Low serum estradiol and high serum progesterone concentrations characterize hypertensive pregnancies at high altitude.

OBJECTIVE: Intrauterine growth retardation and preeclampsia are more common at high than at low altitude. Because altered hormonal profiles have been linked with these disorders, we asked whether placental steroid hormone concentrations were altered during pregnancy at high altitude. METHODS: We measured progesterone, unconjugated estradiol, and estriol (by radioimmunoassay) at weeks 20, 30, and 36 of pregnancy in 18 women at low altitude (1600 m) and 40 women at high altitude (3100 m). RESULTS: Women at 3100 m compared with 1600 m had lower serum estradiol concentrations at week 36 of pregnancy, and lower estriol and higher progesterone concentrations throughout pregnancy. As a result, the progesterone/estradiol ratio was greater in the high- versus the low-altitude women. Estradiol fell between weeks 30 and 36 in women who developed transient hypertension or preeclampsia. The fall in estradiol was accompanied by a marked increase in progesterone concentrations among the preeclamptic women. At 3100 m, estradiol correlated negatively (r = -0.37, P < .05) and progesterone positively (r = 0.46, P < .05) with mean arterial pressure at week 36 of pregnancy. CONCLUSIONS: We speculate that reduced placental oxygen pressure (PO2) at high altitude may decrease placental aromatase activity and thereby lower estradiol and estriol concentrations. The factor(s) responsible for the rise in progesterone is unknown. Possibly, high progesterone relative to estradiol concentrations contributes to the development of preeclampsia at high altitude.

Spina bifida: the personal and financial cost of incontinence.

Spina bifida is a congenital defect in the spinal column in which one or more vertebrae fail to form. The effect of spina bifida on continence is usually complicated by the effect of hydrocephalus on the ability to learn and master the necessary skills to overcome the problems. The childhood and adolescence of some individuals who have spina bifida has been ruined by professionals who chose to ignore the wider implications of severe congenital disability and its effects upon the child's education, personal and social development, and the quality of family life. A holistic approach to management, from birth onwards, should facilitate the interdisciplinary cooperation which is vital if these human tragedies are to be avoided.

The nurse lecturer's role in mentoring the mentors.

AIM: This article reports on the results of a survey conducted early in 1999 investigating the effectiveness of current arrangements for mentor preparation and ongoing mentor support provided within adult placement areas within Greater Glasgow Health Board. METHOD: A sample of 150 mentors was surveyed with a response rate of 47 per cent (n = 71). RESULTS: Results indicate that although mentors are generally satisfied with the current approach to mentorship preparation, the issue of support from both managers and academic staff is problematic. CONCLUSION: Respondents indicated that they wished to see lecturers visiting the practice placement areas and providing support, particularly in relation to assessment of students. Also highlighted in this study is the need for more effective communication between placement areas and academic staff.

Managed care and case management: an overview.

This article presents a brief overview of managed care and discusses the role of case management. Characteristics of each are given along with a brief history, description of current models, and a forecast of the next generation. Major issues and implications for social work and discharge planning are also covered.

The acceptability of pneumococcal vaccine to older persons in Ireland.

A study was carried out to demonstrate in an Irish population whether older persons at risk from pneumococcal disease would accept an offer of the pneumococcal vaccine at the same time as their influenza vaccination. Of the 450 patients from 2 practices invited to attend for vaccination, 367 (81.6%) accepted both vaccines, a further 17 (3.8%) accepted the influenza vaccine only and a further 3 (0.7%) accepted the pneumococcal vaccine only. Three hundred and seven (68.2%) patients had received influenza vaccine at some time in previous years and these were statistically more likely to accept either vaccine than those who had not. This study has demonstrated that older persons at risk from pneumococcal disease will respond positively to a written invitation from their GP to avail of the pneumococcal vaccine.
