The effect of large-dose prednisone therapy on IgG subclasses in multiple sclerosis.

The effect of large-dose prednisone therapy (3960 mg over 56 days) on IgG subclasses in the cerebrospinal fluid and sera, as well as on their intrathecal synthesis, was studied in 15 patients with clinically definite multiple sclerosis. The concentration of IgG subclasses was measured using ELISA with monoclonal antibodies against human IgG subclasses, secondary biotinylated antibody and avidin-biotin-peroxidase complex. There was a decrease of IgG1, IgG3 and IgG4 in the CSF of MS patients after the treatment, but the differences did not reach statistical significance. The IgG index was decreased about 34% (p < 0.01) after the therapy. This was mainly due to diminished synthesis of IgG1 and IgG3. The significance of IgG subclasses in the pathogenesis of MS is discussed.

Reduced expression of erythrocyte complement receptor (C3bR) in MS.

The study was carried out in patients with MS (n-121) and in control group (n-519). On the basis of haemagglutination intensity and results of radioimmunoassay three phenotypes of complement receptor were classified: high HH, medium HL, and low LL. Considerable differences were observed in the distribution of these phenotypes between controls and MS in whom the low phenotype was more frequent. Family studies suggest that reduced expression of the complement receptor on the erythrocytes depends in MS on the disease process in the first place, and not on genetic factors.

Free sterols in senile human brain.

Autopsy material of 18 patients, who died between 66-86 years of age and of 4 ones, who died between 20 and 38 years of age was investigated. The white matter of frontal lobe, corpus callosum and cerebellum was studied using histological and biochemical methods. According to the results of neuropathological studies, the material of aged patients was divided into two subgroups: patients with vascular changes only and brains with senile atrophy of Alzheimer type. Chemical changes in all aging brain regions studied included higher proportion of desmosterol and differences in the pattern of cholesterol isomers. These findings are interpreted as suggesting some chemical transformations of cellular membranes in the cerebral white matter of the aging. The above mentioned changes were almost identical despite presence of the signs of Alzheimer's type senile atrophy, therefore it is not possible to explain their significance in developing of dementia process.

Myelin lipids and proteins in experimental global ischemia.

The experiments were performed on white rats, in which clinical death was induced according to the method described by Korpatchev et al. (1982). After cardiac arrest and cessation of respiratory function lasting 5 minutes resuscitatory action was performed. The lipids and proteins of the cerebral myelin fraction were studied in animals sacrificed 1, 9 and 14 days after global ischemia. The obtained results lead to the following conclusions: 1. Myelin lipids in postreanimation syndrome are characterized by a marked increase in cholesteryl esters and a mild one of lysophosphatidylcholine content. 2. Myelin lipids in the predemyelinating period demonstrate a general pattern of reaction, notwithstanding to the character of the primary noxious agent. 3. After global ischemia a great fall of small basic protein in the myelin fraction and subsequently of SBP to LBP ratio occurred. 4. Various noxious agents affect different proteins of the myelin membranes and the resulting changes seem to be characteristic for various pathological processes.

Analysis of heavy metals content in the brain with the use of electron paramagnetic resonance in a clinically unusual case of hepato-lenticular degeneration.

An analysis of heavy metals content in the brain of a clinically unusual case of hepatolenticular degeneration was performed with the use of paramagnetic resonance. The studies established a manyfold increase of copper content in the brain in the form of free ions, and, less significant, also in the form of copper multi-ion clusters. The content of iron in the studied brain was lower than in controls. These results confirmed significantly the neuropathological diagnosis of hepato-lenticular degeneration.

Electron paramagnetic resonance analysis of heavy metals in the aging human brain.

Electron paramagnetic studies of heavy metals were performed with the use of a Radiopan spectrometer on autopsy material of 5 aged patients (mean age 80.2 years) and 15 control patients (mean age 29 years). The obtained results lead to the following conclusions: 1. The aging brain is characterized by a tendency to decrease in concentration of isolated Cu2+ ions, and a marked decrease in concentration of Fe3+ ions as well as of free radicals, whereas the concentration of Cu2+ clusters is significantly increased. 2. The cases showing both arteriosclerotic and senile degenerative changes are characterized by higher concentrations of Cu2+ clusters than the brains with dominance of the arteriosclerotic process, whereas the concentrations of isolated Cu2+ and Fe3+ ions as well as that of free radicals do not differ between the two subgroups. 3. The diminished concentrations of Cu2+ and Fe3+ ions and free radicals observed in brains of old persons and concomitant with increased concentration of multi-ion aggregates (clusters), more marked in cases of senile atrophy of Alzheimer type, seem to result from some slow-down of metabolic processes in the aging brain.

[Early changes in the visual system in subacute sclerosing panencephalitis].

This paper presents our observations of changes in the visual system in the early period of SSPE in the group of 23 children at the age of 3 to 17 years (18 boys and 5 girls). The most frequent changes were papillary stasis (9 patients). Decreased visual acuity with no changes in the optic fundus or with chorioretinitis and simple atrophy of the optic nerve were observed in 8 children. Convergent squint, visual hallucinations and visual agnosia were additional symptoms in single cases. Changes in the visual system which appeared in early period of the disease as single clinical symptom usually create serious diagnostic problems. The greatest problem appeared when papillary stasis coexisted with other symptoms of increased intracranial pressure. Neuroradiological examinations were decisive in these cases. Diagnosis of SSPE was confirmed by further course of the disease and others additional and typical results.

Myelin proteins in aging human brain.

Autopsy material of 13 persons who died between 70 and 89 yrs old and of patients who died between 27 and 44 yrs old was studied. White matter of temporal, parietal, and occipital lobes was investigated using histological and biochemical methods. According to results of neuropathological studies, the material of aged patients was divided into two subgroups: (a) brains with vascular changes only and (b) patients with senile atrophy of Alzheimer type. Chemical changes found in all studied brain lobes included a mild decrease in Wolfgram protein content with reciprocal increase in large basic protein content, together with a marked decrease in myelin yield. The abovementioned chemical changes were almost identical whether they were only vascular changes or whether senile atrophy of Alzheimer type was also present. It seems, therefore, that the degeneration of vessels is the decisive factor in the pathogenetic mechanism of myelin lesions in the aged brain.

Myelin lipids in ischemic stroke.

Slices of ischemic focus (infarct area) and of the contralateral frontal lobe were submitted to histological and biochemical studies. The obtained results indicate that in stroke cases the necrotic focus as well as contralateral brain hemisphere are characterized by a marked decrease of cholesterol and cerebrosides content and an increase of lysophosphatidylcholine and cholesterol esters in the myelin fraction. We conclude that ischemia as well as the degenerative aging process are both responsible for the abnormal lipid pattern in the myelin of the white matter in stroke cases. The long lasting hypoxia resulting from cerebral vessel atheromatosis contributes to biochemical changes in the myelin of the apparently healthy white matter of the contralateral hemisphere in brain infarction.

Pattern of myelin proteins in moderate hypoxia.

Myelin proteins isolated from the brain of Wistar rats subjected to moderate hypoxia (7% of oxygen in a respiration gaseous mixture) for 30 min were investigated. Among the various protein fractions Wolfgram protein happened to be most markedly affected, demonstrating considerably lowered percentages in all experimental groups, starting from 24 h after hypoxia. Our findings compared with those obtained in other experimental conditions lead to the conclusion that changes in the myelin protein pattern are characteristic for particular morbid processes evoked by various pathogenic factors. The demyelination is concomitant with severe loss of basic proteins, whereas myelin lesions of other types are connected with changes in protein of higher molecular mass.

Epidemilogy of stroke in the Poznań district of Poland.

The incidence of cerebrovascular disease was established in the Poznań district in Poland for 1985. Mortality from cerebrovascular disease during this year was compared with mortality during 1977. The autopsy material from the years 1925 to 1985 was analyzed to differentiate ischemic from hemorrhagic stroke. The incidence of stroke was 198.3 per 100,000 inhabitants greater than 20 years of age. Mortality from cerebrovascular disease decreased significantly in both males and females from 1977 to 1985. Analysis of the pattern of cerebrovascular disease in the autopsy material from 1925 to 1965 demonstrated an overwhelming percentage of hemorrhagic stroke. In the years that followed, an inversion was noticed in the ratio of ischemic to hemorrhagic stroke. It should be assumed that the growing efficacy of antihypertensive therapy largely explains the decreasing incidence of stroke-induced mortality.

[Epidemiology of Alzheimer's disease in the selected region of Wielkopolska (town and commune Steszew)].

An epidemiological survey of Alzheimer's disease (senile dementia of Alzheimer type) was conducted on the territory of town and commune Steszew, lying in the Poznań district, with a population of 13,023 on the appointed day, December 31, 1988. Clinical diagnosis was established by detailed anamnesis, neurological, general and psychological examinations. Also a clinical questionnaire containing ischemic scale after Hachinski et al., (1975) was evaluated. The detected overall prevalence rate of Alzheimer's dementia on the appointed day (December 31, 1988) was 1105.7 per 100 thousand population, whereas in the group of persons over 65 years was 10059.2 per 100 thousand. The prevalence rate of severe cases of Alzheimer's disease was established in the group of population over 65 year on 2662.7 per 100 thousand. The detected overall incidence rate of Alzheimer's disease in the year 1988 was in the age class 45-65 years 201.5 per 100 thousand, and in the group over 65 years 2588.8 per 100 thousand. The obtained results of our investigations in the population of western Poland are in the lower part of dispersion of the epidemiologic data on Alzheimer's disease in the world scale. The results indicate that too rarely is diagnosed Alzheimer's disease as a cause of dementia in elderly people, and too often the cerebrovascular disease. The high prevalence rate of demented people in senility indicates on great importance of organization of Inpatients Senior Clinics in which a part of those people shall find a place.

Influence of experimental hypoxia on content and composition of free fatty acids in cerebral white matter.

The content and spectrum of free fatty acids were studied in the cerebral white matter, on Wistar rats exposed to acute hypoxia (2% of oxygen in a respiratory mixture) for 3 min. The total content of fatty acids, especially of tetraenoic ones, rose sharply already 4 min after hypoxia and persisted elevated even 2 months after the hypoxia. The results showed that the degradation of phospholipids-containing biological membranes in the nervous tissue of the white matter starts immediately after hypoxia, and is continued for months.
