In vitro fertilization and embryo transfer as related to endogenous luteinizing hormone rise or human chorionic gonadotropin administration.

After ovarian stimulation with clomiphene citrate combined with human menopausal gonadotropin for in vitro fertilization, the appearance of a spontaneous luteinizing hormone (LH) surge before fulfillment of the minimal criteria of follicular maturity (at least one follicle greater than 19 mm and serum estradiol [E2] greater than 400 pg/ml/follicle greater than 17 mm) is associated with reduced pregnancy rates. In these cases, follicles are smaller and serum E2 values are lower at the time of the LH surge. Oocyte recovery rate is reduced, embryonic anucleate fragments are more frequently observed, and the level of luteal progesterone on day 4 after oocyte retrieval is lower. Hyperandrogenic patients are more prone to display such premature spontaneous LH surge. We concluded that in case of an untimely LH rise, laparoscopy for oocyte retrieval should be cancelled.

Acute lymphoblastic leukemia at relapse in a child with acute myeloblastic leukemia.

We describe a child diagnosed as having acute myelogenous leukemia (AML) at 25 months of age who relapsed with acute lymphoblastic leukemia (ALL) 1 year later. The AML was morphologically M2 by the French-American-British classification, periodic acid Schiff (PAS) stain negative and peroxidase positive. The ALL was L1 by this classification, PAS positive, and peroxidase negative. The initial AML was associated with a small segment deletion of the long arm of chromosome 11 [46, XY, de (11) (q23)] not seen in the relapse ALL, which had a normal karyotype. The child was rapidly reinduced with vincristine and prednisone, and remains in remission on maintenance lymphoma type (LSA2-L2) therapy more than 2 years later. These findings suggest the development of a new leukemic clone, rather than a phenotypic modulation of the initial leukemia.

Liability to cleft palate in trisomy 19 mouse embryos.

A mouse model is available which permits both the measurement of the underlying liability for cleft palate and the induction of trisomic states. By using this model the palate development has been investigated and compared in normal and trisomic embryos. These studies indicate that palate development of trisomy 19 embryos is significantly delayed compared to that of their normal littermates. Furthermore, the delay in palate development observed in trisomy 19 embryos is in direct relationship with the observed pattern of palate development in normal littermates. This suggests that the increased expression of major congenital malformations associated with aneuploidy is due to a change in their underlying liability.

A taxonomic approach to the del(4p) phenotype.

The findings of 23 patients with the del(4p) phenotype are compared systematically. Three patients with a small deletion evident only on analysis of extended chromosomes have a phenotype comparable to 13 patients with a more extensive chromosomal deletion. Two patients with no detectable deletion also fit into the phenotypic spectrum of patients with del(4p), suggesting the same etiology. Five patients with fewer typical and more atypical findings probably represent a heterogeneous group of other syndromes. Numerical analysis of the phenotype allows one to identify patients who most likely have a deletion requiring a more intensive cytogenetics analysis.

Histological and cytogenetic findings in a malignant tumor of the chest wall and lung (Askin tumor).

This report describes a histological and cytogenetic study of a malignant tumor involving the chest wall and lung (Askin tumor) of a young girl. Although initially considered to represent a variant of Ewing's sarcoma, immunocytochemical studies disclosed neuron-specific enolase in neoplastic cells. Ultrastructural study revealed rare cells which contained microtubules and/or dense core neurosecretory type granules. Cytogenetic analysis of neoplastic cells disclosed a reciprocal translocation (11;22)(q24;q12) and occasional extrachromosomal structures interpreted as double minute chromosomes. The latter finding, an indication of gene amplification, is commonly identified in neural crest-derived neoplasms. These ultrastructural, immunocytochemical, and karyotypic data provide evidence in support of a neuroepithelial histogenesis for the Askin tumor.

Growth rate of follicular estrogen secretion in relation to the outcome of in vitro fertilization and embryo replacement.

Estradiol growth rate (EGR) during active follicular development was calculated for 89 stimulated in vitro fertilization cycles by exponential curve fit (r = 0.83). Cycles could be divided into four groups with very low, low, moderate, and high EGR values. Cases without oocyte fertilization and/or embryo replacement did not occur in the moderate EGR range, which also corresponded to a significantly better ratio of replaceable embryos versus oocytes recovered. This group was also endowed with a pregnancy rate amounting to 28.5% per laparoscopy and per replacement, i.e., about three times higher than in the three other groups. Very low or high EGR entailed significantly higher percentages of missed oocyte recovery and/or fertilization failure. The frequency of occurrence of a spontaneous luteinizing hormone peak was negatively correlated with EGR. The data indicate that a better outcome of in vitro fertilization may be expected when the estrogen rise starts early in the cycle and adopts a moderate growth rate (0.3 to 0.4).

Fanconi's anemia, medulloblastoma, Wilms' tumor, horseshoe kidney, and gonadal dysgenesis.

An 18-month-old female infant with clinically and cytogenetically documented Fanconi's anemia was found to have two neoplasms previously unreported in this syndrome to our knowledge: a medulloblastoma and a Wilms' tumor, with the latter arising in a horseshoe kidney. An additional feature was pure gonadal dysgenesis. These unusual associations are discussed in the context of certain syndromes suggestive of an axial predisposition for neoplasia (kidneys-central nervous system and kidneys-gonads).

[Depot medroxyprogesterone acetate: clinical and metabolic effects (lipids, glucose, hemostasis)].

Medroxyprogesterone acetate (MPA), a potent progestagen, is used as a very efficient contraceptive agent. The method of administration is by a threemonthly or sixmonthly intramuscular injection. The method is particularly convenient when the desired number of children has been reached. Fertility is sometimes reestablished only slowly after stopping the injections. The main side effects are linked to endometrial atrophy (blood loss, amenorrhoea). Other side effects are infrequent and subjective. A controversy has risen over the product, by reference to its effects on beagle dogs. We have reviewed the literature. We also present the characteristics of 313 patients treated during (all together) 8,000 months. These patients belong to a rather poor sample of the population (amongst them many immigrant women), aged about 30, with above average pregnancy and parity rates. We also studied, in 31 patients, serum levels of glucose and lipids, and haemostasis. Side effects and reasons for discontinuing the drug are reported. The Pearl index was 0,75% women-years. There was no average effect on weight, blood tension, glycemia. Triglycerides increased slightly, and there was a discreet activation of coagulation with inhibition of the fibrinolytic activity, but without clinical consequences. We conclude that we can go on prescribing the drug, in view of our study and after reviewing the literature.

[Treatment of non-specific menometrorrhagia with naftazone].

The authors carried out a double blind study of the action of naftazone in non-specific menometrorrhagias caused by a uterine device, the oral contraceptive and medroxyprogesterone, in 25 patients. They show a great improvement with the periods becoming normal and metrorrhagia disappearing in 12 out of 13 cases where naftazone was used and in only 1 out of 12 cases where the placebo was used.

[Voluntary inerruption of pregnancy in adolescents from 13 to 18 years of age. Clinical and sociologic analysis of 295 cases].

The study reports on 295 cases of induced abortion in teenagers (13-18 years) performed in Belgium (where abortion remains illegal), in 1979. This group was compared with a group of 2968 women aged 19-48, also aborted in 1979, for clinical and sociodemographic analysis. The comparative situations of the young patient and he partner were also studied. The latter were elder (62% over 25 years), had higher levels of income, of education, and had better jobs. No difference was found between younger and older patients as concerns access to centres, mean ages of pregnancy (although teenagers presented more often advanced pregnancies: 9.5% between 12 and 16 weeks and 4.5% between 17 and 21), religion, instruction, psychological reactions. We found that different people came with patients at different age groups (parents with the younger, partners with the older). There were fewer foreigners in the younger group and of course there were differences in numbers of previous pregnancies, parity, marital status and income. A stable relationship was rarer in the younger (39%) than in the older (66%). Contraception was indeed used by young patients (61%) but they chose less effective methods (61%) and often admitted that they used the methods wrongly (66%). It is no surprise that young patients mainly ask for abortion (54%) because of socio-economic motives. The second reason was because there was no stable relationship (17%). The discussion covers the special problems of abortion in adolescents and emphasizes the very difficult social situation, the need for support by health workers. Better education should lead to conscious acknowledgment of the consequences of sexual activity.

Balanced translocations among couples with two or more spontaneous abortions: are males and females equally likely to be carriers?

Chromosome studies were performed on a series of 177 couples referred for genetic counseling following two or more spontaneous abortions to clarify the relationship between karyotype and fertility in males and females and to provide risk figures for genetic counseling. The results of these investigations, when combined with those in the literature, suggest that 2-3% of individuals in couples experiencing early fetal losses carry a balanced translocation and that this is not markedly influenced by the number of losses greater than two. Females are more likely than males to be the carrier, reflecting the fact that structural abnormalities of the chromosomes that are compatible with fertility in the female may be associated with sterility in the male. Karyotyping should be performed on both members of a couple following two spontaneous abortions and the products of conception should be studied whenever possible to begin to determine the biological basis of the association between parental rearrangement and fetal loss.

Chromosomal breakage in normal and fragile X subjects using low folate culture conditions.

To investigate whether the fragile X syndrome is associated with a generalised chromosomal instability, we compared the frequency and distribution of chromosomal breakage in lymphocytes grown in low folate medium from normal subjects and from patients with the syndrome. Although low folate conditions increased the rate of chromosome breakage, no difference in frequency or distribution of chromosomal breakage was found between the two groups. This suggests that the fragile X syndrome is not associated with a generalised chromosome instability expressed in folate deficient medium and assessed in terms of chromosomal breakage.
