Systemic and local effects of heat dissipation in the thermally powered LVAS.

A thermally powered left ventricular assist system (LVAS) requires 20 W of heat dissipation either to the blood or to the surrounding tissues, such as the lung subcutaneous tissues. Postoperative systemic and local effects of heat dissipation were studied in 5 thermally powered LVASs and 2 heated blood pumps implanted in calves, and compared with 10 pneumatically powered left ventricular assist devices (LVADs). Postoperative mean temperatures of the heat dissipation group were as follows: 1. rectal (RT), 39.1 x 0.4 degrees C (NS versus control); 2. Lung interface, 41.6 +/- 0.6 degrees C (P less than 0.01 versus RT); 3. tissue interface, 40.2 +/- 1.2 degrees C (P less than 0.01 versus RT); and 4. blood, 39.2 +/- 0.8 degrees C (NS versus RT). There was no significant postoperative change in heart rate, respiratory rate, or rectal temperature between the two groups. No difference was shown between the two groups in hematologic parameters, plasma free hemoglobin, liver and renal function, fibrinogen levels, or coagulation processes at the same postoperative time. Heat dissipation to the pump-blood interface promoted a thinner neointima when compared with the nonheated surface. Angioneogenesis was observed in the tissue capsule adjacent to the heat dissipating surface. These results indicated that the thermal LVAS induced no deleterious local or systemic effects on recipient animals.

In vivo evaluation of a permanently implantable thermal ventricular assist system.

The System 7 pump/actuator/engine combination has demonstrated biologic compatibility and physiologic effectiveness and responsiveness in five TVAS in vivo studies. The recent 75-day implant supports the feasibility of extended reliability of the system components. Postoperative system maintenance was necessary only because of the nonhermetic design of System 7. However, the ability to pump 11 l/min at 120 beats/min, synchronize at beat rates of 144 beats/min, and pump against mean arterial pressures of more than 150 mmHg while maintaining hematologic and biochemical values within physiologic range sets a very optimistic stage for System 8 hardware. A System 8 concept is shown in Figure 14. The System 8 hardware will include a thermal salt package capable of providing completely untethered circulatory support for a nominal 8-h period. Recharge will be accomplished with a transcutaneous transformer in 1 h. Thermal management problems become minimal, with an average energy input of 15.8 W (peak of 24.9 W). The human preclinical testing of System 8 will start in 1988.

Bleomycin (NSC-125066) and CCNU (NSC-79037) in the combination chemotherapy of mopp-resistant hodgkin's disease.

Twenty-two patients with MOPP-resistant stage IVB Hodgkin's disease were treated with a combination of bleomycin and CCNU. The response rate in 18 patients surviving at least 1 month was 72% with 11 partial and two complete responses. The mean duration of response and survival in partial responders were 12.2 and 17.5 months respectively. The two complete responses resulted in survivals of 20 and 36 + months. Bleomycin toxicity contributed to two deaths, one pulmonary and and one hypotensive. Severe CCNU toxicity occurred after three of 82 administrations but there were no CCNU-related deaths. The majority of patients in the study tolerated the regimen without serious toxicity. Although highly effective in the temporary control of advanced resistant Hodgkin's disease, the program will hopefully be improved by the addition of longer-acting agents.

Food and nutrition in Canadian "prime time" television commercials.

Television is, arguably, the most influential mass medium and "prime time" viewing attracts the largest audiences. To assess the type, number and nutritional content of foods advertised on TV, commercial breaks during "prime time" (7:00 to 11:00 p.m.) on five Canadian channels (CBC-English, CBC-French, CTV, CFPL, Much Music) were recorded and analyzed. A similar analysis of Saturday morning children's TV commercials was also performed. Commercials for foods and food products constituted between 24-35% of all commercials, the largest advertising output for any group of products. The combination of food presented in commercials reflected average current consumption patterns. Of special concern was the emphasis on low nutrition beverages, especially beer, as well as snacks and candy on Much Music. While further government intervention to restrict advertising practices may be an impractical option, there is scope for increasing the alternative promotion of healthy dietary choices.

European Nosocomial Infection Survey: analysis of Irish data. Irish Intensive Care Nosocomial Pneumonia Survey Group.

The European Nosocomial Infection Survey enrolled 9127 patients in 14 countries providing epidemiological data on pneumonia in 325 Intensive Care Units (ICU). Ten ICUs took part in Ireland recruiting 205 patients. Using strict diagnostic criteria, the pneumonia rate for the Irish group was 11.7%; European-wide it was of 11.8%. The ICU mortality Ireland was 14.2%, in the overall survey it was 14.3%. The pneumonia mortality rate in the overall survey was 30%.

Disulfiram-cotrimoxazole reaction.

The disulfiram-ethanol reaction is a well-known clinical phenomenon occurring as a result of acetaldehyde accumulation in the blood. Symptoms usually begin within 5-15 minutes after ingestion of ethanol in patients who have taken disulfiram 3-123 hours earlier, and generally occur in the following order: flushing, sweating, palpitations, dyspnea, hyperventilation, increased pulse rate, fall in blood pressure, nausea, vomiting, and drowsiness. Patients need not experience all these symptoms, and recovery is generally complete. Trimethoprim-sulfamethoxazole (cotrimoxazole) is a commonly prescribed antimicrobial agent that may produce a reaction similar to that of disulfiram when taken by patients who drink ethanol. This drug-chemical interaction may result in accumulation of acetaldehyde in the blood.

A comparison of two methods of oral endotracheal tube stabilization in neonatal patients.

OBJECTIVE: To determine if an endotracheal tube stabilization method using an umbilical clamp reduces accidental extubations when compared with a more traditional method of endotracheal tube stabilization. STUDY DESIGN: From January 1, 1994 through April 30, 1997, the frequency of accidental extubations per 100 ventilator days was tracked for the Neonatal Intensive Care Unit (NICU) Quality Assurance Committee of William Beaumont Hospital (Royal Oak, MI). From January 1, 1994 through December 31, 1995, all endotracheal tubes were stabilized using a traditional taping method. From March 1, 1996 through April 30, 1997, the endotracheal tubes were stabilized using a modified umbilical clamp and tape. The incidence of extubation using each stabilization method was compared by chi2 analysis. RESULTS: The umbilical clamp resulted in a significantly lower (p=0.001) rate of accidental extubations. CONCLUSION: The umbilical clamp taping method appears to have lowered the incidence of accidental extubation in our NICU when compared with a standard method of oral endotracheal tube stabilization.
