[Urbach-Wiethe disease. Apropos of 2 cases].

The Urbach-Wiethe disease is a rare condition, where a hyaline substance of an unknown biochemical nature is accumulated in both teguments and mucous membranes. It has an autosomic and recessive genetic transmission. Cutaneous and mucous lesions (especially on the upper digestive tract) are described, insisting about the laryngeal location and the neurological troubles (with their main symptom: intracranial calcifications). The association of the disease with congenital indifference to pain is to be noted. Two particular cases are reported.

[Mechanisms of aspirin intolerance].

Aspirin intolerance brings on cutaneous and/or respiratory reactions. The mechanisms are to be elucidated precisely, but are not IgE-mediated. 193 patients with nasal polyps are studied. With systematic aspirin oral challenge, 27% belong to the classical triad: asthma, polyps and aspirin sensitivity. Other drugs or chemicals intolerance are associated in 42%: alcohol, metabisulfites, benzoates, tartrazine, codeine... A link with the non-allergic eosinophilic rhinitis is suggested. A short review of several pathogenic hypothesis is discussed. Among a comparative study, the authors showed that the aspirin-intolerant patients' platelets have a quite normal arachidonic acid metabolism. The clue is probably towards an abnormal instability of cells membranes and excessive receptors sensitivity to leukotrienes.

[Arteriovenous fistulas of the cranial dura mater. Apropos of 3 cases].

Three cases of arteriovenous fistulas (A.V.F.) of the cranial dura mater are reported. The authors point out that the first symptom is often in the O.R.L. field: vascular tinnitus with sometimes headaches. Diagnosis can be perform by selective arteriography. In the same time, embolization is possible.

Arachidonic acid metabolism and inhibition of cyclooxygenase in platelets from asthmatic subjects with aspirin intolerance.

Exaggerated inhibition of cyclooxygenase has been proposed as a mechanism of drug-induced bronchospasm in aspirin-intolerant patients. This study, using platelets, shows that inhibition of prostaglandin biosynthesis by aspirin is unmodified in patients when compared with healthy subjects. The ratio of cyclooxygenase:lipoxygenase products is similar in platelets from patients and control subjects. We conclude that the cyclooxygenase alteration observed in cells from the respiratory tract is not generalised to other cells such as platelets. We also propose that the major abnormality in NSAID-intolerant patients would affect receptivity to lipoxygenase products more than their biosynthesis.

[Evaluation of the therapeutic efficacy of vastarel 20 mg (trimetazidine) in cochleovestibular syndromes. A double-blind study versus placebo].

334 patients with functional cochleovestibular symptomatology were treated with trimetazidine (40 mg daily) or with placebo in a double-blind study. The decrease in subjective symptoms (tinnitus, hypoacusis, vertigo, disturbances of equilibrium, neurovegetative disturbances, bad discrimination in background noise) was significantly more important with trimetazidine than with placebo. Objective signs such as index deviation, walking tests and positive Romberg test were also significantly more improved with trimetazidine. Puretone audiometry in patients with perceptive hypoacusis showed an improvement of auditory level with trimetazidine; progression with trimetazidine is significantly better than that with placebo, at frequencies equal to or above 2 000 Hz. Overall and cardiovascular clinical acceptability is similar in both groups. This study underlines the therapeutic efficacy of trimetazidine in cochleovestibular syndromes, especially on functional symptomatology.

[Histopathologic study of chronic sinusitis].

The conventional histopathology of the sinus is a criterium for the therapeutic indication, since it is possible to distinguish between granulomatous chronic sinusitis, chronic sinusitis with oedema and nasal polyposis. Each one of these clinical pictures has his own etiology and requires a specific therapeutic approach.
