Melanocyte colonization of adenocarcinoma arising in an ovarian dermoid.

An adenocarcinoma which arose in a dermoid cyst of the ovary displayed areas of melanocyte colonization and pigmentation. Ultrastructural study revealed the presence of epithelial tumour cells and melanocytes; many tumour cells contained compound melanosomes, but not premelanosomes, suggesting transfer of melanin from melanocytes to tumour cells. Melanocyte colonization of malignant tumours is a curious phenomenon the significance of which remains to be elucidated.

Malignant lymphoma of the heart in acquired immune deficiency syndrome.

Malignant lymphoma of the heart was found at autopsy of two homosexual men with acquired immune deficiency syndrome (AIDS). Both patients had symptoms and signs of cardiac dysfunction, and it is likely that the immediate cause of their deaths was related to the cardiac tumors. In both cases, there was lymphomatous involvement of other organs, but the heart was the predominant site of disease. There was prominent endocardial and myocardial involvement, and both tumors were high grade large cell lymphomas with plasmacytoid features. In both instances, the lymphomas were diagnosed only at the autopsy, despite extensive antemortem cardiac evaluation in one patient. The clinicopathologic correlation, gross pathologic and histologic findings of the tumor, and their pattern and distribution suggest a de novo origin of the lymphoma in the heart in these two patients. Lymphomatous infiltration of the heart should be suspected in patients with AIDS who have cardiac symptoms.

Sarcoma metastatic to the central nervous system parenchyma: a review of the literature.

Sarcoma metastatic to cerebral parenchyma, although rare, occurs more frequently than generally recognized. With increased duration of survival due to multi-modal therapy, more CNS metastases are being found. A literature search occasioned by a patient with metastatic sarcoma has produced some interesting results.

Prognostic factors in malignant mesodermal (mullerian) mixed tumors of the uterus.

Clinicopathologic studies of 16 homologous and 19 heterologous malignant mesodermal (Mullerian) mixed tumors of the corpora uteri showed that homologous tumors were more common in black women than in white women; in the former, tumors were diagnosed at a younger age and in more advanced stages than in the latter. Fifty-five percent of women with clinical stage I disease had a higher surgicopathologic stage. The most significant prognostic factors were pathologic extent of disease and vascular invasion in the myometrium. There was no difference in outcome between homologous and heterologous tumors. Initial surgery for staging is essential for the adequate evaluation and treatment of these patients.

Fatal hemorrhage from rectal varices. Report of two cases.

We report two cirrhotic patients who succumbed to massive rectal bleeding. The source of this hemorrhage remained undiscovered clinically despite endoscopy, a bleeding scan, and celiac angiogram in one patient. Autopsy revealed that the source of the bleeding was rectal varices in both cases.

Malignant lymphoma of skin associated with postmastectomy lymphedema.

Malignant lymphoma developed in the dorsum of the hand in a patient with a long-standing postmastectomy lymphedema. The tumor subsequently spread to involve skin of the forearm and arm. Growth and spread of malignant cells within the stagnant dilated lymphatics are the likely cause of the characteristic nodular dissemination of the tumor in a lymphedematous extremity, regardless of the type of neoplasm.

Radiological manifestations of Strongyloides stercoralis.

Nine patients with radiological changes due to Strongyloides stercoralis (SS) are described. A wide variation in appearance exists ranging from mild edema of the duodenal and small bowel mucosa to grossly enlarged, prominent valvulae conniventes. Small bowel dilatation is significant, and in overwhelming infestation toxic dilatation with paresis results. Spasm, ulceration, and stricture are encountered in addition. The appearances usually improve and reverse with treatment. Ampullary involvement is responsible for reflux of barium into the pancreatic duct and biliary tree through a patulous sphincter. In 1 patient the colonic changes resembled ulcerative colitis.

Malignant fibrous histiocytoma of the colon associated with diverticulitis.

We report the second case of a malignant fibrous histiocytoma of the large bowel. The neoplasm constituted an incidental finding during the diagnostic work-up for diverticulitis of the sigmoid colon. There was a massive abdominal recurrence of the tumor four months after colectomy, and the patient died nine months after the initial operation. The literature on fibroblastic sarcomas of the colon is also reviewed.

Heterologous malignant mixed Müllerian tumor of the cervical stump.

A 76-year-old woman presented with vaginal bleeding 37 years following supracervical hysterectomy for a benign disease. Extensive malignant mixed Müllerian tumor was found in the remaining uterine cervix. This appears to be the first documented case of the malignant mixed Müllerian tumor arising in a cervical stump. Possibility of derivation of this tumor from endometriosis notwithstanding, the cervical Müllerian neoplastic potential parallels that of the endometrium because of the developmental kinship of the two. This potential has been fully retained and expressed as a tumor in the cervical stump in this case.

Initial experience with universal pacemakers.

Thirty-five patients were implanted with the Sequicor DDD pacemaker between September 1, 1981 and March 1, 1982. The mean age was 61.7 years (range, 17-88). According to their ECGs, the patients fell into two distinct groups--those with A-V block and those with sick sinus syndrome. Our initial experience has indicated an excellent electronic component function (except for 1 discrepant reed switch); high battery drain of the Sequicor; complications similar to those seen with DVI or VVI pacing; and an increase in the hemodynamic benefits resulting from DDD as compared to DVI or VVI pacing in 70% of patients.

Concurrence of malignant and benign heterologous mixed tumors of the uterus.

A large polypoid uterine mas was composed to two distinct and separate parts: a malignant mixed müllerian tumor (MMMT) and a benign mixed mesenchymal tumor (BMMT). It was considered a collision of two neoplasms rather than a malignant degeneration of the BMMT. Malignant transformation of benign mesenchymal uterine tumors is a controversial concept which is difficult to prove or disprove. Concerning the histogenesis of MMMT it appears, at least in some cases, that they originate not from a single multipotential mullerian cell, but rather from two, a stromal cell and an epithelial cell. It is suggested furthermore, that in certain instances presence of one neoplastic component, e.g., adenocarcinoma, can incite proliferation of the second, sarcomatous element of the MMMT.

Composite mucinous ovarian neoplasms associated with Sertoli-Leydig and carcinoid tumors.

The authors describe two mucinous ovarian tumors that were hormonally active due to Sertoli-Leydig elements within the septal stroma. Carcinoid was also present in each case. The mesenchymal stromal cells within the septa of the mucinous tumors have undergone neoplastic proliferation with the Sertoli and Leydig differentiations, leading to the formation of arrhenoblastoma. The carcinoid component of these tumors is thought to have originated from argentaffin cells of the "intestinal" mucinous epithelium.
