Functional activation of cerebral blood flow and metabolism before and after global ischemia of rat brain.

The effect of somatosensory stimulation on the local CBF (LCBF), CMRglu (LCMRglu), tissue pH, and tissue content of ATP, glucose, and lactate was studied in chloralose-anesthetized rats before and after 30 min of near-complete forebrain ischemia. In nonischemic rats LCBF in primary somatosensory cortex increased by 33%, LCMRglu increased by 55%, tissue glucose content decreased by 21%, and lactate increased by 30%. Local ATP and tissue pH did not change. Functional activation of the intact chloralose-anesthetized rat, in consequence, is associated with the stimulation of "aerobic" glycolysis but does not result in disturbances of energy or acid-base homeostasis. After 30-min ischemia and 3-h recirculation, somatosensory stimulation did not evoke any metabolic or hemodynamic alterations, although EEG and primary somatosensory evoked potentials recovered. The maintenance of normal energy state despite constant metabolic rate suggests that the postischemic generation of evoked potentials does not require measurable amounts of energy. Stimulation of glycolysis in the intact animal, therefore, may serve other purposes than fueling the energy requirements of evoked cortical activity.

[A study of the expression of class II antigen on uterine cervical cancer cells and cell lines].

An immunohistochemical method with monoclonal antibody to DR antigen was used to study class II antigens and HLA-DR antigen in 20 patients with uterine cervical cancer. Eight patients had various amounts of DR antigen. Regional infiltrating lymphocytes were also examined with anti-Leu 1, Leu 2a, Leu 3a, and Leu 10. Among the many infiltrating T cells, Leu 3a-positive cells were relatively predominant surrounding the cancer nests which contained DR antigen. Cervical cancer cell lines OMC-1 and OMC-4 both demonstrated DR antigen. Interferon (IFN)-gamma treatment enhanced the expression of DR antigen in OMC-1 and OMC-4. The DR antigen in OMC-1 and OMC-4 were capable of stimulating allogenic lymphocytes in mixed lymphocyte reaction (MLR), and their stimulatory activity was significantly enhanced by IFN-gamma treatment. These results demonstrated the expression of class II antigen, especially DR, on some cervical cancer cells and cell lines and showed that the DR antigen in uterine cancer cell lines can stimulate MLR.

[Clinical evaluation of contact Nd.YAG laser therapy of dysplasia and early cancer of the uterine cervix].

For the past four years, cone and cone with cautery have been carried out by the contact Nd.YAG laser method after diagnosis with colpo.cervicoscopy in 174 cases of dysplasia and early cancer of the uterine cervix. The subjects were 82 patients with dysplasia, 75 patients with carcinoma in situ and 17 patients with microinvasive carcinoma. The cure rate based on the follow up for 6 to 50 months was 97.7%. There were 7 (4.1%) residual cases, 6 with residual mild dysplasia that required a follow up. Three cases with mild dysplasia disappeared during a follow up. In the remaining case, a semiradical hysterectomy was performed owing to Ib"occ" (8mm). Postoperative histological findings confirmed 76.2% of the preoperative findings; the rate of underdiagnosis was 23.8%. The rates of incomplete excision and underdiagnosis decreased in the cases where preoperative biopsy findings agreed with or overestimated cytologic and colpo.cervicoscopic findings. The results suggest that the laser cone is very useful as conservative therapy for early cervical neoplasia, and the cone specimen can be used to evaluate the success of the therapy and to improve future preoperative diagnosis.

[Immunohistochemical studies on epidermal growth factor receptor in experimental squamous cell carcinoma of the uterine cervix of mice].

The tissue localization of epidermal growth factor receptor (EGF-R) in experimental squamous cell carcinoma of the mouse uterine cervix was examined immunohistochemically. Carcinoma was induced by the insertion of a 20-methylcholanthrene (MC)-impregnated thread into the cervical canal of the mouse. Tissue sections (of normal columnar epithelium, proliferation, atypical proliferation, early invasive carcinoma, invasive carcinoma and metastatic carcinoma) were stained by the avidin/biotin immunoperoxidase technique using anti-EGF-R monoclonal antibody. Normal columnar epithelium was negative for EGF-R, whereas proliferation was partly positive. The lesions of atypical proliferation and early invasive carcinoma had a positive staining for EGF-R. The staining for EGF-R declined in the lesion of invasive carcinoma. Metastatic carcinoma was not stained for EGF-R. These results suggest that EGF-R may play an important role in the early stage of carcinogenesis of the mouse uterine cervix induced by 20-MC.

[EGF receptor and effects of EGF on growth and tumor marker secretion in uterine cervical cancer cells].

Effects of EGF on proliferation and tumor marker secretion of cervical cancer cells are reported together with the characteristics of EGF receptors on the cells. TA-4 producing cell line (OMC-1) originating from cervical squamous cell carcinoma and CA-125 producing cell line (OMC-4) originating from cervical adenocarcinoma, were used. Scatchard plot of EGF binding to OMC-1 indicated a single class of binding sites with a dissociation constant (Kd) of 360pM, whereas that of OMC-4 was curvilinear suggesting two classes of binding sites with a Kd of 170pM and 510pM. The theoretical maximum number of binding sites of OMC-1 and OMC-4 was 2.4 X 10(4) and 1.6 X 10(5), respectively. Effects of EGF on growth were studied by monitoring cell number and the incorporation of 3H-thymidine into the DNA of the cells. OMC-1 was stimulated by EGF at low concentrations (0.01-0.1nM) and inhibited at higher concentrations. OMC-4 was not stimulated by EGF. The TA-4 secretion of OMC-1 was slightly stimulated by EGF at low concentrations (0.01-1nM) and significantly stimulated at high concentration (10nM). The CA-125 secretion of OMC-4 was not stimulated by EGF. These results suggest that there are some differences between cervical squamous cell carcinoma and adenocarcinoma in the mechanisms of regulation of proliferation and tumor marker secretion by EGF.

Stromal reactions to squamous cell carcinoma of the cervix.

The characteristics of the stromal reaction to invasion by cancer were studied in 23 cases of cervical squamous cell carcinoma (stage Ib to IIb) and 86 control cases of benign disease of the uterus. Tissues taken from sites adjacent to tumor or from comparable sites in the nonmalignant uteri were analyzed for number of round cells, density of collagen and reticular fibers, and collagenase activity. The results were correlated with the depth of invasion and the histologic appearances of the tissues adjoining the cancer. Plasma cells and reticular fibers were most numerous in patients with slight invasion and no direct contact between the malignant and benign tissues (C type in CPL classification of Imai). Glycosaminoglycans (mainly hyaluronic acid and chondroitin sulfate) were present in the greatest amounts in C type stroma. The prognosis showed no correlation with either collagen content or collagenase activity but it was a better prognosis correlated with the collagen/collagenase ratio. It is concluded that increased numbers of plasma cells and reticular fibers and a high collagen/collagenase ratio are among the mechanisms protective against the invasiveness of cervical carcinoma.

Vector U loop in patients with right ventricular overloading.

The U loop of the vectorcardiogram (VCG) was examined qualitatively and quantitatively in 126 normal subjects, 15 subjects with complete right bundle branch block (CRBBB group) and 58 patients with right ventricular overloading (RVO group), using a direct-writing vectorcardiograph with memory function. In normal subjects the U loop was directed similarly to the T loop, i.e., to the left, anteriorly and inferiorly. In the CRBBB group, maximum U vector was smaller, but its direction was not significantly different from that in normal subjects. In the RVO group, the U loop tended to be displaced posteriorly and to the left and was significantly greater in magnitude than that in normal subjects in the horizontal (P less than 0.01) and frontal (P less than 0.001) planes. In the RVO group, a good correlation was found between the direction of maximum U vector and right ventricular systolic pressure. In some cases of the RVO group, the U loop was the only abnormality suggesting right ventricular overloading. These findings suggest that abnormality of the U loop is a good indicator in a diagnosis of right ventricular overloading.

Individualization of patients for adjuvant chemotherapy after surgical treatment of cervical cancer.

Adjuvant chemotherapy using Tegafur (1-(2-tetra hydrofuryl)-5-fluorouracil) was given to 52 of 216 patients with recurrence risk of cervical cancer after primary surgical treatment, the degree of risk of recurrence having been formulated as a discriminant function by computer analysis of risk factors involved in disease recurrence. Using the formula, we have managed outpatients in three groups--no recurrence and recurrence after, and before 5 years. Under existing circumstances the power of the discriminant function we have achieved is not yet entirely satisfactory although clinically of value in patient management. Oral, long-term adjuvant chemotherapy in risk groups was of value, with a 37.4% instantaneous recurrence rate and mild side-effects. An instantaneous recurrence rate of 37.4% signifies that the recurrence rate in the group of patients given chemotherapy would be 37.4% if it were 100% in the patients not so treated--an improvement factor of just less than 3.

Small, bilateral, calcified intrarenal aneurysms--a case report.

With increased use of renal angiography and ultrasonography, intrarenal aneurysms and arteriovenous malformations have been easily diagnosed. The authors report a case of small, bilateral intrarenal aneurysms with calcifications that were associated with a visible pulsatile wall motion recorded by M-mode ultrasonography. The finding of selective renal angiography was consistent with bilateral intrarenal aneurysms. Accelerated atherosclerosis and long-term history of hypertension were thought to have caused them.
