HTLV-I-associated myelopathy with adult T-cell leukemia.

We report a 42-year-old Japanese woman with HTLV-I-associated myelopathy (HAM) combined with adult T-cell leukemia (ATL). Combination of the 2 diseases has been extremely rare. The infrequency is explained by HLA types unique to each disease. Our patient suggests that the HAM-associated HLA haplotype does not prevent the development of ATL.

Biphasic accumulation of leukocytes in rat cardiac infarct tissue caused by leukotriene B4 and complement.

An initial increase (3-12 hr) in the polymorphonuclear leukocytes (PMN) counts after ligation of the rat left main coronary artery was reduced by 49.4% (at 12 hr) by a 5-lipoxygenase inhibitor, AA-861 (80 mg/kg, p.o., 1 hr before ligation). Depletion of the complement components induced by cobra venoma factor (CVF) (i.v.), given one day before, resulted in significant reduction in the PMN accumulation after 12 hr (by 63.6% at 24 hr). The combined treatment (CVF+AA-861) suppressed the PMN accumulation by 69.7% (24 hr). The infarct size at 48 hr was also reduced by approximately 36% by either AA-861, CVF or combined treatments.

[Case report of periodontal disease with periodic neutropenia].

A case of periodic neutropenia with specific symptoms of gingiva and alveolar bone loss in primary dentition is reported. The patient was a boy, and it was followed between the ages of 3- and 9 years-old. At the period of neutropenia he was suffered from recurrent redness, swelling and bleeding of the gingiva, and also with cold-like symptoms with fever. Although the cause of these symptoms was not definite at first, even in medical exploration, we could diagnose this disease as periodic neutropenia by the process of dental therapy. By alveolar bone resorption, pocket formation and severe tooth mobility, the patient had lost more than half of his primary teeth at the end of 2 years after the initial examination. However throughout this period we motivated and instructed the patient and his mother how to brush his teeth. His oral hygiene was improved in the mixed and permanent dentition stages. Then there was no evidence of bone resorption around the teeth of the permanent dentition stage, but recurrent gingival symptoms are continuing now. The specific feature of this case is found in the gingival findings. There was no ulceration of the oral mucosa, but a bright red jelly-like appearance was found periodically in the area of the marginal and attached gingiva. The junction between the reddened attached gingiva and the alveolar mucosa was well-defined. The serial examination of peripheral blood in this case revealed the cycle of neutropenia as 16 to 21 days. During this examination, and at the almost time of neutropenia of this patient, hyperplasia of monocytes and lymphocytes occurred. In this situation there was no clinical signs except for gingival redness. However at other times, generally marked signs were recurrent. This may have occurred due to the lack of both neutrophils and monocytes. From the above results we think that periodic neutropenia may be diagnosed first in the dental clinic by the distinctive redness in the gingival area as one specific sign of this disease. We also think that plaque control procedures may retain the teeth.

Behavior of 3,4-endiol form of 2,3-diketo-gulono-delta-lactone formed from dehydro-L-ascorbic acid in deoxygenated and neutral solution.

The formation of L-ascorbic acid (AsA) was observed when dehydro-L-ascorbic acid (DHA) was dissolved in neutral buffer solutions under N2 bubbling at room temperature. The reduction of DHA was done with the lactonized compound of 2,3-diketo-L-gulonic acid (DKG), that is, the 3,4-endiol form of 2,3-diketo-gulono-delta-lactone (3,4-End DKGL). 3,4-End DKGL was formed from DHA or DKG (yield about 10%) under N2 bubbling in neutral buffer solution (pH 7.2). This material was not stable in neutral or alkaline solutions. 3,4-End DKGL suppressed more strongly the linoleic acid (LA) peroxidation in the medium containing 20% EtOH and 10 mM LA than did AsA. This may suggest the possibility that 3,4-End DKGL reproduces AsA from DHA in physiological status.

Effect of atrial natriuretic factor on renin and aldosterone.

Atrial natriuretic factor (ANF) inhibits basal and stimulated aldosterone production in vitro and in vivo. Infusions of ANF have been reported to inhibit renin release in several species, including humans. A 4-h infusion of ANF into 12 patients with mild hypertension reduced plasma aldosterone at 2 and 4 h, with a rebound increase 2 h after the end of the infusion. Plasma renin activity also showed a mild but significant reduction at 2 h with a rebound increase at 6 h. The effect of ANF on renin release in vitro is controversial. Using a superfusion system of rat renal cortical slices and of dispersed juxtaglomerular (JG) cells, we found that ANF did not affect basal or isoproterenol-stimulated renin release by the kidney slices or dispersed JG cells. These studies suggest that ANF does not inhibit renin release by a direct effect on the JG cells and that the inhibition of renin release found in vivo studies may be mediated through a macula densa mechanism. Since calcium plays an important role in aldosterone secretion, we investigated the effect of ANF on calcium fluxes in rat adrenal glomerulosa cells. ANF did not affect either calcium influx or calcium efflux in rat adrenal glomerulosa cells. These studies suggest that the inhibitory effect of ANF on aldosterone production is distal to the calcium messenger system.

[Study on thermal pulp testing of immature permanent teeth].

The duration of reaction to cool stimulation was investigated in 185 normal maxillary right and left central and lateral incisors from 55 pediatric patients aged from 6 to 15 years, who were brought to the Department of Pedodontics of Nippon Dental University Hospital, and from 9 students aged from 19 to 20 years in the School of Dental Technology and Hygiene affiliated with Nippon Dental University by using Dichloro-Difluoro-Methane coolants [PULPER (G-C Dental Industrial Corp.) and PULPOFLUORANE (Septodont Co.)] in order to determine the efficacy of thermal pulp testing for mature and immature permanent teeth. The results obtained are as follows: 1) The mean duration of reaction to cool stimulation according to the type of teeth was 0.93 sec for the maxillary left lateral incisors, 0.91 sec for the maxillary left central incisors, 0.88 sec for the maxillary right central incisors and 0.95 sec for the maxillary right lateral incisors, showing no statistically significant difference in the duration between the tooth types. 2) The duration of reaction to cool stimulation according to age was shortened with aging except for the 6-year-old patients. 3) The patients were divided into those 10 years old or younger and those older than 10 years because the tooth roots of the maxillary anterior teeth are developed at 10 years of age. The mean duration of reaction to cool stimulation was 1.02 sec in the group with immature permanent teeth, which consisted of patients aged 10 years or younger, while it was 0.82 sec in the group with mature permanent teeth, which consisted of patients aged 11 years or older.(ABSTRACT TRUNCATED AT 250 WORDS)

Study on the behavior management of children at dental treatment. Various factors affecting the frequency of training.

Search was made using quantification method of the first and second type to search the factors affecting the frequency of training, and to estimate it. The following conclusions were led; 1. Factors giving large effect on the decision of the frequency of training were 'Frankl classification', 'ages at the initial examination' and 'if children have any experience of dental treatment or not'. 2. With the factors concerned with educational environments including mothers, 'number of brothers and sisters', 'mother's educational history' and 'if mothers works or not' showed high correlation. 3. Among the factors concerned with character analysis, 'emotional stability' 'if children can control themselves or not' gave much effect. 4. As the factors to estimate the frequency of training, the reliable factors include 'Frankl classification', 'ages at initial examination', and 'sexes'. 5. With the factors concerned with educational environments including mothers, 'mother's educational history', 'number of brothers and sisters' 'birth order' were highly reliable. 6. With the factors concerned with character analysis, 'emotional stability', 'if children fants are degenerative or productive' showed high reliability. 7. Multiple correlation coefficient for all the 27 factors was 0.823, which was found to be highly reliable for the estimation of the frequency of training.

[Dental study of hypophosphatasic child].

Hypophosphatasia is a hereditary disease characterized by congenital deficiency of tissue alkaline phosphatase. One of its dental features is the shedding of teeth. We have experienced a case which was referred to our clinic with the chief complaints of loss and shedding of lower deciduous central incisors and was diagnosed to be highly suspected of hypophosphatasia as a result of hematological examination at the pediatric clinic. As a result of close systemic and dental examination and further study of changes in the course for two and a half years, the following findings were obtained: 1. Low serum alkaline phosphatase level and hypercalcemia were the systemic findings of the disease. 2. There was no delayed ossification in terms of carpal bone age. 3. X-ray cephalography revealed no particular cephalic abnormality, except the lack of development of frontalis and mandibular. 4. Dental findings comprised marked resorption of upper alveolar bone in the front teeth area and pronounced instability of front deciduous teeth. 5. As for changes in oral symptoms, extraction of A because of pronounced instability was followed by spontaneous shedding of A. About one year later, A was extracted because of marked instability. At the time of this writing, A was slightly instable without any abnormality in the other teeth. 6. Pathohistological study of the extracted teeth disclosed only a little resorption of cement of root apex and dysgonic periodontium without any marked tissue changes.

[Cognition for visual perception of a student apprentice of dental hygiene to the presence of a parent beside the dental chair for dental treatment].

With a view to finding out how the dental hygienist looks at and recognizes the changes in facial expression and behavior of a mother who is staying by her child just receiving dental care in the treating room, actions which the mother takes near her child were recorded and subjected to observation by a dental-hygienist apprentice. The eye movements of the apprentice were then recorded and analyzed, using the Visicon-Eye-Camera. The results obtained are as follows: 1. In all scenes of the test image, the subject closely observed the mother's face, the child's face and trunk and the dentist's face at a high frequency. 2. In all scenes of the test image, the subject tended to observe closely the mother's face in the former half of his observation and the child's face and the dentist's face in the latter half. 3. In a scene during which the mother looked anxious, the subject tended to look a longer time per observation. 4. In a scene during which there occurred only a change in the mother's expression, the subject frequently took the mother's face, the child's face or the dentist's face as his primary point of observation. 5. The scanning path of the subject was not influenced by a change in the mother's expression, but rather was influenced by the addition of new elements into the scene such as her grasping of the child's hands. 6. In case of the mother beginning to make a body movement, the subject frequently took the mother's face and her child's face as his primary point of observation. 7. The subject tended to make close observation of the dentist's face less frequently with more changes in the mother's behavior, suggesting his tacit cognition of the dentist being there. 8. The above results reveal that the dental hygienist is not visually influenced by the co-existence of a mother and her child just receiving dental care in the treating room if only she is looking at her child in a relaxed manner without any anxious expressions and attitudes.

[Changes on pronunciation using the removal appliance for children with early loss of deciduous teeth. Comparison between good occlusion children with missing anterior-posterior deciduous teeth].

The purpose of this study was to examine missing anterior teeth and posterior teeth for possible differences according to the where the teeth were missing site, a comparison was made between children with such defects and those with relatively good occlusion. The following results were obtained: 1. Compared with children with missing anterior teeth, children with missing posterior teeth showed more differences in the frequency range for vowel formants from those with relatively good occlusion. 2. As for vowel formant, missing anterior teeth and missing posterior teeth presented more such differences for formant i and formants i and e, respectively. 3. For each formant, more such differences were noted for the first formant and the third formant compared with comparatively fewer such differences noted for the second formant. 4. In terms of the strength of voice components in each frequency range, consonant pronunciation rather than vowel pronunciation proved to be influenced by missing either anterior or posterior teeth. 5. For vowel pronunciation, more such differences were noted at low to intermediate frequency ranges. 6. Pronunciation of words containing the letter t and k tended to be improved for missing anterior and posterior teeth, respectively, but pronunciation proved to be improved in a difficult manner. The above results revealed that compared with missing anterior teeth, missing posterior teeth presented more differences from children with relatively good occlusion, suggesting that not only space maintenance and masticatory function but also speech function must be considered in preparing the removal appliance for missing posterior teeth of children.

[Laboratory and clinical studies of clarithromycin in pediatric fields].

We have carried out laboratory and clinical studies on clarithromycin (TE-031, A-56268). The results are summarized as follows. Serum and urinary concentrations of TE-031 were determined in 5 children with ages between 6 and 11 years given single oral doses of 5, 6.7, 10 and 15 mg/kg. Serum concentrations peaked at 1 hour after administration of 5, 6.7 and 15 mg/kg, and respective peak values were 1.98 micrograms/ml, 2.21 micrograms/ml and 5.58 micrograms/ml. Biological half-lives for the drug at 5, 6.7 and 15 mg/kg dose were 2.99 hours, 2.08 hours and 2.09 hours, respectively. Mean serum concentrations peaked at 2 hours after administration of 10 mg/kg, and peak values were 3.91 +/- 1.64 micrograms/ml. Biological half-lives were 3.00 +/- 0.58 hours. The 6-hour urinary recovery rates ranged from 22.7% to 23.8% after administration of 10 mg/kg, and the 6-hour urinary recoveries were 30.1%, 20.5% and 39.1% after administration of 5 mg/kg, 6.7 mg/kg and 15 mg/kg, respectively. Therapeutic responses were recorded as excellent or good in 35(89.7%) of the children, comprising 5 with tonsillitis, 3 with pharyngitis, 7 with bronchitis, 5 with pneumonia, 15 with Mycoplasma pneumonia, 1 with whooping cough and 3 with Campylobacter enteritis. The microbiological effectiveness of TE-031 on identified pathogens comprising 2 strains of Streptococcus pneumoniae, 5 strains of Haemophilus influenzae, 2 strains of Haemophilus parainfluenzae, 5 strains of Mycoplasma pneumoniae and Campylobacter spp. was satisfactory as evidenced by an eradication rate of 82.4%. No significant side effect due to the drug was observed in any cases. In conclusion, TE-031 was found to be efficacious and safe for the treatment of bacterial infections in children.

[Evaluation for the training and behavior of dental treatment in our special patients center].

With a view to modifying the respective behaviors of patient, protector and operator so that a relationship of reliability can be established among them, we have adopted the training for dental treatment in the medical care system for handicapped children. We took this opportunity to classify the present handling methods at the practice of medical care into 4 groups (A, B-1, B-2, C) and to examine the training effect in each group and its features. [Method] On 145 patients having received treatment in the Dental Center for the Handicapped Children in our Hospital, number of patients by group, average age at the first examination, average training frequencies before and after treatment, average treating frequency, behavior in the training and relationship between each group and the disorder were examined for the card. [Results] 1) About 40% of the subjects for examination became capable of receiving treatment without any controlling appliance before the final treatment. 2) Average training frequency and average treating frequency had higher values of B-1 group than for other groups. 3) Behavioral estimation during the training revealed that A and B-1 groups showed better performance in mouth washing, brushing and cleaning by brushing than B-2 group, but the case was the contrary with oral use of three-way syringe and the vacuum. [Conclusion] Application of various behavior-modifying techniques to the training for dental treatment in children with psychosomatic disorder made its effect and features.

[Research for our special patients center. Relation to the developmental screening table and the first training check list of B-1, B-2 group].

We have adopted a behavior-modifying method in the medical care system for children with psychosomatic disorder, using the developmental screening table and the first training check list to make the method effective. Also in order to know the effect of training, patients were classified into 4 groups (A, B-1, B-2, C). We have recently made a comparative examination of both the developmental screening table and the first training check list between B-1 and B-2 groups out of these 4 groups. [Method] Subject for investigation are 32 patients belonging to B-1 and B-2 groups out of those patients who have received medical examination in the Dental Center for the Handicapped Children in our Hospital. Developmental ages and proportions permitting each behavior were determined from the table for the developmental screening table and the first training check list, respectively. On the basis of the about the developmental screening table and the first training check list, behaviors between at the consultation room and at home were compared for 2 items of greeting and gargling and the condition of applying these items was also examined. [Results] 1) The B-1 group had well-matched 5 items compared with the developmental age for the B-2 group and was easier in communication with the operator because of its high cognitive ability. 2) The B-2 group, whose speaking and sociality were low, was more difficult in communication with the operator in the practice of dental treatment, and was evident from the first training check list. 3) Lack of patient's adaptability and communication with the operator often represented patient's capability of greeting and gargling at home but not at the consultation room. 4) Some need for improvement occurred in non-answering the developmental screening table and non-entering some items of the first training check list.

[Clinical study of gingival hyperplasia in epilepsy patient].

Diphenylhydantoin, which was first introduced in clinic by Meritt and Putnam in 1938, has been in wide use for the treatment of epilepsy because of its excellent antispasmodic action. On the other hand, Diphenylhydantoin induced gingival hyperplasia has been examined by many authors in the dental field since gingival hyperplasia was reported by Kimball as its side effect. Recently, we had an opportunity for performing gingivectomy on hypertrophic gingivitis in an epileptic with mental retardation and visual disturbance, on Phenytoin (5,5-Diphenylhydantoin) medication. Some information obtained is reported, together with its therapeutic course.

[Hemostatic changes before and after the onset of disseminated intravascular coagulation].

We performed a blood coagulation study during the course of disseminated intravascular coagulation (DIC) in 34 patients with hematological malignancies. Risk factors of DIC such as increasing tumor mass, anti-tumor therapy and severe infections were frequently observed at onset of DIC, and influenced the prognosis of DIC. Before the onset of DIC, the DIC score and FDP value were slightly elevated, and they were significantly increased after the onset of DIC. Before the onset of DIC, the level of fibrinogen was significantly increased but it was decreased after the onset of DIC. These hemostatic abnormalities continued for about 2 weeks. Patients with DIC showing prolonged APTT and PT had a poor prognosis. The abnormalities of PT, FDP, fibrinogen, DIC score, FDP-D-dimer and fibrinopeptide A were significantly greater in DIC than in Pre-DIC defined as the period one week before the onset of DIC. FDP-D-dimer was also higher in Pre-DIC patients than in those without DIC. Although protein S and C 4 b binding protein were not decreased in DIC or Pre-DIC, Protein C activity decreased during the course of DIC, suggesting that FDP-D-dimer and Protein C activity were useful for diagnosis of Pre-DIC and DIC.
