Lymphatic abnormalities in protein-losing gastropathy, especially in Ménétrier's disease.

Histopathologic and lymphographic studies were performed in 4 patients with protein-losing gastropathy to elucidate the degree of lymphatic disorders and their contribution to clinical features. Histologic examination of gastric mucosa showed various degrees of hyperplasia of the surface epithelium and cystic dilatation of glands, which were considered to be characteristic findings of protein losing gastropathy. Moreover, in 3 of 4 patients there were edematous change of the lamina propria or the submucosa, and marked lymphangiectasia was seen in the submucosa of 3 patients and in the lamina propria of the other. Performed lymphograms showed an increase in the number of lymphatic vessels and lymph nodes in the iliopelvic and lumbar regions of 2 of the patients. This high incidence of lymphatic abnormalities in patients with protein-losing gastropathy suggested that such systemic lymphatic disorders were an important etiologic factor. However, such abnormalities in lymphatic system were not demonstrated in all cases of protein-losing gastropathy. Therefore, lymphatic disorders were not considered to be the primary cause of gastric protein loss, but the important factor that modifies the variety of lost protein and its clinical features.

Home blood pressure and circadian variation of blood pressure in the evaluation of hypertensive patients.

Clinical blood pressures measured at clinic by physician were higher than home blood pressures measured at home by patients in the majority of untreated patients with essential hypertension, but equivalent or lower in some patients. Clinic minus home blood pressure (delta P) were correlated with the levels of clinic blood pressure (r = 0.51, p less than 0.005 for systolic; r = 0.35, p less than 0.02 for diastolic blood pressure, respectively). The systolic delta P might be greater in the middle-aged women, especially in the fifties of females than the age-matched males (p less than 0.05). The delta P could not be altered by any antihypertensive drugs with the exception of systolic delta P with diuretic alone. The blood pressure tended to remain more stable throughout the 24-hour period in proportion as the severity of hypertension increased. The observation of circadian variation in blood pressure disclosed that the blood pressure was lower in the morning, but increased gradually, resulting in the relatively high blood pressure between the afternoon and evening in the low renin and volume expanded type of hypertension. On the contrary, the blood pressure was already high in the early morning in hypertensive patients characterized by the accelerated renin-angiotensin system and contracted volume factor.

Hypertension, hypokalemia and hypoaldosteronism with suppressed renin: a clinical study of a patient with Liddle's syndrome.

A 24-yr-old woman with hypertension, hypokalemic alkalosis, low plasma renin and hypoaldosteronism was studied. Plasma aldosterone, renin and potassium returned to normal and blood pressure fell after sodium restriction or the administration of triamterene. Thiazide therapy also normalized her blood pressure while dexamethasone, spironolactone and furosemide did not improve her symptoms. Plasma aldosterone levels were low and responded poorly to a short term ACTH injection, but responded well to the maximal adrenal stimulation by ACTH-Z. Plasma levels of cortisol, corticosterone and deoxycorticosterone were within the normal range. Adrenal scintigram with 131I-adosterol and abdominal computed axial tomography did not reveal the presence of a sizeable adrenal tumor. In addition, the urinary kallikrein excretion was low after sodium restriction and showed no response to saline infusion. These findings suggest that the excessive secretion of unusual mineralocorticoids may not exist in this case. From these observations and the results of the therapeutic responses to the diuretic agents, we conclude that the primary cause of the disorder of this patient seems to be a renal defect in the distal tubule in handling sodium and potassium which is similar to that in Liddle's syndrome.

Studies on the effects of 3-acetyl-4"-isovaleryltylosin against multiple-drug resistant strains of Staphylococcus aureus.

The macrolide-resistance of multiple-drug resistant strains of Staphylococcus aureus was divided into two types; the decreased sensitivity of ribosomes (type I) and the decreased uptake (type II). Both types were resistant to erythromycin, tylosin and 3-acetyltylosin, and their resistance was not inducible. 3-Acetyl-4"-isovaleryltylosin inhibited the growth of both types. Protein synthesis on ribosomes of type I in vitro (S. aures MS-9610) was inhibited by 3-acetyl-4"-isovaleryltylosin, but little or no inhibition was seen with either tylosin or 3-acetyltylosin. Ribosomes of type II in vitro (S. aureus MS-8710) were sensitive to all macrolides. 3-Acetyl-4"-isovaleryltylosin accumulated about twice as much as 3-acetyltylosin in intact cell of type II.

The screening of beta-lactamase inhibitors: inhibition by fatty acids produced by bacteria.

An active principle inhibiting beta-lactamases, which was found in the culture of a bacillus strain was a mixture of known C14-C17 fatty acids. The mixture was separated into five components by high-performance liquid chromatography. Among these components, 12-methyltetradecanoic acid showed the strongest activity (I50: 20-146 microM). The anteiso fatty acids having the 1-methylpropyl group exhibited an interesting activity inhibiting beta-lactamases.

Histoclinical long-standing follow-up study of hyperplastic polyps of the stomach.

In a series of 2,013 gastric polyps in 1,201 patients, morphological and histopathological studies have been performed. Ninety-three hyperplastic polyps in 56 patients have been followed-up endoscopically and histopathologically for five to 12 years. The incidence of detection of gastric polyps has increased: 1.4% in 1967 to 8.7% in 1979 year by year. Thirty patients (54%) among the 56 showed changes in number, size or shape of polyps during follow-up. Twenty patients (37%) revealed numerical changes (increase in 16 patients, reversion in three patients and vanishing in two patients). Twenty-eight polyps (30%) showed morphological changes, six of them showed continuous enlargement, 18 lesions repeated enlargement, stationary or reduction stages, three lesions were reversed and two polyps disappeared. Histopathologically, three lesions showed transformation from the hyperplastic type to adenoma while demonstrating morphological enlargement. Two of these showed increase in cellular atypia, from the hyperplastic type through adenoma with severe atypia and finally to carcinoma in the polypectomy specimens. From this study, it was concluded that although hyperplastic polyps show changes in size, shape or number with passage of time, malignant changes occur in only a few cases.

[Echocardiographic evaluation of the left ventricular contraction in cases with atrial fibrillation (author's transl)].

Echocardiographic studies were performed in order to examine the effect of preceding R-R interval (PI) and pre-preceding R-R interval (PPI) on the changes in end-diastolic and end-systolic dimensions (Dd and Ds) in 12 patients with atrial fibrillation. As PI prolonged, Dd increased and Ds decreased, however, these were constant at the PI of more than 0.80 up to 1.20 sec. At the PI of less than 0.80 sec, Dd and Ds showed a wider range of distribution. When PPI remained constant, Dd increased and Ds decreased with the prolongation of PI. When PI was kept constant, Dd increased and Ds decreased as PPI became shorter. The greater the degree of PPI shortening and PI prolongation, the larger the increase of Dd and decrease Ds. Thus, the change of contractile state of the left ventricle in atrial fibrillation was affected not only by the preload of the individual PI, but also by that of the PPI. This is thought to be the same mechanism as in post-extrasystolic potentiation.
