Comparisons of EEG sleep state-specific spectral values between healthy full-term and preterm infants at comparable postconceptional ages.

Differences in state-specific electroencephalographic (EEG) spectral values are described between groups of preterm and full-term neonates at comparable postconceptional term ages. Eighteen healthy preterm neonates of < or = 32 weeks gestation were selected from an inborn population of a neonatal intensive care unit. Twenty-four-channel recordings were obtained at a full-term age and compared with studies of 22 healthy full-term neonates. The initial three hours of each 12-hour study were recorded on paper from which EEG sleep state scores per minute were visually assessed. Six mean spectral values (i.e. total EEG, electromyogram, delta, theta, alpha and beta energies) were calculated from each corresponding minute of digitized data, which was also assigned one of six EEG sleep states. Each neonatal group displayed statistically significant differences among sleep-state segments for all spectral values. The alpha- and beta-range spectral values of the preterm group, compared to the full-term control group, were lower during all sleep state segments. Spectral values for the theta band were lower during both quiet sleep segments only, whereas spectral values for delta were lower during all sleep stages, except tracé-alternant quiet sleep. Significant differences in EEG spectral values were noted among states of sleep for both preterm and full-term infants of similar postconceptional term ages. These data also suggest differences in central nervous system maturation between neonatal populations. These findings strengthen our previously stated contention that there is a functional alteration in brain development of the preterm infant as reflected in sleep organization that results from a prolonged extrauterine experience and/or prematurity.

Modification of a whole room indirect calorimeter for measurement of rapid changes in energy expenditure.

Whole room indirect calorimeters are among the most accurate devices for measurement of human energy expenditure and have provided useful data about determinants of total daily energy expenditure. However, a limitation of whole room indirect calorimeters has been the inability to detect acute (usually < 10-15 min) changes in energy expenditure. This precludes using these devices to study the acute effects of food, drugs, or exercise on energy expenditure. We describe modifications made to our whole room indirect calorimeter (respiratory chamber) to allow accurate measurement of energy expenditure over time periods as short as 1 min. The modifications involve changes in the system design and use of signal processing techniques. With these modifications, we can measure energy expenditure in 1-min intervals throughout the day. This allows accurate study of the acute effects of food, exercise, or drugs on energy expenditure in subjects moving freely inside the respiratory chamber. The ability to use respiratory chambers for these types of studies should improve our understanding of how body weight is regulated.

Cardiorespiratory behavior during sleep in full-term and preterm neonates at comparable postconceptional term ages.

Cardiorespiratory behavior during sleep has been investigated by comparing visually analyzed minutes of EEG sleep with the digitized values of these two physiologic variables for each corresponding minute. Continuous 3-h nighttime sleep studies on 37 full-term and preterm neonates at comparable postconceptional term ages were acquired under controlled conditions, using a 24-channel computerized monitoring system and an automated event-marker program. Five thousand, two hundred ninety-four minutes were assigned an EEG state by traditional criteria. Eighteen preterm infants were compared with 19 full-term infants with respect to six cardiac and six respiratory measures: two nonspectral calculations (i.e. average per minute and variance of the means) and four spectral calculations of the cardiorespiratory signal (i.e. bandwidth, spectral edge, mean frequency, and ratio of harmonics). The relative capabilities of these measures to predict a sleep state change were investigated using discriminant analysis. A stepwise selection algorithm in discriminant analysis was used to identify the order of significance for the remaining variables. Eight cardiorespiratory measures were then submitted to multivariate analysis of variance to assess sleep state or preterm-full-term differences: mean frequency, bandwidth, average per minute, and ratio of harmonics for cardiac signals; and spectral edge, mean frequency, logarithm of variance, and ratio of harmonics for respiratory signals. Differences among the sleep states and between neonatal groups were highly significant (p < 0.0001). Interaction between sleep state and neonatal group was also significant (p < 0.034). Two variables differentiated preterm from full-term respiratory behavior: ratio (p < or = 0.001) and mean frequency (p < or = 0.02).(ABSTRACT TRUNCATED AT 250 WORDS)

Screening anti-HIV Chinese materia medica with HIV and equine infectious anemic virus reverse transcriptase.

The inhibitory activities of human immunodeficiency virus reverse transcriptase (HIV-RT) inhibitors PFA and Suramin against HIV-RT and equine infectious anemic virus reverse transcriptase (EIAV-RT) were studied in this paper. The 50% inhibitory concentration (IC50) of HIV-RT and EIAV-RT treated by PFA and Suramin were 0.2 mumol, 9.8 mumol and 17 mumol, 19.9 mumol, respectively. More than thirty Chinese medicines, including recipes, herbs, extracts of traditional materia medica and isolated compounds were tested using HIV-RT and EIAV-RT as target enzymes. It was found that 5 crude extracts such as Rhizoma Polygoni Cuspidati, and Radix Notoginseng, 7 isolated compounds like Flavone of Ramulus Visci, Flavone of Ajuga Decumbens Thumb, and Aristolochic acid, as well as the extract of the complex prescription Xiao Chai Hu Tang have shown various inhibitory actions on these two enzymes. The activities of the two enzymes were comparable, but HIV-RT was more sensitive, suggesting that HIV-RT can be used as index for the screening of anti-aids drugs.

A method for measuring mechanical work and work efficiency during human activities.

A major limitation in understanding human energy balance is the ability to accurately quantify the amount of physical activity or work performed by human subjects. We describe the development and validation of a force platform (2.5 x 2.5 m) system for measuring external mechanical work performed by human subjects. The force platform system was constructed inside a whole-room indirect calorimeter, thus, allowing simultaneous measurement of the energy expenditure associated with physical activity. We demonstrated the accuracy of the system for measuring work performed using a solenoid (32 kg) to deliver controlled amounts of energy to the platform. In tests with human subjects we demonstrated that the system can accurately and reproducibly measure work performed, energy expenditure associated with work performed, and work efficiency (work performed divided by energy expenditure). Overall, we obtained high correlations (an average of 0.932 for 33 subjects), between mechanical work performed and energy expenditure. We believe this mechanical work-energy expenditure system will be useful in determining the importance of individual differences in amount and cost of physical activity in the regulation of body weight and in development of obesity.

Long-term immunity to poliovirus in children immunized with live attenuated and enhanced-potency inactivated trivalent poliovirus vaccines.

Eighty-six children who completed immunization with the two trivalent poliovirus vaccines, live attenuated (OPV) and enhanced potency inactivated (EIPV), in one of four schedules (OPV-OPV-OPV, EIPV-EIPV-EIPV, EIPV-OPV-OPV, and EIPV-EIPV-OPV) at 1 year of age were monitored serologically over the subsequent 4 years and challenged with OPV at 5 years of age. Each of the immunization groups exhibited an initial 10- to 100-fold decline in neutralizing antibody to poliovirus types 1, 2, and 3 during the first 2 years of follow-up; thereafter antibody titers stabilized. The EIPV-EIPV-OPV group maintained the highest antibody levels throughout the observation period, including the response to OPV challenge at 5 years of age. These data suggest that immunization with OPV, EIPV, and combinations of the two vaccines confers long-term immunity. Optimal systemic immunity was associated with two or more doses of EIPV.

Osteogenic sarcoma with clinically detectable metastasis at initial presentation.

PURPOSE: Chemotherapy and surgery have improved the length of survival for patients with osteogenic sarcoma (OS) who present without metastatic disease. We reviewed our experience with patients with OS who presented with clinically detectable metastasis to determine the prognostic factors and the effects of surgery on the primary tumor and on metastatic disease. PATIENTS AND METHODS: From 1975 to 1984 we treated 62 patients who had previously untreated OS with metastasis detected at presentation. All of these patients received intensive chemotherapy that included high-dose methotrexate; doxorubicin; and bleomycin, cyclophosphamide, and dactinomycin (BCD). Selected patients also received cisplatin. The intent of surgery was resection of the primary tumor and metastatic disease. RESULTS: Survival was extremely poor; only 11% of patients survived, with a median survival of 20 months. Survival was not affected by use of preoperative chemotherapy versus immediate surgery, and did not correlate with serum lactate dehydrogenase (LDH) level, alkaline phosphatase level, or the site of the primary tumor. Survival did correlate with age, location of metastatic disease, histologic response to preoperative chemotherapy, and completeness of surgical resection of all sites of tumor. Resection of all sites of tumor identified at initial presentation was necessary for survival. CONCLUSION: OS that presents with metastatic disease has a very poor prognosis with therapy, although therapy has achieved good results for patients without metastasis detected at diagnosis. Aggressive surgical resection of tumor is necessary for survival. The use of novel therapies at initial presentation is justified with this group of patients.

Effect of immune RNA on NK activity of infants born to mothers with cytomegalovirus infection.

The effect of specific immune RNA (i-RNA) on the activity of natural killer (NK) cells in the peripheral blood of 27 newborn infants was studied by lactic dehydrogenase (LDH) release assay. Peripheral blood mononuclear cells (PBMC) were used as effector cells with K-562 tumour cells as target cells. The results showed that in the therapeutic group (n = 17) the average activity of NK cells after therapy (24.13 +/- 4.62%) was 13.67 +/- 5.79% higher than before therapy (10.47 +/- 3.57%). In the control group (n = 10) these were only 10.78 +/- 3.94% and 11.58 +/- 3.37%, the difference was not statistically significant (P > 0.05). However, NK activity in the therapeutic group and control group was markedly different (P < 0.01). The data indicate that specific i-RNA can enhance the activity of NK cells in the peripheral blood of newborn infants. Therefore, we believe that specific i-RNA may play an important role in defence against CMV infection. The mechanism of i-RNA augmenting NK cellular activity is also discussed.

[Blood rheology and cognitive function in elderly patients with uncomplicated isolated systolic hypertension].

The relationship among blood pressure, blood rheology and cognitive function were evaluated in 40 elderly patients with isolated systolic hypertension (ISH) by control-case study. The cognitive function was lower in the patient group than in the control group (79.92 + 17.05 vs 91.24 + 14.06, P < 0.01). Among the hypertensive patients, the weak correlations were observed between the cognitive function and systolic blood pressure (r = -0.11, P < 0.05), but the correlations were strengthened when the two groups were considered together (r = -0.44, P < 0.01). Blood viscosity at low shear rate and plasma viscosity in the patient group were greater than in the control group (P < 0.05, P < 0.01). A close relation existed between blood viscosity at low shear rate and cognitive function (r = -0.82, P < 0.01), whereas the patients with normal cognitive function had viscosity similar to that in control subjects (r = -0.20, P < 0.05). Thus, increased blood viscosity may be a determinant or a response to decrease cognitive function in the elderly patients with uncomplicated systolic hypertension.

Peptidolytic monoclonal antibody elicited by a neuropeptide.

We report evidence that a monoclonal antibody raised by immunization with a vasoactive intestinal peptide (VIP)-carrier protein conjugate selectively hydrolyzes VIP and a fluorescence quenched decapeptide (FQ14-22D), representing the region of VIP most susceptible to autoantibody-mediated cleavage (residues 14-22). A high affinity of the antibody for VIP and a lower affinity for FQ14-22D were revealed by kinetic studies and further substantiated by potent inhibition of FQ14-22D cleaving activity by full-length VIP. Sequencing of FQ14-22D hydrolysis products indicated selective cleavage at one peptide bond. These observations suggest that antibodies induced against naturally occurring polypeptide antigens can express peptidolytic activity targeted for specific sequences in the recognition epitope.

Chemical investigation of the metabolites from the Canadian tuckahoe, Polyporus tuberaster.

The metabolites of the Canadian tuckahoe, the sclerotium of Polyporus tuberaster, have been investigated. The ten-membered lactone tuckolide [1] was isolated, and its structure was determined by spectroscopic methods and confirmed by X-ray crystallography. Ergosterol, ergosterol peroxide, and several unidentified ergosterol derivatives were also obtained, along with an unidentified disaccharide.

Nutrient balance and energy expenditure during ad libitum feeding of high-fat and high-carbohydrate diets in humans.

To study the influence of diet composition on regulation of body weight, we fed 21 weight-stable subjects (11 lean, 10 obese) high-carbohydrate (HC) and high-fat (HF) diets for 1 wk each. Although diet composition was fixed, total energy intake was unrestricted. Subjects had a higher energy intake on the HF (11,039 +/- 2700 kJ/d) than on the HC (10,672 +/- 2617 kJ/d) diet (P less than 0.05), but energy expenditure was not different between diets. On day 7 of the HC diet, carbohydrate (CHO) oxidation was significantly related to CHO intake with the slope of the regression line 0.99, suggesting that overall CHO balance was near zero. However, the slope of the regression line was greater for obese than for lean subjects. On day 7 of the HF diet, fat oxidation was significantly related to fat intake but the slope of the line was 0.50, suggesting that overall fat balance was positive. However, this relationship was due entirely to lean subjects, with obese subjects showing no relationship between fat intake and oxidation.

Short-term effects of dietary-fat ingestion on energy expenditure and nutrient balance.

Joule for joule, dietary fat may promote obesity more than protein or carbohydrate. In this study we determined whether the addition of 50 g dietary fat to a standard breakfast would increase energy expenditure or fat oxidation during the immediate 6-h postprandial period or over the ensuing 18 h. We also determined whether subjects with a high level of aerobic physical fitness would show a greater increase in fat oxidation after the ingestion of the extra fat than would less fit subjects. Adding fat did not increase fat oxidation or energy expenditure either during the immediate 6-h postprandial period or over the following 18 h. This was true regardless of the subject's fitness level. Acutely, dietary fat ingested in excess of its usual rate of oxidation appears to be stored in the body. Being physically fit does not appear to provide an advantage in avoiding short-term storage of excess dietary fat.

Relationship between aerobic fitness level and daily energy expenditure in weight-stable humans.

The relationship between exercise and energy expenditure is unclear, with some suggestions that exercise leads to increased energy expenditure over and beyond the increase due to the exercise itself. In this cross-sectional study, we examined the relationships among aerobic fitness level, body composition, and total daily energy expenditure in 78 subjects. Daily energy expenditure (determined in a whole room calorimeter) was significantly correlated with both fat-free mass (FFM) and aerobic fitness (estimated from maximum aerobic capacity or VO2max). However, multiple-regression analysis demonstrated that, after accounting for FFM, VO2max did not explain a significant amount of the remaining variation in energy expenditure. In addition, the relationship between resting metabolic rate and both FFM and VO2max was evaluated using data from 214 weight-stable subjects analyzed retrospectively. The results were identical with the results obtained from the 78 subjects in that VO2max did not have effects independent of FFM on energy expenditure. We conclude that aerobic fitness does not have a direct effect on energy expenditure. However, it may have effects that are mediated through body composition, since in both populations studied here, VO2max was positively correlated with FFM and negatively correlated with adiposity.

[Effects of ANF and ACTH on aldosterone secretion in cultured human adrenal tissue].

We studied the effects of various concentrations of ANF and ACTH on aldosterone secretion in human normal adrenal tissue over different response times in vitro. We found that ANF 10(-8) mol/L at first stimulated but 24 h later inhibited basal aldosterone secretion. ANF 10(-8) mol/L also inhibited the ACTH-stimulated secretion of aldosterone. ACTH 10(-8) mol/L did not stimulate aldosterone secretion, while ACTH concentrations of 3 x 10(-8) and 5 x 10(-8) mol/L did. When ACTH and ANF were added simultaneously, the ANF-induced inhibition of ACTH-stimulated aldosterone secretion could be overcome by increasing the concentration of ACTH. Our results show that ANF caused a dose-dependent and time-dependent inhibition of basal and ACTH-stimulated increases in aldosterone secretion.
