Function and morphological picture of the liver in obese patients before and after jejunoileostomy.

In 40 obese patients, the liver function and the morphological picture were examined before and after jejuno-ileostomy. In 94% of the patients, distinct fatty liver was observed already before the small bowel exclusion, and the function tests showed an impaired function of this organ in 25% of the subjects before the operation. No clinical symptoms of liver insufficiency were recorded during the postoperative period. During one to three months after surgery, an increased impairment of the liver function was ascertained but, later on, a gradual improvement. More than one year after jejuno-ileostomy, the function test results were considerably better than before the operation. The degree of steatosis increased in the majority of the patients within 6 months after surgery, and some time later a considerable decrease in fatty liver was observed. Within 18 to 24 months after the jejunoileostomy, the morphological picture of the liver did not differe from the normal. The impaired function and the increased degree of steatosis were noted during diarrhea and rapid loss of body weight. The reson for this is most probably the protein malnutrition caused by the radical reduction in the absorption surface of the small bowel. The improved function and morphological picture of the liver are related to the progress of adaptation changes of the active part of small bowel. The results of the author's research do not confirm the hypothesis of permanent, harmful effect of jejuno-ileostomy on the state of the liver. The symptoms observed are definitely of a periodical and transient character, and are therefore not contra-indicated in the application of this operation in morbid extreme obesity treatment.

[Surgical treatment of obesity by jejuno-ileal bypass (author's transl)].

50 patients were operated on; 42 women and 8 men at the age of 17 to 57. The weight of the patients was 100 to 184 kg. and the overweight amounted in most cases to 70 to 188%. Long lasting conservative treatment proved to be inefficient. The endocrynological basis of the obesity was excluded. 6 patients were operated upon according to the Salmon-method and 44 according to the Payne-De Wind-method. 3 patients died. 47 patients recovered without complications. For two years after the operation the loss of weight was 28 to 50% compared with the initial weight. Diarrhoe stopped six months after the operation. During this time a decrease in the concentration of potassium and calcium in the blood serum was observed. Judging from the positive changes the decrease of the cholesterol concentration in the blood was observed as well as the general improvement of the patients health. All patients are in good health and have taken up their jobs again.

[Influence of jejunoileostomy on the lipid fractions in morbidly obese patients (author's transl)].

In 35 morbidly obese patients, operated by Payne-De Wind's jejunoileostomy, the serum concentration of the lipid fractions was examined before and after operation. Before the operation an increase of total cholesterol and beta-lipoproteins was noted in 1/3 of the patients. Six months after the jejunoileostomy a considerable decrease of total cholesterol, beta-lipoproteins, total lipids and free fatty acids has been observed.

[Liverfunction and serum concentration of electrolytes in patients operated for morbid obesity (author's transl)].

50 patients with morbid obesity underwent a Jejuno-Ileostomy. The liver function was tested before operation and postoperatively up to 24 months. Simultaneously the concentration of potassium, natrium, calcium, magnesium and chloride was examined. The concentration of bilirubin did not show any important changes. Thymol turbidity was within the limit. 3 months after the operation an increase of the activity of SGPT and SGOT was noted, as well as the increase of BSP retention. 6 months after the operation a decrease of concentration of potassium and calcium in serum was observed. 3 patients developed tetany. 12 months after the operation no changes in the concentrations of ions in the blood serum have been noted and the tests of the liver showed an improved function compared with the pre-operative period.

[Surgical treatment of idiopathic megacolon].

The authors discuss own experience in diagnosis and surgical treatment of 32 patients with idiopathic megacolon. Three surgical techniques were compared: 1) partial excision of the colon, 2) nearly complete excision of the colon with cecum-rectal anastomosis, and 3) total colectomy with ileorectal anastomosis. Basing on the authors' own experience and available literature the third approach is recommended as the most appropriate in the treatment of the persistent, chronic constipation accompanying idiopathic megacolon.

[Evaluation of the analgesic effectiveness of tramadol hydrochloride in surgical practice].

Tramadol hydrochloride was administered to: a) 90 patients who underwent various abdominal surgery (group A) immediately after operation; b) 50 patients with inoperable tumors of pharynx, stomach, pancreas, liver and biliary tract and colon (group B); c) 32 patients in surgical out-patient clinic (group C). Excellent and favourable results (group A - 87.8%; group B - 74%, and group C - 75%) and negligible adverse reactions advocate the use of tramadol hydrochloride both in in- and out-patient surgical clinics as an effective analgesic agent.

Spirometric, gasometric and electrocardiographic studies in patients with extreme obesity prepared for surgical treatment.

Spirometric, gasometric and electrocardiogarphic studies were carried out in 17 patients at rest, who were treated surgically for extreme obesity (overweight from 52 to 175 per cent in relation to the normal weight). The measurements were made before operation. It was found that the mean spirometric values were within the range of acceptable deviations from the normal value. The residual volume showed a tendency towards decreasing, especially in young patients. The values of dynamic parameters indicated a tendency towards ventilatory changes of a restrictive type in both groups. Minute ventilation at rest increased proportionally to obesity. Decreased values of respiratory equivalent indicated a good economy of respiration in obese persons when at rest. The mean gasometric values in capillary blood at rest were also within the acceptable range of abnormalities. Electrocardiographic tracings taken at rest showed considerable changes in the group of more obese persons.

Surgical treatment of obesity.

Thirty-two patients, 28 women and 4 men aged from 17 to 57 years were treated surgically for obesity. Their body weight ranged from 90 to 184 kg, with an excess weight from 42% to 180%. In 6 cases the Salmon operation and in 26 the Payne-de Wind operation were performed. Two patients died. In the remaining cases the postoperative course was uneventful and relatively mild. The longest follow-up period was 22 months. Weight loss rate was highest in the first period after the operation and the monthly rate of the weight loss in the first year after the operation was 3.5 to 6 kg. Diarrhea disappeared usually after 4 months. In some patients a transient fall in serum potassium and calcium level was observed. Other favorable results included a significant fall in the serum levels of cholesterol and beta-lipoproteins and improved value of BSP retention test. The condition of the patients is good and they have returned to work.
