[Comparative analysis of blood and urinary electrolytes in normotensive and hypertensive normal, overweight and obese subjects aged 50-54 in the Bulgarian army].

Objective of the study is to investigate the hypotheses regarding the correlation of serum levels and urinary excretion of electrolytes with arterial hypertension. 211 subjects aged 50-54 from Bulgarian Army were studied. Evaluated were following parameters: SBP, DBP, BMI, Na+, K+ in blood serum; Na+, K+, NaCl, KCL, Na/K ratio, Ca++, Mg++ in 24-h urine excretion. Statistical analysis of variance, dispersion and correlation analysis (using 6D program from statistical pack BMDP) were applied. In the blood, no correlation was found between Na+ and both SBP and DBP. A significant positive correlation between SBP and K+ was found only in non-obese hypertensive females (R = 0.59, p < 0.05). Correlation between K+ and DBP was negative in separate groups with normal BMI (R = -0.84, R = -0.65, p < 0.05). In the 24-h urine excretion, no correlation was found between K+, NaCl, KCL and SBP, as well as between Na+ and SBP and DBP. Significant positive correlation was found between K+ and DBP in obese hypertensive subjects (R = 0.56, p < 0.05) and between NaCl and DBP (R = 0.47 up to R = 0.66 in separate groups, p < 0.05). Significant positive correlation between Ca++ and DBR (R = 0.47) was found in obese hypertensives. Mg++ correlates positively with DBP (R = 0.62 in some groups). Urine levels of Ca++ and Mg++ were significantly higher in normatensives. Electrolytes in blood and urine were closely related to DPB rather than to SBP. K+ levels in blood affect only DBP. Our data do not confirm the hypothesis for the positive correlation between urinary Na(+)-excretion and BP (except in obese subjects). The hypothesis about the inverse relation of K(+)-intake to BP, especially to DBP is confirmed by our data. The data regarding the Ca++ and Mg++ urinary excretion and their correlation with DBP and SBP are very discordant and do not give an opportunity to make definite conclusions. Our data confirmed the hypothesis about the natriuretic effect of Mg++, which possibly is of a protective value for development of AH in some obese persons.

Anticardiolipin antibodies in spontaneously hypertensive rats (SHR), stroke-prone SHR and normal Wistar rats.

1. Anticardiolipin antibodies (ACA) can be detected in the serum of patients with autoimmune disturbances, ischaemic heart disease, myocardial infarction, neurological disorders and other medical conditions. Elevated values of these autoantibodies can be associated with recurrent fetal loss, arterial and venous thrombosis and thrombocytopenia. 2. In the present study, we investigated the presence of ACA in three rat strains, namely normal Wistar rats (WR), spontaneously hypertensive rats Okamoto-Aoki (SHR) and stroke-prone SHR (SHRSP). All animals were examined at four ages: 1, 4, 10 and 12 months of age. Anticardiolipin antibodies were determined by ELISA. 3. Anticardiolipin antibody levels in normal WR, which were used as controls, were lowest at 1 month and increased significantly from the 4th month on. At the prehypertensive age (1 month), ACA levels in SHR and SHRSP were significantly higher compared with control WR, decreased with age and were significantly lower at 4, 10 and 12 months compared with age-matched WR. 4. These differences may be a result of immunological disorders in SHR.

[Prenatal diagnosis of bilateral fetal renal polycystosis].

Congenital abnormalities of the renal and urinary tract (CARUT) of the foetus constitute about 20-30% of all abnormalities. The frequency of infant mortality of genito-uretral malformations is about 10 to 100,000 lye-burts. The early and opportune antenatal diagnosis of these abnormalities by ultrasound examination of the foetus improves considerably the prognosis of children having such divergences in their renal and urinary tract. The authors represent a clinical case of antenatal diagnosis of a foetus with congenital bilateral renal polycystosis in the first half of the period of pregnancy and emphasize the importance of systematic ultrasonographic screening of the pregnant patients. A bibliographical survey of the CARUT is made together a review of the potentialities of ultrasonographic screening for diagnosing in time for due action in the presence of any abnormalities of the renal and urinary tract.

[Inborn laryngeal cysts - description of a case with fatal outcome].

The authors describe a case of a newborn baby with inborn laryngomucocaele, living 14 days after birth. The newborn baby was looked after with artifical lentilation. Because of the inability of self breathing the baby underwent tracheostomia. The autopsy revealed the presence of laryngomucocaele.

[The late results of the surgical treatment of stress urinary incontinence].

The severe stress urinary incontinence is a condition, which traumatize women psychology. Amongst numerous well-known operative techniques for treatment of the stress urinary incontinence authors apply the following ones: suprapubic colpouretherosuspension by Burch, colpouretherosuspension by Ratz, sling operation with synthetic stuff--Ampoxen. The evaluation of late results from the applied operative approaches in 30 patients displaced a long term overcome of stress incontinence. The authors emphasized that during clinical follow up of all cases in the course of 3 years the best results were obtained after application of suprapubic and sling methods. Our conclusion is that the choice of operative approach have to be individual and is in dependence with every clinical case. The postoperative outcome depends on the exactness of the operation.

[Combined diagnostic methods for women in the pre- and postmenopause].

UNLABELLED: During one-year period, 20 women with pre- and post-menopausal problems were investigated. The diagnostic examination included transvaginal sonography (TS), hysteroscopy with separative abrasion and histopathological examination. The data from the three diagnostic methods were compared. RESULTS: In 6 patients the endometrial thickness (ET) was lower than 8 mm. The hysteroscopic finding (HF) comprised in endometrial atrophy, confirmed also histologically. ET was higher than 8 mm in 14 patients, and the HF revealed endometrial polyps and endometrial hyperplasia. The hystological examination confirmed cystic endometrial hyperplasia in 4 cases, endometrial secretary phase in 5 cases, endometrial polyp in 3 cases and endometrial carcinoma in 2 cases. The authors accepted that the combination of 3 diagnostic methods of examination, applied in the described consequence, gave the possibility for precise diagnostic investigation and adequate treatment.

Treatment of moderate or severe arterial hypertension with losartan.

Losartan is the first drug of a new class antihypertensive drags--Angiotensin II receptors antagonists. According to the data in the literature the drug shows a very good efficiency and tolerability. Fifteen patients at the average age of 48.9 are the object of this study. Our study establishes considerable lowering of the systolic BP with 15.9% and of the diastolic BP with 20.3%. No considerable effects on the hematopoiesis, the carbohydrate and the lipid metabolism, and the renal function were observed during treatment with Cozaar.

[Combined gynecological operations].

During the objective examination of patients admitted for operation, changes in the gynaecological status, corresponding to primary disease and demanding also operation, were determined. The authors studied clinical status of 33 women with the following primary diseases: myoma uteri, static anomalies of female genitalia and accompanying diseases like urinary stress incontinence, uterine cervical laceration and the accompanying static anomalies. Authors applied combined operative techniques, abdominal and vaginal (hysterectomy with colpouretherosuspension by Burch, vaginal hysterectomy with colpouretherosuspension by Ratz, myomectomy with colpouretherosuspension and ligamentopexy by McColl, etc.), in order to treat definitively the primary disease and the accompanying ones as well. The results showed that in appropriately selected cases and operative techniques, a good therapeutic outcome was achieved. In conclusion we may say that the operative approach should include simultaneously all pathological deviations of women genitalia.

Synaptic vesicle glutamate uptake in epileptic (EL) mice.

The ATP-dependent glutamate uptake system of synaptic vesicles was investigated in epileptic (EL) mice to determine whether glutamate uptake activity correlates with seizure susceptibility or development. Given the focal seizure onset, glutamate uptake activity was measured in four separate brain regions: cerebrum (minus hippocampus), hippocampus, cerebellum, and brain stem. The EL values were compared to those of age-matched controls; DDY and ABP/LeJ (ABP) mice. The glutamate uptake specific activity for EL cerebrum was significantly higher than that for the control mice (approx. 400 days old), but was not elevated prior to seizure onset (46 days old). No difference in glutamate uptake was observed between the strains in the other brain regions. We conclude that increased synaptic vesicle glutamate uptake is brain-region specific (cerebrum) and is associated with the development or maintenance, rather than the initial cause, of seizures in the EL model of epilepsy.

Successful crosses and molecular tetrad and progeny analyses demonstrate heterothallism in Mycosphaerella graminicola.

Monospore isolates of Mycosphaerella graminicola considered to originate from one ascus were analysed by the polymerase chain reaction (PCR) with 32 RAPD primers. Eighteen of these revealed three classes of polymorphisms, which enabled a RAPD-based tetrad analysis. Four pairs of isolates resulting from a single diploid nucleus were determined. A procedure to cross these isolates was developed to investigate the mating system. Three of six crosses were successful, and the segregation of mating types in accordance with the tetrad analysis strongly points to a bipolar heterothallic mating system in M. graminicola. Random ascospore progenies from the successful crosses, each comprising 54 isolates, were studied with three primers to determine the mode of inheritance of the RAPD markers. Mendelian segregation and recombination of RAPD markers was observed in all progenies.

[The current aspects of hospital infections in maternity and neonatal wards].

The epidemiologic, etiologic and clinical structure of HAI in the maternity and neonatal wards in the country for the 1982-1992 period has been specified. 934 women in child-birth and 2357 neonates acquire nosocomial infections at an average annually. A comparatively constant level in HAI epidemic process intensity is observed with inconsiderable diversions in the beginning and by the end of the studied period. Staphylococci like causative agents of HAI take up a leading place in both types of wards with especially marked incidence rate among the newborn children. The predominant clinical forms in the women in child-birth are the surgical wound infections, skin and genital infections and in the neonates--the staphylodermatites, upper respiratory airway infections, pulmonary and enteric infections.

Eradication of poliomyelitis in Bulgaria--problem encountered.

The authors analyze the effectiveness of the National Vaccination Programme with regard to poliomyelitis eradication in Bulgaria and the difficulties encountered in the 1980s and at the beginning of the 1990s. An account is presented on the eradication achieved, interrupted by an outbreak of 46 cases in 1991. The critical analysis points out the negative influence of several factors, disrupted by the failure in 1991. Based on these fact conclusions are drawn on a prolongation of the period needed to achieve the Eradication Programme goal. The main reasons for the delay are as follows: the intensive migration process, the existence of small non-immunized groups of population (gipsy population) as well as some shortcomings in the organization of the health services. According to the authors, stricter epidemiologic supervision is imperative for the country as well as coordination of similar approach to the problem in countries of the region.

[Cardiac tamponade from right ventricular rupture in severe lipomatosis].

A case of an 80 years old woman with II-degree obesity is presented. The patient showed no hemodynamic disturbances prior to her death despite bilateral cardiac overload causes by chronic pulmonary inflammatory processes and arterial hypertension. The rupture of the right ventricular cardiac wall with cardiac tamponade was due to the massive fatty infiltration of the cardiac wall and to the sharp increase of the hydrostatic pressure in the right ventricle in the course of pulmonary embolism.

Effect of calcium blocking agent verapamil on blood pressure, ventricular contractility, parathyroid hormone, calcium and phosphorus in plasma, catecholamines, corticosterone and plasma renin activity in spontaneously hypertensive rats.

In four-month-old spontaneously hypertensive rats (SHR) the effect of a calcium blocking agent verapamil on blood pressure, ventricular contractility indices, parathyroid hormone (PTH), plasma renin activity (PRA), plasma and adrenal corticosterone content and catecholamines in hypothalamus, myocardium and adrenal gland was evaluated. Calcium and phosphorus in plasma were also determined. Verapamil treatment resulted in a significant decrease in systolic and diastolic blood pressure and a reduction in maximum left ventricular pressure. Verapamil exerted a negative inotropic effect, evaluated by a decrease in dP/dt max and dP/dt neg. PRA was elevated, calcium tended to decrease, and no changes in PTH and phosphorus were found. The hypotensive effect of verapamil in SHR was accompanied by a decrease in plasma and adrenal corticosterone content, and a fall in catecholamine concentration in adrenal glands and myocardium.

Effect of captopril on some ventricular contractility indices in spontaneously hypertensive rats (SHR).

Two groups of SHR were treated with captopril (Squibb): the first group (1 month old)--30 mg/kg i.p. twice daily for 77 days, and the second group (8 months old)--20 mg/kg p.o. twice a day for 23 days. The recording of electrocardiogram (ECG), systolic blood pressure (SBP), diastolic blood pressure (DBP), and some isometric indices of the left-ventricular contractility indicate that in all treated animals the values of most of the parameters are considerably lowered compared to the controls. The ratio between velocity of pressure increase and intraventricular pressure/equal to 50 mmHg--(dP/dt)/ICP50/does not change significantly. It can be concluded that in SHR, captopril practically does not influence the inotropic condition of the myocardium.

Treatment of resistant ventricular tachycardia with endocavitary fulguration and antiarrhythmic therapy, compared to antiarrhythmic therapy alone: experience in 111 consecutive cases with a mean follow-up of 18 months.

Endocavitary fulguration was performed in 31 cases of high-risk ventricular tachycardia (VT) for which antiarrhythmics, including Amiodarone and Class-I antiarrhythmic agents, given alone or in combination, proved ineffective. Permanent VT was incessant in nine patients at the time of fulguration: five of these were moribund, and two were unconscious. Included in the series were ten cases of arrhythmogenic right ventricular dysplasia, 13 cases of chronic VT after myocardial infarction, five cases of idiopathic dilated cardiomyopathy, two cases of idiopathic VT, and one case that involved a congenital anomaly. Combined fulguration and antiarrhythmic therapy succeeded in preventing VT in the 27 patients who survived the initial period of treatment. Thirteen patients needed two or more sessions, however, before their VT was brought under control. Three of the four early deaths were probably related to an imperfect technique. Four late deaths were due to spontaneous evolution of the disease. One patient without cardiac failure died suddenly. The follow-up period ranged from a minimum of 13 to a maximum of 34 months, with an average follow-up of nearly 2 years. The success rate for fulguration alone or for antiarrhythmic drugs is about 90%. This group of patients is compared with a group that involved 73 less severe cases treated with drugs. Fulguration appears to be the better form of treatment.
