[Diagnosis and treatment of trauma to blood vessels around the knee joint (author's transl)].

Eighteen arterial reconstructions after trauma in the region of the knee joint were performed between 1970 and 1976. In seven cases there had been fracture of the distal femur, in five of the head of the tibia, in three dislocation of the knee joint or severe contusion of the popliteal artery. In 12 cases referral had been eight hours or later after injury. The concomitant vessel injury was not recognised in six. Vascular reconstruction by direct suture with a venous patch was performed in eight, interposition of a venous autograft in five, and saphenous vein bypass in four. Results depended upon length of ischaemia time and the level of the vascular injuries. Complete restitution to normal was achieved in only two cases, while in the remaining 12 there were defects of motor and sensory function as seen in the post-ischaemia syndrome. In four cases above-knee amputation was necessary despite successful reconstruction of the artery because there was irreversible ischaemic damage with tissue necrosis. To improve these results careful examination of blood vessels and, in questionable cases, angiography is recommended for every trauma victim.

Emergency procedure for digoxin radioassay.

A rapid but precise radioassay for digoxin has been developed by optimizing the conditions for antigen-antibody interaction and for separation of bound and free fractions. This new procedure involves incubation of radioactively labeled antigen, antibody, and standard or sample at 3 degrees C for 10 min. Charcoal is then added, the mixture centrifuged for 5 min, and the radioactivity of the supernate counted. Results are available in less than 1 h after receipt of the specimens, making the procedure suitable for emergency determinations. Results correlate well with those by our routine procedure (Becton Dickinson) (r = 0.9739, y = 0.0356 + 0.9915x, n = 49). Analytical recovery of added pure digoxin exceeds 94%, both within and between assay CV's are less than 8%.

[Intraoperative instrumental autotransfusion. 1st experiences in vascular surgery and traumatology].

Intraoperative autotransfusion with the Bentley-ATS system was performed in 22 patients, saving a total of 98 000 ml of their own blood. In addition 77 000 ml of homologous blood were given, intraoperatively. Eleven patients underwent major vascular surgery and 11 were operated for major traumatic injuries. There was a total of 11 deaths overall but none related to the autotransfusion. It is concluded that intraoperative autotransfusion is a safe and simple method of rapid transfusion which can save significant volumes of precious homologous blood.

[Hyperextending ruptures of vessels in course of arthroplasties of the hip and knee (author's transl)].

The authors referred specific complications in the course of arthroplasties of the hip and of the knee: hyper-extending-ruptures of vessels in sequence of indirect lesions. Causes of these so far not described complications are extreme passive articulations of hip respectively knee during operation. Clinical symptoms are enumerated, the pathogenetic mechanisms explicated and directions for treatment given. Important are the knowledge of these complications and the measure for their prophylaxis.

[Duplication of the external and common iliac veins with a venous trunk anterior to the artery (author's transl)].

Short duplications of the stem forming an island where the artery crosses it have been described as isolated variants of the v. iliaca communis. V. iliaca externa and interna opening into the vena cava separately and a femoral vein opening into the ipselateral internal iliac vein are isolated observations of irregular junctions. An observation of duplication of the v. iliaca externa and to some extent also of the v. iliaca communis with concomitant duplication of the v. femoralis is reported, one of the two iliac venous trunks taking a course ventral to the external and common iliac arteries. These numerical abnormalities in combination with the irregular position of the v. iliaca externa and partly also of the common iliac vein are observed here for the first time.

Metabolic and hormonal studies comparing three parenteral nutrition regimens in infants.

Metabolic and hormonal studies were performed in 6 infants during the first 3 months of life while receiving 3 different types of parenteral nutrition: 1) 20% glucose and a nitrogen source (Dudrick's method) 2) 12% glucose, a nitrogen source and soybean fat emulsion (Intralipid method) and 3) 12% glucose, a nitrogen source and 1% alcohol (Babson's method). All three regimens provided positive nitrogen balance of similar magnitude. The substrate-hormone relationships were appropriate. After parenteral fat free nutrition (primary caloric source glucose) the plasma glucagon levels were significantly lower and the growth hormone levels significantly higher than after the fat emulsion therapy period. The Dudrick and Intralipid methods resulted in a higher caloric intake and weight gain than the Babson method. The former two regimens cannot be completely endorsed, however, since septic and central vein complications are unavoidable with the Dudrick method particularly in the small infant; and the long term effects of intralipid, particularly on the liver are still unknown.
