[Clinical studies with a new non-ionic contrast medium (iopamidol) in intravenous pyelography in aged patients].

Iopamidol (IOPAMIRON 300) was examined for excretory urography on 21 patients, between 70 and 94 years old, with reference to visualization, safety and urinary excretion. The results about visualization and safety were compared with those obtained with 60% Urografin. Iopamidol was demonstrated visualization nearly equivalent to that with Urografin, and 19% better pyelogram. Especially, calix showed a clearer contrast than those obtained with Urografin. None of the 21 old patients experienced any side effects or adverse reactions. No abnormalities were noted in serum chemistries. Iopamidol was not quickly excreted into the urine, only 19% during the first 2 hours, in elder patients.

[A case report of multiple endocrine neoplasia type 2b].

A 10-year-old girl was admitted displaying a medullary thyroid carcinoma, accompanying neurinomas of the tongue, marfanoid habitus, megacolon, and scoliosis. Although her adrenal glands were found to be unremarkable on both CT and ultrasonogram examination an MIBG scintiscan showed a 131-1 uptake, suggesting the presence of medullary hyperplasia. Remarkable nodal involvement with invasive features witnessed in the upper mediastinum during tertiary surgery four years later, following a total thyroidectomy. The TCT and CEA profiling was not sufficient enough to predict a recurrence. At the present state there is difficulty in providing an early diagnosis; through node dissection, however, is considered to be necessary at time of primary thyroid operation in cases with MEN type 2b.

[A case of malignant paraganglioma arising from the retroperitoneum].

We report the case of 38-year-old male with a malignant and functional paraganglioma arising from the retroperitoneum. He was admitted to our hospital with a pathological fracture of the right femur, A diagnosis of paraganglioma was made after taking a biopsy specimen and discovering a hypervascular retroperitoneal tumor. The level of catecholamine in his urine was greatly increased, accompanied with hypertension, rapid pulse and sweating. He was treated with chemotherapy, radiotherapy and surgical therapy. However, he died from pulmonary metastasis.

[Fever attack in percutaneous nephrolithotomy and transurethral ureterolithotripsy].

We studied the fever attacks and their factors in the cases of percutaneous nephrolithotomy (PNL) and transurethral ureterolithotripsy (TUL). In PNL, fevers were significantly higher and more frequent in the cases with infected urinary stones than those with sterile stones, but prophylactic administration of antibiotics did not lower the fever or its frequency of attack. There was no significant difference between the cases of single procedure and those of multiple procedures although multiple procedures were likely to increase the frequency of fever attacks. In TUL, single J-catheter indwelling decreased fever attacks, but infection and administration of antibiotics did not affect the fever significantly. However, careful control of urinary tract infections before and after operation and of intrarenal pressure during operation are thought to be most important for prevention of fever attacks and urosepsis.

[Transurethral ureterolithotripsy using a rigid ureteroscope].

Eighty patients with ureteral stones underwent transurethral ureterolithotripsy using a rigid ureteroscope. In 68 of the patients (85%), the stones were removed successfully. Most of the 12 failures were removed by a percutaneous approach. The main complications of transurethral ureterolithotripsy were high fever (35%), ureteral injury (10%) and ureteral stricture (1.6%). Ureteroscopic procedures are thought to be useful in stone removal and diagnostic evaluation of abnormality of upper urinary tract.

[Clinical statistics on inpatients and operations in the urological department of the Omori Red Cross Hospital (January, 1981 to December, 1985].

The statistics obtained for the 716 inpatients and 550 operations at our clinic between January 1981 and December 1985 were reviewed. The patients were most frequently in their sixties, and there was a predominance in males. The most frequent diseases of the inpatients were benign prostatic hypertrophies and bladder tumors. The most performed operations were also those for urological tumors.

[Usefulness of the bladder assist device (BLAD)].

We investigated the effectiveness of the Bladder Assist Device on urinary tract infection of patients with indwelling catheters. The patients, except for those with cord bladder and vesicoureteral reflux, could urinate intermittently under the BLAD system. Irrigation of the bladder using BLAD made urine clear and decreased urinary bacteria. Because of intermittent urination in patients who had been using the BLAD system for two weeks, the urine became somewhat clear and the number of bacteria decreased. Combination of bladder irrigation and intermittent urination using this system may inhibit urinary tract infection of an indwelling catheter. BLAD is considered to be a useful device for patients with continuous indwelling catheters.
