Immunopathological studies of an autopsy case with Goodpasture's syndrome and systemic necrotizing angiitis.

A rare autopsy case of Goodpasture's syndrome with systemic necrotizing angiitis is reported. The patient, a 56-year-old male, died of respiratory failure with massive pulmonary hemorrhage and renal failure. The autopsy showed widespread hemorrhage in both lungs, diffuse crescentic glomerulonephritis, and systemic necrotizing angiitis in the small arteries. Immunofluorescence studies demonstrated a linear deposition of IgG along the glomerular basement membrane (GBM) as well as the alveolar basement membrane. A granular deposition of C3 was also found along the GBM. Electron microscopy showed that the GBM was irregularly thickened and wrinkled, but electron-dense deposits were indistinct. Anti-GBM antibody activity was detected in the patient's serum and had cross reactivity with normal alveolar basement membrane. The renal eluates contained IgG antibody activity for normal human GBM. These results suggest that glomerulonephritis and pulmonary hemorrhage in the present case were mediated by anti-basement membrane antibodies. We also discussed whether anti-basement membrane antibody is involved in the pathogenesis of systemic necrotizing angiitis.

A Japanese Burkitt's lymphoma with t(2;8) and EBNA.

A 56-year-old Japanese man was admitted to the hospital with a large mass in his right axilla. Histological investigation revealed that it was a Burkitt's lymphoma. Ultrastructures of the tumor cells showed immature lymphoid features with frequent lipid droplets within the cytoplasms. Virological studies before the treatment revealed that the lymphoma was closely related to EB virus infection, being positive for EBNA (more than 95% of all tumor cells) and IgG antibodies to VCA (X 5,120), EA (X 640), and EBNA (X 160). The tumor cells exhibited low levels of cytoplasmic IgM and other properties of B cells. They were positively stained with L26, L27, Leu 14, and HLA-DR MAb. In cultured tumor cells, L25 and CALLA antigens were demonstrated, but no surface Ig was shown. In contrast, the tumor cells were negative for T cell markers including AcP, E-receptor, and Leu 1, 2, and 3. Cytogenetic studies demonstrated that the karyotype of the tumor cells was 46, XY, dup (1q), t(2;8)/46, X, -Y, t(2;8), +mar. VEMP therapy was immediately conducted. However, following a two-month partial remission, a relapse with bone marrow infiltration occurred. Thus, a case of Japanese Burkitt's lymphoma with EBNA (+) and t(2;8) properties is described, and the relationships among primary sites, phenotypes, and genotypes are discussed.

[Clinical trials of imipenem/cilastatin sodium in the field of obstetrics and gynecology].

Many different infections including urinary tract infections occur in the field of obstetrics and gynecology. Furthermore, in most cases, it is practically impossible to clearly identify causative organisms as in cases of pelvic peritonitis and parametritis. In many incidents, causative organisms consist mainly of Escherichia coli, and recently, Enterococcus faecalis, Bacteroides fragilis, Klebsiella pneumoniae, Pseudomonas aeruginosa, etc. have also been identified. Because the new antibiotic, imipenem/cilastatin sodium (MK-0787/MK-0791), is very effective against these bacteria, and because of its wide spectrum, we believe it is especially effective against infections of obstetrics and gynecology. The distribution of this antibiotic into various organs and tissues was similar to other drugs, and it reaches a high level in internal genital organs, hence it should be effective for the treatment of obstetric/gynecological infections. Its toxicity is low also, and no side effects nor abnormal laboratory findings were observed in our trials. Results of our trials are summarized below: Of 9 cases of obstetric/gynecological infections, the antibiotic showed excellent effectiveness against 2, good against 6, for the efficacy ratio of 89%. No side effects nor abnormal laboratory findings were observed.

Genetic differences between two substrains of NZB mice.

We have compared the NZB/B1NJ (/NJ) and NZB/B1Pt (/Pt) mouse substrains with respect to a number of immunological and biochemical polymorphisms. In agreement with earlier findings by other workers, we detected histocompatibility antigen difference(s) between the two substrains. Serological analysis indicated that /NJ and /Pt expressed the same allotypes of H-2, Thy-1 and Lyt-1 antigens, while they differed for the Lyt-2 antigen. Amongst 15 biochemical polymorphisms, which we assayed, /NJ and /Pt differed for four, namely Pep-3, Mup-1, Gpd-1 and Hbb. In addition, the two NZB substrains differed for the Hc marker (haemolytic complement).

[Clinical trial of aztreonam in the field of obstetrics and gynecology].

Clinical effect and safety of aztreonam (AZT) in the obstetrics and gynecology were studied in 17 cases of obstetric and gynecological infections. AZT was administered at a dose of 2 g per day by intravenous drip infusion, for 4-8 days depending on severity of the disease. The results showed that AZT was effective in 14 out of 17 cases and the overall efficacy rate was 82.4%. E. coli, Enterobacter, E. faecalis, etc. were detected in 8 out of 17 cases, and after AZT treatment other strains appeared in 3 clinically effective cases. There was a slight elevation of GOT and GPT in 1 case but no other appreciable side effect or abnormal laboratory value was observed.

[Clinical studies on cefpimizole in the field of obstetrics and gynecology].

Cefpimizole (AC-1370), a new cephem antibiotic, was studied clinically in the field of obstetrics and gynecology, and the following results were obtained. AC-1370 was administered to 7 patients with gyneco-obstetrical infections, and the therapeutic efficacy was 85.7% (6 cases of 7 cases). E. coli was isolated as causative pathogen in 4 cases out of 7 cases, and was eliminated in all cases after the AC-1370 treatment. S. faecalis was eliminated in 2 cases out of 3 cases, and B. fragilis was eliminated in all 3 cases. As abnormal laboratory findings, transient elevation of GOT, GPT and A1-P was observed in 1 case but became normal after cessation of AC-1370 administration. The therapeutic effect was poor for this case.

[Results of the clinical use of cefminox in the field of obstetrics and gynecology].

Cefminox (CMNX, MT-141) was used in the treatment of obstetrical and gynecological infections with following results. In the treatment of a total of 8 cases including 4 cases of intrauterine infections, 2 cases of pyosalpinx and 2 cases of pelvioperitonitis, clinical results obtained were rated as excellent in 4 cases (50%) and good in 2 cases (25%). When good and excellent results were combined as representing effectiveness, the effective rate was 75% (6/8 cases). The 2 cases, which gave poor results, were under treatment for ovarian carcinoma. CMNX was found effective for intrauterine infections (postpartal) and pyosalpinx, especially those caused by E. coli. None of the cases experienced side effects.

[Clinical efficacy of ceftriaxone on gyneco-obstetric infections].

Ceftriaxone (Ro 13-9904, CTRX) was administered to 13 cases with gyneco-obstetric infections and the following results were obtained. The responses were excellent in 4 cases (30.8%) and good in 7 (53.8%) out of the 13 cases, the efficacy rate (good or above) being 84.6% (11/13). Two cases showing a poor response were considered inappropriate for a study on the efficacy of an antibiotic, as they had received anticancer drugs concomitantly for treatment of their underlying disease of peritonitis carcinomatosa due to ovarian cancer. From a bacteriological viewpoint, CTRX was effective against E. coli, S. aureus, S. faecalis, Pseudomonas, Staphylococcus, K. pneumoniae, S. epidermidis, etc. No specific side effects were observed.

[Absorption and excretion studies of S6472 (sustained release preparations of cefaclor)].

In order to see absorption and excretion of S6472 (sustained release preparations of cefaclor (CCL], 3 studies regarding, 1) influence of meals (single dose of 375 mg in capsule form), 2) dose response (single dose of 375 mg vs. 750 mg in granule form) and 3) continuous administration (twice-a-day administration of 750 mg in granule form for 8 days) were conducted in 15 healthy adult male volunteers using capsule and granule forms of S6472. The following is the summary of the results from the above studies: Tolerance. In the above 3 studies, none of the 15 volunteers complained of subjective abnormalities. In the clinical laboratory tests performed before the administration of S6472 and at the next day of the completion of the studies, the values from the laboratory tests were within normal range. Influence of meals. The average time for peak plasma level of CCL and decrease in the plasma level following the administration of S6472 were the fastest in the fasted volunteers, followed by the volunteers with light meals and usual meals. From this, it was confirmed that plasma levels of CCL were maintained for longer period of time in the non-fasted volunteers than in the fasted volunteers. In the volunteers who had light and usual meals, the peak plasma levels, AUC of the plasma levels and urinary recovery rate were almost the same. Influence by amount of meals was scarcely observed. Dose response. Mean serum levels and their AUC in the volunteers receiving 750 mg of S6472 were approximately twice as much as those in the volunteers receiving 375 mg of S6472, and dose response between the 2 doses was confirmed. Continuous administration. Mean serum levels and their AUC at the first dose and the 15th dose (final dose) were almost the same. In the continuous administration of S6472 for 8 days (15 doses), no accumulation of CCL in the body was observed.

Autoimmune disease and malignant lymphoma associated with host-versus-graft disease in mice.

BALB/c mice neonatally injected with (BALB/c X C57BL/6)F1 hybrid spleen cells develop host-versus-graft disease (HVGD) with immunopathological features characteristic of either systemic lupus erythematosus (SLE), or immunoblastic lymphadenopathy (IBL) in man. HVGD mice manifest polyclonal hypergammaglobulinemia with various autoantibodies, generalized lymphadenopathy, hepatosplenomegaly, immune complex glomerulonephritis and evolve in the end to malignant lymphoma. The necessary prerequisite for HVGD induction between donor and host can be summarized as follows: Histoincompatibilities in the H-2 region between donor and host are needed; predominant F1 donor cells needed for HVGD induction are, if not sole, steroid resistant, nylon wool nonadherent and Thy-1 positive T cells; the role of donor T cells is not only to present H-2 complex to the host, but also to interact with and proliferate in the host; strain differences are found for the susceptibility of HVGD induction in the host. It has been found that HVGD evolves to a malignant lymphoma of host T cell origin. The HVGD mouse model may, therefore, contribute to the understanding of the cell to cell interactions at work in the pathogenesis of IBL, as well as SLE, in man.

Rat lymphoid cell lines with human T cell leukemia virus production. I. Biological and serological characterization.

Cocultivation of spleen cells, lymph node cells, and thymocytes of female Wistar-King-Aptekman rats with short-term cultured male adult T cell leukemia (ATL) cells in the presence of 5-bromo-2'-deoxyuridine (BrdUrd) resulted in the establishment of rat lymphoid cell lines, TARS-1, TARL-2, and TART-1. Cytogenetic analysis of the three cell lines showed a female rat karyotype with 42 chromosomes. The surface phenotypes of TARS-1 and TART-1 were those of rat T cells. TARL-2 was only positive for rat Ia and leukocyte common antigens. The cell lines continuously produced a type C retrovirus, human T cell leukemia virus (HTLV) and expressed ATL-associated antigens. TARS-1 and TART-1, but not TARL-2 were transplantable into newborn syngeneic rats and nude mice. These results strongly indicate that HTLV not only immortalizes, but also transforms rat T cells in vitro. Adult rats immunized with either TARS-1 or TARL-2 produced antibodies specific for HTLV. The biochemical analysis of the antigens that reacted with rat sera revealed that they are the two HTLV-specific polypeptides, p24 and p28.

Lupus nephritis. Clinicopathology and immunopathology of 80 biopsy cases.

The clinicopathology and immunopathology of 80 cases of lupus nephritis in a total of 1,187 biopsies were presented. Histologically, they consisted of 11 cases of mesangial lupus nephritis (13.8%), 51 cases of diffuse proliferative lupus nephritis (63.7%), and 18 cases of membranous lupus nephritis (22.5%). No cases of focal proliferative type, minimal changes or end-stage kidneys were included. Ten cases were male and they had no qualitative and quantitative differences with female ones in any respects. The microtubular structures were found in glomerular endothelial cells with a 70% frequency and in the interstitial endothelial cells with a 46% frequency. In the latter case, microtubular structures tended to be localized in the perinuclear space as well as in the endoplasmic reticulum. Spherical microparticles were found in 46% of all cases and the incidences were significantly higher in the membranous type. Organized deposits with straight parallel microfilaments were found only in a single case. Electron-dense deposits with fingerprint pattern were not seen. Immunohistochemically, various immunoglobulins, complement components, and fibrinogen were found deposited more frequently in diffuse proliferative lupus nephritis. IgG was the predominant immunoglobulin deposited in capillary loops in membranous types, and IgA in the mesangium in mesangial types. Extraglomerular deposition of immunoglobulins and complements were found in 26.3% of all cases.

[Clinical trial of sulbactam/cefoperazone in the field of obstetrics and gynecology].

Sulbactam/cefoperazone (SBT/CPZ) was administered to 19 patients with various infections in the field of obstetrics and gynecology. The clinical efficacy was judged as excellent in 1 case and as good in 16 cases; hence, the efficacy rate was 89.5%. SBT/CPZ was not effective in a patient with end-stage carcinomatous peritonitis and nor in a patient with pyosalpinx caused by E. coli and Peptococcus magnus. Except a case by E. coli, SBT/CPZ was highly effective for pyosalpinx and pelvic peritonitis where E. coli or K. pneumoniae was the causative organism. No side effects were observed.

A case of primary B-cell lymphoma of the breast. Light and electron microscopy, and immunologic cell markers.

A case of primary non-Hodgkin's lymphoma of the breast was studied with light and electron microscopy and for immunologic cell markers. With light microscopy, the tumor was seen to be a poorly differentiated lymphocytic lymphoma, diffuse, according to Rappaport's classification. With electron microscopy, the tumor was seen to consist of a cytologically uniform cell population. The cells were characterized by irregularly shaped nuclei with infrequent cytoplasmic projections and "microvilli-like" structures, mimicking small cleaved follicular center cells. The immunologic cell marker study showed that the cells were positive for surface IgA and HLA-DR antigen; negative for SRBC receptors, Leu 1, Leu 2a, and Leu 3a antigens. The collective evidence indicated the tumor was of B-cell origin.

[Clinical use of ceftazidime in the field of obstetrics and gynaecology].

Clinical effect and safety of ceftazidime (CAZ) in the obstetrics and gynaecology were studied in 7 cases of intrauterine infections, 8 cases of parametritis, 1 case of Bartholin's abscess, and 4 cases of intrapelvic infections totalling 20 cases. CAZ was administered at a dose of 2 g per day by intravenous drip infusion, for 4--9 days depending on severity of the disease. The results showed that CAZ was effective in 13 out of 20 cases (overall efficacy rate 65%), and especially in intrauterine infections, it was effective in all the cases. However, effects were not satisfactory in 8 cases of parametritis and 4 cases of intrapelvic infections for various reasons such as influences of underlying diseases including cancer. Neither adverse reactions nor abnormal laboratory findings attributable to CAZ were observed during the treatment.

[Clinical use of piperacillin on obstetrical and gynecological infections (Second report)].

We administered piperacillin (PIPC) to patients with obstetrical and gynecological infectious diseases and obtained the following results. 1. Sixteen patients were administered PIPC at a dose of 4 or 9 g by dripping infusion 2 or 3 times a day for a period of 4 approximately 14 days. Eight cases had cancer as their underlying disease (including cervical cancer etc.). The clinical effect was good in 3 of these 8 patients with cancer so that the efficacy rate was 37.5%. But the other 8 cases without cancer all responded effectively, so their efficacy rate was 100%. The efficacy was good in 11 out of 16 cases so the overall efficacy rate was 68.8%. 2. No side effect was observed. 3. No adverse reaction in laboratory findings (including hematological findings, hepatic function tests, renal function tests) was observed.
