[The effect of disopyramide phosphate in extrasystole at rest and on exercise (author's transl)].

The antiarrhythmic action of disopyramide phosphate was compared to placebo in a randomized double blind trial at rest and under physical exertion in two groups each of 10 patients with extrasystoles. The exercise was carried out on the bicycle ergometer in a recumbent posture in stages of 25 watts at 2 minutes each without a break to the limit of the individual. The extrasystoles were counted at fixed registration periods. It was shown that the number of extrasystoles in the disopyramide group was significantly lower in comparison to the placebo group (P less than 0.05).

[Mechanics of circulation in lowering the blood pressure in different hemodynamic forms of hypertension (author's transl)].

The high blood pressure of essential hypertension sustained by an increased peripheral resistance or by cardiac output, or by both of these magnitudes. To examine the mechanics of lowering the blood pressure in these different forms of hypertension on analysis of circulation was carried out on three patients with a cardiac output hypertension and on seven patients with a resistance hypertension before and after lowering the blood pressure by the compound preparation Briserin. The results showed that the influence on the hemodynamics was different. The therapeutic result was due partly to a decrease of the cardiac output and partly to a reduction in vascular peripheral resistance. The increased parameter of circulation decreased during the treatment. Occasionally however the cardiac output as well as the peripheral resistance were decreased or while one of the parameters decreased the other even increased.

[Segmental wall movement of the left ventricle in healthy persons and myocardial infarct patients studied by a catheter-less nuclear medical method (camera-cinematography of the heart)].

Camera-Kinematography is a nearly noninvasive method to investigate regional motion of the myocard, and allows evaluation of the function of the heart. About 20 min after injection of 15-20 mCi of 99mTC-Human-Serum-Albumin, when the tracer is distributed homogenously within the bloodpool, data acquisition starts. Myocardial wall motion is represented in an appropriate quasi three-dimensional form. In this representation scars can be revealed as "silent" (akinetic) regions, aneurysms by asynchronic motion. Time activity curves for arbitrarily chosen regions can be calculated and give an equivalent for regional volume changes. 16 patients with an old infarction have been investigated. In fourteen cases the location and extent of regions with abnormal motion could be evaluated. Only two cases of a small posterior wall infarction did not show deviations from normal contraction pattern.

[Clinical studies on the effect of drugs used in the treatment of angina].

The report is based on a four-centre study on 25 patients with coronary insufficiency (ECG signs). In random sequence the following were given to each patient: a nitrate (isosorbide dinitrate), carbocromene, practolol, and oxyphedrine. The initial criterion for inclusion was S-T depression of 0.2 mV on stepwise exercise (bicycle ergometry). S-T depression, systolic blood pressure and heart rate changes were used as criteria. For each drug there was an acute test and a long-term test of 12 days. The Friedman test was used for the statistical evaluation of the results. Isosorbide dinitrate decreased the extent of S-T depression in the acute tests while practolol did so in the long-term test. Practolol reduced systolic pressure on long-term administration, with no difference in the acute experiment. Heart rate was reduced by practolol both acutely and long-term. These results give no indication for any possible differential treatment of coronary insufficiency, but this may be due to the selection of patients in this study.

[Camera-cinematography of the heart (author's transl)].

By "camera-cinematography" of the heart, we mean an isotope method which permits detailed observation of cardiac mechanics without the use of a catheter. All that is necessary is an intravenous injection of 10 to 15 mCi 99mTc human serum albumen followed after ten minutes by a five to ten minute period of observation with a scintilation camera. At this time the isotope has become distributed in the blood. Variations in the precordial impulses correspond with intra-cardiac changes of blood volume during a cardiac cycle. Analysis of the R-wave provides adequate information of cyclical volume changes in limited portions of the heart. This is achieved by a monitor with a pseudo-3-dimensional display; contraction and relaxation of the myocardium can be shown for any chosen longitudinal or horizontal diameter of the heart. Our programme allows simultaneous presentation of the movement of any point on the myocardium as a time-activity curve. The method is recommended as an addition to chest radiography, heart screening or cardiac kymography before carrying out cardiac catheterisation.

The effect of radar on cardiac pacemakers.

The susceptibility of 16 noncompetitive cardiac pacemakers to radiation from a powerful radar system was investigated in the laboratory and in the vicinity of its prototype. From comparative in vitro tests in air, fat, water, and saline it was concluded that only tests in fat or air represent the worst case condition after implantation. In air all pacemakers showed signs of interference at pulse power densities between 0.025 mW/cm2 and 62.5 mW/CM2. Three of six implanted pacemakers were triggered or inhibited depending on their mode of operation when tested at a location 1.2 km away from the radar station by the radar beam occurring every 5.5 sec. Because interfering radiation can enter the pacemaker circuitry directly along the electrode, acting as an antenna, metal encapsulation of the pulse generator does not provide sufficient shielding against microwave radiation. However, pacemakers modified by metal encapsulation and a low-pass filter at the electrode remained undistrubed at pulse power densities of greater than 10 W/cm2 when tested under worst case condition in air.

Disturbed myocardial calcium metabolism: a possible pathogenetic factor in the hereditary cardiomyopathy of the Syrian hamster.

In the BIO 8262 inbred strain of cardiomyopathic Syrian hamsters, a latent disturbance of their myocardial calcium metabolism could be evidenced. Whereas the myocardial calcium content of untreated young cardiomyopathic hamsters with prenecrotic hearts did not differ from that of healthy control animals, it was distinctly elevated 6 hours after injection of isoproterenol (1 mg/kg body weight s.c.) in cardiomyopathic animals remaining unchanged in healthy controls. However, the same dose of isoproterenol induced elevated myocardial 45Ca uptake in both strains, although that of the cardiomyopathic hearts was distinctly greater. Later, during the stage of spontaneous progressive necrotization of the hearts, a spontaneously increased myocardial uptake of 45Ca and calcium content became manifest. By combined treatment with isoproterenol and verapamil, a substance which is known to decrease the calcium conductivity of myocardial cell membranes without blocking beta-adrenergic receptors, the isoproterenol-stimulated 45Ca uptake by prenecrotic cardiomyopathic hearts as well as the increase of their calcium content could be inhibited. Long-term treatment with verapamil alone beginning during the prenecrotic phase of the cardiac condition, was fully effective in preventing myocardial overload as well as necrotization. These findings demonstrate that overload with calcium of cardiomyopathic cells of the hamsters can be influenced beneficially. Therefore, the disturbed myocardial calcium metabolism in the hereditary cardiomyopathy of the Syrian hamster is considered a decisive pathogenetic factor.

Left ventricular function in Syrian hamsters of different ages with hereditary cardiomyopathy.

Hemodynamic data of cardiomyopathic hamsters of the strain BIO 8262 aged 70 days and more than 200 days were compared with those of control groups. Left ventricular systolic pressures and heart rate revealed no great differences between the groups. Left ventricular diastolic pressure was lower in both older groups than in both younger groups. Contractility, as measured by dp/dt max as well as VCE and Vmax, showed considerably lower values in the myopathic groups. Although congestive heart failure connot be observed in the BIO 8262 strain, a decrease in cardiac performance can be postulated particularly in the 70-day age group in the stage of acute necrosis. The hypothesis is supported that circulatory congestion seen in the BIO 14.6 hamster is a separate entity, not necessarily connected to the myocardial lesions.
