[Activity of procoagulant and fibrinolysis in homogenate of leukemic cells].

We examined activities of procoagulant and fibrinolysis in homogenate of leukemic cells. Procoagulant activity (PCA) was increased in patients with acute myelocytic leukemia (AML) and acute promyelocytic leukemia (APL), but it was significantly decreased in patients with chronic myelocytic leukemia (CML) and adult T cell leukemia. In CML, PCA was increased in the blastic phase. Plasminogen activator activity (PLGAA) was also increased in patients with AML, APL and acute lymphocytic leukemia (ALL) associated with disseminated intravascular coagulation (DIC). Elastase-like activity, trypsin-like activity and chymotrypsin-like activity (CTLA) were increased in those with myelocytic leukemia, but they were low in those with lymphocytic leukemia. PCA, PLGAA and CTLA were significantly higher in patients with DIC than in those without DIC. Measurement of procoagulant and fibrinolytic activity in leukemic cells homogenate may be useful not only for studying hemostatic abnormalities but also for classification of leukemic cells.

[Comparison between children with missing anterior deciduous teeth and posterior deciduous teeth by analysis of speech sounds].

Growing human offsprings are creatures that communicate by language. If rapidly growing children lose their deciduous teeth very early in life, their language and pronunciation functions may be seriously affected. The authors conducted a series of tests to find how the voice changes when deciduous teeth are extracted. The results may be summarized as follows. 1. There was no significant difference in the formants of vowels on "a-gyo", the first line of the Japanese syllabary, but an appreciable difference was recognized between children (four to six years old) with missing anterior teeth and posterior teeth when there were consonants before and after these vowels. 2. In the formants of the vowels, there was a marked difference in the vowel "i" when children were fitted with no appliance. 3. The strength of voice components in each frequency range was compared between children with missing anterior teeth and posterior teeth. It differed widely in "o ka a sa n" (which means "Mother") and "a-gyo", when children were fitted with no appliance. These findings indicate that the pronunciation of "o ka a sa n" and "a-gyo" can be recovered to some extent if the children are provided with an appliance. However, a sound analysis indicates that their pronunciation ability of sounds on "ka-gyo", "sa-gyo" and "ta-gyo" (the second, third and fourth lines of the Japanese syllabary) can hardly be fully recovered.

[Effect of a protein-bound polysaccharide, PSK, on human hemopoietic progenitors].

Using in vitro clonal culture assays, we investigated the effects of PSK, a protein-bound polysaccharide derived from the cultured mycelium of CM101, Coriolus versicolor (Fr.) Quél in Basidiomycetes, on human hemopoietic progenitors. PSK alone did not stimulate colony formation by human bone marrow progenitors. Although 1-100 micrograms/ml of PSK had no effects on colony formation stimulated by erythropoietin and medium conditioned by phytohemagglutinin-stimulated leukocytes, more than 1 mg/ml of PSK inhibited all types of colony formation. In contrast, medium conditioned by PSK-stimulated leukocytes significantly stimulated formation of various types of colonies including erythroid bursts, granulocyte and/or macrophage colonies, eosinophil colonies, megakaryocyte colonies and mixed hemopoietic colonies. It is speculated that administration of the optimal dose of PSK can reduce the hematological suppression of antitumor drugs.

[Relationship among child, protector and operator at the training-room. The peculiar character of external behavior].

We have studied the external behaviors of the child, protector and operator at the dental care room from the aspects of the child's characteristic features and operator's characteristic features and the protector's degrees of anxiety. The courses from that the child and protector had entered the training-room with the operator till the child began to brush his teeth in the room were divided into four steps, so their external behaviors were investigated at each step. The conclusions were: 1. When compared the protector who had entered the dental care room with the child and had not entered, the former's degrees of anxiety were higher than the latter. 2. The child who had entered the dental care room with the protector had more negative behaviors than the child had entered by himself. 3. The children classified as the 'dependent, standard and independent type' by the characteristic features had equal rates of that their protectors had entered the dental care room. And they had the degrees of negative behavior equally. The protector's degrees of prompt were most in the children classified as the independent type. 4. When the protector had not entered the dental care room, the operator's degrees of prompt during the transfer from the waiting room to the training-room were more than the other steps. 5. When the protector had entered the dental care room, on the operator classified as the standard type by the characteristic features the degrees of prompt during the transfer from the waiting room to the training-room and in the room were more than the other steps. On the other side the operator classified as the stable type had more prompt in the entrance.

[Cognition for visual perception of a student apprentice of dental hygiene to the presence of a parent beside the dental chair for dental treatment. The study of cognitive style and personality traits].

With a view to knowing how the dental hygienist looks at and makes cognition of a mother who is kept standing by her child just receiving his dental treatment, cognition from visual perception was examined, by dental hygienist apprentice. Possible difference in scene cognition process according to different cognition style of these subjects was also analyzed from the elements of eye-movement. On the basis of the results of a YG character test performed together, cognition styles and personality traits were examined additionally. The results obtained were as follows: 1. From the number of total scanned sites upon the observation of test movie, 9 and 6 subjects having reflective cognition style and impulsive cognition style, respectively could be divided out of all 43 subjects. 2. When the mother's countenance turned from calmness to uneasiness, the reflective style-group showed increased gaze at the mother's face. 3. When the mother's countenance turned from calmness to uneasiness, the impulsive style-group showed especially increased gaze at the child's face, with a tendency to gazing obviously biased toward his face. 4. The reflective style-group tended to have visual scanning strategies by which it observes each of the sites under test movie, while always contrasting them. 5. The impulsive style-group tended to have visual scanning strategies by which it observes one of the sites under test movie and then the other. 6. Compared with the reflective style-group, the impulsive style-group showed personality traits such as less depression, low general activity and weak initiative. 7. The above results revealed that a subject having impulsive cognition style found a difficulty in taking a wide view of the situation in which its mother is just present in the treating room and was easily put under visual influence of any change in its mother's countenance.

[Adrenal cyst: report of a case].

This is a case report of an adrenal pseudocyst which developed in a 46-year-old man. The patient experienced dull headache, vertigo and palpitation. The diagnosis of the right adrenal cyst was established by echography and CT scan. The fluid of the cyst obtained preoperatively by percutaneous needle aspiration under ultrasonic guidance was 30 ml of a white, cloudy fluid and contained higher concentrations of cortisol and aldosterone than those in the plasma. The adrenal cyst was removed surgically: it measured 4 x 4 x 4 cm and contained yellow and serous fluid. Histological study revealed that the wall of the cyst consisted of thick collagen fibrous tissue without endothelial cell lining and so this adrenal cyst was classified to pseudocyst of the adrenal grand. The percutaneous needle aspiration under ultrasonic guidance is useful in the diagnosis and treatment of adrenal cyst. This is the 81st case of adrenal cyst reported in the Japanese literature.

[The pharmacokinetics of cisplatin and its influence on renal function according to different infusion methods (report II)--alleviation of renal impairment by bismuth subnitrate combined with ginseng and tang-koui ten].

In the previous report, we discussed in vivo dynamics and renal impairment resulting from the difference in cisplatin CDDP administration. In the present study, we investigated in vivo dynamics and renal impairment under 1 h of continuous drip infusion of CDDP. The free-type Pt concentration ratio, which is considered to indicate antitumor activity in the overall plasma Pt concentration at the end of the administration, was noted to be approximately 64%. This was significantly higher than other administration methods, making this an effective administration procedure. However, NAG, which is considered to be an index of renal impairment along with the amount of beta 2-microglobulin urine excretion, showed higher values than those by other administration methods. Thus, using the 1-hr continuous CDDP drip infusion, we attempted to alleviate the renal impairment, combining bismuth subnitrate with ginseng and Tang-koui ten. As a result, the amount of beta 2-microglobulin excretion in the urine was diminished as well as NAG, and a reduction of the renal impairment was noted. These alleviational effects of the renal impairment were attributed to the method of consistent oral administration by which alleviation could take place with little load on the patient.

SEM study on the dentin and cementum of oreodont teeth.

An SEM study on the dental hard tissue was conducted on the canines and molars removed from a fossil oreodont. The orthodentin was densely distributed with dentinal tubules measuring 2.2 microns in diameter; the tubules were filled with their related odontoblastic processes. They coursed in a parabolic fashion from the pulpal aspect towards the tooth surface. There were many fine lateral offshoots that branched from the tubules, such that the mantle dentin measuring 50 microns in thickness was mostly distributed with these terminal branches measuring 0.7 micron in diameter. However, they did not intrude into the enamel to form spindles and rods. SEM study indicated that the uneven surface of the tubular wall was due to a crisscross of the underneath dentinal matricial fibers, and the peritubular dentin was not observed. The superficial circumpulpal dentin was distributed with interglobular dentin, while the radicular dentin beneath the cementum was additionally distributed with a granular layer. We also observed concentric lamellae corresponding to incremental lines in the dentin, as well as a flat wavy dentinoenamel junction. We noticed the radicular dentin was covered with an acellular cementum except at the apical portion, where it was paved with cellular cementum.

[Laboratory and clinical studies of sulbactam/ampicillin in pediatric field].

We have carried out laboratory and clinical studies on sulbactam/ampicillin (SBT/ABPC). The results are summarized as follows. SBT/ABPC was given by 30-minute drip infusion to 1 child at a single dose of 15 mg/kg and to 2 children at a single dose of 30 mg/kg. After the 30-minute drip infusion, peak serum levels of ABPC(SBT) obtained for the 2 dose levels were 18.0 micrograms/ml (12.4 micrograms/ml) for the former dose level and 81.0 micrograms/ml (53.7 micrograms/ml) and 300 micrograms/ml (200 micrograms/ml) for the latter at the end of injection, and half-lives were 0.84 hour (0.82 hour) for the former and 0.96 hour (1.44 hours) and 0.93 hour (1.19 hours) for the latter. In another trial, SBT/ABPC was given to 1 child at a single dose of 60 mg/kg. After the 30-minute drip infusion, peak serum level of ABPC (SBT) was 82.3 micrograms/ml (45.9 micrograms/ml), and half-life was 1.20 hours (1.36 hours). The urinary excretion rates of ABPC (SBT) were 51.3% (49.5%), 55.8 +/- 10.4% (65.3 +/- 9.1%), 74.0% (76.1%) up to 6 hours after the 30-minute drip infusion of 15 mg/kg, 30 mg/kg and 60 mg/kg, respectively. Treatment with SBT/ABPC was made in 21 cases of pediatric bacterial infections: 8 cases of tonsillitis, 4 cases of bronchitis, 3 cases of pneumonia and 1 case each of pharyngitis, peritonsillar abscess, lymphadenitis, impetigo, abscess and urinary tract infection. Results obtained were excellent in 14 cases, good in 7 cases. No significant side effect due to the drug was observed in any cases except 1 case of fever and rash.

[HTLV-I associated myelopathy (HAM) with adult T-cell leukemia (ATL)].

A 42-year-old female case of HTLV-I associated myelopathy (HAM) combined with adult T-cell leukemia (ATL) was reported. The patient noticed paresthesia in the soles at the age of 17, and gait disturbance and urinary retention at the age of 20. These symptoms progressed slowly. The patient showed indurations in both hypothenar regions and leukocytosis of 17,900/microliters in the peripheral blood at the age of 41. On admission to our hospital, she had spastic gait, muscle weakness of four extremities, increased deep tendon reflexes, pathologic reflexes, mild decreased of vibration sense and vesico-rectal disturbance. Anti-HTLV-I antibody (PA method) was increased to 1,024 fold in serum and 64 fold in spinal fluid. Thus, this patient was diagnosed as having HAM. In addition, leukocyte count in the peripheral blood was increased to 17,200/microliters including 33% atypical lymphocytes, and lymphocytes subset analysis showed 96.1% CD4+, 1.8% CD8+ and marked increase in CD4/CD8 ratio. Southern blot analysis of HTLV-I provirus DNA in the lymphocytes was of complete type with a monoclonal mode of integration. These findings indicated a diagnosis of ATL for this patient. HLA typing revealed A2, A11, Bw22, Bw46, Cw1, Cw3, being compatible with HAM type reported by Usuku et al. HAM and ATL were reported to be caused by an identical virus. However, there has been no case report of combination of the two diseases. Usuku et al proposed that the development of each disease was based on the differences in HLA haplotypes and HTLV-I-specific immune responsiveness.(ABSTRACT TRUNCATED AT 250 WORDS)

[Laboratory and clinical studies of cefpodoxime proxetil in pediatric field].

We have carried out laboratory and clinical studies on cefpodoxime proxetil (CS-807, CPDX-PR). The results are summarized as follows. CPDX-PR was given via oral administration to each 2 children at a single dose of 3 mg/kg and to each of 3 children in a 100 mg tablet. After the oral administration, mean peak serum levels of CPDX obtained for the 2 dose levels were 1.86 +/- 0.35 micrograms/ml and 2.16 +/- 0.63 micrograms/ml at 2 hours, respectively, and mean half-lives were 1.31 +/- 0.02 hours and 1.47 +/- 0.18 hours, respectively. The mean urinary excretion rate of CPDX was 32.8 +/- 1.0% in the first 12 hours after the oral administration of 3 mg/kg. When a dose of 100 mg tablet was given to each of the 3 children, urinary excretion rates in the first 12 hours were 43.5%, 48.6% and 24.8%, respectively. Treatment with CPDX-PR was done in 38 cases of pediatric bacterial infections; 19 cases of tonsillitis, 3 cases of pharyngitis, 1 case of bronchitis, 3 cases of pneumonia, 3 cases of scarlet fever, 2 cases of impetigo, 4 cases of UTI and 1 case each of phlegmone, subcutaneous abscess and balanitis. Results obtained were excellent in 23 cases, good in 15 cases. No significant side effect due to the drug was observed in any cases.

[A cyclic thrombocytopenia associated with Sjögren syndrome].

A 51-year-old women in menopause was admitted because of intermittent purpura. She was diagnosed as having cyclic thrombocytopenia. One platelet cycle lasted for about 22 days with platelet count fluctuating from 1.0 X 10(4)/microliters to 31.0 X 10(4)/microliters. The platelet-associated immunoglobulin was correlated with the platelet cycle. The rheumatic factor, antinuclear antibody, antimicrosome antibody, anti-thyroglobulin antibody and platelet binding immunoglobulin were all positive but did not fluctuate during the cycle. The platelet life-span was shortened. The mean platelet volume fluctuated during each cycle. The lipid emulsion test was prolonged. Symptoms of Sjögren syndrome and purpura appeared at the same time. It was strongly suggested that purpura was caused by cyclical destruction of platelets by reticuloendothelial system.

Endothelin: a new inhibitor of renin release.

Endothelin is a recently-discovered vasoconstrictor peptide which is produced by endothelium and acts on vascular smooth muscle cells. At present its actions on other organs or cells are unknown. We studied the effect of endothelin on renin release in a dynamic superfusion system of dispersed rat juxtaglomerular (JG) cells. Endothelin in concentrations of 10(-11) M or more inhibited renin release dose-dependently and this inhibitory action vanished in the absence of extracellular Ca. It is suggested that endothelin is an inhibitory regulator of renin secretion from JG cells and its action is Ca-dependent.

Enterotoxin of Clostridium perfringens type A forms ion-permeable channels in a lipid bilayer membrane.

The enterotoxin of Clostridium perfringens type A was found to form ion-permeable channels in a lipid bilayer. A patch clamp technique was used to detect channel activities in an asolectin bilayer with incorporated enterotoxin. About 20% of the lipid bilayer patches examined showed rectangular or stepwise shift of membrane current. The shifts indicated the gating of ion-permeable channels in the patches. The channels showed high conductance (40-450 pS), no rectification in current-voltage curves and occasional long-lasting events. The significance of these findings is discussed in relation to the mechanism of action of the toxin.

Synergism of BSF-2/interleukin 6 and interleukin 3 on development of multipotential hemopoietic progenitors in serum-free culture.

We investigated the effects of B cell stimulatory factor 2/interleukin 6 (BSF-2/IL-6) on the development of murine hemopoietic progenitors using serum-containing culture and serum-free culture. In serum-containing culture, BSF-2 mainly supported multipotential blast cell colonies from spleen cells of normal and 5-fluorouracil (5-FU)-treated mice. In serum-free culture, no colony growth was seen in the presence of BSF-2. Addition of BSF-2 to the serum-free culture containing IL-3 resulted in a significant increase in the number of colonies formed from multipotential progenitors in spleen cells and bone marrow cells of 5-FU-treated mice, whereas no effects were seen on the number of single or oligolineage colonies formed by the spleen cells of normal mice. These results suggested that BSF-2 and IL-3 act synergistically on the multipotential progenitors but not on the maturer progenitors. When BSF-2 was added to a culture containing low concentrations of IL-3 (1 U/ml, 4 U/ml), which had little effect on colony formation, the number of total colonies formed by the spleen cells and bone marrow cells of 5-FU-treated mice increased significantly. The combination of BSF-2 and 40 U/ml of IL-3 resulted in a significant enlargement of GMM colonies. Thus, BSF-2 appears to enhance the sensitivity of multipotential hemopoietic progenitors to IL-3.

Gelsolin is Ca2+-sensitive regulator of actomyosin system in platelet.

Effects of gelsolin on the actomyosin system in platelet have been studied. MgATPase activity of platelet actomyosin is enhanced up to two folds by 200 nM of platelet gelsolin in the presence, but not in the absence of Ca ion. The half maximum enhancement is observed at the concentration of Ca2+ around 10(-5) M. The effect of gelsolin to enhance the ATPase activity of actomyosin is potentiated by tropomyosin, which is a Ca2+-insensitive actomyosin enhancer. The results indicate that gelsolin may control the activity of actomyosin system in platelets.
