Effects of chronic overload training and aging on left ventricular systolic function.

To examine the effects of age and chronic overload training on left ventricular systolic function during static exercise, Doppler echocardiography studies were performed in 14 young (28 +/- 6 years) and 10 older (51 +/- 3 years) weight lifters and also in 14 young (29 +/- 6 years) and 10 older (52 +/- 1.1 years) normal individuals during upright dead-lift isometric exercise, at 30% of maximal voluntary contraction for 3 min. At rest, older weight lifters demonstrated impaired left ventricular systolic function as compared with the other three groups. During exercise, peak and mean velocity values increased significantly (p < 0.05) from resting values in the young groups, but remained unchanged in the older groups. Flow period and acceleration and deceleration times were significantly (p < 0.05) lower in the younger subjects than those observed for the old ones, with the highest (p < 0.05) values obtained by the old weight lifters. This study suggests that the differences between the groups are related to differences in impedance and not to fundamental changes in the properties of the myocardium.

Treatment of open tibial fractures with primary suture and Ilizarov fixation.

Thirty-two open tibial fractures were treated by irrigation, debridement, antibiotic therapy, primary closure when possible, and fixation with Ilizarov device. In the Gustilo classification, 11 were Grade I, 10 Grade II, 8 Grade IIIA, and 3 were Grade IIIB. No Grade IIIC fractures were treated in the period studied. Fifteen were in patients with multiple trauma, and in 17 patients the fracture was a single injury. All fractures healed with good anatomic alignment, except for 1 with 5 degrees angulation and another with 1-cm shortening. The healing time was 21.9 weeks in the single injury and 25.7 weeks in the patients with multiple trauma. Eleven patients had pin tract infection, and in 4 the pin had to be replaced. One patient had dermatitis near the fracture site. This treatment of open tibial fractures proved to be successful for anatomic restoration of the tibia and had a low complication rate. Primary closure should be considered a possible method of treatment in Types I to IIIA open fractures, together with the Ilizarov external fixator, if the wound edges can be approximated easily with no tension at the suture line.

[Screening for scoliosis in western Galilee schools].

Scoliosis screening is still under debate. It is generally agreed that it is not cost-effective, but some still favor it as integral to preventive medicine. Among 2940 school children, aged 9 - 13, from several Moslem-Arab villages in the western Galilee, 55 (1.9%) were found to have some deformity on physical examination and were referred for spinal X-ray. Of these, 24 were X-rayed and in 20 of them there was a scoliosis of more than 7 degrees. The prevalence of in-family marriage in those with deformities was 31%, not significantly different from the 35% found in their communities. Using the worst-case analysis, the Moslem-Arab population of western Galilee cannot be regarded as a population at risk for scoliosis. Considering the poor cooperation of the parents with further investigation, we conclude that scoliosis screening should not be performed in this community.

[Magnuson-Stack operation for chronic anterior shoulder instability].

Between the years 1975-1992 we operated on 30 cases of recurrent anterior dislocation of the shoulder by the Magnuson-Stack procedure, using the subscapularis tendon. 23 patients (25 shoulders) have since been followed for an average of 5 years. We developed a comprehensive shoulder rating scale to evaluate functional status of involved shoulders. By this rating system the results were excellent in 73.9% of the cases, good in 17.4% and fair in 8.7%. The operation is technically simple, rapid, had no complications and is at least as effective as other operations.

Effect of low-power He-Ne laser on fracture healing in rats.

BACKGROUND AND OBJECTIVE: Helium-Neon (He-Ne) laser radiation has been found to accelerate fracture healing in laboratory animal models as well as in cultures of cells involved in this process. We investigated the radiological, biomechanical, and histological effects of He-Ne radiation on fracture healing in a rat model. STUDY DESIGN/MATERIALS AND METHODS: Sixty-two rats underwent bilateral open osteotomies of the tibiae followed by internal fixation with intramedullary wires. The right leg received He-Ne laser radiation of 0, 2, or 4 Joules every other day for 2 to 6 weeks while the left leg served as a control. RESULTS: Radiological and histological examinations of the osteotomy sites failed to show any enhancing effect of He-Ne laser radiation on the bone healing process. Biomechanically, the irradiated bones of two of the six test groups were significantly weaker than the controls. CONCLUSION: These results fail to support the previously reported enhancing effect of He-Ne laser radiation on fracture healing.

Influence of the time of day on physical performance in patients with coronary artery disease.

The exercise training workload for cardiac patients is determined from the peak heart rate achieved safely during a stress test. Circadian rhythms may play a key role in changing physiological responses to the stress test. Therefore, the purpose of this study was to evaluate the influence of the time of day on cardiopulmonary and metabolic responses in highly trained men with coronary artery disease. A group of 15 patients with coronary artery disease [53.5 (SD 6) years] performed two sessions of graded tests to exhaustion: one session was performed at 10 a.m. and the second at 5 p.m. in randomized order. Treadmill velocity was kept constant at a speed of 4.8 km h-1 starting with an elevation of 0% which was increased thereafter by 2.5% every 3 min. At rest the results revealed that only oxygen uptake was significantly lower (P < 0.05) in the morning compared to that observed in the evening [2.9 (SD 0.4) compared to 3.5 (SD 0.5) ml O2.kg-1.min-1, respectively]. During exercise, differences due to time of day were found in the variables of maximal oxygen uptake which were significantly higher (P < 0.05) in the evening than in the morning [34.2 (SD 2.6) and 40.8 (SD 2.5) ml O2.kg-1.min-1, respectively]. These data indicated that in these well-trained coronary artery disease patients there was a significant time of day effect associated with metabolic responses following stress-testing.

[Stabilization double osteotomy of the knee].

Stabilization osteotomy of the knee is a single or double 1-stage osteotomy done to correct ligamentous deficiency of the knee. The concept of stabilization osteotomy depends on the residual ligamentous and capsular tissues which envelope the circumference of the joint. The technique relies on open wedge osteotomy to restore alignment of the joint. Satisfactory results in 6 patients who underwent stabilization osteotomy as a salvage procedure during the past 5 years encouraged us to publish the principles of this concept.

Studies on a novel anterior cruciate ligament polyethylene fiber prosthesis: the histomorphological pattern of organization and bony anchorage of a polyethylene fiber prosthesis in the stifle of the goat.

The anterior cruciate ligament of goats was substituted by a high-tensile polyethylene fiber prosthesis. The animals were sacrificed after 6-12 months. Histologically, the implants were separated from a newly formed bony shell by a thick fibrous interface membrane, which was anchored to the bone by Sharpey-like fibers. Within the joint cavity, the implants were enclosed in thick fibrous sheaths, which were continuous with the intra-osseous interface membranes. While the inner granulomatous layer of the interface membrane extended in between the polyethylene fibrils for a short distance, the bulk of the prosthesis was poorly organized. A thick central fibrous band accompanied the intra-osseous and intra-articular portions of the implants throughout their entire lengths. Oblique fibrous tracks linked the interface membranes with the central fibrous bands. The intra-osseous tunnels were considerably expanded when compared to the initially drilled tunnels.

[Intramedullary nailing of mid-femoral fractures].

75 consecutive cases of mid-shaft femoral fractures were treated by open intramedullary nailing, using the solid, diamond-shaped Hansen-Street nail. The average age was 32.5 years; road accidents accounted for 43 cases; average operative time was 40 minutes. There were 2 systemic complications but no infections. 56 of the 75 were studied after an average of 5 years and 19 were followed for at least 6 months. At 4 months all fractures were united except 1. Average time to sound bone union was 4 months. There was no residual deformity in varus-valgus or in the sagittal plane. Malrotation was rare and did not exceed 10 degrees. There was shortening of 2 cm in 3 cases of comminuted fractures. All patients followed up returned to regular daily function. The Hansen-Street nail used in an open procedure is an accurate method of anatomic reduction. As opposed to close techniques, we found the procedure to be simple and quick, without undue exposure of the surgical team to radiation and without other complications, such as nerve damage due to traction. This device is indicated for use only in fractures of the middle third of the femoral shaft.

The use of the Hansen-Street intramedullary nail in midshaft fractures of the femur.

A retrospective study was conducted on 75 consecutive femoral midshaft fractures treated with intramedullary nailing using the solid-diamond-shape Hansen-Street nail with an open technique. The average age of patients was 32.5 years. Road accidents accounted for 43 cases. The average operative time was 40 minutes. There were two systemic complications. No infection occurred. Fifty-six cases could be gathered for this study, with an average follow-up of 5 years; 19 were followed for at least 6 months. At 4 months, all the fractures were united except one. The average time to sound bony union was 4 months. There was no residual deformity in varus-valgus or in the sagittal plane. Malrotation was rare and did not exceed 5 degrees. Shortening of 2 cm was found in 3 patients where the fracture was comminuted. All followed patients returned to regular daily function. The use of the Hansen-Street nail in an open procedure is an accurate method for anatomic reduction. As opposed to closed techniques, the procedure proved in our hands to be simple and quick, without undue exposure to radiation of the surgical team, and without such other complications as nerve complications due to traction. The procedure with this device is indicated for mid-third fractures of the femoral shaft only.

Knee arthroplasty rating.

A number of rating systems used to evaluate the results of total knee arthroplasty exist. Many of these systems are based on different concepts, and might be expected to give divergent results. To see if this was so, the authors examined a consecutive series of 235 posterior stabilized knee arthroplasties recording the results according to five rating systems: HSS (The Hospital for Special Surgery), Brigham, Freeman, BOA (British Orthopaedic Association), and the VENN diagram. In spite of their apparent differences, all point systems and the BOA gave almost identical results, while the VENN diagram proved to be the most stringent. The authors suggest that any of the current point systems may be used to "score" arthroplasties, but the results should also be rated with the VENN diagram in order to see the quality of the arthroplasty and a comparison between the different series.

[Laser therapy in the prevention and treatment of mucositis caused by anticancer chemotherapy].

The appearance of mucositis is a frequent and painful secondary effect of anticancer chemotherapy. Patients who develop oral toxicity during the first course of treatment will almost assuredly show identical side effects during each subsequent course unless the drugs are changed or the doses are lowered. In the absence of an efficacious antidote or preventive prophylaxis for such lesions to date, this report presents the results of a preliminary retrospective non-randomized study of the effect of soft-laser treatments on mucositis in cancer patients receiving combination chemotherapy, including 5-fluorouracil. Iatrogenic mucositis was observed during 43% of 53 chemotherapy cycles in the case control population. Curative laser therapy reduced the time to repair lesions and the rate of therapeutic modifications. For patients who received soft-laser therapy as a preventive measure, the incidence of oral complications was reduced to 6% during 101 cycles of chemotherapy. All of these patients, even those who have encountered mucositis before receiving preventive laser therapy, terminated their cancer therapy as originally scheduled. Well designed and carefully controlled trials will be necessary to define the place of helium-neon laser therapy in the repair and prevention of oral complications due to cancer chemotherapy.

Ten-year survivorship of cemented ceramic-ceramic total hip prosthesis.

In the first 187 consecutive alumina-alumina combination hip arthroplasties performed from 1977 to 1979, both components were cemented with conventional techniques. At ten-year follow-up evaluation, 87 patients were reviewed or interviewed by telephone, 37 were dead, 39 were lost to follow-up evaluation, and 24 failures were reoperated on before the end of ten years. The major cause of failure was aseptic loosening of the acetabular component (15 failures). Fracture of the socket and of the femoral head occurred in five patients in this series. However, these complications were not seen with components manufactured after 1979. At the end of ten years, survivorship analysis depicted a 82.59% survival rate when reoperation was considered as failure and a 88.57% rate when reoperation for aseptic loosening was considered as failure. The femoral component had a 99.16% survival rate and the acetabular component had an 88.57% survival rate when reoperation for aseptic loosening was considered as failure. Age, appearance of a two- or three-zone demarcation at the intermediate follow-up evaluation, and outer diameter of the acetabular component were the major parameters influencing the results. Better results observed in the population younger than 50 years of age may be related to the small amount of wear debris produced by the alumina-alumina combination. This combination in hip prosthesis is secure, but should be implanted in young and active patients; the outer diameter of the acetabular component must be at least 50 mm. The major problem that remains is the socket's fixation. It could be improved by a design modification, by choosing another mode of fixation, or both.

Histological features of the interface membrane of failed isoelastic cementless prostheses.

The interface membranes of both components from two isoelastic cementless hip arthroplasties, which were revised for aseptic loosening, were examined histologically. The membranes consisted of densely structured fibrous tissue interspersed with giant cell granulomas, sheets of histiocytes and chronic inflammatory infiltrates. The granulomas and the histiocytes contained polymeric and metallic wear products, respectively. The morphological features of these membranes were compared with those of cemented joint replacements. Apart from the deposits of acrylic cement bordered by macrophages, there were no distinctive differences between the membranes of failed cementless and cemented arthroplasties. The release and deposition of wear products, of whatever nature, are apparently responsible for the formation of exuberant interface membranes.

Helium-neon laser treatment transforms fibroblasts into myofibroblasts.

The differentiation of myofibroblastic cells from normal human gingival fibroblasts in vitro has been established by transmission electron microscopy and quantitated by immunohistochemistry, using antigelsolin monoclonal antibodies. Untreated control cultures were compared to cultures exposed to Helium-Neon (He-Ne) laser irradiation. A direct and massive transformation of the cultured fibroblasts into myofibroblasts was observed as early as 24 hours after laser treatment, whereas control cultures were comprised of only resting fibroblasts and active fibroblasts. This in vitro induction of myofibroblasts may be analogous to that which occurs in vivo. Therefore we undertook a similar study using biopsies from gingival tissues after wisdom tooth extraction. Myofibroblasts were present in the connective tissue of laser-treated gums 48 hours after irradiation, but not in untreated contralateral control tissues. These data provide evidence that the primary biologic effect of the Helium-Neon laser on connective tissue is the rapid generation of myofibroblasts from fibroblasts. The induction of a phenotype with contractile properties may have clinical significance in the acceleration of the wound-healing process.
