[7 cases of congenital multicystic kidney with special reference to its embryogenesis].

We have experienced 7 cases of multicystic kidney. The latest two cases, etiologically of interest, are reported herein with special reference to its embryogenesis. Case 6: A 4-year-old girl was referred to our clinic for further evaluation of mild azotemia and nonvisualization of left kidney. Left kidney was strongly thought to be multicystic kidney from abdominal CT, whereas her contralateral kidney exhibited hydrocalycosis resembling infundibular stenosis with diminished calyceal numbers. Nephrectomy of her left kidney was performed and histological studies confirmed renal dysplasia (primitive duct, metaplastic cartilage, etc.). Case 7: A 7-year-old girl was referred to Hakodate Kyokai Hospital for the evaluation of azotemia and low stature. Her right kidney was not visualized on IVP and her left kidney exhibited hydronephrosis with diminished calyceal numbers. Her right kidney was diagnosed as multicystic kidney on CT-scan. Pyeloplasty of her left kidney was performed. Multicystic kidney is a rather rare congenital disease. Association of various anomalies in contralateral kidney has been emphasized as well as the notion that infundibulopelvic stenosis is a linked in the clinical spectrum extending from cystic dysplasia of the kidney to hydronephrosis. Our last two cases seem to be included in this category of obstruction theory. As shown by microdissection technique (Potter), however, severe ampullary inhibition early in fetal life is also an attractive hypothesis. Diminished calyceal number of contralateral kidneys seen in our recent two cases is compatible with possibility of ampullary damage. Recent experimental study also showed that renal dysplasia is not solely caused by simply mechanical obstruction to urinary drainage, even when the obstruction is imposed at an early stage of renal development.

[A biological rhythm in a patient with normal pressure hydrocephalus--comparative studies in pre- and postoperative patients by a polygraphy].

The authors had studies the correlation between the appearance of the pressure waves and the level of the sleep in pre- and postoperative patients with normal pressure hydrocephalus (NPH). The changes from preoperative findings to postoperative ones were discussed in detail with the relation of the pathophysiological state in these patients. Seventeen patients were evaluated for the suspected diagnosis of the disease of NPH. Thirteen patients of them were treated with ventriculo-peritoneal shunts. Four representative cases among them were evaluated by pre- and postoperative polygraphic studies. A polygraphic overnight study includes a monitoring of an intracranial pressure (ICP), electroencephalography (EEG), electrooculography (EOG), respiratory movement and electromyography (EMG). Each data was recorded for the analysis on data recorder using a computer system. A pressure waves (largely B type) appeared accompanied with an apnea at a resting state (so-called sleep) of each patients. An arousal response in EEG was also observed in the raising period of the pressure waves. At the peak respiration was resumed with transient activities in EMG and continued to the disappearance of the pressure wave. Pressure waves were observed frequently and continually in hours in the resting state of these patients. As a result the level of sleep alternated frequently between an awake stage and a stage 1, including extremely rare appearance of stage 2. In short, the level of sleep was frequently interrupted by the appearance of the pressure waves and apneas. Such pathological states of patient's sleep were involved in the entity of the sleep apnea syndrome.(ABSTRACT TRUNCATED AT 250 WORDS)

A monoclonal antibody that binds preferentially to human suppressor T cells.

A monoclonal antibody termed JM3-3-3A was produced by somatic cell hybridization. The reactivity was assessed by indirect immunofluorescence. JM3-3-3A was reactive with 41% human peripheral blood T cells and 48% non-T cells. Among various human lymphoblastoid cell lines (MOLT 4F, JM, and TALL-1), TALL-l was found not to be reactive with JM3-3-3A. Human peripheral blood T cells fractionated by JM3-3-3A-coated dish panning were submitted to functional studies. JM3-3-3A positive T cells responded to mitogens, concanavalin A and phytohemagglutinin-P much better than JM3-3-3A negative T cells in the presence or absence of adherent cells. JM3-3-3A positive T cells showed suppressor activity and JM3-3-3A negative T cells showed helper activity in pokeweed mitogen-induced Ig production. More than fifty percent of the JM3-3-3A positive T cells were reactive with OKT8 which binds to suppressor/cytotoxic T cells, whereas 18% of JM3-3-3A negative T cells were reactive with OKT8. In addition, immunoprecipitation experiments identified a protein with an approximate molecular weight of 43,000 as a cell surface antigen for JM3-3-3A. Thus, the reactivity of JM3-3-3A showed a wide distribution but human peripheral blood T cells could be dissected functionally by this antibody.

[Enhanced immunogenicity of xenogenized tumor cells in rats pretreated with cyclophosphamide (author's transl)].

Immunization with xenogenized tumor cells, tumor cells infected with murine leukemia viruses produced strong antitumor immunity. In this study, the immunogenicity of xenogenized tumor cells was compared in cyclophophamide-treated and nontreated rats. CY treatment 3 days before immunization with xenogenized tumor cells enhanced antitumor cell-mediated immunity and antibody responses. However, CY treatment after immunization had no effect. Transfer of thymus cells from normal rats before the tumor challenge abolished the CY-induced augmenting effect. From these results, it is suggested that the elimination of precursors of suppressor T-cells results in enhanced immunogenicity of xenogenized tumor cells.
