcillin were found in 10 up to 97% of the tested strains. Acinetobacter calcoaceticus var. antitratus and Enterobacter cloacae usually displayed a wide resistant pattern. Some strains were resistant as to 16 antibiotics. The incidence of multiresistant Staph. aureus was studied. The time course of multiresistance was paralleled by the incidence of a 6-fold resistance to Benzylpenicillin, Oxacillin, Tetracyclin(T), Gentamycin(G), Erythromycin(E), and Sulfadiacin(S). The probability of simultaneous resistance to 2 of the 4 antibiotics (T, G, E, S) ranged between 0.7 and 0.98. 98 out of 336 Staph. aureus isolates showed a simultaneous resistance to T, G, E and S (29%).

Red cell and serum protein polymorphisms in three population groups of South Korea.

Genetic markers ACP1, PGM1 with subtypes, ESD, GLO1, PGD, GPT, PGP, C3, TF and GC with subtypes, BF, HP, AMY, PLG and PI, were studied in three populations in South Korea, one being the population of the industrial capital Seoul, the second a rural group from Taejon and the third the population of Cheju Island. For the polymorphic systems studied in the present work, a general similarity was observed among the three populations, with the exception of GPT and ACP1 (Taejon vs. Seoul) and subtypes of GC (Taejon vs. Cheiu).

[Comparison of 4 methods of bacterial count determination in burn wounds].

The present study includes twelve patients with second and third degree fresh burns involving 10-70 per cent total body surface area. Standardized surface swab, gaze pad contact, Rodac plates, and burn wound biopsy cultures were obtained twice per week. The various techniques were compared by 48 independent measurements. Confluent growth occur in more than 70 per cent of the Rodac agar plates. There was a significant (alpha less than 1%) correlation between the results of the surface swab and the gaze pad method. A comparison of the number of viable bacteria detected with the burn wound biopsy technique and the surface methods (gaze pad and surface swab) showed no significant correlation. In addition, the number of colony forming units in 8 parallel cultures obtained from different sites of the burn wound was determined. The standard error of mean (SEM) varied between 1.01 log (biopsy), 0.91 log (gaze pad) and 0.61 log (surface swab). Several case reports emphasize that the quantitative biopsy culture seems to give most reliable data in predicting burn wound sepsis.
