Solitary intraosseous neurofibroma of the mandible: report of a case.

A case of solitary intraosseous neurofibroma of the mandible is reported. The tumor was located in the body of the right mandible. Segmental mandibulectomy including the surrounding tissue was performed and the mandible was reconstructed with the free scapular osteocutaneous flap. From histological examination of the resected mandible, typical findings of a tumor invading the adjacent bone along the mandibular nerve were observed. Immunohistochemical examination of the tumor showed positive staining of the tumor cells for anti-S100 protein antibodies. Numerous nerve-like fibers were also noted by Bodian stain.

[Experimental acute lung injury in guinea pigs after aerosol challenge with sonicated Pseudomonas aeruginosa whole cells].

Acute hemorrhagic alveolitis was elicited in the lungs of guinea pigs by aerosol challenge with sonicated P. aeruginosa whole cells. Histological findings showed the severe inflammatory changes, which were characterized by the inflammatory infiltration of alveolar macrophages (AM), polymorphonuclear leukocytes (PMN) and eosinophils in the peribronchial and the alveolar space at 8 hours after the challenge. A marked increase of PMN as well as AM was noted in the bronchial alveolar lavage fluid (BALF) at 8 hours after the challenge and the increase of the numbers of AM and PMN in the BALF remained until 24 hours after challenge. The complement titer (CH50) and C3 component in the serum decreased at the early stage, and CH50 maintained low level and C3 component increased gradually. Polyethyleneglycol precipitation-complement consumption test (PEG-CC) of the BALF showed the existence of immune complexes formed in the airway after aerosol inhalation. These data suggest that the immune complexes of P. aeruginosa activate the complement system in the lungs, which is followed by the infiltration of inflammatory cells. Guinea pigs which were pretreated with cobra venom factor significantly reduced the extent of the inflammatory changes in the lungs. The results suggests that the complement system might act as an important factor of the acute lung injury in this model.

[Subacute subdural hematoma--reexamination of mechanism by CT and MRI findings].

We have already reported about the importance of establishing the concept of subacute subdural hematoma. But the mechanism by which this disease develops has not as yet been elucidated fully. In one case of subacute subdural hematoma, we were able to perform CT and MRI over time and obtained findings which were of use in studying the mechanism of development. The case was a 56-year-old male. He developed with seizure. CT on admission revealed acute subdural hematoma and subarachnoid hemorrhage. But neurological deficits were absent. So he was treated conservatively. On the 16th hospital day there appeared seizure, anisocoria and an increase in the mass sign due to subdural hematoma was noted on CT, so a diagnosis of subacute subdural hematoma was made. Cerebrospinal fluid was considered accountable for the increase in the mass sign judging from the findings of CT and MRI each performed over time. It was surmised that subdural effusion developed concurrently in the subacute stage. Three conditions, namely, presence of (1) arachnoid tear, (2) clots, (3) no intracranial hypertension are considered important as the mechanism by which subacute subdural hematoma develops.

[Targeting therapy using monoclonal antibody directed against growth factor receptors].

Targeting therapy is expected to be a new effective anti-cancer treatment in near future. Major advances are achieved by considerable effort in this area. Monoclonal antibodies that recognize tumour-associated antigens were conjugated to chemotherapeutic drugs, toxins and radionuclides with various procedures. To apply this new therapy for clinical use, many problems still remain to be clarified. These problems consist of human anti-mouse antibody, antigenic heterogeneity and low tumour uptake. By our experiment, growth factor receptor was an ideal target for targeting therapy and immunoconjugate directed against growth factor receptor showed selective cytotoxicity to cancer cells with expression of growth factor receptor. Moreover, cytotoxic effect was positively correlated with expression level of growth factor receptor.

Characterization of epidermal growth factor (EGF) receptor and biological effect of EGF on human uterine cervical adenocarcinoma cell line OMC-4.

In this paper, we investigated how epidermal growth factor (EGF) acts on growth and regulation of extracellular matrix components and their degenerative enzymes in uterine cervical adenocarcinoma cells in vitro. Effects of EGF on cell growth, DNA synthesis, and laminin, collagen IV, and tissue plasminogen activator (t-PA) production of human uterine cervical adenocarcinoma cell line OMC-4 were examined, together with the characteristics of its EGF receptors. Scatchard plot of EGF binding to OMC-4 indicated two classes of binding sites with a dissociation constant of 170 pM and 510 pM. The total binding sites were 1.6 x 10(5)sites/cell. The number of OMC-4 cells did not increase in the presence of EGF, whereas 3H-thymidine incorporation was inhibited by EGF at concentrations of 1 and 10 nM. The production of laminin and collagen IV by OMC-4 cells was inhibited by EGF, whereas that of t-PA was significantly promoted at the physiological concentration of 0.1 nM. These results suggest that EGF is closely associated with regulation of proliferation and extracellular matrix degradation of uterine cervical adenocarcinoma cells.

[Clinical study on judgement of brain death by the nasopharyngeal lead with the aid of an automatic EEG analysis system].

We investigated the relationship between scalp electroencephalogram (EEG) and nasopharyngeal EEG using Automatic EEG Analysis System in 10 patients with the age between 17 and 66 who were clinically presumed brain death because of flat line EEG and loss of auditory brainstem response (ABR). Among 10 patients, nasopharyngeal lead EEG showed low voltage and slow waves of 10-20 microV in 4 cases and flat line EEG in 6 cases. However, Automatic EEG Analysis system-assisted analysis of EEGs recorded on a magnetic tape revealed that equivalent electric potentials in the scalp EEG from these patients were smaller than those of healthy volunteers, but those in the nasopharyngeal EEG from all patients showed low voltage within the range of distribution in normal volunteers (delta:6, theta 1:6, theta 2:2, alpha 1:1, alpha 3:10, beta 1:6 in 10 cases). The present results show that in addition to the routine scalp EEG, it is clinically useful to employ simple and easy nasopharyngeal lead EEG method and to analyze data using Automatic EEG Analysis System for the judgement of brain death.

[Clinical experience in 3 cases, 5 events of thrombosed Björk-Shiley mitral prostheses].

We have successfully treated 3 cases, 5 events of thrombosed Björk-Shiley mitral prostheses. Case 1: Treatment with urokinase thrombolytic therapy failed to improve the valve opening, and then submergent surgical thrombectomy was performed successfully. Case 2: This case was recurrent thrombosed valve. First time thrombosis was successfully managed by urokinase thrombolytic therapy. Second time thrombosis was treated with emergent surgical thrombectomy. Third time thrombosis was treated with urokinase thrombolysis and elective replacement of the Björk-Shiley valve with a 25 mm SJM valve. Case 3: Urokinase thrombolytic therapy failed to improve the valve function, and then emergent replacement of the Björk-Shiley valve with a 25 mm SJM valve was performed successfully. These cases suggested that the surgical treatment for the thrombosed Björk-Shiley mitral valve should be performed without delay in the patient whose condition is rapidly deteriorating.

[Clinical and pathological studies on superficial esophageal carcinoma by flow cytometric analysis of nuclear DNA content].

Correlation between histological type, depth of invasion, nodal involvement, macroscopic type, survival rate and nuclear DNA content was studied by flow cytometer using paraffin-embedded materials from 52 cases of superficial esophageal carcinoma. There were 23 cases (44.2%) of diploid and 29 cases (55.8%) of aneuploid. There was no correlation between DNA aneuploidy and depth of invasion or size of the carcinoma. Carcinoma with aneuploid had a high frequency of nodal involvement and well-differentiated histological type. Carcinoma with deep of highly elevated macroscopic type has a tendency to have higher DNA content. There was no correlation between survival rate and nuclear DNA ploidy patterns.

[A study of plasma atrial natriuretic peptide, antidiuretic hormone and cerebral vasospasms in patients with aneurysmal subarachnoid hemorrhage].

The relationship between plasma atrial natriuretic peptide (ANP) and antidiuretic hormone (ADH) both of which show high values after subarachnoid hemorrhage and cerebral vasospasm was studied. The subjects were 23 patients who were admitted because of aneurysmal subarachnoid hemorrhage during three years from March, 1989 to March, 1992 and in whom plasma ANP and ADH levels could be determined over time. Cerebral vasospasm was evaluated by the finding of cerebral angiography, clinical symptoms, and presence or not of low density areas on CT. Hyponatremia was defined as the serum sodium level of 130 mEq/l or less for two days or more. Angiographical vasospasm was found in 17 patients (85%), symptomatic vasospasm in 15 patients (65.2%), low density areas on CT in 9 patients (40.9%) and hyponatremia in 8 patients (34.8%). Symptomatic vasospasm was noted in 7 of the 8 patients (87.5%) with hyponatremia, low density areas on CT in 4 patients (50%), the detection rate being high. The plasma ANP and ADH levels were 76.7 +/- 32.1 pg/ml and 2.2 +/- 0.7 pg/ml respectively in the patients with symptomatic vasospasm against 38.3 +/- 21.3 pg/ml and 2.4 +/- 0.6 pg/ml respectively without symptomatic vasospasm, the plasma ANP level being significantly high in the patients with symptomatic vasospasm (p < 0.01). The plasma ANP and ADH were 71.2 +/- 33.8 pg/ml and 2.0 +/- 1.1 pg/ml respectively in the patients with low density areas on CT against 51.2 +/- 31.3 pg/ml and 1.8 +/- 0.5 pg/ml respectively without low density areas on CT.(ABSTRACT TRUNCATED AT 250 WORDS)

Preclinical studies for adoptive immunotherapy in bone marrow transplantation. Generation of anti-CD3 activated cytotoxic T cells from normal donors and autologous bone marrow transplant candidates.

In order to obtain T cells for adoptive immunotherapy after autologous bone marrow transplantation (ABMT) for patients with resistant hematological malignancies, a culture system was developed for growing T cells and inducing non-MHC-restricted cytotoxicity using anti-CD3 monoclonal antibody (OKT3) activation. In this investigation, we show that (1) peripheral blood lymphocytes or bone marrow mononuclear cells (BMMNC) from normal donors and cancer patients can be activated with OKT3 and grown in interleukin-2; (2) normal BMMNC activated with OKT3/IL-2 exhibited non-MHC-restricted cytotoxicity and surface markers comparable to that exhibited by normal PBL activated with OKT3/IL-2; (3) both proliferation and cytotoxic functions were IL-2-dependent; (4) PBL activated with OKT3/IL-2 after cryogenic storage grew and killed comparable to PBL activated with OKT3/IL-2 prior to cryopreservation; (5) OKT3/IL-2-activated PBL and BMMNC obtained from 5 patients with non-Hodgkin's lymphomas (NHL) and 1 patient with acute myelogenous leukemia (AML) increased cell numbers 41-75-fold in 2 weeks of culture; 5 of 6 patients with NHL or AML had PBL and BMMNC that exhibited cytotoxic activity; and (6) contaminating leukemic cells did not overgrow in OKT3/IL-2-activated cultures and could no longer be detected on cytospin specimens after 3 weeks of culture. These data show that T cells in PBL or BMMNC from ABMT candidates can be activated with OKT3/IL-2 for adoptive immunotherapy in combination with ABMT.

[Pathologic changes at the coronary artery-bypass graft anastomosis--an immunocytochemical study].

We have performed immunocytochemical investigations on proliferative tissues observed in anastomotic sites of the saphenous vein aortocoronary bypass grafts to the coronary arteries. Nine anastomotic sites were obtained from 5 necropsy patients who had died after bypass grafting. Interval between the operation and death in these patients varied from 5 days to 9 months. The anastomotic sites were fixed in methanol-Carnoy's fixative, and embedded in paraffin. Six monoclonal antibodies specific for muscle cell actin (HHF35), smooth muscle cell actin (CGA7), vimentin, desmin, macrophages (HAM56) and endothelial cells were used for the immunocytochemical study. Of the 9 anastomotic sites, 4 were totally occluded, while the remaining 5 were patent. The occluded sites were characterized by a presence of fresh or organized thrombus. Immunocytochemically, the latter was composed of macrophages, smooth muscle cells and neovascularization. In contrast, a patent site at 5 days after grafting showed an early stage of intimal proliferation composed of macrophages and spindle-shaped cells. Two patent sites at 33 days after grafting revealed a distinct intimal thickening consisting almost entirely of smooth muscle cells. At this stage, smooth muscle cells at a deeper intima were stained positive with both HHF35 and CGA7, whereas the cells at the luminal site were stained positive with HHF35 but negative with CGA7. In older lesion at 9 months after grafting, almost all smooth muscle cells within intimal thickening showed positive with HHF35 and CGA7. Moreover, these smooth muscle cells were stained positive with vimentin but were negative with desmin, irrespective of the lesion's age.(ABSTRACT TRUNCATED AT 250 WORDS)

Expression of p53 protein is an early event in ultraviolet light-induced cutaneous squamous cell carcinogenesis.

BACKGROUND AND DESIGN: p53 Is known to be a tumor-suppressor gene and has been suggested to play an important role for multistep carcinogenesis of cutaneous squamous cell carcinoma (SCC). To evaluate the role of ultraviolet light (UV) in p53 mutation in squamous cell carcinogenesis, paraffin-embedded sections of SCCs were immunohistochemically stained with the CM-1 antibody for p53 protein. Positive staining suggests mutation of the p53 gene since the mutant p53 protein gains prolonged half-life to be detectable by this method. The specimen included SCCs induced by UV (SCCs on sun-exposed areas, SCCs on patients with xeroderma pigmentosum), roentgen rays, scar, and miscellaneous causes. In addition, solar keratoses that are precancerous lesions of UV-related SCCs were also analyzed. RESULTS: Fourteen (54%) of 26 UV-related SCCs were positive, whereas five (19%) of 26 UV-unrelated SCCs were positive. Among the UV-related SCCs, five (45%) of 11 well-differentiated SCCs and nine (60%) of 15 moderately to poorly differentiated SCCs were positive. Eleven (48%) of 23 solar keratoses were positive for p53. CONCLUSIONS: These results indicate that UV acts as a mutagen for the p53 gene, and this event may occur at a relatively early stage of multistep UV carcinogenesis.

[Evaluation on our procedure for autotransplantation of parathyroid glands by the intact-PTH].

In thyroid cancer surgery since 1978 we have made parathyroidal autotransplantation which has been to resect the entire parathyroid, cut fine to mud and autotransplant into greater pectoral muscle. To evaluate our procedure and consider the mechanism of hypocalcemia and tetanus following surgery, we examined the intact-PTH recovered to about 80% of its preoperative value on 14-postoperative-days and thereafter remained almost constant. It remained below the sensitivity of measurement from immediately to 3-postoperative-days, and with our procedure, parathyroid was considered to be totally resected. Comparison between 2 (6 cases) and more autotransplanted glands (11 cases) revealed that the former showed slightly later functional recovery, but recovered to the almost some extent as the other 21-postoperative-days. Comparison between the group requiring calcium supplement therapy (12 cases) and the otherwise (5 cases) revealed almost the same course of recovery. Thus, our procedure seems to enable us to make satisfactory functional preservation and expect functional recovery by 2-gland autotransplantation at least. The supplement therapy was considered not to be detrimental to the take of autotransplanted glands and functional recovery, and no correlation was noted between the onset of tetanus and intact-PTH.

Effect of hyperbaric oxygen therapy on osseointegration of titanium implants in irradiated bone: a preliminary report.

Four patients who had undergone combined surgical and radiotherapeutic treatment for maxillofacial cancer had osseointegrated titanium implants placed in the bone of the treated region at various intervals after irradiation. Preoperatively and postoperatively, hyperbaric oxygen therapy was administered to the patients at the level of 2 or 3 atmosphere absolute (ATA). Of the 21 fixtures placed, one was lost because of lack of osseointegration, rendering the survival rate 92.3%. Hyperbaric oxygen therapy seemed to be a viable method to improve the survival rate of fixtures in irradiated bone tissue.

Effects of a novel calcium antagonist, S-(+)-methyl-4,7-dihydro-3-isobutyl-6- methyl-4-(3-nitrophenyl)thieno[2,3-b]pyridine-5-carboxylate (S-312-d), on ischemic amino acid release and neuronal injury in stroke-prone spontaneously hypertensive rats.

The effects of a novel dihydrothienopyridine Ca antagonist S-(+)-methyl-4,7-dihydro-3-isobutyl-6-methyl-4-(3-nitrophenyl)-thieno [2,3-b]pyridine-5-carboxylate (S-312-d), on the amount of amino acid release during cerebral ischemia and delayed neuronal death in the hippocampal CA1 region of stroke-prone spontaneously hypertensive rats were studied and compared with those of nimodipine. The released amino acids were measured by high-performance liquid chromatography after microdialysis. Cerebral ischemia was produced by occlusion of the bilateral common carotid arteries for 20 min. Intraduodenal administration of 0.3 mg/kg of S-312-d at 60 min before the carotid occlusion significantly decreased the ischemic release of glutamate and taurine, but did not influence their basal release. However, nimodipine did not inhibit the ischemic glutamate release even at a dose of 10 mg/kg. Similar peripheral hemodynamic effects were observed before and during bilateral carotid occlusion in groups treated with S-312-d or nimodipine. During the carotid occlusion, almost no cerebral blood flow was observed in either group. Therefore, the inhibitory effect of S-312-d on ischemic amino acid release probably arises from its potent direct action on neuronal cells. The neuronal cell densities of the CA1 subfield at 7 days after 20-min bilateral carotid occlusion significantly decreased in the vehicle-control group compared with the sham-operated group. Intraperitoneal administration of 0.1 mg/kg of S-312-d at 60 min before ischemia prevented the decrease of neuronal cell density compared with the vehicle control. These results show that S-312-d can offer marked neuronal protective effects against ischemic injury.

Peroxisomal acetoacetyl-CoA thiolase of an n-alkane-utilizing yeast, Candida tropicalis.

Two genes encoding acetoacetyl-CoA thiolase (thiolase I; EC 2.3.1.9), whose localization in peroxisomes was first found with an n-alkane-utilizing yeast, Candida tropicalis, were isolated from the lambda EMBL3 genomic DNA library prepared from the yeast genomic DNA. Nucleotide sequence analysis revealed that both genes contained open reading frames of 1209 bp corresponding to 403 amino acid residues with methionine at the N-terminus, which were named as thiolase IA and thiolase IB. The calculated molecular masses were 41,898 Da for thiolase IA and 41,930 Da for thiolase IB. These values were in good agreement with the subunit mass of the enzyme purified from yeast peroxisomes (41 kDa). There was an extremely high similarity between these two genes (96% of nucleotides in the coding regions and 98% of amino acids deduced). From the amino acid sequence analysis of the purified peroxisomal enzyme, it was shown that thiolase IA and thiolase IB were expressed in peroxisomes at an almost equal level. Both showed similarity to other thiolases, especially to Saccharomyces uvarum cytosolic acetoacetyl-CoA thiolase (65% amino acids of thiolase IA and 64% of thiolase IB were identical with this thiolase). Considering the evolution of thiolases, the C. tropicalis thiolases and S. uvarum cytosolic acetoacetyl-CoA thiolase are supposed to have a common origin. It was noticeable that the carboxyl-terminal regions of thiolases IA and IB contained a putative peroxisomal targeting signal, -Ala-Lys-Leu-COOH, unlike those of other thiolases reported hitherto.
