In the second half of the year 1985, during the rainy season, an epidemic of Dengue Fever was recognized in Nicaragua. A total of 17,483 cases were reported by the health services. The highest morbidity and attack rates were reported between August and November of the same year. Regions II (Leon and Chinandega), III (Managua) and IV (Masaya, Granada, Carazo y Rivas) reported 89% of the cases; these regions are precisely the more densely populated in the country, located in the Pacific Coast. Leon and Chinandega were the more affected cities reporting 41% of the cases of the epidemic. Sixty seven percent of the cases were adults, 57% were female. The national attack rate was 55.4/10,000 inhab. An intensive campaign for the control of the vector was launched immediately after the epidemic was recognized and by the month of October 1985 morbidity decreased and an endemic situation was established. Seven fatal cases were reported in adults. They were considered as DHF/DSS cases by a multidisciplinary group of pathologists and clinicians, considering the criteria of WHO and the experience obtained during the cuban DHF/DSS epidemic, in 1981. The outbreak was considered as a Classical Dengue Fever epidemic with 7 fatalities. Dengue-1 and -2 were isolated from acute sera and Dengue-1 from one of the dead.
A retrospective seroepidemiologic survey is made in the municipalities of Cienfuegos and Palmira to determine the presence of neutralizing antibodies to Den-1 and -2 virus in the population. It was found that 21.4% of the population studied had antibodies to Den-1, 12.3% to Den-2, and that 5.5% had been infected by the two serotypes (secondary infection). It became apparent that in the Palmira municipality the percentage of infection due to Den-1 was higher and it was found that in 1980-1981 there was a late dengue (Den-1) outbreak. Therefore, this municipality was probably the only in the country with 2 epidemic outbreaks due to two dengue serotypes (1 and 2) in a 6 month interval. The potential relationship between this fact and the high lethality of hemorrhagic dengue fever in this municipality is established. The influence of environmental, social, and sanitary factors on the risk of infection is suggested. It was found that Whites and Blacks are equally infected and that infection is not a random phenomenon, instead it is favored by the presence of vectors in and around the housing.

A preservative attitude towards splenic traumatism is globally no more discussed. But the best way to succeed for the general surgeon has to be precise: non operative either coeliotomy for preservative surgery? About 55 observations of splenic traumatisms, the authors compare advantages with drawback of each position and try to show out the factors acting on each choice to aim a preservative score the highest possible. Now the global preservative score is 47%. The non operative proceeding lead 5/12 to failure. 10 preservative surgery initially performed lead to 10 success: preservative surgery, in our hands give the best results. Preservative surgery for a greater number of cases, and mainly for polytraumatisms (or multiple contusions) for which the collaboration of surgeons from different specialties make more difficult, for more stratified, the supervision in case of non operative proceeding.
Hybrid intratracheal pulmonary ventilation (h-ITPV) is a continuous flow ventilatory technique that uses a "reverse thruster" catheter to redirect the flow of gas away from the carina. We report here the use of h-ITPV in a pediatric patient with acute sickle cell chest syndrome who required venoarterial ECMO support because of refractory hypoxemic respiratory failure. Her ECMO course was complicated by air leaks, coagulopathy, cardiac tamponade, and necrotizing tracheobronchitis. She could be weaned from ECMO only by maintaining high pressure conventional ventilatory support. To prevent ventilator induced barotrauma, we initiated h-ITPV and weaned her from ECMO bypass. After 12 days of h-ITPV, with tidal volumes of 2-3 ml/kg at carinal peak inspiratory pressures of 25-30 cm H2O, the air leaks ceased and h-ITPV was discontinued. Dead space ventilation fraction (VD/VT) as low as 0.29 was achieved with this technique. Post-h-ITPV bronchoscopy displayed a dramatic resolution of the necrotizing tracheobronchitis. The patient survived and was discharged from the hospital. We conclude that the use of hybrid ITPV may facilitate weaning from ECMO to low pressure conventional ventilation and prevent the development of pulmonary barotrauma.

We describe a patient with a non-0:1, non-0:139 Vibrio cholerae septicemia associated with ecythema gangrenosa-like skin lesions. The patient acquired the infection in Puerto Rico. Given the high fatality rate, it is important for the medical community to consider the diagnosis in high risk patients with exposures in Puerto Rico tropical waters.
