Value of early ambulation in patients with and without complications after acute myocardial infarction.

A prospective, controlled, randomized study was done to compare the effect of early and late ambulation in hospitalized patients with acute myocardial infarction. All patients surviving longer than the first five days were studied; 64 patients were mobilized on day six and discharged on day 12, and 65 were mobilized on day 13 and discharged on day 19. Follow-up observation lasted from six to 52 weeks. Of patients without complications until day six, eight out of 32 in the early and 16 of 35 in the late groups manifested complications during the follow-up period (p smaller than 0.05). Of those who had complications before day six, seven of 32 and 26 of 30 still had or acquired new complications until last seen (p small than 0.0001). The number of serious complications in the two groups was eight and 24 respectively (p smaller than 0.001). We conclude that early ambulation is beneficial irrespective of complications on admission.

Recurrent inguinal hernia. Follow-up study of 100 postoperative patients.

This review of recurrent inguinal hernia evaluates the possible causes of recurrence and the most effective operative procedures for successful repair. Emphasis is placed upon precise dissection technic so that whatever fascia is available after the primary operation may be utilized for the subsequent repair. A detailed study is devoted to the histologic features of fascia, emphasizing its lack of vascularization as the possible reason for its strength and permanency throughout life. In operations for recurrent inguinal hernia, rectus sheath pedunculated grafts should be used more frequently, and in extreme cases cord excision may be required. A follow-up study of one hundred postoperative cases, with a failure rate of 7 per cent, is presented.

Interaction of ribosomes and the cell envelope of Escherichia coli mediated by lysozyme.

Evidence is presented suggesting the existence of a natural ribosome-membrane complex. A reconstruction system is described wherein free ribosomes form a complex which appears to involve cell fragments. The reconstructed complex is similar in stability to the inferred natural complex. The reconstructed complex is generated by lysozyme, and it is concluded that at least part of the inferred natural complex is also generated by lysozyme. These results are discussed with reference to existing data concerning certain membrane-associated systems in bacteria.
