Small-bowel endoscopy.

Up-to-date, small-intestinal endoscopy has been developed along three main lines (a push-type fiberscope, a rope-way-type fiberscope, and a sonde-type fiberscope), and each fiberscope has both merits and demerits. We can observe small-intestinal diseases sufficiently, wherever they arise int he small intestine, with a suitable scope, chosen according to the shape of the lesions or the location in the small intestine. Now we have a basis for the correct diagnosis of small-intestinal diseases in using three types of fiberscopes. Further advances in recent enteroscopy are the application in pediatric enteroscopy, endoscopic surgery for polyps of the small intestine, magnifying enteroscopy and so on. Further problems in enteroscopy are how to utilize it for studies of pathophysiology of the small intestine and how to improve its technique to make it easier.

Analysis of the clinical features of ischemic colitis.

Although ischemic colitis is not rare in the elderly with episodes of ischemia involving the heart and brain, clinical features of mild cases (transient type) have seldom been reported. Whereas the diagnostic findings of ischemic colitis are said to be longitudinal ulcers and stricture, longitudinal ulcers were noted only in 38% of the transient type in our series. For the correct diagnosis of the transient type, it was necessary to observe slight mucosal changes such as edema, congestion and petechiae by colonoscopy. It is emphasized that for the diagnosis of the mild transient type of ischemic colitis emergency colonoscopy must be performed as soon as possible after the onset of abdominal pain or rectal bleeding.

Metabolism of 4-hydroxyaminoquinoline-1-oxide and its binding to DNA in chick embryos.

The metabolic pathway of 4-hydroxyaminoquinoline-1-oxide (4HAQO) and its binding to DNA was studied in 2-day chick embryos administered [G-3H]4HAQO in a shell-less culture. The 4HAQO rapidly metabolized into non-carcinogenic compounds and 1 h after administration only very small amounts of free 4HAQO could be detected in the embryo cells. The amount of DNA-bound 4HAQO in the embryo cells reached a maximum 2 h after administration, then began to decrease. The maximum extent (mu mol/mol P of nucleotide) was 18.2, equivalent to 1 molecule of 4HAQO-purine adducts per 2.8 X 10(4) base pairs of DNA. It was possible to detect removal of 4HAQO-purine adducts from DNA in chick embryo cells in a shell-less culture. A dose-response relationship for the killing effect of 4HAQO on 2-day embryos was observed in the range of 0.24-24 nmol 4HAQO per embryo. The practicality of the present method of administration of 4HAQO for 'flash administration' of compounds to chick embryo and the advantages of the shell-less culture method which provides access for biochemical and developmental studies of chick embryos were also discussed.

Effects of phospholamban phosphorylation catalyzed by adenosine 3':5'-monophosphate- and calmodulin-dependent protein kinases on calcium transport ATPase of cardiac sarcoplasmic reticulum.

To elucidate the role of 22000-dalton protein phospholamban, a putative regulator of Ca2+-dependent ATPase of cardiac sarcoplasmic reticulum, we examined the relationship between cyclic AMP- and calmodulin-dependent phosphorylation of phospholamban and their effects on ATPase activity and calcium transport of cardiac sarcoplasmic reticulum. Cardiac microsomes were incubated with [gamma-32P]ATP or unlabeled ATP, catalytic subunit of cyclic AMP-dependent protein kinase and/or exogenous calmodulin, and subsequently assayed for ATPase activity and calcium uptake by cardiac sarcoplasmic reticulum. Cyclic AMP-dependent phosphorylation of phospholamban was independent of Ca2+, whereas calmodulin-dependent phosphorylation of phospholamban was dependent on Ca2+ within a range between 0.2 and 50 microM. Cyclic AMP- and calmodulin-dependent phosphorylation of phospholamban occurred independently; when both kinases were operative, the amounts of phosphorylation were additive. Under these conditions, the phosphoproteins formed by cyclic AMP- and calmodulin-dependent protein kinases electrophoretically migrated as 11000-dalton components when sodium dodecyl sulfate-solubilized phosphoproteins were boiled prior to polyacrylamide gel electrophoresis. The ATPase activity was stimulated by either cyclic AMP- or calmodulin-dependent phosphorylation of phospholamban at Ca2+ concentrations up to 2 microM. The extents of stimulation of ATPase activity were additive when both types of phosphorylation were functional. Calcium uptake was similarly augmented by cyclic AMP- and/or calmodulin-dependent phosphorylation of phospholamban. These results indicate that Ca2+-dependent ATPase and calcium transport of cardiac sarcoplasmic reticulum are regulated by phospholamban phosphorylation catalyzed by cyclic AMP- and calmodulin-dependent protein kinases, thus suggesting a dual role of phospholamban in active calcium transport.

Actions of amino-beta-carbolines on induction of sister-chromatid exchanges.

Synthetic 3-aminoharman and 3-aminonorharman (amino-beta-carbolines) caused slight but definite induction of sister-chromatid exchanges (SCEs) in human lymphoblastoid cells NL3 and Chinese hamster cells CHO-K1. These amino-beta-carbolines are ranked between 2-amino-alpha-carboline and 2-amino-6-methyl-9a-aza-delta-carboline (Glu-P-2) and much lower than 3-amino-gamma-carbolines (Trp-P-1 and 2) in inductive activity. 1-Amino-beta-carboline, harman and norharman had very weak, if any, SCE-inducer activity. Norharman had a synergistic effect with aromatic amines such as Trp-P-2 and aniline on SCE induction, while 3-aminoharman suppressed SCE induction by more potent inducers such as Trp-P-2 and benzo[a]pyrene.

Presence of 3-amino-beta-carbolines (3-amino-9H-pyrido[3,4-b]indoles), effectors in the induction of sister-chromatid exchanges, in protein pyrolysates.

3-Amino-9H-pyrido[3,4-b]indole (3-aminonorharman) and 3-amino-1-methyl-9H-pyrido[3,4-b]indole (3-aminoharman), effectors for induction of sister-chromatid exchanges in human lymphoblastoid cells, were identified in pyrolysates of casein by high-performance liquid chromatography and UV spectrophotometry, together with some other mutagenic/carcinogenic carbolines. In pyrolysates from a preparation of zein, which contained little tryptophan, only norharman and harman were found in small amounts.

Transient collateral augmentation during coronary arterial spasm associated with ST-segment depression.

To examine the possible existence of collateral circulation during coronary artery spasm, we attempted to visualize the transient appearance of collateral vessels that could serve to salvage otherwise jeopardized ischemic areas. In three patients with vasospastic angina, total spastic obstruction of a major coronary artery was associated with transient collateral augmentation, which was supplied by the nonspastic artery during anginal period associated with ST-segment depression. These collateral vessels disappeared when the angina and ST changes resolved after nitroglycerin administration. These findings suggest that the collateral blood supply could transiently occur through preexisting vessels to perfuse the ischemic area during coronary artery spasm and that such collateral flow could have a role in preventing transmural myocardial ischemia, resulting in a lesser degree of ischemia associated with ST-segment depression.

Arteriographic features of angina pectoris associated with ST segment depression during coronary arterial spasm.

To examine the angiographic features of vasospastic angina associated with ST segment depression, we attempted to analyze the coronary arteriograms of 12 patients who exhibited ST segment depression during the ergonovine provocative test. Right and left coronary arteriograms were obtained successively within a short period when the ergonovine administration revealed ST segment depression. Eight out of 12 patients showed non-total spastic obstructions in one of the major coronary arteries. Among them, a collateral augmentation was found only in one patient. Two cases exhibited the well-developed collateral channels during non-anginal periods and in one case a collateral blood supply was reduced by the spasm occurred in the donating artery. In another one, the collateral circulation did not change during anginal period. Three out of 4 patients who showed total spastic obstructions demonstrated transiently augmented collateral circulation which was supplied by the non-spastic artery. These findings may indicate that ST segment depression during coronary artery spasm could attribute to a subendocardial ischemia caused by an incomplete occlusion of large coronary artery and transient reduction or augmentation of collateral blood flow.
