Comparison of SP-A and LPS effects on the THP-1 monocytic cell line.

Surfactant protein A (SP-A) increases production of proinflammatory cytokines by monocytic cells, including THP-1 cells, as does lipopolysaccharide (LPS). Herein we report differences in responses to these agents. First, polymyxin B inhibits the LPS response but not the SP-A response. Second, SP-A-induced increases in tumor necrosis factor-alpha (TNF-alpha), interleukin-1beta (IL-1beta), and IL-8 are reduced by >60% if SP-A is preincubated with Survanta (200 microgram/ml) for 15 min before addition to THP-1 cells. However, the LPS effects on TNF-alpha and IL-8 are inhibited by <20% and the effect on IL-1beta by <50%. Third, at Survanta levels of 1 mg/ml, SP-A-induced responses are reduced by >90%, and although the inhibitory effects on LPS action increase, they still do not reach those seen with SP-A. Finally, we tested whether SP-A could induce tolerance as LPS does. Pretreatment of THP-1 cells with LPS inhibits their response to subsequent LPS treatment 24 h later, including TNF-alpha, IL-1beta, and IL-8. Similar treatment with SP-A reduces TNF-alpha, but IL-1beta and IL-8 are further increased by the second treatment with SP-A rather than inhibited as with LPS. Thus, whereas both SP-A and LPS stimulate cytokine production, their mechanisms differ with respect to inhibition by surfactant lipids and in ability to induce tolerance.

Epithelioid sarcoma: a puzzling soft tissue neoplasm in a child.

Epithelioid sarcoma (ES) is a rare soft tissue neoplasm that usually occurs on the upper extremity of adolescents and young adults. It rarely occurs during childhood. ES is a slowly growing tumor with a high propensity for recurrences and metastases. This neoplasm is likely to be confused with a variety of benign and malignant conditions. Recognition of ES is important. The treatment consists of wide excision. Misdiagnosis can lead to inappropriate treatment and local recurrences or metastases. We report the case of a 5-year-old girl with an ES on the right forefinger.

[Detection of Prader-Willi syndrome by methylation-specific PCR].

OBJECTIVE: Using a faster and more efficient method to diagnose Prader-Willi syndrome(PWS). METHODS: Differential DNA methylation at several sites in the PWS critical region where the maternal homologue is unmethylated and transcriptionally active. Methylation-specific PCR(MSPCR) is based on sodium bisulfite treatment of DNA, which converts unmethylated but not methylated cytosine residues to uracil, and PCR primers specific for the maternal and the paternal allele. Methylation analysis by Southern blot is made to validate the MSPCR result. RESULTS: Bisulfite-modified DNA from PWS patients amplified only with methylated allele-specific primer pair showed only maternal 174bp PCR product. MSPCR and PW71B methylation studies showed an abnormal pattern consistent with the clinical diagnosis of PWS. Untreated DNA does not produce a PCR product. CONCLUSION: MSPCR can be used to detect all presently testable causes of PWS (deletion, uniparental disomy, and imprinting mutation) in an efficient first step for stepwise diagnostic testing, compared with either Southern blot analysis which is more timeconsuming, or fluorescence in situ hybridization(FISH) which can not detect uniparental disomy or imprinting mutation.

[Atopic dermatitis and domestic animals].

Several arguments are raised attributing to aeroallergens an important role in atopic dermatitis. The aeroallergens that penetrate the epidermis could be fixed by IgE on the Langerhans cells and then induce a cellular mediator reaction comparable to that of allergic contact eczema. Patch tests have been developed to evaluate the role of aeroallergens (dust mites, animal dander, etc.). Preventive anti-dust mites measures in the home of atopic patients are recommended. Eviction of domestic animals (cat, dog, etc.) or avoidance measures for animal dander in the home can produce improvement in atopic dermatitis. Oral specific immunotherapy is being validated as a treatment for this disease.

[Evaluation of exercise performance and quality of life after lung volume reduction surgery for severe emphysema].

OBJECTIVE: To evaluate grade of dyspnea, exercise performance and quality of life pre and post-lung volume reduction surgery in patients with severe emphysema. METHODS: Serial measurement of lung function, cardiopulmonary exercise test and 6-minute walk distance, and the dyspnea grade assessment by modified dyspnea scale of English Medical Research Council and quality of life by Notting-ham Health Profile were all done before and at 3, 6, 12 months after surgery. RESULTS: FVC, FEV1, MVV increased (P < 0.01) and RV, TLC, RV/TLC decreased at 3, 6, 12 months after surgery (P < 0.01), WATT, TET, VO2max, VO2max/HR, VEmax, VTmax, 6MWD were all improved (P < 0.01), and the dyspnea grade was declined as well as the NHP score(P < 0.01 or < 0.05). CONCLUSION: Lung volume reduction surgery in patients with severe emphysema may improve patient's exercise performance and quality of life.

[Craniovertebral decompression and posterior fossa reconstruction treatment of Chiari syringomyelia complex].

OBJECTIVE: To study the results of Chiari malformation/syringomyelia (CM-SM) complex treated by craniovertebral decompression and posterior fossa reconstruction (PFR). METHODS: 37 patients of CM-SM complex were treated surgically from 1994 to 1999. All patients underwent craniovertebral decompression and posterior fossa reconstruction. The procedure consisted of suboccipital craniectomy and laminectomy of C1 (when necessary C2), exploration and plugging of the obex, and resection of arachnoid adherence. A wide dural graft was used to reconstruct the cisterna magna artificially. Syrinx shunt was not performed. RESULTS: Follow-up for 0.5 to 4.5 years showed that 31 patients (83.8%) had their symptoms improved, 5 (13.5%) stabilized, but 1 (2.7%) deteriorated. Magnetic resonance was used to evaluate the morphological results. The shrinkage of the syrinx, upward migration of the hindbrain, the formation of an artificial cisterna magna were observed. CONCLUSIONS: Craniovertebral decompression and posterior fossa reconstruction in restoring the craniovertebral junction are recommended in the treatment of CM-SM complex.

[Clinical evaluation of the paste of erythromycin ethylsuccinate-metronidazole CP for root canal sterilization].

OBJECTIVE: The infected root canal was sterilized by anti-bacteria compound paste and the efficacy was compared with FC. METHODS: According to the mixed infection in infected root canal of anaerobic bacteria and obligate anaerobe, the paste made of erythromycin ethylsuccinate, metronidazole and CP was selected to seal and sterilize the root canal. RESULTS: The clinical observation of 180 patients with entirely developed root apex of acute and chronic apical periodontitis shown that there was no significant difference comparing erythromycin ethylsuccinate metronidazole-CP with FC in root canal sterilization, therefore, the irritability and poisonousness of the paste can be reduced by using erythromycin ethylsuccinate metronidazole CP instead of FC. CONCLUSION: The root canal sterilization drug of erythromycin ethylsuccinate metronidazole-CP was a safe and effective method to promote the restoration of root apex diseases.

Hormonal control of protein expression and mRNA levels of the MaxiK channel alpha subunit in myometrium.

Large conductance voltage-dependent and Ca(2+)-modulated K(+) channels play a crucial role in myometrium contractility. Western blots and immunocytochemistry of rat uterine sections or isolated cells show that MaxiK channel protein signals drastically decrease towards the end of pregnancy. Consistent with a transcriptional regulation of channel expression, mRNA levels quantified with the ribonuclease protection assay correlated well with MaxiK protein levels. As a control, Na(+)/K(+)-ATPase protein and RNA levels do not significantly change at different stages of pregnancy. The low numbers of MaxiK channels at the end of pregnancy may facilitate uterine contraction needed for parturition.

BRCA2 is required for ionizing radiation-induced assembly of Rad51 complex in vivo.

Mutations in BRCA1 and BRCA2 account for the majority of familial breast cancers. Cells with mutated BRCA1 or BRCA2 are hypersensitive to ionizing radiation (IR) and exhibit defective DNA repair. Both BRCA1 and BRCA2 have been reported to bind Rad51, a protein essential for homologous recombination and the recombinational repair of DNA double-strand breaks. In normal cells, a redistribution of Rad51 protein, manifested as formation of Rad51 nuclear foci, is seen upon IR treatment. Here we demonstrate that IR-induced Rad51 foci formation is aberrant in BRCA2- but not BRCA1-deficient tumor cells. In Capan-1 cells, which do not express functional BRCA2, there was little Rad51 foci formation in response to a wide range of radiation dosages. Moreover, forced expression of a fusion protein containing green fluorescent protein and the first Rad51-binding BRC repeat of BRCA2 in cells with wild-type BRCA2 rendered them hypersensitive to IR and cisplatin and compromised IR-induced Rad51 foci formation. In HCC1937 cells, which harbor mutation of BRCA1, IR-induced Rad51 foci were readily detected. This study suggests a requirement of BRCA2 protein for the IR-induced assembly of Rad51 complex in vivo.

Breast masses in African American teenage girls.

PURPOSE: The aim of this study was to evaluate the incidence of malignancy in breast lumps excised from African American teenagers. METHODS: The authors reviewed the pathology records at King's County Hospital Center between January 1982 and December 1992. The pathology reports and charts of all patients who had breast masses excised during this period were reviewed. Data for this study were derived from the group of African American and black Caribbean American teenage patients who underwent breast biopsies and whose pathology reports and medical records were available. The age of patients, size of the lesion, and diagnosis were recorded. RESULTS: Medical records from 155 African American and black Caribbean American girls between the ages of 13 and 19 years (inclusive) who underwent breast biopsies between January 1982 and December 1992 were reviewed. Fibroadenoma was the most common diagnosis (127 of 155; 82%). This was followed in frequency by fibrocystic mastopathy (18 of 155; 11.6%) and breast abscess (3 of 155; 2%). None of the patients had a malignancy. Review of tumor registry data from the same time period at Kings County Hospital Center and the University Hospital of Brooklyn showed that the youngest African American or black Caribbean American patient diagnosed with breast cancer was 21 years of age. CONCLUSIONS: The incidence of malignant breast lumps in African American and black Caribbean American teenagers is distinctly low. Conservative treatment in this population is warranted, and diagnosis can be made easily in most cases with either needle aspiration for cytology or core biopsy of any lesions discovered.

Effect of stem stiffness and bone stiffness on bone remodeling in cemented total hip replacement.

The hypothesis in this study is that the stem stiffness-to-bone stiffness ratio influences the incidence and type of bone remodeling and fixation with cemented total hip arthroplasty. Ninety-one patients with 99 hips had cemented stems using 3 different anatomic porous replacement designs. The APR I and APR II titanium stems with proximal porous coating on the proximal one fourth of the stem were cemented into 49 and 35 patients. The APR II-C stem, which is a cobalt-chrome stem only for cemented fixation, was cemented into 15 patients. These 3 different stem designs were used to study different metals as well as different stem shapes. The average follow-up was 4.3 years (range, 2-10 years) with all hips having 2 years' follow-up and 42 hips at least 5 years' follow-up. Bone remodeling was measured as stress shielding, calcar resorption, and distal hypertrophy on anteroposterior and lateral radiographs of the hip. Stress shielding was measured by the 4 grades described by Engh. A stem stiffness-to-femoral bone stiffness ratio was calculated from the plain radiographs with the stem stiffness known from the manufacturer and the bone stiffness calculated using measurements of the outer and inner diameters of the femur. There was no statistical difference for bone remodeling and fixation between the 3 stem shapes or 2 metal types used in these hips. No stem was loose, and only 10 had radiolucent lines. Stress shielding was statistically related to stem stiffness but was more strongly related to the axial stiffness ratio, mediolateral bending stiffness ratio, anteroposterior stiffness ratio, and torsional stiffness ratio. Stress shielding grade 3 and 4 was present in 20% of hips with a torsional stiffness ratio < 0.33, in 38% of hips with a torsional stiffness ratio of 0.34 to 0.5, and in 70% of hips with a torsional stiffness ratio > 0.5. Five-year results showed no statistical change in stress shielding, calcar resorption, and distal hypertrophy from the 2-year observations. The stem stiffness-to-bone stiffness ratio influenced bone remodeling but not fixation of these cemented stems.

Tinea capitis in two black african adults with HIV infection.

Tinea capitis is rare in adults, although a few cases have been described in HIV-infected patients. We present two cases in black African adults who were HIV positive. In one, the condition led to the diagnosis of HIV infection. It is possible that the rarity of tinea capitis in HIV-positive adults could be related to increased colonization of their scalp by Pityrosporum (Malassezia) spp. In patients well controlled with an antiviral therapy, the treatment of tinea capitis seems no more difficult than in non-immunosuppressed patients. There is possibly a relation between clinical presentation and degree of immunodeficiency.

High prevalence of cagA-positive strains in Helicobacter pylori-infected, healthy, young Chinese adults.

BACKGROUND: Cytotoxin-associated gene A (cagA) has been implicated as a potential pathogenic marker for Helicobacter pylori-induced severe gastroduodenal diseases. Although the prevalence of cagA-positive strains has been reported in patient populations from developed countries, only limited information from developing countries is available. METHODS: Polymerase chain reaction (PCR) in combination with immunoblot analysis was used to determine the prevalence of cagA and its adjacent cagE genes and to evaluate the expression of CagA protein in 55 H. pylori clinical isolates from China. RESULTS: The expected PCR products derived from H. pylori cagA and cagE genes were identified in all Chinese H. pylori clinical isolates. Similarly, the CagA protein was detected in all 40 isolates tested. CONCLUSIONS: These results demonstrated that the presence of the cagA gene correlated well with expression of the CagA protein in all surveyed Chinese H. pylori isolates and that infection with cagA-positive H. pylori strains is highly common in China and independent of clinical presentation.

Amelioration of rat cerulein pancreatitis by guamerin-derived peptide, a novel elastase inhibitor.

Increased activity of various proteases is observed in both human and experimental pancreatitis; however, the information on the effects of specific protease inhibitors on the disease is limited. In this study we show that a novel elastase inhibitor, guamerin-derived synthetic peptide (GDSP), improves the parameters of cerulein-induced acute pancreatitis in the rat. The effects of GDSP on pancreatic weight, serum amylase and lipase, morphologic changes in the pancreas, neutrophil infiltration, and nuclear factor KB (NF-KB) activation were measured in rats infused with supramaximal dose of cerulein (5 (g/kg/h) for 6 h. The effects of GDSP were also measured on superoxide formation by activated human neutrophils. The effects of GDSP were compared with those of another elastase inhibitor, elastatinal. GDSP significantly inhibited edema formation, neutrophil infiltration, acinar cell damage, and plasma lipase and amylase increases caused by cerulein. GDSP also completely inhibited superoxide formation in the human neutrophils stimulated by N-formyl-methionine-leucine-phenyl-alanine (fMLP) or 12-O-tetradecanoylphorbol-13-acetate (TPA). Elastatinal had some of the same effects as GDSP but was less potent and effective. These results demonstrate a beneficial effect of GDSP, a novel specific elastase inhibitor, on the development of rat cerulein pancreatitis.

Intracisternal TRH analog increases gastrin release and corpus histidine decarboxylase activity in rats.

Thyrotropin-releasing hormone (TRH) acts in brain stem nuclei to induce vagally mediated stimulation of gastric secretion. The effects of intracisternal injection of the TRH analog RX-77368 on plasma gastrin levels and corpus histidine decarboxylase (HDC) activity were studied in 48-h fasted conscious rats. RX-77368 (25-100 ng) increased plasma gastrin levels by threefold at 30 min, which remained significantly higher than control at 2 and 4 h postinjection. Corpus HDC activity began to increase at 2 h and reached a peak at 4 h postinjection with a 21-fold maximum response observed at 50 ng. Morphological changes in the appearance of corpus HDC-immunoreactive cells correlated well with HDC activity. Pretreatment with gastrin monoclonal antibody completely prevented RX-77368 stimulatory effects on HDC activity. Atropine significantly attenuated gastrin increase at 30 min by 26%. These results indicated that in conscious fasted rats, TRH analog acts in the brain to increase corpus HDC activity in the enterochromaffin-like cells, which involves gastrin release stimulated by central TRH analog.

An unusual case of squamous cell carcinoma of the nose.

A 49-year-old man presented with a tumoral lesion of the tip of the nose which had been manifest for 2 months. Within a few weeks, the tumour increased in size and became infiltrated. The biopsy showed a squamous cell carcinoma. Treatment consisted of a radical surgical excision.

Pharmacology of LY315920/S-5920, [[3-(aminooxoacetyl)-2-ethyl-1- (phenylmethyl)-1H-indol-4-yl]oxy] acetate, a potent and selective secretory phospholipase A2 inhibitor: A new class of anti-inflammatory drugs, SPI.

LY315920 is a potent, selective inhibitor of recombinant human, group IIA, nonpancreatic secretory PLA2 (sPLA2). In a chromogenic isolated enzyme assay, LY315920 inhibited sPLA2 activity with an IC50 of 9 +/- 1 nM or 7.3 x 10(-6) mole fraction, which approached the stiochiometric limit of this assay. The true potency of LY315920 was defined using a deoxycholate/phosphatidylcholine assay with a mole fraction of 1.5 x 10(-6). LY315920 was 40-fold less active against human, group IB, pancreatic sPLA2 and was inactive against cytosolic PLA2 and the constitutive and inducible forms of cyclooxygenase. Human sPLA2-induced release of thromboxane A2 (TXA2) from isolated guinea pig lung bronchoalveolar lavage cells was inhibited by LY315920 with an IC50 of 0.79 microM. The release of TXA2 from these cells by N-formyl-methionyl-leucyl-phenylalanine or arachidonic acid was not inhibited. The i.v. administration of LY315920, 5 min before harvesting the bronchoalveolar lavage cells, resulted in the inhibition of sPLA2-induced production of TXA2 with an ED50 of 16.1 mg/kg. Challenge of guinea pig lung pleural strips with sPLA2 produced contractile responses that were suppressed in a concentration-dependent manner by LY315920 with an apparent KB of 83 +/- 14 nM. Contractile responses induced by arachidonic acid were not altered. Intravenous or oral administration of LY315920 to transgenic mice expressing the human sPLA2 protein inhibited serum sPLA2 activity in a dose-related manner over a 4-h time course. LY315920 is a potent and selective sPLA2 inhibitor and represents a new class of anti-inflammatory agent designated SPI. This agent is currently undergoing clinical evaluation and should help to define the role of sPLA2 in various inflammatory disease states.
