Acute intoxication with verapamil.

A healthy 18-year-old man was admitted to our unit two hours after a suicidal ingestion of 2 gm of verapamil. There was mild hypotension, depression of the sinus node, atrioventricular dissociation, changes of repolarization, and first-degree intranodal atrioventricular block (the His bundle electrogram revealed an atrio-His [A-H] interval of 155 msec). Twenty-four hours after ingestion, the patient was well, and the electrocardiogram was completely normal. This case gave us a good opporturnity to study the electrocardiologic effects of a huge oral dose of verapamil on a healthy young heart.

[Antifungal activity of aromatic essential oils (author's transl)].

"Paragerm" is commonly used in hospitals where its bactericide action is well known. Preliminary tests have been carried out in the laboratory in order to study the antifungal power of this product. Variation of the different concentrations in the culture medium, variation of the time of contact between "Paragerm" and the fungus and both the mechanical and chemical effects of the product have shown that, among the species selected, certain were more sensitive and others more resistant to this solution and that, in our experimentation, "Paragerm" had a fungistatic effect.

Denaturization of allergen P: effect on allergenicity, antigenicity and immunogenicity.

When allergen P was denatured by 8M urea, the modified molecule still reacted with IgE specific for the native allergen but not with hemagglutinating antibodies. Heating at 100 degrees C abolished the reaction in both cases. The results suggested differences between allergenic and antigenic capacities which may be based on structural differences of the antigenic determinants.

Incidence of pneumothorax and pneumomediastinum in patients with aspiration pneumonia requiring ventilatory support.

In a prospective study of patients with ventilatory support, six (38 percent) of 16 patients who had ventilatory support because of aspiration pneumonia developed pneumothorax and pneumomediastinum. In contrast, the incidence of barotrauma in the entire group of patients who had ventilatory support during a year was 4 percent (22/553) (P less than 0.001). This greater incidence in the group with aspiration pneumonia was also observed when patients who were receiving ventilatory support with positive end-expiratory pressure were excluded.

[Diagnosis and assessment of therapeutic response of bone metastases by bone scintigraphy and PET study].

Nuclear medicine plays an important role in staging and evaluation of the initial extension and response to treatment of bone metastases. In order to accurately read Bone Scintigraphy (BS) and Positron Emission Tomography scan (PET) procedures, it is essential to understand the different behavior of these studies. We present a case report of a woman treated for breast cancer, with suspicion of recurrent disease due to increase of tumor markers. Initial BS showed non-conclusive findings, whereas PET study was consistent with a spread of bone metastases. The patient underwent both procedures again after a course of chemotherapy. Post-treatment BS showed progression of bone lesions, while PET showed good therapeutic response. The PET demonstrates lesions earlier and more effectively than the bone scintigraphy in the evaluation of the therapeutic response of bone metastases.

[Cancer incidence estimation in Spain: period 1993-1996].

OBJECTIVE: To estimate the number of incident cases of cancer in Spain between 1993 and 1996. METHODS: We used data on the incidence of cancer from nine Spanish population-based cancer registries published in the monograph Cancer incidence in Five Continents, vols. VI and VII (period 1983-92). The National Institute of Statistics provided mortality data (period 1983-96). Generalized linear mixed models were used to estimate the number of incident cases. The incidence/mortality ratio was modeled. To account for heterogeneity, the providence of residence was considered as a random effect. Other factors analyzed were sex, age and period. Model parameters were estimated using a Bayesian approach with BUGS software. Estimates were valdated by comparing the observed number of cases with those predicted by the model in the regions with cancer registry data. RESULTS: The average number of incident cases per annum for all cancer sites except non-melanoma skin cancer was 78,440 for men and 55,480 for women. The most frequent neoplasm in men was lung with 15,480 cases followed by bladder with 9,445 cases and colorectal with 8,876 cases. In women the most frequent cancer was breast with 13,490 cases followed by colorectal with 8,274 cases. These absolute frequencies showed an increasing time trend for all cancers and for the most frequent sites, with the exception of stomach and uterine cervix. Internal validation of the estimates allowed calculation of a relative error smaller than 10%. IMPLICATIONS: This is the first time that the number of incident cases of cancer in Spain has been estimated with methods that account for heterogeneity. These figures show the importance of cancer as a public health problem in our community.

[Rapid diagnosis of cytomegalovirus in immunocompromised patients using monoclonal antibodies that detect early viral antigens].

A technique was applied to detect early fluorescent antigens (DEFA) of cytomegalovirus (CMV) using the E13 monoclonal antibodies in 52 immuno-compromised patients hospitalized in the Nephrology Institute of Havana. Of the 75 urine or blood (buffy coat) samples taken, 15 were found positive to CMV. Using classical diploid human fibroblast isolation technique, 12 CMV strains were isolation of previously detected positive samples by DEFA. In addition, CMV was isolated from one sample reported to be negative by DEFA. A coincidence of 80% was found between both techniques. With the ELISA test, all the sample studied have IgG antibodies to CMV.

Dengue 2 virus enhancement in asthmatic and non asthmatic individual.

During the 1981 dengue hemorrhagic fever/dengue shock syndrome (DHF/DSS) Cuban epidemic, bronchial asthma (BA) was frequently found as a personal or family antecedent in dengue hemorrhagic fever patients. Considering that antibody dependent enhancement (ADE) plays an important role in the etiopathogenic mechanism of DHF/DSS, we decide to study the Dengue 2 virus (D2V) capability of replication in peripheral blood leukocytes (PBL) from asthmatic patients and healthy persons. In 90% of asthmatic patients and 53.8% of control group it was obtained PBL with a significant D2V enhancing activity (X2 p < 0.01). Power enhancement was higher in asthmatic group. This is the first in vitro study relating BA and the dengue 2 virus immuno enhancement. The results obtained support the role of BA as a risk factor for DHF/DSS as already described on epidemiological data.

[Infection associated with intravascular catheter in 1998 at the University Clinic of Navarra].

PURPOSE: To study the intravascular catheter related infections (CRI) since January of 1998 to January of 1999 in our hospital. METHODS: We studied 540 catheter tips using a modified combination of the cuantitative method of Cleri and the semicuantitative method of Maki. The catheters were classified into two groups according to the presence or absence of criteria for CRI. RESULTS: 74.5% of the retired catheters because of infection suspice did not satisfied criteria for CRI. 44.7% of the patients with criteria suffered a catheter-related bacteriemia while just 1.7% of the patients without criteria suffered a bacteriemia. The most common isolated microorganisms were coagulase-negative Staphylococci, Corynebacterium species and S. aureus. DISCUSSION AND CONCLUSIONS: At least, 74.5% of the patients with a suspice of catheter related infection could undergo a non invasive diagnosis procedure that would have showed that the catheter was not infected.
