Activation of mitochondrial functions by malotilate in relation to accelerated liver regeneration in partially hepatectomized rats.

Malotilate was orally administered to rats at a dose of 100 mg/kg 2 hr after partial hepatectomy. Mitochondrial state 3 respiration of the liver increased significantly from 23.4 nmol/min/mg protein in the control rats to 29.3 nmol/min/mg protein in the rats administered with malotilate at 3 hr after the hepatectomy (1 hr after the administration). The administration also resulted in higher tendencies in the respiratory control ratio after 3, 6 or 20 hr (1, 4 or 18 hr after the administration) than in the control rats. Although partial hepatectomy made hepatic ATP concentration remarkably low, it gradually increased (from 1.53 mumol/g liver after 3 hr) to a level of 2.03 mumol/g liver after 20 hr in the rats administered with malotilate. No increase in ATP concentration was observed in the control rats. Correspondingly, the adenylate energy charge also showed higher tendencies in the malotilate administered rats. From these result, it is supposed that malotilate activates the mitochondrial functions which results in the increases of ATP concentration and adenylate energy charge. These changes in energy metabolism can be associated with accelerated regeneration of the liver by malotilate.

[Imaging diagnosis of head and neck cancer--diagnosis of laryngeal cancer, hypopharyngeal cancer and cancer of the cervical esophagus by CT x-ray].

Fifty-six patients with laryngeal cancer were examined. The rate of correct diagnosis of clinical staging was 64.3%. Whereas the rate of correct diagnosis of staging by CT was markedly improved, 89.3%. In particular, CT facilitated correct diagnosis of T4. Supraglottic T3 cancer is diagnosed on the basis of deep infiltration or fixation of the hemi-larynx. Disappearance of the pre-epiglottic space or ulcer formation was recognized as a CT finding of deep infiltration. Disappearance of the paraglottic space and dislocation of the arytenoid cartilage were used as the standard for CT findings of laryngeal T3 cancer. CT allowed diagnosis to be made in 84.4% of cases which were pathologically determined to be T4. It can be said that CT is a very effective method for staging diagnosis in patients with laryngeal cancer. Fifty-seven patients with hypopharyngeal cancer and cancer of the cervical esophagus were also examined. When 40 post-surgical patients were compared according to pathological classification of T, the rate of correct diagnosis of clinical staging was 77.5% while that of staging diagnosis by CT was 82.5%. The rate of correct diagnosis by postoperative CT was 80.0% compared with the pathological determination of depth of invasion. The diagnosis of depth of invasion by CT on initial examination is thus an effective means for the determination of prognosis.

[Some problems of treatment of hypopharyngeal and cervical esophageal cancer].

201 cases with hypopharyngeal and cervical esophageal cancer undergoing treatments at our hospital from 1969 to 1985 were analysed from the clinical viewpoints. 112 out of 201 cancer cases received radical operations and the resultant 5 year survival rate was 33.5%. Concerning the resection of primary tumor lesions, both procedures of pharyngo-laryngo-esophagectomy and radical neck dissection were performed for the cases with hypopharyngeal cancer. For cervical esophageal cancer, the operative procedures include partial resection of sternum and upper mediastinal dissection. As for reconstruction of cervical esophagus, we used mainly four materials such as gastric tube, free jejunum, free forearm flap and deltopectoral flap.

[Relationship between TNM-classification and survival rate in cases of carcinoma of the oral cavity: a comparison between UICC and AJCC TNM staging systems].

We compared TNM classification between UICC and AJCC in the case of oral cancer. T4 definition of UICC was indistinct and AJCC seemed better for T-classification. N-classification of AJCC was rational, since it distinguished between "solitary" and "multiple" nodes. It was inappropriate to equalize T3N0 for T3N1 in stage grouping and also to regard T4N0 as Stage IV because T4N0 had better prognosis than T3N1. Five-year survival rates of all cases were as follows. Tongue (66.7%), floor of mouth (62.4%), buccal mucosa (69.1%), lower gingiva (66.7%), upper gingiva (50.0%), hard palate (62.3%), lip (83.3%).

[Malignant mucosal melanomas of the head and neck--collective review from six cancer hospitals].

Forty-nine patients with mucosal malignant melanoma of the head and neck treated at six cancer hospitals were reviewed. Of the 49 melanomas, 25 were located in the oral cavity and 24 in the nasal or paranasal cavity. Thirty of them were classified as stage I, 14 as stage II, and five as stage III. The initial treatment modality included surgery, radiotherapy, chemotherapy, cryotherapy and immunotherapy in varying combinations. The five-year survival rate was 17.4% for oral melanoma and 19.1% for nasal and paranasal melanoma.

[Effect of the administration of liver extract after surgical operations in urology].

The effect of liver extract administered to 22 patients after urological operations was evaluated with reference to 19 controls. No significant difference was observed between the cases and the controls. We rather supposed, however, that the liver extract was effective as a liver protection drug, because the results of liver function in the cases were less changed than the controls throughout the post-operative course.

Diverse involvements of Ni protein in superoxide anion production in polymorphonuclear leukocytes depending on the type of membrane stimulants.

Effects of islet-activating protein (IAP) were examined to assess the involvement of the guanine nucleotide-binding regulatory protein responsible for inhibition of adenylate cyclase system (Ni protein) in the superoxide anion (O-2) production in polymorphonuclear leukocytes (PMNL) stimulated with various agents. N-Formyl-methionyl-leucyl-phenylalanine (FMLP)-stimulated O-2 production was inhibited by the pretreatment with IAP. O-2 production induced by each of phorbol myristate acetate, concanavalin A, and A23187, however, was rather resistant to the pretreatment with IAP. This observation indicates that the Ni protein does not involve in the common pathway for the O-2 production. in PMNL, and the involvement is rather specific for the FMLP-induced production. O-2 production in PMNL stimulated with various membrane perturbing agents was also diverse in the requirement of extracellular Ca2+.

Percutaneous microexplosion nephrolithotripsy.

We developed the technique of microexplosion for the destruction of bladder stones and have used it since 1981 with good results. The procedure is safe and simple. A renal pelvic stone in a seventy-one-year-old woman was successfully destroyed by microexplosion using a percutaneous approach. We believe this is the first clinical report on the use of microexplosion for upper urinary tract calculi.

A combined modality for anaplastic large-cell carcinoma of the thyroid.

The survival rates of 55 patients with anaplastic large-cell carcinoma of the thyroid with various combinations of treatment were analyzed to estimate the most effective way to cure the disease. A total of 21 patients with infiltration into subclavial vessels, plexus brachialis, vagus, sympathics, phrenic nerve, and/or hypoglossus died within 7 months after the diagnosis and most of them died within 2 months. The prognosis of patients with such tumor infiltration was very poor. Among 34 patients without infiltration of the above-mentioned structures, 12 were treated by radiation therapy alone or with chemotherapy, 8 were treated by surgery plus radiation therapy, and 13 were treated by a combination treatment consisting of operation, radiation, and chemotherapy. The post-therapeutic one-year survival rates were 7.7, 12.5, and 75%, respectively. Therefore, it is concluded that a combination treatment of surgical operation and radiotherapy plus chemotherapy should be the best for treatment of this tumor.

CT scan as a diagnostic tool to detect cancer invasion in the carotid artery.

The presence of cervical lymph node metastasis in head and neck tumors is usually determined by manual neck palpitation. However, this method cannot ascertain whether carotid artery invasion is present or not. Actually, fixed lymph node metastasis means nothing but deep fascia invasion of the metastasis. Contrast CT scan, on the contrary, can present exact information about the carotid artery. We investigate the comparison between CT scan results and surgical results in this paper.

Congenital absence of flexor pollicis longus without hypoplasia of thenar muscles.

A 7-year-old girl who was unable to flex her left thumb at the interphalangeal joint proved to have an extremely hypoplastic flexor pollicis longus with normal thenar muscles, which is very rare. The flexor digitorum superficialis of the ring finger was transferred to the inserting portion of the flexor pollicis longus tendon with good results. The patient's cooperation seems to be a factor determining the prognosis.
