Waldenström's macroglobulinemia. Report of an autopsy case presenting with a pulmonary manifestation.

An autopsy case of Waldenström's macroglobulinemia is reported, in whom an abnormal pulmonary shadow had already existed 2 years before the diagnosis of the disease and was proved to be pulmonary involvement. Immunoelectrophoresis demonstrated a monoclonal increase in immunoglobulin M with kappa light chain. A chest X-ray film showed a reticulo-nodular shadow in the right lower lobe of the lung. A bronchial biopsy specimen revealed a diffuse and dense lymphocytic infiltration. Bone marrow aspirate revealed no remarkable change except for a slight increase in plasma cells (1.7%) and an appearance of atypical lymphocytes (0.5%). At autopsy, more than half of the right lower lobe of the lung was occupied by a pale whitish, viscid and glossy tumour mass. Heptosplenomegaly and lymph node enlargement were not observed. Histological findings of the tumour tissue were similar to those of the biopsy specimen. Lymphocytic infiltration was observed also in the liver, kidneys, spleen, bone marrow and lymph nodes, but was of minor degree. Other reported cases of Waldenström's macroglobulinemia accompanied by pulmonary involvement are reviewed.

Scirrhous carcinoma of the stomach: a clinical and pathological study of 106 surgical cases.

Clinical and pathological characteristics of scirrhous carcinoma of the stomach were studied in 106 cases treated by gastrectomy between 1973 and 1983. The male to female ratio was 0.58. The percentage of scirrhous carcinomas to all gastric carcinomas resected in the same period was three times higher in females than males. The age distribution of the patients suggested that there were two peaks in the forties and sixties in the male, and in the thirties and fifties in the female. The incidence of scirrhous carcinoma in all types of gastric carcinoma was significantly higher in the twenties, thirties and forties compared to the lowest incidence in the seventies. In the female group the primary lesion had a tendency to be adjacent to the fundic gland area and to avoid intestinal metaplasia. In the male the opposite was recognized. Cancer nests with single cells or only several cells were common in this type of carcinoma. These findings suggest that there might be two biologically different scirrhous carcinomas both in the male and the female, the appearance of single carcinoma cells might be favored by female sex hormones and young ages, and not only the original gastric mucosa but also mucosa with intestinal metaplasia could be precursors of single carcinoma cells.

3 beta-hydroxyandrost-4-en-6-one derivatives as aromatase inhibitors.

The 3-formate (II), 3-acetate (III), 3-bromoacetate (IV), 3-propionate (V), 3-methyl ether (VI), and 3-deoxy-derivative (VII) of 3 beta-hydroxyandrost-4-ene-6,17-dione (I) were synthesized and tested in human placental microsomes for their ability to inhibit aromatase. II, III, and VII of this series were potent inhibitors of aromatase with the IC50's (1.7 and 3.3 microM) of the latter two comparable to that (1.2 microM) of 4-hydroxyandrostenedione. Kinetic studies showed that the three steroids are competitive inhibitors of the enzyme with Ki's of 16.0, 5.5, and 0.61 microM for II, III, and VII. Furthermore, II showed a time-dependent, pseudo-first order rate of inactivation of aromatase with Ki of 20.5 microM and kinact of 1.54 x 10(-2) min-1, while III gave a time-dependent, biphasic loss of the enzyme activity. NADPH and oxygen were required for the time-dependent inactivation and the substrate, androstenedione, prevented it.

An electrical method for examining remaining dentine thickness.

Cylindrical cavities were prepared on the buccal surfaces of 78 dog teeth. Impedance between cavity floor and oral mucous membrane was measured with a caries meter and correlated with remaining dentine thickness (RDT). Statistically significant, positive correlations (P less than 0.01) were found to exist between RDT and impedance in upper and lower premolars, however, a considerable degree of variation was observed in the RDT at an impedance value of less than 30 k omega. It is concluded that measurement errors make it difficult to determine RDT with electrical methods and that an impedance of 40 k omega is required to prevent bacterial contamination of pulpal tissues.

Treatment of malignant melanoma by selective thermal neutron capture therapy using melanoma-seeking compound.

As pigment cells undergo melanoma genesis, accentuated melanogenesis concurrently occurs in principle. Subsequent to the understanding of intrinsic factors controlling both processes, we found our selective melanoma neutron capture therapy (NCT) using 10B-dopa (melanin substrate) analogue, 10B1-p-boronophenylalanine (10B1-BPA), followed by 10B(n, alpha)7Li reaction, induced by essentially harmless thermal neutrons, which releases energy of 2.33 MeV to 14 mu, the diameter of melanoma cells. In vitro/in vivo radiobiological analysis revealed the highly enhanced melanoma killing effect of 10B1-BPA. Chemical and prompt gamma ray spectrometry assays of 10B accumulated within melanoma cells after 10B1-BPA administration in vitro and in vivo show high affinity, e.g., 10B melanoma/blood ratio of 11.5. After successfully eradicating melanoma transplanted into hamsters with NCT, we advanced to preclinical studies using spontaneously occurring melanoma in Duroc pig skin. We cured three melanoma cases, 4.6 to 12 cm in diameter, by single neutron capture treatment. Complete disappearance of melanoma was obtained without substantial side effects. Acute and subacute toxicity as well as pharmacodynamics of 10B1-BPA have been studied in relation to therapeutic dosage requirements. Clinical radiation dosimetry using human phantom has been carried out. Further preclinical studies using human melanoma transplanted into nude mouse have been a useful model for obtaining optimal results for each melanoma type. We recently treated the first human melanoma patient with our NCT, using essentially the method for Duroc pig melanoma, and obtained similar regression time course leading to cure.

Concentrations of beta-phenylethylamine in plasma and plateletes of schizophrenics.

The plasma and platelet PEA levels of 20 normal subjects and 17 schizophrenic patients were investigated using a high-performance liquid chromatography. In the normals the mean plasma and platelet levels of PEA were 4.9 +/- 1.9 ng/ml and 1.78 +/- 1.01 ng/mg protein, respectively, while in the schizophrenics, those were 12.1 +/- 7.9 ng/ml and 0.77 +/- 0.5 ng/mg protein, respectively. The plasma PEA levels of the schizophrenics were significantly higher than those of the normals, and the platelet PEA levels of the schizophrenics were lower than those of the normals.

Ventilatory and heart rate responses to hypoxia and hypercapnia in patients with diabetes mellitus.

The ventilatory response to isocapnic progressive hypoxia and hyperoxic progressive hypercapnia in 24 diabetic patients were compared with those of sex and age matched normal control subjects. The heart rate response to hypoxia was also measured in both groups. In diabetic patients the ventilatory and heart rate responses to hypoxia were significantly lower than those in the control group (0.10 v 0.24 l/min/% fall/m2 and 0.5 l v 1.27 beats/min/% fall respectively). The ventilatory response to hypercapnia was significantly higher (1.09 v 0.76 l/min/mm Hg/m2) in the diabetic patients. There was a significant correlation between the hypoxic ventilatory response and the heart rate response in diabetic patients (r = 0.56), but not in the control group (r = 0.28). In addition, both the ventilatory and the heart rate responses to hypoxia in diabetic patients had weak but significant correlations with the heart rate variation during deep breathing. It is concluded that the ventilatory and heart rate responses to hypoxia in diabetic patients are impaired, whereas the ventilatory response to hypercapnia is well preserved.

Coronary angiographic abnormalities in mucocutaneous lymph node syndrome: acute findings and long-term follow-up.

Coronary arterial changes in mucocutaneous lymph node syndrome were evaluated by retrospectively analyzing 62 coronary angiograms of 42 patients with abnormal coronary arteries. A total of 108 aneurysms on the initial studies were classified as follows: eight diffuse, 18 saccular, and 50 fusiform aneurysms and 32 localized areas of dilatation. These were also classified as large, medium, or small aneurysms. Approximately one-third of the aneurysms were in the right coronary artery and two-thirds in the left. Seventy percent of the aneurysms were located in the proximal portion of the coronary artery, 16% in the middle portion, and 14% in the distal portion. Sixteen patients were followed up with angiography for periods ranging from 4 months to 8 years 11 months. At follow-up, 48% of the 62 aneurysms had regressed to normal, 37% were diminished in size, and 10% were unchanged. Small and medium-sized aneurysms frequently showed complete regression. In contrast, large aneurysms tended to persist, and stenotic lesions formed at or adjacent to the aneurysm in 41%. Aneurysms larger than 9 mm in diameter, especially those that were diffuse in shape, carried a high risk of subsequent total occlusion.

Screening for molluscicidal activity in crude drugs.

Thirty-four extracts of crude drugs and medicinal plants have been screened for activity against Oncomelania nosophora, the intermediate host of the Japanese strain of Schistosoma japonicum. Strong molluscicidal activity was found in the MeOH extract of Anemarrhenae Rhizoma. Although timosaponin A-III, one of the main saponins of Anemarrhenae Rhizoma, showed very strong killing activity, markogenin-3-O-beta-D-glucopyranosyl (1-2)-O-beta-D-galactopyranoside having the same glycosidic linkage was found to be inactive.

Time-dependent inactivation of human placental aromatase by bromoacetoxy 4-androsten-3-ones in the presence of reduced nicotinamide adenine dinucleotide phosphate (NADPH).

6-Bromoacetoxyandrostenediones (1 and 2) and 17 beta-bromoacetoxy-4-androsten-3-one having a 6 beta-bromo (3), 6-keto (4), or 6 beta-methoxyl (5) substituent were evaluated as mechanism-based inactivators of human placental aromatase. All of these compounds except the 6 alpha-bromoacetate 1 showed a time-dependent, pseudo-first-order inactivation of aromatase in the presence of reduced nicotinamide adenine dinucleotide phosphate (NADPH) with apparent Ki's of 40, 50, 30, and 34 microM and kinact's of 0.048, 0.364, 0.267, and 0.040 min-1, respectively, for compounds 2, 3, 4, and 5. The enzyme inactivation with compounds 3 and 4 was blocked by the addition of the substrate androstenedione to the incubates, and NADPH and oxygen were required for their effective time-dependent inactivation.

Chemotactic responses of neutrophils in cats with spontaneous feline infectious peritonitis.

The culture supernatant of peritoneal exudate cells (PEC) from cats with effusive feline infectious peritonitis (FIP) was chemotactic for peripheral blood neutrophils (PBN) from healthy cats, magnitude of the chemotactic activity being approximately 10-fold lower than that in zymosan-activated fresh serum of healthy cats (ZAS). The migration profile of PBN from healthy cats was slightly different between the PEC culture supernatant and ZAS. These findings suggest that the chemotactic activity detected in the PEC culture supernatant is distinct from that in ZAS. The chemotactic responses of PBN from FIP cats to ZAS were reduced, as compared with that from healthy controls. In contrast, the neutrophil chemotactic response and sensitivity to the PEC culture supernatant in FIP cats were not remarkably different from those in healthy controls. Furthermore, the chemotactic responsiveness of PEC from FIP cats to ZAS was slightly different from that of PEC to the PEC culture supernatant. These results suggest that neutrophils from FIP cats have altered reactivities against these chemoattractants.

Ovarian aromatase activity in scorbutic mutant rats unable to synthesize ascorbic acid.

Osteogenic disorder syndrome rats are unable to synthesize ascorbic acid owing to the lack of l-gulonolactone oxidase, and become scorbutic within a few weeks without the supply of ascorbic acid. We studied effects of ascorbic acid deficiency on the ovarian aromatase activity in vivo using osteogenic disorder syndrome rats. The ovarian aromatase activity in ascorbic acid-deficient osteogenic disorder syndrome rats was significantly higher than that in normal or ascorbic acid-supplied osteogenic disorder syndrome rats. The activity in hypophysectomized immature rats was extremely low, but increased after treatment with pregnant mare serum gonadotropin, regardless of the presence or absence of ascorbic acid. The extent of the increase was the same among experimental groups. The present results indicate that ascorbic acid at physiological level lowers the ovarian aromatase activity, whereas it does not impair the responsiveness of the aromatase activity to gonadotropins.

Concentration of free thyroxin in serum during nonthyroidal illness--calculation or measurement?

We measured the free thyroxin (FT4) concentration in serum from 79 patients with nonthyroidal illnesses (NTI), using two analog RIAs and two calculation methods, and compared the results with those by ultrafiltration (UF) as a reference. The percentage of NTI patients with subnormal concentrations of FT4 was 10.1% by UF; 20.3% by mathematical calculation (MC) according to mass-action theory; 19.0% by FT4 index (FT4I), calculated by the triiodothyronine-albumin uptake method expressed as the bound:free ratio; 37.2% by Amerlex M FT4 (P less than 0.001 vs UF, P less than 0.025 vs MC); and 33.3% by Coat-A-Count FT4 with albumin blocker (P less than 0.001 vs UF, P less than 0.025 vs MC). No significant correlations were found between FT4 (UF) and T4-binding proteins (TBP) in serum, but FT4 (MC) correlated with T4-binding globulin (TBG) (r = 0.383, P less than 0.001), FT4I with TBG (r = 0.345, P less than 0.001), FT4 (Amerlex M) with albumin (r = 0.491, P less than 0.001), and FT4 (Coat-A-Count) with TBG (r = 0.636, P less than 0.001). All FT4 measurements correlated well with each other, but correlations were stronger between both calculated FT4 values and FT4 (UF), r = 0.702 to 0.760, than between both values for RIA and FT4 (UF), r = 0.475 to 0.665. These results indicate that in comparison with the UF method, both calculated FT4 values are little affected by TBP concentrations, and FT4 (MC) or FT4I by T3-albumin uptake may be more appropriate than analog RIA for evaluation of circulating FT4 concentration in NTI.

[An autopsy study of immunoblastic lymphadenopathy (IBL)-like T-cell lymphoma in a patient previously treated with chemotherapy in colon cancer].

A 77-year-old man was admitted to our hospital, due to systemic lymph node swelling. Nine months before his admission, he had been given a right hemicolonectomy for a colon cancer, that had been followed by chemotherapy (MMC and Tegafur). Laboratory testing revealed these findings: RBC 217 x 10(4)/mm3, Hb 8.3 g/dl, haptoglobin less than 10 mg/dl, positive Coombs test, cold hemagglutinin titer, 1:512, and polyclonal hyper r-globulinemia. A biopsy of a lymph node specimen exhibited the histological appearance of an IBL-like T-cell lymphoma described by Shimoyama et al. Although treatment with prednisolone was started for autoimmune hemolytic anemia, the patient died of severe anemia two months after the appearance of his lymph node symptoms.

Aromatase activity and progesterone metabolism in ovaries of scorbutic mutant rats unable to synthesize ascorbic acid.

Aromatase activity and progesterone metabolism were studied in ovaries of scorbutic ODS rats hereditarily unable to synthesize ascorbic acid (AsA). When ODS rats were kept on an AsA-deficient diet for 23 days, the scurvy developed with undetectable levels of AsA in ovaries. A significant increase in ovarian aromatase activity was observed in scorbutic ODS rats. However, there was no difference in the production of testosterone from progesterone between ovaries of scorbutic and ascorbutic ODS rats. These data suggest that AsA may play a role as a modulator of aromatase activity in vivo.

[Jaw and perioral reflexes elicited by tooth tapping in the cat].

The reflex responses elicited by tapping the maxillary canine were studied in 12 adult cats anesthetized with alpha-chloralose. Short (4-8 msec) and long (14-22 msec) latency responses were recorded from both efferent nerves innervating a digastric muscle and an inferior portion of perioral muscles. These two responses were also observed from a single fiber teased from the efferent nerve as two successive groups of spike discharges. The long-latency responses from the digastric and the perioral muscle were both elicited by a light tooth tapping and disappeared at about the same time following infiltration of local anesthetic over the tap-stimulated tooth root or following inhalation of ether narcosis. The long-latency responses from the above muscles may be, therefore, elicited by the same sensory information of the periodontal afferent, and have a common reflex center.
