[Personal monitoring method for volatile chloroorganic compounds].

A method for measurement of volatile chloroorganic compounds (VCOCs) in air by using porous polymer resin beads, Porapak N was developed as follows: 1. Air was sampled in a glass collection tube packed with Porapak N (1.2 g) at flow rate of 30 ml/min for 24 hours at room temperature. 2. The collection tube was set in a desorption device. The VCOCs adsorbed on the Porapak N were desorbed by heating in a nitrogen flow (180 degrees C, 50 ml/min) and trapped in 10 ml n-hexane cooled by ice. Sufficient recoveries were obtained at desorption time of 8 min. 3. The VCOCs were determined by a gas chromatograph with an electron capture detector (GC-ECD). The present method was applied to determine personal exposure levels for VCOCs. Personal exposure levels of 1,1,1-trichloroethane, carbon tetrachloride, trichloroethylene and tetrachloroethylene were 1.1-6.9, 0.1-0.9, N.D-0.4 and N.D.-4.2 ppb, respectively.

[A case of marked effectiveness using medroxyprogesterone acetate (MPA) in lung metastases of cancer of the uterine body].

This case is a 71-year-old female. As hormonotherapy, 400 mg/day of MPA alone was orally administered in a lung metastatic case of cancer of the uterine body in the IV stage. Although progressing for 2 years and 1 month after the initial administration, lung metastatic foci disappeared, and recurrence was not noted, and serious side effect by using MPA was also not noted. Progesterone therapy such this case is considered as one of therapeutic methods which can be expected remarkably effective rate in the lu metastatic foci of cancer of the uterine body.

Changes of intrahepatic localization of hepatitis B core antigen in HBeAg positive patients with chronic active hepatitis B treated with interferon.

The intrahepatic distribution of hepatitis B core antigen (HBcAg) was studied in 14 HBeAg-positive patients with chronic active hepatitis B who were treated with interferon. Patients received 5 to 6 million units daily of human lymphoblastoid interferon or human diploid fibroblast interferon for 28 days. Intrahepatic HBcAg was detected by the indirect immunoperoxidase technique. Before interferon therapy, the intrahepatic HBcAg was detected almost equally in both the nucleus and cytoplasm. The expression of nuclear HBcAg decreased significantly (p less than 0.001) after interferon therapy, irrespective of the outcome for DNA polymerase and/or HBeAg/anti-HBe status. The expression of cytoplasmic HBcAg decreased significantly (p less than 0.05) only in the patients who lost DNA polymerase after treatment, whereas it increased in patients with DNA polymerase and/or HBeAg in the serum. These findings suggested that a shift of intrahepatic HBcAg from the nucleus to the cytoplasm occurred in HBeAg-positive patients with chronic active hepatitis B receiving interferon therapy.

[A clinical evaluation of the CA 72-4 serum level as a possible tumor marker].

The level of CA 72-4 in serum has been measured in various diseases by an immunoradiometric assay using CA 72-4 RIA Kit for an evaluation of its clinical significance as a possible tumor marker. CA 72-4 showed a high positive rate in patients with ovarian and bile duct cancers. In contrast, the false positive rate was low in benign gynecological and digestive diseases. Serum CA 72-4 was found to be increased in patients with a mucinous cystadenocarcinoma and in other histological types in the early stages of an ovarian cancer. Further, the serum CA 72-4 level changed in parallel with the clinical course of the ovarian cancer. Therefore, our results suggest that measurement of CA 72-4 be combined with measurement of CA 125, so as to provide a better sensitivity and specificity in monitoring an ovarian cancer.

[Cerebral blood flow and event-related potential in patients with cerebral infarction].

Cerebral blood flow (CBF) and P 300, a long-latency positive component of the scalp-recorded event-related potential (ERP) associated with cognitive processing were investigated in patients with cerebral infarction to elucidate the relationship between CBF and cognitive function. Twenty-five patients with cerebral infarction with a mean age of 66.8 years were studied. ERP was recorded during auditory discrimination tasks. The latency of the P 300 from Pz region was recorded. The regional CBF was measured by the 133Xe inhalation method. The F1 (cortical gray matter flow) values were calculated using the Obrist's method. The mean F1 values in patients with cerebral infarction were significantly lower compared to the values obtained for the 15 control subjects (mean age, 68.4 years) (p less than 0.01). Patients with cerebral infarction showed significant prolongation of the P 300 latency compared to the 20 control subjects (mean age, 64.4 years) (p less than 0.02). There was a significant correlation between the P 300 latency and the mean F1 values in patients with cerebral infarction (p less than 0.01). Present results suggest that the cognitive impairment with respect to ERP may be related to the reduction of cortical blood flow in patients with cerebral infarction.

[A case of an operation for lung cancer after PTCA].

A 64-year-old lung cancer patient combined with angina pectoris was admitted to our hospital. Chest roentgenogram showed a coin lesion in the left upper lobe. Broncho-fiberscopic examination proved adenocarcinoma. Coronary angiography revealed 90% stenosis of the left circumflex artery. Percutaneous transluminal coronary angioplasty (PTCA) was done and resulted in success. Eight days after PTCA, left upper lobectomy for lung cancer was undergone. Postoperative course was uneventful. Compared with coronary artery bypass grafting, PTCA was very useful for the patient having malignant disease combined with angina pectoris because of little surgical stress and no delay of operation.

[Isotonic contraction of rabbit superior rectus muscle].

It is well known that the mammalian extraocular muscle has slow muscle fiber morphologically. But the contraction speed of slow muscle fiber has not been mentioned in previous reports. We studied the isotonic contraction of slow muscle fiber and compared it with fast muscle fiber. Each fiber bundle was isolated from the rabbit superior rectus muscle. Both light microscopic findings and the reaction to isotonic contraction in Ca2+ free Ringer's solution could identify each of the two muscle fiber types. Contraction speed depended on the frequency of stimuli (40-200 Hz) in both slow and fast muscle fiber. Maximum velocities of slow and fast fiber bundles were 4.3 +/- 1.53 mm/sec and 26.9 +/- 3.48mm/sec at 200Hz, respectively. It was suggested that the relaxation of slow muscle fiber did not interfere with the contraction of the fast muscle fiber. The relation between contraction velocity and afterload showed an approximately right angle hyperbolic curve.

[Hemolytic anemia and acute renal failure caused by blood transfusions].

We report a case of 33 years old woman who developed hemolytic anemia after blood transfusion. In this case no abnormalities were observed by the cross-match test performed before blood transfusion. She was undergone the surgical resection of liposarcoma of her right arm in June, 1984 and 8 units of the concentrated red blood cells were transfused during the operation. During the following 2 months 13 units of the concentrated red blood cells were transfused. Immediately after blood transfusion, she developed icterus and oliguria and was diagnosed acute renal failure associated with hemolytic anemia. At this time we examined the antigen type of her red blood cells and of three donors' red blood cells which had been given to her. As the result, either C (Rh-hr), Pi (P), Lea (Lewis), Fyb (Duffy) antigens were found in some of donors' RBC, but no such antigens were detected in her RBC. The half life span of RBC measured using 51Cr labeled RBC was 6.2-14.5 hours, in respect of the antigen-positive RBC, where as it was 49.4 hours in the RBC without the antigens. It is more likely that the blood transfusion was the cause of hemolytic episode in this case, because the half life span of the donors' RBC was extremely shortened, compared with that of her own blood which was 16 days.

[Adrenal tumors associated with renal cell carcinoma].

We experienced two cases with renal cell carcinoma who showed enlargement of the contralateral adrenal gland. In case 1, the enlarged adrenal gland was a non-functioning adrenal adenoma, and in case 2, it was a metastatic adrenal tumor. Non-functioning adrenal adenomas are benign tumors of the adrenal cortex often found incidentally at autopsy or on computed tomography (CT) studies of the upper abdomen. Adrenal adenomas have been reported to occur in 9.5-14% of patients with renal cell carcinoma, while the incidence being 1.5-8.7% in the general population. Since metastases of renal cell carcinoma to the adrenal glands are not uncommon, it is important to distinguish between non-functioning adrenal adenomas and metastatic tumors. Therefore, we studied the incidence of adrenal tumors (including metastatic tumors and benign tumors) in patients with renal cell carcinoma who underwent the abdominal CT study in our hospital. From 1982 to 1989, CT had been performed in 67 patients with renal cell carcinoma. Six adrenal masses were identified with CT in these patients. Three out of the 6 patients probably had metastatic tumors and one of the 3 adrenal masses was pathologically diagnosed as metastatic tumor. The other three masses were benign on pathology and only one of them was pathologically diagnosed as non-functioning adrenal adenoma. The incidence of adrenal adenomas on the CT study was lower than that of previous studies based on autopsy. The reasons of this difference in results between their studies and ours are not clear.(ABSTRACT TRUNCATED AT 250 WORDS)

[Biosynthesis of guanidinoacetic acid in isolated rat tubules].

Guanidinoacetic acid (GAA), a precursor of creatine, is an essential substrate for muscle energy metabolism. GAA is formed from arginine and glycine by glycine-amidinotransferase (GAT) mainly in the kidney. But mechanism of GAA synthesis in the kidney has not been defined. The author isolated renal tubules from rats and incubated them with various substrates. GAA was separated by HPLC and measured fluorometrically after reacting with 9, 10-phenanthrenequinone. The following results were obtained: 1. GAA synthesis in the isolated renal tubules was recognized without addition of any substrate. 2. A remarkable increase of the amount of GAA was observed in the presence of glycine and several amidine donors, such as arginine and canavanine. 3. The amount of GAA tended to increase until 3 hours. 4. D.L-norvaline and ornithine suppressed GAA synthesis in renal tubules. 5. Methionine suppressed GAA synthesis. 6. Creatine suppressed GAA synthesis. Negative feedback control of GAA synthesis by creatine was recognized in vitro. 7. GAA synthetic activity was not recognized after the addition of hydroxyurea, citrulline and argininosuccinic acid in the medium. 8. Low concentrations of methylguanidine and guanidinosuccinic acid decreased the amount of GAA. Concentrations used in the experiment were similar to those in uremic serum.

[Involvement of central nervous system in patients with diabetes mellitus detected by evoked potentials].

Somatosensory (SSEPs) and brainstem auditory evoked potentials (BAEPs) were recorded for 44 patients with diabetes mellitus (age, 63.0 +/- 9.3 years, mean +/- SD) and 20 age-matched healthy controls (age, 67.3 +/- 9.1 years) to elucidate the involvement of the central nervous system (CNS) in diabetes mellitus. Furthermore, relationships between SSEP or BAEP and clinical variables such as autonomic nervous function, peripheral nerve conduction velocity, duration of diabetes, metabolic control (fasting blood sugar level, HbA1 and HbA1c levels) and therapeutic method, were studied. The central conduction time (CCT), which represents the peak latency between N13 and N20, in patients with diabetes mellitus was significantly longer than that of the healthy controls (p less than 0.005). Likewise, diabetic patients showed significant prolongation of interpeak latency between waves I and V (I-V IPL) compared to the healthy controls (p less than 0.005). Significant correlations between CCT and motor conduction velocity of median nerve (p less than 0.05) were observed although the correlation between CCT and sensory nerve conduction velocity was not found to be significant (p less than 0.1). Furthermore, I-V IPL was significantly correlated with the duration of illness (p less than 0.05). There were no significant correlations between CCT or I-V IPL and autonomic nervous dysfunction as determined by orthostatic hypotension and the coefficient variation of the R-R interval or metabolic control or therapeutic method. From the present results, it would appear that there is CNS involvement even in diabetic patients not manifesting overt CNS signs and symptoms and that it is correlated somewhat with impairment of the peripheral nervous system and duration of illness.

Isolation of mutants showing temperature-sensitive cell growth from embryonal carcinoma cells: control of stem cell differentiation by incubation temperatures.

Embryonal carcinoma(EC) cells, the undifferentiated stem cells of teratocarcinomas, have many properties in common with pluripotent embryonic cells, and thus provide an excellent system for studying the early events involved in embryonic development and stem cell differentiation. We have isolated three novel mutants with temperature-sensitive(ts) cell growth that were able to differentiate at a non-permissive temperature for cell growth. These mutations affect the progression of the cell cycle, leading to the transient accumulation of cells in a specific phase, the S phase, of the cell cycle, which is likely to be the primary cause of stem cell differentiation of EC cells at non-permissive temperature. Isolation of these mutants strongly supports the notion that there is a close association between the inhibition of DNA synthesis and EC cell differentiation.

Inhibition of DNA synthesis causes stem cell differentiation: induction of teratocarcinoma F9 cell differentiation with nucleoside analogues of DNA-synthesis inhibitors and their inducing abilities counterbalanced specifically by normal nucleosides.

Nucleoside analogues inhibiting DNA synthesis can induce cell differentiation in teratocarcinoma cells. We have examined how their abilities to induce F9 cell differentiation were specifically counterbalanced by their corresponding normal nucleosides. We have also compared the differentiation inducing ability of the wild type F9 cells with that of its thymidine kinase-less mutant using plasminogen activator, as a differentiation marker, which is expressed at a very early stage of endodermal cell differentiation and can be assayed quantitatively. The results obtained were clearly explainable by the conventionally accepted action mechanisms of the nucleoside analogues, thus strongly suggesting that their abilities to induce cell differentiation were direct consequences of the inhibition of DNA synthesis; thus this confirms the notion that a close association exists between the inhibition of DNA synthesis and the induction of teratocarcinoma stem cell differentiation.

Lysosome instability in aged rat brain.

The study of the age-dependent change in lysosomal enzyme activities of the cerebral tissue showed the significant increase of cathepsin D in the aged rat brain, while those of beta-glucuronidase and acid phosphatase remained unchanged. The subcellular distribution study of cathepsin D and beta-glucuronidase revealed the increased activity of these enzymes in the cytosolic fraction from the aged brain. In vitro incubation of the lysosome fraction from the aged rat brain resulted in more leakage of these two enzymes, indicating the instability of the lysosome in the aged brain, which resembled the effect of L-Leu-methyl ester to the lysosome.

Extracellular product of Nocardia amarae induces bacterial cell flocculation.

The fact that Nocardia amarae YK1 produced a bacterial flocculation-inducing substance (designated as FIX) was discovered. FIX had a function of flocculating proliferous cells. FIX-induced flocculation was inhibited by making cells resting, but not completely by adding chloramphenicol. FIX worked widely on Gram-positive to -negative bacteria. In the presence of FIX, Achromobacter cycloclastus IAM1013, Acinetobacter calcoaceticus IAM1517, Bacillus subtilis IAM1069, Escherichia coli C600-1, E. coli IAM1239, Flavobacterium lutescens IAM1667, Klebsiella pneumoniae IAM1102, Micrococcus luteus IAM1313 and Pseudomonas putida IAM1002 formed flocs. B. cereus IAM1029, however, exhibited no flocculation.

Elevated levels of 2',5'-oligoadenylate synthetase activity in peripheral blood mononuclear cells and serum during acute exacerbation of chronic hepatitis B.

We measured 2',5'-oligoadenylate synthetase activity in peripheral blood mononuclear cells and serum of 14 HBsAg- and HBeAg-positive patients with chronic hepatitis B with or without acute exacerbation. Elevated levels of 2',5'-oligoadenylate synthetase in peripheral blood mononuclear cells and serum were found in seven chronic hepatitis B patients with acute exacerbation, whereas in the remaining seven chronic hepatitis B patients without acute exacerbation, both levels were similar to those of normal subjects despite active hepatitis B virus multiplication. 2',5'-Oligoadenylate synthetase levels in peripheral blood mononuclear cells and serum, which were not statistically different from those of normal subjects prior to acute exacerbation, increased during acute exacerbation from 3- to 23-fold over initial levels following elevations in ALT activity. 2',5'-Oligoadenylate synthetase levels fluctuated over a normal range while ALT levels were elevated, and they returned to a baseline with ALT normalization. This suggests that the in vivo interferon system may be activated during acute exacerbation, and that this activation may not be a result of hepatitis B virus multiplication alone, but also of a host-immune response to hepatitis B virus multiplication. Three patients were treated with interferon during acute exacerbation. All three had elevated levels of 2',5'-oligoadenylate synthetase in peripheral blood mononuclear cells and serum just before treatment. 2',5'-Oligoadenylate synthetase levels increased only 1.1- to 2.2-fold over initial levels during treatment, with none of the patients clearing HBeAg during and after treatment.(ABSTRACT TRUNCATED AT 250 WORDS)

Decreased interferon binding capacity of peripheral blood mononuclear cells during interferon treatment of chronic hepatitis B with special reference to (2'-5') oligoadenylate synthetase activity.

The levels of (2'-5') oligoadenylate synthetase (2-5 AS) activity and interferon (IFN) binding capacity were measured in peripheral blood mononuclear cells (PBMC) obtained from 9 patients with chronic hepatitis B (CHB) during IFN treatment. Six of the patients received human lymphoblastoid IFN (HLBI) or IFN-beta daily for 4 weeks at doses of 6 million units (MU); of the remaining three, 2 were treated on alternate days at doses of 3 MU for 5 weeks, and the other given daily IFN doses of 3 MU for 4 weeks. IFN binding was measured using HLBI (2 X 10(8) IU/mg protein) and completely purified 125I-labeled HLBI (290 Ci/mmol specific activity). The levels of 2-5 AS activity in PBMC were measured using radioimmunoassay. Before IFN treatment, the levels of IFN binding capacity were similar to those of normal subjects. During treatment, however, IFN binding capacity decreased significantly from 40 to 60% of initial levels 1 to 2 weeks after the start of treatment in the 7 patients receiving daily IFN, while in the remaining 2 patients given IFN on alternate days, IFN binding capacity did not decrease during treatment. On the other hand, 2-5 AS activity in PBMC increased during IFN treatment in all patients. Even after IFN binding capacity decreased in the 7 patients, the levels of 2-5 AS activity remained high during the remainder of treatment. These finding suggest that some mechanism other than IFN binding capacity may be responsible for the determination of 2-5 AS levels.
