Early assessment of reperfusion therapy using cardiac troponin T.

OBJECTIVES: The purpose of this study was to investigate the utility of cardiac troponin T for early assessment of reperfusion therapy. BACKGROUND: Several biochemical markers are used for early noninvasive detection of reperfusion during intravenous thrombolytic therapy. However, cardiac troponin T, a new myocardial-specific marker, has not been used previously for this purpose. METHODS: We measured troponin T and creatine kinase, MB isoenzyme (CK-MB) levels in 38 patients with acute myocardial infarction whose infarct-related artery was totally occluded before reperfusion therapy. Subjects comprised 14 patients with successful angioplasty (group 1), 12 patients with successful thrombolytic therapy (group 2) and 12 patients with unsuccessful attempted reperfusion (group 3). Blood samples were taken every 15 min, and coronary angiography was performed every 5 to 8 min until 60 min after reperfusion (groups 1 and 2) or after the initiation of treatment (group 3). We calculated the increase in troponin T (delta troponin T) and CK-MB (delta CK-MB) 60 min after treatment was initiated and 60 min after reperfusion in groups 1 and 2. RESULTS: Mean (+/- SD) delta troponin T and delta CK-MB levels were 9.35 +/- 7.83 ng/ml and 125 +/- 83 mU/ml in group 1 and 3.23 +/- 3.08 ng/ml and 130 +/- 137 mU/ml in group 2, respectively, 60 min after treatment and were 10.1 +/- 8.35 ng/ml and 131 +/- 84 mU/ml in group 1 and 6.84 +/- 8.30 ng/ml and 158 +/- 146 mU/ml in group 2, respectively, 60 min after reperfusion. These values were significantly higher than those 60 min after treatment in group 3: 0.16 +/- 0.19 ng/ml and 10 +/- 9 mU/ml, respectively. The predictive accuracy for detecting reperfusion using a threshold value of 0.50 ng/ml of delta troponin T and 25 mU/ml of delta CK-MB was 100% in group 1 and 92% in group 2 60 min after treatment, respectively. There was significant correlation between delta troponin T and delta CK-MB. CONCLUSIONS: Serial measurements of cardiac troponin T as well as of CK-MB are useful for early assessment of reperfusion therapy.

Rapid diagnosis of coronary reperfusion by measurement of myoglobin level every 15 min in acute myocardial infarction.

OBJECTIVES: The purpose of this study was to examine whether coronary reperfusion can be diagnosed rapidly and accurately by myoglobin measurements. BACKGROUND: When intravenous thrombolysis is used for acute myocardial infarction, it is important to determine coronary reperfusion rapidly and noninvasively so that further treatment can be initiated. METHODS: We determined myoglobin, creatine kinase (CK) and creatine kinase, MB fraction (CK-MB) isoenzyme levels in 63 patients with acute myocardial infarction with total occlusion of the infarct-related artery that was confirmed by coronary angiography. Myoglobin was measured by turbidimetric latex agglutination, which has an assay time of 10 min. We measured myoglobin, CK and CK-MB every 15 min in 45 patients with and 18 patients without reperfusion. The condition of the infarct-related artery was confirmed every 5 to 8 min by coronary angiography. RESULTS: The rate of increase in myoglobin, CK, and CK-MB at 15, 30, 45 and 60 min after treatment and reperfusion was significantly higher in the reperfused than in the nonreperfused group. In the reperfused group, the rate of increase in myoglobin was significantly higher than the corresponding rate of increase in CK and CK-MB at 15, 30 and 45 min after reperfusion. When reperfusion was evaluated on the basis of a cutoff level (myoglobin > or = 2.0, CK > or = 1.8, CK-MB > or = 1.5), the predictive accuracy of myoglobin (95%) was significantly higher than that of CK (68%) and CK-MB (73%) at 15 min after reperfusion. CONCLUSIONS: Coronary reperfusion can be rapidly and accurately detected by measurement of the plasma myoglobin every 15 min.

The anthocyanin responsible for purplish blue flower colour of Aconitum chinense.

The anthocyanin in the purplish blue flowers of Aconitum chinense was identified as delphinidin 3-rutinoside-7-O-(6-O-(4-(6-O-(4-hydroxybenzoyl)-beta-D-glucosyl)oxyb enzoyl- beta-D-glucoside), violdelphin by means of chromatography, FAB-MS including MS-MS, 1H and 13C NMR. The anthocyanin is stable in neutral or slightly acidic aqueous solution and the absorption spectra in the pH range of 4.0-6.2 exhibited three characteristic bands at 534, 569 and 620 nm, which are practically identical with those of purplish blue petals of A. chinense. Thus the purplish blue flower colour is manifested by intramolecular co-pigmentation of the anthocyanin.

Regulation by androgen of levels of the beta subunit of nerve growth factor and its mRNA in selected regions of the mouse brain.

Our previous studies showed that the concentration of the beta subunit of nerve growth factor (beta-NGF) in nervous tissues is higher in male than in female mice. To identify the brain regions that are affected by androgens, the amounts of beta-NGF protein and its mRNAs were measured in male, female, and castrated male CD-1 mice and testicular feminization mice at 3-4 months of age. Among tissues examined, the hypophysis of males contained the highest average concentration of beta-NGF protein. In most regions of the brain, individual levels were more variable in males than in females. However, after the castration, such variations in beta-NGF levels disappeared. Average levels of beta-NGF protein in males were higher in the cerebellum (eightfold higher), olfactory bulb (12-fold higher), hypothalamus (sixfold higher), and hypophysis (72-fold higher) than those in corresponding regions of females. No significant differences were observed in levels of beta-NGF protein in the hippocampus, cerebral cortex, striatum, septum, and brainstem. The castration of male mice caused a reduction in levels of beta-NGF protein in the hypothalamus and hypophysis, but not in the cerebellum and olfactory bulb, to the female levels. The concentrations of beta-NGF protein in testicular feminization mice were similar to those in female CD-1 mice in all regions.(ABSTRACT TRUNCATED AT 250 WORDS)

Accumulation of blood-borne horseradish peroxidase in medial portions of the mouse hippocampus.

The intracerebral distribution of intravenously injected horseradish peroxidase (HRP) in young adult DDD mice was examined. HRP-tetramethylbenzidine reaction products were observed in the medial portions of the hippocampus, particularly the medial CA1 region and medial dentate gyrus. Reaction products were observed in the subfornical organ in mice decapitated 5 min after HRP injection, and then also progressively more caudally in the medial portions of the hippocampus as postinjection survival time increased. These findings suggest that blood-borne macromolecules have ready access to the medial portions of the hippocampus, particularly the medial CA1 region and medial dentate gyrus.

Serotonin relaxes porcine pial veins.

The effect of serotonin [5-hydroxytryptamine (5-HT)] on pial venous tone of the pig was examined using in vitro tissue bath techniques. Isolated pial venous rings exhibited spontaneous rhythmic contractions (SRC) on mechanical stretching and/or applications of several vasoactive substances, including norepinephrine. On the other hand, KCl induced sustained active muscle tone (SAT) without SRC. The SRC induced by mechanical stretching were not affected by tetrodotoxin, nitro-L-arginine, alpha- and beta-adrenergic, histaminergic, and muscarinic receptor antagonists, indicating that the SRC in porcine pial veins are of myogenic origin. The SRC induced by stretching or applications of vasoactive substances and SAT induced by KCl were inhibited by 5-HT in a concentration-dependent manner. The inhibition was prevented by methysergide and methiothepin but not by ketanserin, propranolol, 3 alpha-tropanyl-1H-indole-3-carboxylic acid ester, hemoglobin, or nitro-L-arginine. The SRC and SAT were inhibited by 5-carboxamidotryptamine (5-CT), 8-hydroxy-2-di-N-propylaminotetralin HBr (8-OHDPAT), 1-[3-(trifluoromethyl)phenyl]piperazine (TFMPP), and 5-methoxytryptamine (5-MT), but not by sumatriptan, alpha-methylserotonin, or 2-methylserotonin. On the other hand, 5-CT, 8-OHDPAT, TFMPP, 5-MT, and sumatriptan constricted the porcine pial arteries exclusively. In 15% of pial venous preparations examined, 5-HT at low concentrations induced ketanserin-sensitive constrictions. These results indicate that the porcine pial venous smooth muscle contains multiple subtypes of 5-HT receptors. The 5-HT inhibition of SRC and SAT is predominant and is mediated by 5-HT1-like receptors, which, however, do not seem to correspond to 5-HT1A, 5-HT1B, 5-HT1C, 5-HT1D, 5-HT1E, or 5-HT1F receptor subtypes.(ABSTRACT TRUNCATED AT 250 WORDS)

Nodular glomerulosclerosis of unknown origin associated with the nephrotic syndrome.

A 35-year-old male patient clinically characterized by massive proteinuria and hypertension without evidence of systemic diseases is reported. Histological investigation of renal biopsy specimens revealed extensive nodular formations in the mesangial areas in every glomerulus. Light-microscopic examination did not allow discrimination between the glomerular changes found in these specimens and the nodular glomerulosclerosis described in patients with diabetes mellitus. Electron-microscopic examination confirmed the presence of massive, nodular, mesangial expansions consisting of finely fibrillar substances without electron-dense deposits and circumferential mesangial interposition. Immunofluorescent examination showed deposition of IgG, C3, fibrinogen and kappa and lambda light chains in mesangial areas, peripheral capillary loops and a part of the nodules. Furthermore, collagen types IV, V, VI and laminin were detected in the nodules. Amyloid was not observed in these nodules. This diagnosis has not been made, and the mechanism of this nodular glomerulosclerosis remains unknown.

A B cell lymphoma line, BALL-1 stimulates T cells to produce a unique eosinophil chemotactic factor.

Human mononuclear leukocytes (MNL), probably OKT4-positive T cells, produced an eosinophil chemotactic factor (ECF) when they were cocultured with irradiated BALL-1, a B cell lymphoma line. Treatment of MNL, with anti-IL-2 antibody failed to suppress BALL-1-induced ECF production. Periodate-lysine-paraformaldehyde-fixed but not acetone- and ethanol-fixed BCLL induced evident ECF production. These results suggested that some cell surface molecules play a role in the induction of ECF production. Isoelectric point of BALL-1-induced ECF was around pH7, whereas that of IL-2-induced ECF was around pH 5. The molecular weight of BALL-1-induced ECF was between 10 and 30 kD. Although a combination of MoAb against IL-3, IL-5, and GM, CSF suppressed the activity of IL-2-induced ECF, it failed to suppress that of BALL-1-induced ECF. Furthermore, BALL-1-induced ECF suppressed fMLP-induced respiratory bursts of eosinophils, while IL-2-induced ECF failed. We propose that at least one reason for eosinophil infiltrate into the stroma of tumors is that the tumor cells stimulate T cells to produce BALL-1-induced ECF, and the eosinophils attracted by the ECF exhibit different functions from those by other ECF.

Selective regulation of chemotactic lymphokine production by monocytes and macrophage cell line cells.

Effects of lipopolysaccharides (LPS) on the production of chemotactic lymphokines for eosinophils and monocytes (ECF and MCF) from antigen- or mitogen-stimulated T cells were examined. Supernatants from monocytes stimulated with various LPS regulated concanavalin A (Con A)- and purified protein derivative (PPD)-induced ECF or MCF production. The regulation varied with LPS used for monocyte stimulation. The supernatant from monocytes stimulated with LPS from Escherichia coli, J-5 strain, selectively potentiated ECF production, whereas that from Salmonella minesota potentiated MCF production. In contrast, supernatant from monocytes stimulated with LPS from Salmonella typhimurium potentiated both ECF and MCF production, whereas that from Vibrio cholerae failed to potentiate production. The supernatants from monocytes stimulated with lipid A of S. typhimurium, S. minesota, and E. coli, however, failed to potentiate ECF and MCF production. The potentiating activity for each lymphokine was recovered from fractions ranging in molecular weight between 10 and 20 kD. Further purification with isoelectric electrophoresis revealed that the potentiating activity for the production of chemotactic lymphokine for eosinophils has a pI value of about 4-5, and that the activity for MCF production is detected in two fractions of pI about 5-6, and 7-8. Macrophage lineage cell line cells, such as THP-1 and U-937, also release similar factors after differential stimulation.

Chemotactic heterogeneity of eosinophils in Kimura's disease.

We evaluated the chemotactic heterogeneity of eosinophils in Kimura's disease. Patients with Kimura's disease were divided into two groups according to their clinical findings: one group had no other symptoms (KD), and another was accompanied with atopic dermatitis (KD + AD). The chemotactic response of eosinophils from two groups to 5 eosinophil chemotactic factors (ECF) derived from STO-2, an established T cell line. Eosinophils from KD were attracted only by ECF-PI5 and PI6 but not by ECF-PI7, PI8 and PI9. On the other hand, eosinophils from KD + AD responded to all 5 ECF. Eosinophils were further fractionated into normodense and hypodense eosinophils, and assessed for their chemotactic response. We thus found that there was little essential difference in their chemotactic responses to STO-2-derived ECF except ECF-PI9, though random migration of hypodense eosinophils was enhanced. The hypothesis that hypodense eosinophils are in the activated form was not always true, especially in the chemotactic response to ECF.

Effects of eosinophilotropic cytokines on differentiation of an eosinophil cell line, YY-1.

Differentiation of an eosinophilic cell line, YY-1, was induced by treatment with 0.3 mM butyric acid (BA). The effects of eosinophilotropic cytokines, such as IL-3, IL-5, and GM-CSF on BA-induced differentiation, especially in the chemotactic response to ECF, was examined. Five STO-2-derived ECFs, IL-5, C5a, and fMLP were used as eosinophil chemoattractants. The effects of the cytokines on the chemotactic response of YY-1 greatly differed according to the ECF used. It was thus suggested that heterogeneity in the chemotactic response of eosinophils depends on the cytokines which act on eosinophils during differentiation.

Differential aging pattern of cerebral accumulation of radiolabeled glucose and amino acid in the senescence accelerated mouse (SAM), a new model for the study of memory impairment.

Using an accelerated senescence-prone model mouse strain, SAMP8, with spontaneously occurring age-related deficits in learning and memory, cerebral glucose and amino acid accumulation were investigated to study the metabolic abnormalities in relation to age. The findings were compared with those in an accelerated senescence-resistant mouse strain, SAMR1, without deterioration of ability in learning and memory. [14C]-2-Deoxyglucose accumulation in the SAMP8 brain was normal at 1 month of age but decreased from 2-3 months of age onwards. In contrast, tyrosine accumulation was unchanged from 1 to 5 months of age. The impairment of memory in the SAMP8 at 2-3 months of age corresponded with the decrease in [14C]-2-deoxyglucose accumulation, but was not related to Tyr. This animal model may help provide new information on the metabolic changes in aging.

Enhancement of the intestinal absorption of a cyclosporine derivative by milk fat globule membrane.

A cyclosporine derivative, dihydrocyclosporine D, was used for the evaluation of milk fat globule membrane (MFGM) as an emulsifier of lipophilic cyclopeptides. As compared with olive oil formulation, MFGM emulsion significantly enhanced the blood and lymphatic fluid concentrations of the cyclosporine derivative after intraduodenal dosing in rats. Thus, it was suggested that MFGM can be used as an intestinal absorption enhancer of cyclosporines.

Calcein release from temperature-sensitive liposome with or without stirring.

Characteristics of the calcein release from temperature-sensitive liposomes with or without stirring were studied. Calcein release from DPPC(dipalmitoylphosphatidylcholine) REV(reverse phase evaporation vesicle) with stirring occurred at a temperature lower by 8 degrees than that without stirring. DPPC-DSPC(distearoylphosphatidylcholine)-mixed REV showed the maximum release(%) for calcein, near 40 degrees C with stirring and 45 degrees C without stirring.

Impaired endothelium-dependent vasodilatory responses in hindlimb blood flow in dogs with congestive heart failure.

The vasodilatory response is known to be diminished in chronic heart failure. In the present study, we used dogs with chronic heart failure to investigate whether endothelium-dependent vasodilation in the resistance vessels in the hindlimb is decreased in chronic heart failure, and if so, to determine if endothelium-derived relaxing factor (EDRF) is involved in the mechanism. We induced heart failure in dogs by continuous rapid ventricular pacing. Under anesthesia, an internal carotid artery-femoral artery bypass system was established and perfusion pressure to the femoral artery was kept constant. Changes in hindlimb blood flow in response to various concentrations of acetylcholine (ACh), adenosine 5'-diphosphate disodium salt (ADP) and nitroglycerin (NTG) were examined in dogs with and without heart failure. The vasoconstrictive responses to L-NG-monomethyl L-arginine monoacetate (L-NMMA) were also studied. In dogs with heart failure, vasodilatory responses to both of the endothelium-dependent vasodilators, ACh and ADP, were decreased, while there were no differences in the responses to the endothelium-independent vasodilator NTG. The vasoconstrictive response to L-NMMA was diminished in heart failure. Thus, endothelium-dependent vasodilation in hindlimb resistance vessels is impaired in dogs with congestive heart failure. This impairment may be related to diminished EDRF/NO production or release in these vessels.

[A case of immature teratoma and yolk sac tumor of the testis associated with pulmonary metastases of 2 distinct histological types].

A 24-year-old man presented with right scrotal swelling and was diagnosed as having a testicular cancer. A high inguinal orchiectomy was carried out. Histological examination revealed immature teratoma and yolk sac tumor of the testis. Eight months later, 2 separate metastatic lesions appeared in the lungs, both of which were removed by operation after a course of combination chemotherapy with Etoposide and Cisplatin. The histology showed one to be composed of immature teratoma, and the another of yolk sac tumor.

Cytostatin, a novel inhibitor of cell adhesion to components of extracellular matrix produced by Streptomyces sp. MJ654-NF4. I. Taxonomy, fermentation, isolation and biological activities.

Cytostatin has been identified as a novel inhibitor of cell adhesion to components of extracellular matrix (ECM) in cultured broth of Streptomyces sp. MJ654-NF4. Though cytostatin did not inhibit EL-4 cell adhesion to ECM components such as laminin and fibronectin; it inhibited the adhesion of B16 melanoma cells to laminin and collagen type IV but not to fibronectin. It exhibited antimetastatic activity on B16 melanoma cells in mice. The cytotoxicity of cytostatin are also reported.
