Even in our times of revolutionary advances in the technology and operative techniques in plastic surgery, the corium is a material with multiple applications and can be found in the patient at varying thickness. The aim of the article is to show that its indication to reinforce flaccid musculature or to cover muscular defects of the abdominal wall is fully justified even in a time of routine use of plastic nets.
A rare case of extensive invasive basilioma of the abdominal wall in a 65-year-old female patient was treated by resection of the affected wall in its entire width and reconstruction by mobilizing the peritoneum and an extended inguinal flap. In combination with a simple appliance an adequately firm wall was established for complete social rehabilitation of the patient.

The authors present a choice of thumb reconstruction method using free transfer of sensitive radial artery flap together with autologous bone graft implantation in a single-time operation. They regard this approach as more advantageous than the standard tubed flap techniques. The advantages are the following: 1. single-time operation, 2. flap tissue sensitivity, and 3. satisfactory shape of the reconstructed thumb, requiring no subsequent modellation. Photographs show the surgical procedure and three other casuistics.
Based on clinical cases, the author shows the advantage of free sensory flaps for covering defects in the region of the planta. As compared with other types of flaps, these are more resistant and tolerate the normal load.
