Intimal fibroelastic thickening--its etiological significance in juvenile cerebral infarction. Case report.

Recurrent occlusive stroke of the vertebrobasilar system is reported in a 16-years-old man. Multiple arterial occlusions, segmental stenosis, or irregular dilatation were found in the angiography involving both the intra- and extracranial vertebro-basilar arteries. Surgical specimen of the extracranial vertebral artery with localized irregular lumen disclosed fibroelastic thickening of the intima with mural thrombus, which were thought to be an embolic source. Etiological significance of this intimal lesion in juvenile cerebrosvascular accident is discussed.

Morphological characteristics of the auricular blood vessels related to idiopathic pseudocyst of the auricle.

We examined the actual circumstances of idiopathic pseudocyst of the auricle and morphological characteristics of the auricular vascular system, utilizing the serial sections of tissue blocks embedded in celloidin. While the origin of pseudocyst of the auricle is considered to have much to do with the vascular system, there are still many unknown factors regarding the auricular vascular system itself. The morphology of the proximal portion has been described in various documents, but the details have not yet been elucidated. In order to examine this, we made close observation of the auricular vascular system. The materials used consisted of eight auricles obtained from corpses for the anatomical dissection. Four auricles were macroscopically dissected and observed after being injected with dye into the vascular system, whereas the remaining four auricles were observed with a new staining process using the celloidin serial section method in which sections are alternatively stained with luxol fast blue-PAS-hematoxylin and modified Masson-Goldner stain. In the macroscopical observation of arteries being distributed in auricles, the arteries perforating the auricular cartilage mentioned in documents were not be confirmed. In the observation of the serial sections, capillary plexuses were able to be confirmed along the periphery, very close to the auricular cartilage. Pathohistological findings with regard to four cases of idiopathic pseudocyst of the auricles were examined and closely compared with the histological findings of normal cases. It was presumed that idiopathic pseudocyst of the auricles were caused by bendings or oppressions of auricles, which perhaps caused circulatory disturbance, leading ultimately to partial split of the auricular cartilage.

Double intracranial tumours of maldevelopmental origin--teratoma at the pineal region and an epidermoid cyst in the fourth ventricle.

A 21-year-old man with double intracranial tumours of maldevelopmental origin, teratoma at the pineal region and an epidermoid cyst in the fourth ventricle, is reported. The tumours were removed totally by multiple operations; the epidermoid cyst was resected by suboccipital craniectomy with the patient in prone position and the teratoma was removed via occipital transtentorial approach with the patient in sitting position. Maldevelopmental tumours located within the cranial cavity with different histological types such as the present case have not sofar been described. Histogenesis of these tumours is confusing because teratoma often contains tissues of epidermoid cyst. Etiological considerations for these tumours are presented and appropriate operative procedures for tumours simultaneously occurring in the pineal and the fourth ventricle are also discussed.

Changes of ADP-ribosylation of GTP-binding protein by pertussis toxin in human platelets during long-term treatment of manic depression with lithium carbonate.

1. The study was done to determine whether long-term treatment with lithium carbonate affected the guanosine triphosphate (GTP)-binding protein in platelet membranes from patients with manic depression. 2. It was found that adenosine diphosphate (ADP)-ribosylation of inhibitory G-proteins (40-41 kDa) by pertussis toxin, islet-activating protein in the cell membranes increased about 45% when long-term treatment with lithium carbonate was stopped. 3. These results support the idea that one site for the therapeutic effects of lithium ions is G-proteins.

Changes in adhesion efficiency and vimentin distribution of fibroblasts from familial Alzheimer's disease patients.

Cultured fibroblasts from familial Alzheimer's disease patients were characterized. Familial Alzheimer fibroblasts showed the same appearance by phase microscopy and the same growth rate with the control fibroblasts cultured from the age-matched healthy control subjects. Fibroblasts cultured from familial Alzheimer's disease patients showed decreased efficiency in adhesion to substrata in limited periods. While 80% of the control fibroblasts finished adhesion within 30 minutes incubation, only 30% of Alzheimer fibroblasts completed adhesion within the same period. When fibroblasts from familial Alzheimer's disease were kept under serum-free media more than ten days, they showed a unique aberration of vimentin fiber distribution, while other cytoskeletal fibers were remained intact. It is indicated that fibroblasts cultured from patients with familial Alzheimer's disease can be used to study pathological processes which affect the cytoskeletal organization in the fibroblasts as well as in the cells of the central nervous system.

Approximation of various canine left ventricular end-systolic relations by a cylinder model.

Assuming a linear end-systolic force-length relation and a cylindrical geometry of the left ventricle (LV), we have developed a mathematical model (cylinder model) and derived formulas for LV end-systolic force-diameter (Fes-Des), stress-diameter (Ses-Des), pressure-diameter (Pes-Des), and pressure-cylinder volume (Pes-Ves) relations. To assess the validity of this model for the study of the intact LV, eight healthy beagles were instrumented with ultrasonic crystals to measure LV diameter and wall thickness and a micromanometer to measure LV pressures. Preload was altered by vena caval occlusion. Contractility was increased by addition of Ca2+. Fes-Des relation was nearly linear under control contractility, whereas after addition of Ca2+ this relation became concave toward the diameter axis. Linear approximation was more accurate for this relation than for the Pes-Des and Pes-Ves relations in control contractility. Predictions from the model formulas more accurately reproduced observed data in the Pes-Des (P less than 0.01) and Pes-Ves (P less than 0.001) relations than those from linear regression equations, whereas the model predictions for the Ses-Des relation were as accurate as those of a linear regression equation. Although the slope of the Fes-Des relation increased with inotropic stimulation, diameter intercepts of this relation did not change. Thus the present results obtained under control contractility were consistent with assumptions of the model. This cylinder model might provide reasonable approximations for various canine LV end-systolic relations.

Learning impairment and microtubule-associated protein 2 decrease in gerbils under chronic cerebral hypoperfusion.

A coiled stainless steel wire clip was made that allowed us to chronically reduce cerebral blood flow in Mongolian gerbils. After 6 weeks of reduced cerebral blood flow in 15 experimental gerbils, we evaluated their learning ability and found it to be impaired relative to that in 15 control gerbils. Eight weeks after surgery, regional cerebral blood flow in the parietal cortex measured by the hydrogen clearance method in the experimental gerbils was 73-76% of that in the control gerbils. Light microscopy showed minimal histologic changes in the brains of the experimental gerbils. Concentrations of brain proteins analyzed using sodium dodecyl sulfate polyacrylamide gel electrophoresis showed that among water-soluble brain proteins, the concentrations of cytoskeletal proteins (microtubule-associated protein 2, calspectin, and clathrin) declined in the experimental gerbils. In particular, the concentration of microtubule-associated protein 2 declined significantly. Our findings show that the reduction of cerebral blood flow via carotid stenosis impairs the learning behavior in gerbils, with an associated decrease in the concentration of microtubule-associated protein 2. We believe that Mongolian gerbils with chronically reduced cerebral blood flow are a useful animal model of chronic brain hypoperfusion.

Selective and competitive histamine H2-receptor blocking effect of famotidine on the blood pressure response in dogs and the acid secretory response in rats.

Famotidine has been already demonstrated to be a competitive H2-receptor antagonist in the stomachs of dogs and cats. The present experiments were carried out to examine the effects of famotidine on changes in blood pressure induced by dimaprit and several other agonists in vagotomized, anesthetized dogs and on changes in gastric acid secretion induced by histamine in stomach-perfused, anesthetized rats. Famotidine caused a parallel displacement of the dimaprit dose-response curve to the right with a DR10 value of 0.059 mumols/kg, indicating that famotidine is 166 times more potent than cimetidine in vascular H2-blocking activity. On the contrary, famotidine did not affect the depressor responses to 2-pyridylethylamine and histamine that were antagonized by mepyramine. The histamine dose-response curve was displaced to the right more markedly after simultaneous administration of mepyramine and famotidine than after mepyramine alone. The effects of methacholine, phenylephrine and isoproterenol on blood pressure were not influenced by famotidine in doses up to 720 nmol/kg. In rats, famotidine also caused a parallel displacement of the acid dose-response curve to histamine to the right with a DR3 value of 24 mumols/kg/hr in stomach-perfused rats anesthetized with pentobarbital, exhibiting a potency 108 times greater than that of cimetidine. Analysis of the acid dose-response curve with the Edie-Hofstee transformation showed that famotidine, like cimetidine, was a competitive H2-receptor antagonist.

Isolated human growth hormone deficiency due to the hGH-I gene deletion with (type IA) and without (the Israeli-type) hGH antibody formation during hGH therapy.

Three Japanese patients with isolated growth hormone deficiency from two different families were shown to be homozygous for deletion of the structural gene for human growth hormone (hGH-I gene). These three patients had the same restriction fragment length polymorphism haplotypes. In patient No. 1, the growth rate initially responded well to pituitary human growth hormone, but growth rapidly ceased concomitantly with the development of high levels of anti-hGH antibodies. He again responded well to recombinant methionyl hGH and recombinant hGH without the methionine residue, even though having high hGH antibodies. Two siblings (Patients No. 2 and 3) showed a rather good response to pituitary hGH treatment without hGH antibodies ever being detected (the Israeli-type). hGH-I gene deletions may not necessarily result in hGH antibody formation. Heterogeneity has been observed in isolated hGH deficiency due to hGH-I gene deletion. hGH-I gene analysis should not be limited to patients with hGH antibody formation and subnormal growth responses to hGH therapy.

Canine hepatic vein branches associated with subendothelial mast cells and an adventitial lymphatic plexus.

Branches of the hepatic veins in the dog are equipped with peculiar, periodically arranged sphincter muscles which are known to constrict in response to hematogenous shock agents, causing the severe hepatic congestion characteristic of this species. As was confirmed in this study, the sphincters are more strongly and effectively disposed in the peripheral portion of the veins, including the sublobular and central veins. Mast cells were numerous around the sublobular branches, being specifically gathered beneath the endothelium as recorded by Fujita (1964). The present observation light-microscopically extended his findings, particularly with regard to the distribution of the mast cells along the entire course of the hepatic vein branches--from the proximal trunks through the sublobular veins to the central veins. In addition, mast cell condensation was especially pronounced in the peripheral branches, apparently in accordance with the development of the sphincters. Electron microscope observation confirmed the subendothelial location of the mast cells and revealed that, through an endothelial gap, the cells may extend a microprocess into the venous lumen, thus enabling the direct detection of hematogenous agents. A suspension of a bacterium, Listeria monocytogenes, was injected into the dog livers to induce hepatitis, and the resulting pathologically altered parts of the organ were examined light-microscopically. A heavy infiltration of inflammatory cells was found around the peripheral branches of the hepatic veins. The lymphatics accompanying the veins often contained lymphocytes and macrophages at two days after the injection. At five days, the lymphatics were extremely distended and twisted. The subendothelial mast cells were not encountered at the sites of severe cell infiltration.(ABSTRACT TRUNCATED AT 250 WORDS)

Periventricular lucencies on computed tomography in multiple cerebral infarcts: correlation with cerebral blood flow measurements.

Forty-nine patients with multiple cerebral infarcts with a mean age of 70.2 years were studied to elucidate the effect of periventricular lucencies (PVLs) on cerebral blood flow (CBF). Patients with multiple cerebral infarcts showed significantly lower mean cortical blood flow (F1) values compared to 15 age-matched normal subjects (p less than .01). The mean F1 values were most significantly decreased in severe group of PVLs and moderately decreased in moderate and mild groups (p less than .05 between severe and mild groups); however, no significant differences in mean F1 values were found between severe and moderate groups. Demented patients showed significantly lower F1 values compared to nondemented patients (p less than .001) and normal subjects (p less than .001). There was no significant difference in the degree of PVL between demented and nondemented groups. As well, the mean F1 values were not significantly correlated with either the number of infarcts or ventricular dilatation. These results suggest that PVLs on CT (especially in severe cases of PVLs) may have some role in the reduction of cortical blood flow in patients with multiple cerebral infarcts.

[Measurements of 127I Mössbauer spectra for iodine compounds].

127I Mössbauer spectra of good quality were obtained at 20 K using a Zn127mTe source produced by high flux neutron irradiation of 66Zn-126Te at reactor core. The Mössbauer spectra for KI and some iodine(III) compounds with absorption intensity of about 5% suggest that the 127I Mössbauer spectroscopy is useful in elucidating the bonding nature of the iodine compounds. Preparative method of the Mössbauer source and experimental technique are reported in detail.

[Urodynamic study of the upper urinary tract in the state of ureteral stenting--diuresis renography].

On 24 reno-ureteral units of 21 patients with a silicon ureteral stent, urodynamic study of the upper urinary tract was performed by diuresis radionuclide urography (DRU). We obtained the following conclusions. 1. A silicon stent was useful to maintain urinary stream in ureteral operation. 2. In extrinsic ureteral stricture due to pelvic malignant tumor, pattern of DRU was still poor after indwelling stent. 3. When a stent of the 5-8F's outer diameter was used for an adult, a too thick stent for the ureteral size might rather interrupt the urinary stream. 4. Patterns of DRU and GFR measured by Gates' method in the state of ureteral stenting could warns that hydronephrotic kidney may develop.

[Study on urinary low molecular protein and enzyme in primary vesico-ureteral reflux patients].

Urinary beta-2-microglobulin (beta 2-MG), urinary N-acetyl-beta-D-glucosaminidase (NAG), urinary creatinine (Cr) and serum creatinine level were measured in 79 patients with primary vesico-ureteral reflux. Comparing the data between conservatively treated group and operated group, between pre and post operation, we obtained the following conclusions: 1. From urinary beta 2-MG and NAG, renal tubular damage tended to increase in both conservatively treated group and operated group in proportion to the grade of reflux. But the degree of tubular damage was more severe in the conservatively treated group than in the operated group. 2. Tubular reabsorption ability was lower in the operated group than in the conservatively treated group. 3. In patients with severe tubular damage, improvement of tubular function was seen at the early stage after operation. 4. Anti-reflux operation was shown to be effective even in improving glomerular function.

[Localizing methods of primary hyperparathyroidism and those results].

The results of several pre-operative localizing methods and pathological findings were compared in 11 patients with primary hyperparathyroidism. 1. The accuracy rate was highest with selective blood sampling from the thyroid vein and MRI, which was 75% with both methods. The 3rd was dual photon scintigraphy, the accuracy rate being 62.5%. The 4th was ultrasonography, 57.1%, and the rate was lowest with CT, 25%. 2. MRI-a new localizing method-has a weak point that it cannot differentiate lymph nodes from parathyroid adenoma/hyperplasia. 3. Not to overlook small lesion of primary hyperplasia, several localizing methods should be performed before initial neck exploration.

[Effect of body temperature changes on evoked potentials].

Short-latency somatosensory (SSEPs) and brainstem auditory evoked potentials (BAEPs) were recorded in 12 patients with fever due to respiratory infection (age, 44.3 +/- 20.9 years, mean +/- SD) to clarify the effect of body temperature change on conduction in the central somatosensory and brainstem auditory pathways. Subjects were studied during episodes of fever (37.9 +/- 0.8 degrees C) and after their body temperature had decreased (36.6 +/- 0.3 degrees C). The central conduction time (CCT), which is the peak latency between N 13 and N 20, was significantly longer after body temperature had decreased than during fever. Likewise the interpeak latency between waves I and V (I-V IPL) was significantly prolonged following decrease in body temperature. These results suggest that increases in body temperature have an effect upon conduction in the central somatosensory and brainstem auditory pathways.

Cardiovascular and bronchial actions of famotidine in anesthetized dogs.

The effects of famotidine (Gaster; CAS 76824-35-6) and cimetidine on cardiovascular and bronchial functions were investigated in anesthetized dogs. Famotidine did not affect heart rate, blood pressure, left ventricular pressure (LVP), max. dLVP/dt, cardiac output or coronary blood flow at i.v. doses of 1 to 30 mg/kg in open-chest dogs anesthetized with pentobarbital or a combination of nitrous oxide, oxygen and halothane (GOF). No hemodynamic changes were either observed after famotidine in pentobarbital anesthetized dogs whose cardiac function was depressed by propranolol (1 mg/kg i.v.). On the contrary, cimetidine dose-dependently decreased heart rate and blood pressure at doses greater than 3 mg/kg, and left ventricular pressure, cardiac output and coronary blood flow at the dose of 30 mg/kg. Regarding the electrocardiogram (ECG), famotidine did not produce any remarkable change at doses up to 30 mg/kg with the exception of a transient increase or decrease in the T-wave amplitude at a dose of 30 mg/kg. Cimetidine prolonged Q-T intervals of ECG in addition to changing the T-wave at a dose of 30 mg/kg. Neither famotidine nor cimetidine showed any effect on resting and histamine-increased bronchoresistance at doses up to 30 mg/kg. It is concluded that famotidine is superior to cimetidine with regard to safety because famotidine has no significant effects on cardiovascular functions in anesthetized dogs.
