Immunotherapy with a mixture of Mycobacterium leprae and BCG in different forms of leprosy and in Mitsuda-negative contacts.

A total of 529 weak or non-reactors to M. leprae, including Mitsuda-negative contacts and patients with leprosy, were vaccinated once or repeatedly, as necessary, with a mixture of 6 x 10(8) purified, heat-killed M. leprae and 0.01 mg to 0.2 mg of viable BCG. Clinical, histopathological and immunological criteria were used to evaluate the response of these individuals. Clinical changes, including sharper definition of borders and progressive flattening and regression of lesions, were observed in 57% of the active LL cases and 76% of the active BL cases. Histopathological study revealed infiltration of the lesions by mononuclear cells, appearance of epithelioid differentiation, and fragmentation of the microorganisms. Delayed-type skin tests with soluble antigen from purified M. leprae became positive in significant numbers of each group studied. These results demonstrate the efficacy of combined immunotherapy in low-resistance forms of leprosy and potential utility in the immunoprophylaxis of the disease.

Effects of bradykinin on intestinal transcapillary fluid exchange.

Bradykinin (50 micrograms.L-1) increases intestinal lymph flow sixfold when infused intraarterially into the cat ileum. The capillary filtration coefficient and capillary pressure increase and interstitial fluid pressure rises from negative to positive values. A slight increase in lymph:plasma protein concentration occurs with a resulting fall in the transcapillary oncotic pressure gradient. These results indicate that the effect of bradykinin on intestinal lymph flow is attributable, at least in part, to increased capillary pressure, exchange vessel surface area, and a reduction in the effective transcapillary oncotic pressure gradient.

Production of monoclonal mouse IgE antibodies with DNP specificity by hybrid cell lines.

IgE anti-DNP antibody-secreting hybridomas were obtained by polyethylene glycol fusion of murine myeloma cells with spleen cells from mice, primed with DNP-KLH and challenged with DNP-Nippostrongylus brasiliensis extract. The in vivo and in vitro continuously growing hybridomas are producing high amounts of monoclonal IgE anti-DNP antibodies, which are heat-labile, hapten-specific and have rat mast cell sensitizing capacity.

[The influence of chronic pentroxifylline medication on ergometric and hemodynamic parameters in intermittent claudication (author's transl)].

In an open clinical trial 14 patients with intermittent claudication caused by an obliteration of the femoral artery received 400 mg pentoxifylline 3 times daily over a period of 6 months. The walking distance, time to peak flow and calf ergometry showed a significant increase. Minor improvement of working hyperemia and peak flow could be found. The systolic pressure gradient over the obstruction didn't change. No patient complained about side effects.

Immunological changes observed in indeterminate and lepromatous leprosy patients and Mitsuda-negative contacts after the inoculation of a mixture of Mycobacterium leprae and BCG.

This investigation was carried out to study the possibility of eliciting favourable immunological changes in small groups of Mitsuda-negative patients with indeterminate leprosy, lepromatous patients who were bacteriologically negative after prolonged treatment with sulphones, and in Mitsuda-negative contacts by means of stimulation with a mixture of autoclaved tissues from Mycobacterium leprae-infected armadillos and living BCG. A radical change was observed in the specific immunological activity of the indeterminate group, all of whom initially had occasional bacilli in cutaneous nerves in biopsies taken from hypopigmented spots, and in the persistently Mitsuda-negative contacts. The 48 hr and 30 day reactions to lepromin, the 48 hr reaction to supernatant antigen from lepromin, the test for bacillary clearence and in vitro lymphocyte transformation (LTT) to M. leprae from human and armadillo lesions all became positive. Of the lepromatous patients studied, only one became positive to all the criteria mentioned above. In the others, the 48 hr reaction to supernatant antigen, the LTT to antigen from a humn source, and the clearance test remained negative, while the Fernandez and Mitsuda reactions became positive. These results are discussed in terms of the possible use of this stimulation procedure in the prevention and immunotherapy of leprosy.

Deaf patients' access to care depends on staff communication.

To ensure equal access to care for deaf patients, hospitals must make certain that their staff members can communicate effectively with these patients. To do so, common misconceptions about deaf people and about how they communicate must be corrected, and resources and techniques for staff members to use in communicating with deaf patients must be identified and learned.

Immunological characteristics of the armadillo, Dasypus sabanicola.

The immunological responses of the armadillo are of interest because of its susceptibility to generalized lepromatoid infection with Mycobacterium leprae. In this study, specimens of Dasypus sabanicola were found to have a typical mammalian distribution of lymphoid cells in thymus, spleen, lymph nodes and blood. Their complement was active in bactercidal, protozoan immobilization and haemolytic systems. Blood lymphocytes responded to phytohaemagglutinin and to pokeweed mitogen. Sensitization with ovalbumin in CFA resulted in the production of circulating precipitins; strong Arthus reactions were detectable in the sensitized animals. Responses of cell-mediated immunity to DNCB and to M. tuberculosis were very discrete. Heat-killed M. leprae elicited granulomatous reactions characterized by microscopic necrosis, but without abundant lymphocytic infiltration; skin tests and lymphocytic transformation were generally negative in the animals injected with M. leprae.

Correlation between human chorionic somatomammotropin and placental weight.

Human chorionic somatomammotropin concentration in serum (S-HCS) during the latter half of pregnancy was measured by radioimmunoassay and correlated to the placental weight, in two groups of normal, healthy, pregnant women. In one group, 228 samples from 47 women were examined, which gives a longitudinal series. In the other group, single samples from each of 346 pregnant women were examined, which gives a cross-sectional series. Both groups were randomized on the basis of a prospective selection. The mean values of S-HCS in each week of gestation were almost identical in the two groups, showing a steady increase until 37-38 weeks and a subsequent decrease. In the longitudinal series there was a positive correlation between the S-HCS and placental weight after 37 weeks' gestation, but not before that time. Before 37 weeks gestation the ratio S-HCS/placental weight was significantly higher with small placentae than with large placentae. This difference between small and large placentae disappeared after 37 weeks. These results point to the existence of some regulatory mechanism tending to keep the S-HCS concentration within certain limits, independent of placental weight. This mechanism appears to be lost after 37 weeks of gestation when the S-HCS concentration starts to correlate with placental weight.

Dermal exudate macrophages. Induction in dermal chambers and response to lymphokines.

Chambers were implanted in the dorsum of guinea-pigs at the dermal-subcutaneous junction. Exudates were induced and harvested. Macrophages obtained were able to migrate in vitro. If procured from sensitized donors, macrophage migration was inhibited by the corresponding antigen. Dermal exudate macrophages are therefore subject to the effect of lymphokines. The chamber model may be useful for in vivo studies of cell to cell and cell-parasite interactions.
