Enhancement by vasoactive intestinal peptide of experimental carcinogenesis induced by azoxymethane in rat colon.

The effects of vasoactive intestinal peptide (VIP) on the incidence and histology of colonic tumors induced by azoxymethane (AOM) were investigated in Wistar rats. Rats were given 20 micrograms/kg body weight of VIP every other day for 12 weeks and from experimental week 3, were given 10 weekly injections of 7.4 mg/kg body weight of AOM. The administration of VIP before and during AOM treatment resulted in a significant increase in the incidence of colonic tumors in week 40. Furthermore, it caused a significant increase in the labeling index of the colonic mucosa during AOM treatment. These findings indicate that VIP enhanced the development of colonic tumors. This effect may have been related to its effect in increasing proliferation of cells in the colonic mucosa during administration of the carcinogen.

Enhancement by propranolol of the inhibitory effect of tetragastrin on gastric carcinogenesis induced by N-methyl-N'-nitro-N-nitrosoguanidine in Wistar rats.

The effects of combined administration of propranolol and tetragastrin on gastric acid secretion and the incidence and histological types of gastric adenocarcinomas induced by N-methyl-N'-nitro-N-nitrosoguanidine were investigated in inbred Wistar rats. Prolonged administration of tetragastrin, 1 but not 0.2 mg/kg body weight in depot form after treatment with N-methyl-N'-nitro-N-nitrosoguanidine significantly reduced the incidence of adenocarcinoma of the glandular stomach. The adenocarcinomas that did develop in rats treated with the higher dose of tetragastrin had high mucin-producing activity and showed little or no typical glandular structure. A combination of propranolol (2 mg/kg) and tetragastrin (1 mg/kg) did not influence the inhibitory effect of gastrin on gastric carcinogenesis. However, concomitant administration of propranolol (2 mg/kg) and tetragastrin (0.2 mg/kg) caused a significant increase in gastric acid secretion and a reduction in the incidence of gastric carcinomas. With this treatment, the incidence of adenocarcinoma was similar to that of treatment with tetragastrin (1 mg/kg). Histological examinations showed that like the cancers in control rats, the adenocarcinomas induced in these rats were all highly differentiated.

Small-cell carcinoma of the esophagus: a case treated by chemotherapy.

A patient with small-cell carcinoma of the esophagus treated with cyclophosphamide and vincristine is described. Within 2 months after the beginning of therapy, two coexisting tumors had disappeared and the main primary lesion and metastases in the liver and lymph nodes were markedly reduced in size. The patient relapsed 5 months after the start of therapy and died with widespread metastases. Autopsy revealed that the patient was in partial remission.

Histological features and recurrence of completely and incompletely healed gastric ulcers classified by the methylene blue dye method.

Histological features and recurrence of gastric ulcers classified as completely or incompletely healed by the methylene blue dye method were investigated. On chromo-endoscopic examination, complete healing was defined as a scarred area completely covered with granular patterns, while incomplete healing was defined as a scarred area surrounded only marginally by granular patterns. The methylene blue dye method was significantly more accurate than routine endoscopy for determining the histological completeness of healing. Recurrences were significantly more frequent in incompletely healed ulcers classified by the dye method than in completely healed ulcers. Heavy smoking, male sex, and location of ulcers were closely related to recurrence. Therapy should be continued until the healed ulcers are wholly covered by granular patterns, and physicians should encourage their patients to stop smoking.

Relationship of erosive gastritis to the acid secreting area and intestinal metaplasia, and the healing effect of pirenzepine.

The extent of acid secreting areas and the distribution of intestinal metaplasia in patients with erosive gastritis, and the healing effects of pirenzepine were examined. Studies were done with the endoscopic Congo red-methylene blue test developed in our hospital. Compared with control patients with no gastroduodenal disease, erosive gastritis was associated significantly more frequently with large acid secreting areas, but little or no intestinal metaplasia was detected in the stomach. A double blind trial was carried out, using 100 mg pirenzepine tablets or placebo for three months in 43 patients with erosive gastritis. Endoscopically, complete healing was significantly more frequent in the pirenzepine treated groups three months after the start of the treatment, as compared with the placebo treated group (p less than 0.05).

CT-pathologic correlation in small peripheral lung cancers.

To evaluate the morphology of small peripheral lung cancers, we studied thin-section CT images of 18 small peripheral lung cancers (14 adenocarcinomas, two squamous-cell carcinomas, one large-cell carcinoma, and one carcinoid) in 17 patients. After surgical resection, specimens were sliced transversely, and the gross morphology and histology were correlated with the appearance of the lesion on preoperative thin-section CT images. CT images showed fine speculations in 78%, a notch in 83%, pleural retraction in 78%, and convergence of peripheral vessels in 83% of the 18 lung cancers. Furthermore, in cases of papillary adenocarcinoma, the lesions had heterogeneous densities (78%) and small cavitations (67%). CT showed a peripheral fluffy zone in most of the well- or moderately differentiated papillary adenocarcinomas. This correlated well with tumor cells lining the alveolar walls, observed in pathologic studies. Comparison of thin-section CT images with pathologic data suggests that the demonstration of the lung-tumor interface and the tumor's internal texture, while not specific for malignant lesions, can make the CT diagnosis of small peripheral lung cancers more accurate.

Scavenging effect of butylated hydroxytoluene on the production of free radicals by the reaction of hydrogen peroxide with N-methyl-N'-nitro-N-nitrosoguanidine.

The scavenging effects of the antioxidant butylated hydroxytoluene (BHT) on a hydroxyl free radical (.OH) produced in the reaction of H2O2 with N-methyl-N'-nitro-N-nitrosoguanidine [(MNNG) CAS: 70-25-7] and on free radicals derived from MNNG (.MNNG) were examined by electron spin resonance with the use of the spin-trapping agent 5,5-dimethyl-1-pyrroline-1-oxide (DMPO). BHT was added to the H2O2-MNNG-DMPO system in 50% acetonitrile and exposed to a tungsten-halogen lamp at an intensity of 0.3 mW/cm2 for 3 minutes at 3 degrees C for the first 2 minutes of irradiation. The amounts of .OH and .MNNG in the system decreased and reached constant levels with increase in BHT concentration. The spectrum of the H2O2-MNNG-BHT system showed the specific signal of BHT, whereas the spectra of the BHT solution and the H2O2-MNNG system did not show any signal. These findings indicate that BHT scavenged .OH and .MNNG, and in its reaction with them formed a stable free radical.

[A comparative clinical trial with tegafur plus lentinan treatment at two different doses in advanced cancer].

In order to evaluate the clinical efficacy of combined tegafur plus lentinan treatment, a comparative trial was performed on patients with advanced cancer using two different doses, a conventional-dose group and a high-dose group. Thirty-four patients were evaluable in this trial. The doses of medication were 600 mg of tegafur p.o. daily and 1-2 mg of lentinan i.v. weekly in the conventional-dose group, and 1,200-800 mg of tegafur p.o. daily and 4 mg of lentinan i.v. weekly in the high-dose group. The response was evaluated using the criteria of Koyama. The response rates were 14.3% for the conventional-dose group and 25.0% for the high-dose group, although no statistical difference was observed. Acute toxicities such as oppression in the anterior chest and dryness of the throat, which were considered to be probably due to lentinan, were noted in patients given rapid administration with 20 ml of solution. However, these effects disappeared with slow-drip infusion using 100-200 ml of solution. These results suggest that the combined tegafur plus lentinan treatment would be better administered at a dose higher than the conventional one for the treatment for advanced cancers.

Inhibitory effect of prolonged oral administration of cetraxate hydrochloride on experimentally-induced intestinal metaplasia in Wistar rats.

The effect of cetraxate hydrochloride (Neuer) on the induction of intestinal metaplasia by intragastric instillation of 5% NaOH solution was investigated in Wistar rats. Oral administration of cetraxate, beginning 2 days after NaOH treatment, resulted in a significant increase in antral mucosal blood flow and a significant reduction in the incidence and amount of intestinal metaplasia in experimental week 25, but no significant changes in basal acid secretion or antral mucosal pH. On histological examination, goblet cell metaplasia were frequently seen. These results show that the increased gastric blood flow produced by cetraxate hydrochloride may be associated with the suppression of induction of intestinal metaplasias.

[Postoperative local thermo-chemotherapy in control of residual pleural diseases after resection of primary lung carcinoma--regulation of dissemination].

Pleural dissemination was proved by intraoperative histodiagnosis in 11 of 121 patients who underwent pulmonary resection for non-small cell carcinoma of the lung between April, 1985 and December, 1986. To control such intrathoracic residual diseases, we devised a means of local thermo-chemotherapy. Ten of 11 patients were treated with intrapleurally administrated cisplatin (50-100mg, bolus) combined with simultaneous radiofrequency hyperthermia (13.56 MHz) for 2 to 3 weeks after surgery. Courses were repeated at 5-to 7-day intervals. Eight patients had N2-disease, one N1 and the other NX. Eight were adenocarcinomas. Thermal burn of the chest wall with hyperkalemia was observed in only one patient who received thermotherapy at the over a magnetrode power of 450 watts for 50 minutes. In the other 9, side effects were minimal under the thermotherapy to obtain a peripleural temperature beyond 42 degrees C. Of the 10 patients, three lived more than 12 months after treatment. Although distant metastases were recognized in 6 cases, none had local recurrence for the median follow-up period of 6 months. One patient had metastasis to the contralateral supraclavicular lymph nodes. Because there was no evidence of intrathoracic recurrences in this patient, radical neck dissection were performed 15 months after the initial operation. This experience warrants further investigation of thermo-chemotherapy as a treatment for controlling pleural dissemination after resection of primary tumor.

Effects of cigarette smoking on the location, healing and recurrence of gastric ulcers.

The effects of cigarette smoking on the location, healing and recurrence of gastric ulcers were investigated by endoscopy. The endoscopic Congo red test developed in this clinic revealed a close relationship between the location of gastric ulcers and the extent of the acid-secreting area, and the smoking index. In general, in patients with a high smoking index, gastric ulcers were located proximally and the acid-secreting area was small. These findings indicate that smoking is associated with the development of fundal gastritis and also with the development of high-lying ulcers. Healing of gastric ulcers was significantly faster, and their recurrence was significantly less frequent in patients who stopped smoking or reduced smoking by a half or more after treatment, than in those who did not.

Pancreas cancer associated antigen from normal colonic mucosa (PCAAc) assayed with monoclonal antibody.

We prepared monoclonal antibodies against pancreatic cancer associated antigen (PCAA) from normal colonic mucosa (PCAAc) and prepared an assay system to detect the circulating PCAAc in sera. 200 patients with cancer including 85 pancreatic cancer and benign disease were compared with 40 normal healthy individuals. Diagnostic rate of pancreatic cancer was 64/85 (75%) but other malignancy also showed elevated PCAAc: 18/19 (94.7%) of hepatoma and 19/37 (51.4%) of gastric cancer. In pancreatitis, 8/17 (47.1%) showed elevated PCAAc. Compared with our previous reports of PCAA assay, these results indicated that diagnostic sensitivity of pancreatic cancer was increased but specificity of it was decreased.

Cell features and patterns in fine-needle aspirates of hepatocellular carcinoma.

The values of the cytologic features of individual cells and cellular patterns in aspirated materials in the diagnosis of 49 hepatocellular carcinomas (HCC) were investigated. Excellent cytologic specimens were obtained by percutaneous aspiration biopsy with a heparinized fine 22-gauge needle. In the well-differentiated type of HCC, a correct diagnosis of malignancy was difficult from the cytologic features of individual cells because of their resemblance to normal hepatocytes. In contrast, in moderately differentiated and poorly differentiated types of HCC, a correct diagnosis of malignancy was easily made from the features of individual cells, but there was little or no cytologic evidence of the hepatic origin of the cells. Comparison of histologic and cytologic findings in aspirated materials obtained from the same patients showed that the cellular patterns seen in cytologic specimens faithfully reflected the histologic structures of HCC. Various characteristic cellular patterns were recognized only in specimens obtained from patients with HCC, but not in those from patients with benign liver diseases. These cellular patterns were very useful not only for diagnosis of malignancy, but also for identification of the hepatic origin of cells. A combination of the features of individual cells and of characteristic cellular patterns raised the diagnostic rates for well-, moderately, and poorly differentiated types of HCC to 90.5%, 100%, and 100%, respectively.

Diagnostic accuracy of ultrasonography for hepatocellular carcinoma.

Accuracy of ultrasonography for the diagnosis of hepatocellular carcinoma (HCC) was examined. The subjects were 5339 patients who underwent ultrasonography of the upper abdomen, during the year 1980 at our hospital. Out of 5339, 113 cases were found to be hepatocellular carcinoma by a 3-year follow-up study, checking against the records of regional cancer registry. Diagnostic accuracy based on 6 months of follow-up was as follows: for identification of liver tumors, sensitivity, specificity, and overall accuracy were 94.9%, 98.7%, and 98.9% respectively; for diagnosis of HCC, sensitivity, specificity, and overall accuracy were 58.9%, 99.9%, and 99.3% respectively. In conclusion, the ability of ultrasonography to detect hepatocellular carcinoma is adequately high, and thus, ultrasonography can be employed as a diagnostic procedure for mass screening.

Value of ultrasonographic examination combined with measurement of serum tumor markers in the diagnosis of pancreatic cancer of less than 3 cm in diameter.

Ultrasonographic examinations and measurements of CA 19-9 and elastase 1 were done simultaneously in 14 patients with resectable pancreatic cancer of less than 3.0 cm in longest diameter and 48 patients with pancreatitis. Although a pancreatic mass was detected ultrasonically in only six (42.9%) of the patients with tumors of less than 3.0 cm in diameter, one or more of the abnormal ultrasonic findings (a pancreatic mass and dilatation of the pancreatic and/or bile duct) was significantly more frequent in patients with pancreatic cancer than in those with pancreatitis. An abnormally high level of CA 19-9 and/or elastase 1 also was significantly more frequent in patients with pancreatic cancer than in those with pancreatitis. The rates of detection of pancreatic cancer by ultrasonography only and by measurement of tumor markers only were 92.9% and 100%, respectively, but the specificities and predictive values of positive results by each of these tests alone were low. Thirteen (92.9%) of 14 pancreatic cancers were found in patients giving positive results by both ultrasonography and measurement of tumor markers, whereas no tumors were found in patients giving negative results in both of these examinations. A combination of these two examinations raised the specificity and predictive value of positive results to 87.5% and 68.4%, respectively, but had little or no influence on the sensitivity or predictive value of negative results. Therefore, patients in whom ultrasonographic findings are abnormal and in whom the serum levels of tumor markers also are abnormally high should be examined more extensively. This combination of examinations indicates the possibility of earlier detection of pancreatic cancer.
