Structural and strategic approaches to psychosomatic families.

Patterns of relatedness characteristic of psychosomatic families are discussed. A step-wise intervention procedure based on structural and strategic approaches is presented. The goals of the procedure are to establish symptomatic relief and modify concurrently the patterns of relatedness. The procedure is applied to a sample of families in which a child presents with psychogenic abdominal pain. This paper is intended as a detailed and practical guide to working with such families; a degree of generalization is possible, as well.

Protocol for determining the auditory percepts of electrical stimulation of the cochlea.

The percepts of electrical stimulation of the auditory nerve can best be determined in man. An eight channel bipolar electrode array has been developed which allows discreet stimulation of segments of auditory nerve via the scala tympani. In addition, a programmable stimulator has been developed which allows the delivery of different waveforms and stimulus patterns. Hopefully this will begin to allow the generation of the elements of speech.

Cushing's syndrome due to adrenocortical carcinoma - a comphrensive clinical and biochemical study of patients treated by surgery and chemotherapy.

Four post-menopausal women had Cushing's syndrome due to adrenal cortical carcinomas. Comprehensive analyses of blood and urinary steroids showed that although the steroid profiles differed between patients, the pattern in each patient remained almost constant as the disease progressed, or remitted due to therapy. Elevations of serum testosterone and oestradiol were commensurate with the extent of virilisation, and the urinary output of aldosterone was associated with the severity of hypertension. A new finding was that all had substantially increased urinary free deoxycorticosterone. Complete surgical removal of the primary tumours was impossible but when most of the tumour tissue was removed, full clinical and biochemical remissions were obtained for a short time in 2 patients. One patient obtained a clinical and biochemical remission from op'DDD. In another patient the drug caused reduction both in blood pressure and in urinary aldosterone excretion, but there were unpleasant side effects. A third patient could not tolerate op'DDD. Metyrapone therapy produced neither clinical nor biochemical improvement in 3 patients. The mean duration of survival was 17 months after the first symptoms and 10 months from the date of operation. Despite advances in drug therapy, adrenal cortical carcinoma remains a lethal disease. Biochemical screening of multiple steroids offers a means of early diagnosis and disease monitoring. Extensive surgical removal of the tumour offers the best chance of a clinical and biochemical remission.

A simple method for incorporating single pass dialysate delivery and controlled ultrafiltration with the RP-6 high flux dialyzer.

A simple hemodialysis protocol has been developed to permit significant "middle molecule" clearance, yet retain normal low molecular weight clearance. The high flux RP-6 dialyzer has been combined with a single pass dialysate delivery system to provide accurate control of ultrafiltration without resorting to specialized or expensive equipment. By operating the RP-6 in the co-current mode, a simple valve on the dialysate output can be used to regulate ultrafiltration. At QB = 200 ml/min and QD = 500 ml/min, CU = 127 ml/min and CB12 = 56 ml/min at zero ultrafiltration rate; these values increase considerably with ultrafiltration. This protocol offers dialysis centers with standard equipment the opportunity to use high flux membranes in a routine manner.

Standards for accurate in vitro characterization of dialyzers: test case the Vivacell.

A standard format is proposed for cataloging dialyzer transport data in order to improve communication and understanding of published results. The need for such a format is obvious from the ambiguity and lack of uniformity in published data. This report for in vitro characterization also describes an adequate experimental set-up for proper dialyzer characterization and an improved means for reporting uncertainty where curve fitting techniques are employed. The Vivacell dialyzer has been evaluated by these means as the first test case.

Fresh-frozen plasma vs. plasma protein derivative as adjunctive therapy for patients with massive burns.

Twenty patients with burn injuries involving 45% or more total body surface area were randomly allocated to receive either fresh-frozen plasma (plasma), 200 ml/m2/d (11 patients), or an approximately equal amount of plasma protein derivative (Plasmanate) (nine patients) during the first 45 days postburn. To study the potential effects of these two adjunctive therapies on host resistance to infection, measurements were made twice weekly of the antibacterial funciton of neutrophils, the opsonic index (ability to opsonize alternative pathway dependent E. coli 075), C3(B), IgG, properdin, factor B, total protein, and albumin. The average size of burn in the plasma group was 61.5% total and 42% 3 degrees compared with 61% total and 46% 3 degrees in the Plasmanate group. Ten and 18 episodes of bacteremia occurred in the plasma and Plasmanate groups, respectively. Analysis of the results indicates only slightly better support of host resistance when plasma is administered, but this is counterbalanced by the increased risk of viral hepatitis.

Flash photolysis of human serum albumin: characterization of the indole triplet absorption spectrum and decay at ambient temperature.

The method of flash photolysis was used to identify the transient absorption spectrum and to characterize the decay kinetics of the indole triplet of human serum albumin. This protein was studied because it contains a single indole side chain which is deeply buried in an expandable oily region and because the phosphorescence of the homologous indole in bovine serum albumin could not be detected at ambient temperatures. The transient was identified on the following basis: (i) its triplet-triplet absorption spectrum was similar to those previously reported for indole and tryptophan; (ii) it was quenched by small quantities of oxygen; and (iii) it was photobleached by 370- to 700-nm light. In a nitrogen-saturated solution at room temperature, the indole triplet decays exponentially for more than a factor of 10 with a lifetime of 0.5 msec. These observations suggest that, because of its exponential decay and relatively long lifetime, the triplet will be more valuable than the indole singlet as an intrinsic reporter group for the study of the structure and dynamics of proteins in solution.
